Estrace Cream Is Safe to Use in Woman Who Had Melanoma ---
DEAR DR. ROACH: I am a 70-year-old woman who has been using Estrace vaginal cream weekly for several years. I am in excellent health, work out daily, run half-marathons and have my own business. I take 10 mg of atorvastatin daily. Ten years ago, I had melanoma removed from my arm. My concern is the use of Estrace. My gynecologist recommended that I use it after a Pap smear, but my primary care physician is not of the same mind. I would appreciate your opinion on this. My mother had breast cancer in her 80s, my sister had uterine cancer, and my brother passed away from bladder cancer. -- C.S. ANSWER: As far as the safety of using estradiol (Estrace), the bioidentical hormone to major natural estrogen, I am definitely of the same opinion as your gynecologist. When used topically, it works on the vulva and the lining of the vagina, keeping the tissue healthy. Without estrogen, the tissues that line the female perineum can become atrophic (thinned), predisposing a woman to discomfort, infections and incontinence. The Pap smear may have resulted in a finding that was suggestive of vaginal atrophy (increased neutrophils). Without treatment, the labia can even fuse together. With long-term use, the estrogen levels in the body are only slightly higher in women who use topical estrogen compared to those who do not, so the absorption into the body is minimal. The only case where I get concerned is with someone who has a personal history of an estrogen-sensitive tumor, such as endometrial cancer and some breast cancers. In those cases, I discuss options with both the patient and their oncologist.
HOW LONG SHOULD A FILLING LAST? Fillings generally last around 10 years, but it depends on the type of filling and your personal chewing, grinding, and oral health habits. Amalgam and ceramic fillings can last for 15 years, while composite fillings last around 7 to 10 years. Glass ionomer fillings last about 5 years. Amalgam (silver mixed with other metals) and composite (acrylic and ceramic resin) fillings are the most common. Composite fillings are popular because the color can be matched with the surrounding teeth. Fillings sometimes crack or wear down over time—and can even fall out on their own—and require replacement. You may not be able to tell if you need a new filling, but your dentist assesses fillings during regular exams.

Not Playgrounds Please let us remind you that senior apartment buildings are not playgrounds. You need to be more respectful of the elderly that live there. Take the children outside to play or to a playground where it's more suitable and safe for the tenants. Appointments I’m furious about, mostly medical offices, when they call to reschedule an appointment that you’ve been waiting months for. They want to reschedule for sometimes 2 to 3 months, if not more, out further. How about “bumping” others that haven’t been waiting so long. If it’s not your fault that the appointment is cancelled but it seems you’re the one that inconvenienced. You have no heart To all the people that dump their animals, you are the lowest. Do you even care that as you drive away, they are thinking what they did wrong? Take them to a shelter so they are warm and fed. How can you even sleep at night.

Do you have any tips on how we can prepare the house for the puppy's arrival in a few weeks? -- Chet in Boise, Idaho DEAR CHET: First, kudos to you for handling the Christmas announcement so well! That's a great way to surprise the kids during the gift-opening while not having to supervise a puppy who has no idea what is going on. And keeping the puppy with its mother until it's at least 8 to 12 weeks old is so important. Involving the kids in preparation for the puppy's arrival is important. Explain to them that puppies like to explore -- often with their teeth. So they need to put away their toys, clothing and anything that they don't want to get chewed up. Puppies can also get into foods and cleaning items that can be dangerous or deadly for them, so those items need to be lifted off the floor or removed from the house's common areas entirely. In the heat of the New Year’s Eve countdown, fueled by enthusiasm and possibly a few glasses of champagne, resolutions may roll off of the tongue. But come the morning of January 1, such resolutions may not hold the same appeal. Between 40 and 45 percent of the American population will make a New Year’s Resolution, according to polling by researchers at the University of Scranton. Research suggests that only about 8 percent of people stick with those resolutions and achieve their goals. The outlook is similar for Canadians, 31 percent of whom set New Year’s resolutions. Among those that do, three-quarters will break them, according to a poll from Ipsos. A survey of online shoppers who visited the website FreeDeliveryLand.co.uk revealed that the average length of time a person can expect to pursue their New Year’s resolution is around three and a half weeks, or 24 days. Maintaining resolutions can be a resolution in and of itself. Resolutions are made and broken every year. Make this the year you realize your resolution.

WOMEN AND STEM The number of women entering the professional fields of science, technology, engineering, and mathematics (STEM) is slowly growing around the world, but there is still a sizable gender gap in these professions. According to the College Board, which produces many standardized tests, only 27 percent of all students taking the AP Computer Science exam in the United Science are women. Similarly, just 18 percent of American computerscience degrees are attained by females. However, this is not the case elsewhere in the world. A paper by Gijsbert Stoet and David Geary published in Psychological Science noted that women who live in countries with traditionally higher gender inequality tend to choose STEM professions more readily. Algeria, for example, has one of the highest ratios of women in STEM professions, at 41 percent. Stoet and Geary surmise that women in these countries may be choosing careers with the strongest path to financial independence. According to a U.S. Department of Education report, students studying science or math in college have a higher employment rate and salary than other majors after graduation. STEM majors typically earn an average of $15,500 more annually than nonSTEM majors. Engineering and engineering technology tends to pay the most. Women eager to secure competitive, stable and well-paying jobs should carefully consider the opportunities available to them in STEM fields.

Quitting vaping may be a life-saving resolution Electronic cigarettes and other vaping devices were once described as potentially “safer” and “healthier” methods of smoking, even by leading health organizations. Many former tobacco smokers even turned to e-cigarettes as a method to quitting traditional cigarettes. However, recent research into vaping and a rash of mysterious lung illnesses and deaths recently linked to vaping has led many to question if vaping is safe. Those who suspect it’s not may be wise to quit.
The illness toll rises The Centers for Disease Control and Prevention says that, as of September 2019, there were 530 cases of vaping-related lung illnesses. Eight people also died in six states, with the first confirmed death occurring in April. The first known vaping-related illness in Canada was documented on September 18, 2019, when a youth from Ontario was put into intensive care for respiratory illness. While federal and state investigators are largely focusing their investigations on vape cartridges that likely contained tetrahydrocannabinol (THC), the psychoactive component linked to marijuana, as a potential source of these illnesses, all electronic cigarette devices are now under scrutiny.
A school epidemic Vaping has quickly become an epidemic in schools. More than 3.6 million American middle and high school students currently use e-cigarettes, according to the latest National Youth Tobacco Study. Teen nicotine vaping rates in 2019 were double those of 2017, the National Institute on Drug Abuse says. Despite the installation of vaping detectors, and some schools going so far as to remove doors from bathroom stalls, opportunities to vape still abound. Older students may leave campus to vape in their cars during lunch breaks. Laws restricting sales of e-cigarettes to youth in certain states have already been implemented. Mystery ingredients Many adults look to e-cigarettes to reduce their dependence on tobacco products. However, Johns Hopkins Medical Center warns e-cigarettes are just as addictive. Also, many e-cigarette users get even more nicotine through extra-strength cartridges or by increasing the voltage to get a greater hit of the addictive substance. The Food and Drug Administration has not approved vaping as a smoking cessation method. People never can ber certain about the ingredients in vaping liquids. And as the popularity of e-cigarettes grows, newer, cheaper, and poorly regulated products are being imported from all over the world to meet the demand. According to the CDC, exposure to such products may cause a range of symptoms, including cough, chest pain, shortness of breath, fatigue, vomiting, diarrhea, and fever. There also have been cases of “popcorn lung,” which derived its name from a former chemical flavoring (diacetyl) used in microwave popcorn linked to scarring of the tiny air sacs in the lungs, resulting in suffocation. Popcorn lung also has been linked to vaping use. More evidence continues to point to vaping as an unsafe practice. Quitting the habit now can have profound, potentially life-saving effects.
