About Us

Adventist HealthCare Washington Adventist Hospital is an acute-care facility located in Takoma Park, Maryland. Opened in 1907, the hospital is Montgomery County’s first cardiac center, performing hundreds of open-heart surgeries and thousands of heart catheterizations each year. 100 YEARS OF CARING FOR THE COMMUNITY... AND COUNTING! In 2007, Adventist HealthCare Washington Adventist Hospital celebrated 100 years of providing care to the communities it serves. As the hospital did a hundred years ago, it continues to strive to remain ahead. For this reason, the hospital is in the process of seeking state approval to build a relocated hospital on a 48-acre parcel of land near White Oak, about six miles from our current location and within the hospital’s service area. The White Oak hospital, along with medical services retained in Takoma Park, will strengthen health care in our region and expand access to health care for the communities it serves in Montgomery and Prince George’s counties and our surrounding areas. LEADING EDGE, STATE-OF-THE-ART PROCEDURES Adventist HealthCare Washington Adventist Hospital has provided the Washington D.C. region with the most advanced, least invasive cardiac and vascular procedures with skill, compassion and experience for more than 50 years. The hospital was the first in the D.C. area to perform open heart surgery, numerous

cardiac procedures, including mitral valvuloplasty, and the first in the mid-Atlantic to perform a revolutionary type of surgery called Mini-Maze that can correct atrial fibrillation (AF), a common form of heart rhythm abnormality and a major cause of stroke. HOSPITAL-BASED RADIATION ONCOLOGY Adventist HealthCare Washington Adventist Hospital’s radiation oncology program is the first hospital-based facility in all of Montgomery and Prince Georges Counties, and the first facility in the Washington Metropolitan area to be accredited by the American College of Radiation Oncology (ACRO). Visitor & Patient Information Our commitment to quality medical care is more than just words. We have implemented a formal quality and safety program throughout the hospital. These processes, which encourage employees to establish goals and continuously look for ways to improve service and patient safety, are designed to ensure that our patients receive the best available care. Visiting Philosophy Adventist HealthCare Washington Adventist Hospital recognizes that visits from family members, close friends and clergy are an important part of your healing process. Our visitation policy provides a safe environment that promotes patients’ physical and emotional well-being, provides adequate patient rest and allows time for medical therapies and nursing care. Visitors are encouraged to communicate with the patient’s nurse to discuss visiting arrangements. Visitors are asked to respect the nurse’s direction in coordinating visiting times according to each patient’s care needs and the needs of the family. If you need to enter the hospital after 8 p.m., please use the entrance next to the Emergency Department. Valet Parking In an effort to better meet our patient’s needs, we now offer valet parking service at Adventist HealthCare Washington Adventist Hospital. This service is intended to significantly alleviate the parking hassles that our patients and visitors face on a daily basis. Patient’s and other drivers will be able to access this service at the main entrance circle of the hospital, under the canopy area.

Planning Your Hospital Stay

What to Expect • Patients are encouraged to have laboratory tests completed 24 to 72 hours in advance of being admitted to the hospital for a surgical procedure. • Bring your hospitalization insurance card, policy or certificate with you when you check in, as well as a list of medications you are currently taking. •Y ou will be asked many questions to ensure that your medical record is complete. All information is kept strictly confidential. • If you have Advance Directives, please bring an original copy to the hospital to be placed in your medical chart. If you need a copy of the form to fill out your Advance Directives, the Admitting Office, Patient and Guest Services or the nursing unit can provide you with a form. •Y ou will be asked to sign several release and consent forms for medical and/or surgical treatment. The law requires this for your protection as well as the hospital’s. Permits for treatment of children under 18 must be signed by parents or legal guardians. •Y ou will be given an identification band to wear on your wrist. As a safeguard, you should keep the band on during your entire stay. • If you have any questions regarding payment of your bill, you may call the financial counselor at 301-891-5351, Monday-Friday, 8 a.m.-4:30 p.m. • Any medicine you need will be prescribed by your doctor and given to you by the nursing staff. Please notify your nurse of any medications you have brought with you. • Be sure to bring only essential personal items such as clothing, glasses, hearing aids and dentures. When these items are not in use, keep them in a case or container placed in the drawer of your bedside table. If you have brought valuables with you, you may choose to send them back home with family members or alert your nurse so they can be properly stored. Valuables may be deposited with the Admitting Office upon admission. The hospital cannot be held responsible for loss of personal valuables.

Preparing to Leave the Hospital

The Social Work and Case Management departments offer assistance to patients and families to develop the post-hospital care plan that is best for each patient. The social workers and case managers are able to assess patient care needs, provide information on aftercare services available in the community, assist in making arrangements for care at home or in another health care facility, and offer counseling to help deal with the emotional and physical demands of the illness. In addition, a medical assistance worker is available to take and process medical assistance applications. The procedure for leaving the hospital is outlined below: 1. Your physician will write a discharge order on your chart. 2. When you have been discharged, you will be asked to stop by the Admitting Office for a brief check-out, to ensure all your insurance and/or contact information is complete and accurate. 3. So that we may prepare your room for the next patient, we ask that you plan to leave no later than 11 a.m. on the day of your discharge. You are charged for your room on the day of admission, but not for the day of discharge if you check out by 11 a.m. Patients are often discharged to other levels of care, such as rehabilitation and nursing centers or home health. These other levels of care provide excellent services to meet the patient’s post-hospital needs. If you have a concern about discharge planning, please call the Social Work/Discharge Planning Department at 301-891-5465 as soon after admission as possible to begin the planning process.

Volunteering Makes a Difference Imagine the possibilities to transform your community by serving the diverse population at Adventist HealthCare Washington Adventist Hospital. You have the chance to uplift, change, and impact the lives of the patients and guests that pass through our doors. Volunteers are currently needed to fulfill our mission to “demonstrate God’s care by improving the health of people and communities through a ministry of physical, mental and spiritual healing.” Hospital volunteerism offers great variety and is not just for the medically-minded. Volunteers work in a multitude of departments and are determined by skill set, availability, and experience. OPPORTUNITIES Volunteers are accepted at a minimum of 15 years old and may be placed in one of the following capacities: • Patient Care - Volunteers with an interest in the medical field may be especially interested to work around patients or assisting techs. Some examples include Transport, Post Anesthesia Care Unit, and several Medical/Surgical units. No previous training is required and spaces are limited. • Office/Clerical Support - Our greatest demand is the need for assistance in filing, paperwork, phones, and other tasks necessary to maintaining an efficient hospital. Work is often stationary and requires strong English proficiency skills. • Guest Relations - Patients are our first priority and volunteers are needed in the guest relations division. Strong interpersonal skills are required. • Mended Hearts - Are you interested in volunteering with cardiac patients? Have you undergone complex heart surgery yourself? Consider working as a Mended Hearts Volunteer in the Adventist HealthCare Washington Adventist Hospital Chapter. Population Health Programs Washington Adventist Hospital has a number of programs and partnerships that aim to address our patients’ broader health needs and spread our reach beyond the hospital’s walls and into the community. The following programs and partnerships are at the core of our mission to improve population health. Hospital Programs Our hospital-wide programs aim to keep patients healthy in the community after an acute-care episode. Transitional Care Program Free to Washington Adventist Hospital patients, the transitional care program is designed to ensure that, upon hospital discharge, patients are returning to a safe environment and have the information and tools necessary to successfully manage their health and prevent an unnecessary return to the hospital. How does it work? Before you are discharged from the hospital, a registered nurse (RN) will meet with you and explain the program in detail and ask if you would like to participate. • If you accept, you will be set up with a scheduled home visit within 48-72 hours of being discharged. • The home visit is then followed by phone calls for 90 days, on a needs basis, until you are stabilized and ready to return to your daily activities. During the 40-45 minute home visit, the nurse carries out a number of tasks intended to empower patients and ensure their safety: U-Turn Program This program is focused on decreasing unnecessary hospital visits and unnecessary returns to the hospital. Upon entry to the Emergency Department (ED), a RN assesses patients for medical and social needs and works to connect them with the appropriate emergency care and community services to be treated and discharged directly from the ED. SeedCo SeedCo is an online software that connects patients and their families to public and private benefits programs. Washington Adventist Hospital is the first hospital in the country to start such an initiative. Often patients are not aware of the benefits programs that they are eligible for. This system identifies and screens patients for a number of benefits such as Medicaid, food stamps, child care subsidies, housing benefits, transportation vouchers, and tax credits. Staff and volunteers follow up with patients and help them through any remaining application processes.

Population Health Programs Tobacco Cessation Program The program consists of RNs identifying patients at the hospital who are smokers, assessing them based on their tobacco cessation history and their willingness to quit, and encouraging them to join the program. The program offers patients free nicotine replacement therapy and over-the-phone counseling for up to a year. The RNs also follow up with them in intervals to coach them and try to help them stay quit. For more information, call 301-891-5004 or visit www.AdventistHealthCare.com/QuitSmoking Remote Patient Monitoring Program The program identifies patients in the hospital with chronic conditions prior to discharge and sends them home with remote home monitoring devices. The devices ask questions each day about symptoms related to their heart and allows them to input biometric data. The devices are Bluetooth or wireless connected, and the data gets sent to a dashboard at Washington Adventist Hospital almost immediately after the patient records it. A nurse then looks gets a list of all of the patients daily. If a patient is having issues, the nurses intervene assist patients in getting them what they need and avoid an unnecessary return to the hospital. Walgreens 340b Drug Program This is a partnership with Walgreens to provide a 30 day supply of discharge medications prior to discharge from Washington Adventist Hospital to ensure all patients have their needed medications in their hand prior to leaving the hospital. Churches and Parish Nurses Adventist HealthCare works with the local parish nurse program, and works closely with local congregations. Around 10-15 religious organizations meet once every two-months in the offices of Washington Adventist Hospital. Parish nurses partner with Adventist on the traditional care needs of discharged patients, and go into the homes of the patients and provide a range of services, including everything from cardiovascular screenings, to taking individuals to the hospital, to trainings on women’s empowerment, self-defense classes, skin cancer screening, and children’s bullying. Prescription Produce Program (partnership with Long Branch Health Enterprise) Washington Adventist Hospital partners with the Crossroad Community Health Market (less than a mile) and Long Branch Health Enterprise to provide a “prescription” for healthy foods for its underinsured/uninsured diabetic patients. Under the “double food stamps” program, the community market gives physicians fresh-checks and they can prescribe them to some of these patients, at half of the cost. The market has commitments from 22 vendors and local farmers.

Diabetes Education Program The Diabetes Education Program at Adventist HealthCare Washington Adventist Hospital and Adventist HealthCare Shady Grove Medical Center provides comprehensive education and training by Certified Diabetes Educators to help patients learn and manage all aspects of diabetes care and prevent complications. Special Programs and Services Look Good Feel Better Cancer Support Group The American Cancer Society’s Look Good Feel Better program is dedicated to improving the selfesteem and quality of life of people undergoing the treatment of cancer. The program’s approach is to improve the self-image appearance through complimentary group, individual, and self-help beauty sessions that create a sense of support, confidence, courage and community. Adventist HealthCare has partnered with the American Cancer Society to bring quarterly twohour sessions are facilitated by a certified cosmetologist who teaches women undergoing cancer treatment about make-up tips and tricks, turban use, wig care and other information related to beauty. A free make-up kit is also given to each participant! Registration Line at 800-542-5096

Community Screening Program Our Community Screening Program includes brief consultation (1-4 minutes) with a Health Practitioner in regards to your heart health risk and desired screenings. We will be offering comprehensive screenings for cancer, cholesterol, heart disease, diabetes and body fat. Some screenings require a fee and fasting. Walgreens Pharmacy Well Transitions Program A long wait at the pharmacy to fill a new prescription is something you come to expect, but it is often the last thing you want to do after leaving the hospital. To tackle this issue and to help ease the transition from hospital to home, Washington Adventist Hospital and Walgreens. WellTransitions Program offers a number of services including: • Medication review – pharmacists review patients’ prescriptions upon admission to the hospital and at discharge; check for possible duplication of therapy and simplify medication regimen • Bedside medication delivery – Walgreens pharmacy staff member delivers medication to patient’s room and offers additional counseling as needed • Patient counseling – pharmacists provide medication counseling to both the patient and caregiver and work in close communication with the medical staff • Regularly-scheduled follow-up calls – pharmacists conduct regular calls to follow up on patient progress, discuss regimen and answer any questions or concerns • 24/7 pharmacist support – pharmacy staff is available to answer patient questions 24/7 either over the phone or online

Special Programs and Services Adventist HealthCare Adventist Medical Group Adventist HealthCare Adventist Medical Group is a local, multi-specialty physician network with convenient locations offering quality primary and specialty medical care. Community Clinic (CCI) Community Clinic (CCI) is one of the oldest and largest providers of medical services to medically under-served people in Montgomery and northwestern Prince George’s counties, CCI offers two major health services programs. In the primary care program, CCI serves more than 10,000 children and adults who are economically disadvantaged, homeless, disabled, with state medical assistance coverage or uninsured. The state of Maryland Women, Infants and Children (WIC) nutrition education and food supplement program enrolls more than 30,000 pregnant and breast-feeding women and children under the age of five. Over half the pregnant women in Montgomery County receive WIC services. Together, the programs serve more than 45,000 people in need who count on CCI for medical and health-related care. Montgomery County Department of Transportation Services for Seniors & Persons with Disabilities Montgomery County is committed to providing mobility for seniors and persons with disabilities. All of Ride On’s buses are wheelchair accessible. The County’s transit system is one of the best in the country. Emergency Care

At Adventist HealthCare, we provide emergency health care services to more than 150,000 patients each year ranging in age from newborns to the elderly. Our facilities are open 24 hours a day, seven days a week and are committed to providing the highest quality emergency care in an efficient and timely manner. Our doors are open to anyone in need of emergency medical services. The following are some guidelines that can help you or your loved ones in an emergency or urgent care situation. When in doubt, never hesitate to call your doctor, go to the emergency department, or call 911 for ambulance assistance. WHEN SHOULD I VISIT THE EMERGENCY DEPARTMENT? These symptoms warrant an immediate trip to the Emergency Department: • Chest pain • Pain in the left arm, neck and jaw • Shortness of breath • Sudden onset headache that can be described as “the worst headache of your life” • Weakness on one side of your body or face. Weakness in one arm or leg • Slurred speech • Bleeding from a cut that will not stop with applied pressure • Vomiting blood or bright red blood in your bowel movements These symptoms warrant a call to your doctor: • Body aches, joint pain, or muscle pain • Flu-like symptoms that persist or get worse over 2 to 3 days

Emergency Care • Any known medication side effect (e.g. nausea, diarrhea, vomiting, mild rash, yellowing of the skin or whites of the eyes) • Any other sign or symptom that is not typical for you WHAT IS THE MOST IMPORTANT THING FOR ME TO BRING TO THE ED? Bring your medical history to the ED. HOW MUCH TIME WILL MY VISIT TAKE? The length of your wait depends on how many patients are in the ED and how ill you are. The ED sees the most ill patients first. Upon arriving at the hospital, patients who did not come via ambulance will first see a nurse who will begin the triage process. Patients will be assessed and prioritized based on the seriousness of illness or injury. Since each individual will require different levels of medical care and treatment, the length of visit will vary for each patient. CAN I HAVE VISITORS WHILE I AM IN THE ED? Due to limited space and to ensure optimal patient privacy, patients are allowed a limited number of visitors. WILL I NEED TO BE ADMITTED TO THE HOSPITAL? Admittance to the hospital is determined after a thorough examination. Most patients seen in the ED do not need to be admitted to the hospital, but patients who need tests or treatments not conducted in the ED are admitted. WHAT HAPPENS IF THE ED DOCTOR IS NOT ABLE TO PROVIDE ALL OF THE TREATMENTS I NEED WHILE I AM IN THE ED? In the ED, our first goal is to treat life-threatening illnesses and pain. Your illness may need more longterm care than we are able to provide in the ED. It is important that you go to your doctor or clinic appointment after leaving the ED, so they may continue to provide treatment for your illness. HOW DO I PAY FOR MY TREATMENT? The registration staff can answer any questions that you may have about how to pay for your treatment. After being seen by our physician and your condition is stabilized, you may be asked for your insurance co-pay or to make payment arrangements.

Receive Care for Life

Continuing care communities offer support for changing health needs

C

ontinuing Care Retirement Communities (CCRCs) offer long-term contracts that guarantee lifelong shelter and access to specified health care services. A staff of medical professionals is on hand to provide different levels of care, from independent living to assisted living and nursing care. A member can move up or down this continuum of care for the rest of their life. Most CCRCs establish requirements for incoming residents based on age, financial assets, income level and physical health. Residents are typically expected to move in while they are still independent to get to know the community. In return for guaranteed lifelong shelter, amenities and health care, residents usually pay a lump-sum entrance fee and regular monthly payments.

There are three basic types of CCRC contracts. An extensive contract covers shelter, residential services, and amenities, plus unlimited long-term nursing care without an increase in monthly payments (except for increases related to operating costs and inflation adjustments). An extensive contract spreads the risk of catastrophic health care costs among all residents, so that no single person faces financial ruin. Entrance fees and monthly costs under this contract are typically higher than those under modified or fee-for-service contracts. A modified continuing care contract covers shelter, residential services and amenities, plus a specified amount of nursing care. After the specified amount has been received, the resident can receive

care at daily or monthly nursing care rates. A fee-for-service continuing care contract covers shelter, residential services and amenities. While emergency and shortterm nursing care is usually included in the contract, access to long-term nursing care is guaranteed only at daily rates. Entrance and monthly fees are lower because residents are responsible for all long-term nursing and health care costs. Most CCRCs provide a variety of residential services that may be covered by monthly fees or be available at extra cost, depending on the contract. Visit the CCRCs you are considering with a trusted family member. Immerse yourself as much as possible in the daily life, and assess the management’s philosophy and its relationship with residents.

Understand Entrance Fees

Knowing the options when paying for Continuing Care Retirement Communities(CCRCs)

M

ost CCRCs require payment of a one-time fee, usually called the entrance fee, before moving to the community. The entrance fee amount often depends on the level of care at the time of entry, type of housing chosen (with larger units requiring a larger fee) and agreement type. The entrance fee allows the CCRC to offer lower monthly fees and provide for the future needs of residents. State regulations can impact the terms of the entrance fee and in some states, refunds are called rebates. Entrance fees are typically structured in one of three ways: Declining scale refunds, also known as amortizing entrance fees, specify a period of time in which the entrance fee will be refundable to the resident on a declining basis. For example, if an entrance fee under this arrangement declines at the rate of 1 percent each month, after 6 months 94 percent of the entrance fee is refundable.

Partially refundable entrance fees guarantee a specific percentage of the refund that will be returned within a certain period of time regardless of the term of residency. For example, 50 percent of the entrance fee may be refundable upon termination of the contract or to the estate upon the resident’s death. Full refunds offer just that, a full refund of the entrance fee. A fixed charge may be deducted before the refund is made, and the agreement generally states how long the refund is valid and under what conditions a refund is due. Entrance fees that offer full refunds are typically more expensive than those without refunds or those that are partially refundable or refundable on a declining basis. he more years you put behind you, the more worries you may have about continuing to maintain your home. When you do not need medical supervision, but feel like washing your hands of endless home-related tasks, independent living is an option that can help. What is Independent Living? Independent living communities are residential communities for active older adults who want an enjoyable lifestyle free of the worries and trouble of home maintenance. They are similar to any apartment, condominium or single-family

development, except that they provide special services, including security—an important consideration for many residents. Most independent living communities are fully secured, with staff members greeting and screening all visitors around the clock. Residents can leave home for extended periods of time knowing that their living quarters will be safe. As many individuals at this stage in their lives are looking to pursue new hobbies and interests with their free time, the activities and amenities available at a community are a major factor for potential residents. Several independent living communities provide a full range of activities such as shopping trips, outings to cultural events and organized gath-

erings that promote socialization. Some communities also have tennis courts, swimming pools, activity rooms and other amenities that keep residents active and engaged. Most offer a variety of meal plans, including the option to prepare meals in your own apartment. Most independent living communities do not offer health care services (such as those provided, for example, by assisted living communities and skilled nursing/rehabilitation facilities), but some independent living communities are affiliated with health care facilities that can provide such care when it is needed. Residents also have the option of purchasing home health care services from outside providers to whom they may have grown accustomed.

Assisted Living Communities Providing supportive services

A

ssisted Living communities offer housing and a variety of supportive services delivered by qualified staff members. As opposed to independent living communities, health care services are available at assisted living communities. The Assisted Living Federation of America (ALFA) notes that assisted living is “a unique transition from independent living without care” that “helps seniors enjoy an improved quality of life with independence and care combined.” Community amenities vary widely but can include private units, state-of-the-art facilities, beautifully decorated common areas, barber shops and beauty parlors, pharmacies, physical therapy services, recreation rooms, libraries, gardening areas, and fitness centers. Consumers will quickly find that assisted

living communities, also commonly called residential care facilities or personal care homes, come in a variety of shapes and sizes. Who Needs It? Residents of assisted living usually include anyone who needs some help with the activities of daily life. These include eating, bathing, dressing, transferring, toileting, medication management and walking. Most communities offer three meals a day served in a common dining area. Many also offer some type of assistance with transportation. Housekeeping and laundry services are standard offerings. Staff members are available to meet resident needs 24 hours a day. Most communities are equipped with security systems, and emergency call systems are often available in all residences. For residents with more acute health care needs, such as Alzheimer’s or

other forms of dementia, special units and/ or specially trained staff members are often available. In addition, communities offer a wide range of social activities to stimulate resident contact. Choosing a Community First and foremost, consider the needs and desires of the potential resident. Narrow the search by reviewing a list of all the local communities. Request a brochure from your top choices, as well as other specific information such as the resident agreement or contract, a list of services and amenities, detailed prices, staffing information, admission and discharge criteria, and community rules. Visit a facility more than once to get a complete feel of the lifestyle at the community. Talk with staff and residents and tour the entire facility.

Nursing & Rehabilitation Centers Specialized care for individuals who need daily assistance

W

hile some individuals are able to continue running their households with little assistance, there are those who may have debilitating physical or emotional conditions that require care beyond what family or professional home care aides can provide. What is Nursing Care? At a nursing home, care is administered by professionals under the direction of a physician. Many facilities also offer subacute, respite, rehabilitation and other short-term care. Rehabilitation services can be especially helpful for individuals who are recovering from surgery, an illness or any other life-changing event. These homes serve as permanent residences for individuals who are too sick

or frail to live at home, or as temporary facilities during a recovery period after a surgery or illness. Who Pays for Nursing Care? Medicare, Medicaid, private insurance and personal assets are all used under various circumstances to pay for services in a nursing care facility. If a facility is not certified by Medicare and Medicaid, the care will have to be paid for entirely with personal funds. In general, Medicare pays for skilled nursing care following hospitalization for the same illness or condition at 100 percent of the cost for days 1 – 20. Some long-term care insurance pays for nursing home care; insurance payments are usually a fixed amount for a specified number of qualified days. The basic daily rate is the standard charge the nursing home bills to all

residents, which covers the fundamental services every resident receives, including rent for the room, housekeeping, meals and general nursing care. It is important to understand all the services and amenities that are not included in the basic daily rate. What to Look for in Nursing Care Before choosing a home, contact the Long-Term Care Ombudsman Program of your Area Agency on Aging. Ombudsman programs promote the highest quality of life and care for residents of nursing facilities and can help families and staff with inquiries and complaints. Tell the ombudsman which options you are considering and request any information they may have. Visit your prospective communities at different times of day. Talk with residents. Be persistent about getting your questions answered.

A Special Care Residence May Be the Right Option ertification standards for nursing homes and adult care residences do not specifically address dementia special care units. You will need to rely on your own judgment when considering a special care unit. Visit more than one special care unit and compare them. Use all your senses when you visit — sight, smell and hearing are all important. Be sensitive to the overall atmosphere and how the staff interacts with the residents. Ask about the number of staff on duty at all times and find out about any special training they have had. Generally, the more staff available, the higher the quality of care. Some reports find a daytime ratio of six resi-

dents to one staff person to be adequate in a special care unit; others feel that a ratio of eight to one can also result in quality care. At night, when residents are in bed, a higher patient to staff ratio may be fine. Get a written description of the services and programs offered by any special care unit along with the charges or fees associated. This description should clearly explain to you how the special care unit differs from the rest of the nursing home or assisted living facility. It should emphasize special training the staff receives and describe the environment. Before choosing a special care unit, you should be confident that the higher price charged for care in the unit will actually result in better care for your relative. You may find that your relative can be adequately

cared for in a regular unit. Finally, talk with someone about special care units. Your state ombudsman, your local chapter of the Alzheimer’s Association or your local Area Agency on Aging are all good resources. They may be able to provide you with a more detailed consumer checklist to help you compare special care units. All dementia special care units should focus on the individual needs of the resident. Understanding Home-Based Care in Maryland

A

ccording to the Maryland Health Care Commission, “Home-Based Care can include a variety of activities such as nursing care, rehabilitation services (for example, physical therapy), assistance with activities of daily living (ADLs), assistance with housekeeping, chores and meal preparation, or assistance with activities to maintain health, such as taking medications.” There are several types of agencies that provide these services throughout Maryland. For individuals needing home-based care, it is important to first determine what type(s) of help you need to narrow your search of agencies and find one that will meet your specific needs. The following information from the Maryland Health Care Commission provides a look at the different agencies and their state regulations. Home Health Agencies Home health agencies provide nurs-

ing services, home health aides and one or more other services such as physical therapy, occupational therapy or social services. Several kinds of health care staff may provide care based on your needs. Home health agencies participate in the Medicare program and many health insurance programs include a home health benefit. A physician referral is required, so it is a good idea to talk to your doctor if you think you might need this type of care. Usually a medical reason is needed for these services to be reimbursed by Medicare or insurance. Generally, home-based care is appropriate whenever a person prefers to stay at home but needs ongoing care that cannot easily be provided by family and friends. Nurse Referral Service Agencies (NRSA) These agencies screen and refer licensed nurses, home health aides, homemakers, live-in caretakers, and companions for individuals needing help in their home. The person seeking assistance contracts directly with the individual who will provide the service and may pay the agency or the

provider for services. Residential Services Agencies (RSA) RSAs provide supportive home care services, that is, assistance with ADLs and/or housekeeping services. They may provide one or more home care services, such as provision of oxygen or medical equipment, which includes wheelchairs, walkers and hospital beds. An RSA cannot participate in the Medicare Program; however, Level III RSAs can provide skilled nursing services. RSAs employ all their caregivers, while NRSAs do not. The person seeking assistance contracts with the agency. RSA services are sometimes paid for by the person using services, but are generally covered under long-term care insurance policies and may be paid through medical insurance in some circumstances. Home Care Patient Rights Federal law requires that all individuals receiving home care services be informed of their rights as a patient. Following is a model patient bill of rights the National Association for Home Care (NAHC) has developed, based on the patient rights currently enforced by law. Home care patients have the right to: n be fully informed of all rights and responsibilities by the home care agency;

prior to the start of any treatment or procedure; n be advised of any change in the plan of care, before the change is made;

n choose care providers; n appropriate and professional care in accordance with physician orders; n receive a timely response from the agency to requests for service; n be admitted for service only if the agency has the ability to provide safe, professional care at the level of intensity needed; n receive reasonable continuity of care; n receive information necessary to give informed consent

Understanding Home Health Care Benefits When it comes to understanding Medicare coverage, many individuals can find themselves confused by a complex web of information. This especially applies to those who want to know if they are eligible for Medicare coverage when entering a nursing home or receiving home health care. Below is some helpful information on whether you can use your home health care benefits if you have Medicare and what services and products it will cover. First, you can use your home health care benefits if all of the following applies to your situation. Your doctor must prescribe that you need medical care at home and create a care plan. The care you need has to be intermittent and in the form of skilled nursing care, physical therapy, occupational therapy or speech therapy. The home health agency must be Medicare-certified and you must be homebound, or unable to leave the home unassisted. Once you have established those requirements, it is important to understand what Medicare will actually cover, which includes the following: • Home health aide services on a part-time or intermittent basis, as long as there is a skilled need • Medical social services • Certain medical supplies • Durable medical equipment (wheelchair, walker, bedside commode, etc.) Given the services and products that Medicare does cover, it is also valuable to take note of what is not covered. Care that is required 24 hours a day will not be covered. Other services not covered include personal care provided by home health aides such as bathing and dressing, along with homemaker services like cleaning and laundry. Also not included are home-delivered meals. If you require care that goes beyond intermittent visits for a limited amount of time, or if your needs are not covered by Medicare, an important option to consider is private duty home care.
