Smoking, Symptoms Biggest Factors in COPD Prognosis

DEAR DR. ROACH: I was diagnosed with COPD a year and a half ago. I seem to be unable to get answers from my pulmonary doctor, as he is always busy. How fast does COPD progress? I’m 78 years old, I’m in reasonably good health, and I exercise regularly. I have chronic bronchitis with a slight amount of emphysema. I had been coughing a little during the day, had mucus in the morning and coughed at night for two to three hours after bedtime -- to the extent of having to use a rescue inhaler to settle down and get back to sleep. After he put me on Trelegy, all of those symptoms disappeared, and I feel perfectly normal except for shortness of breath. My other question is, what is the life expectancy of a person diagnosed with COPD? I’ve read answers to this all over the map, from two or three years after diagnosis to 15 or more years. I would presume that it depends on other factors such as physical condition, exercise, weight, etc. I would like some kind of guideline for someone in my condition. -- T.J. ANSWER: There are two major types of chronic obstructive pulmonary disease: chronic bronchitis, where the primary problem is productive cough; and emphysema, which is destruction of the small airways. Shortness of breath is a common feature of all types of COPD. The most prevalent cause is smoking. I can’t answer your questions on rate of progression and overall prognosis without much more information, but it is certainly a good sign that many of your symptoms are well-controlled on treatment. The single biggest factor in rate of progression of COPD is whether you are a smoker, but not being able to exercise, being male and being very underweight are additional risk factors for faster progression. In someone with COPD, the decline over months to years can cause symptoms to occur first with exercise, and then when at rest. Once people have severe symptoms at rest, the prognosis becomes quite poor. No medication can reverse lung damage. Inhaled steroids reduce inflammation and can slow progression somewhat. Inhaled bronchodilators open airways and make breathing easier. Inhaled anticholinergics reduce secretions. Trelegy contains all of these medicines and is appropriate for people with severe COPD or those who do not have good symptom control with a less intensive regimen.

CHOOSING AN ELECTRIC TOOTHBRUSH Many electric toothbrush models provide a deeper clean than manual ones. Choosing an electric toothbrush is, in some ways, much like choosing a manual toothbrush: the head size should be small enough to reach your back teeth, the bristles should be soft and contoured instead of hard and straight, and the handle should provide a firm grip. But when it comes to head and bristle vibration, an oscillating motion is best. Bristles that simply vibrate provide no benefit over a manual toothbrush. Another feature to look for is a range of settings, such as different cleaning modes, a pressure censor, and a self-timer. Without these personalized settings, electric toothbrush oscillations may feel uncomfortable or even painful.

Started many years ago to help low-income individuals with their phone costs, it’s been expanded to include internet at home, offering a $9.25 monthly discount. That $9.25 savings doesn’t sound like much, but if you’re like me, nowadays, you’re converting every dollar saved into food. That $9.25, for example, could turn into a loaf of bread and a gallon of milk, or even a pound of ground beef, a package of hamburgers buns and most of a can of beans. You’ll need to qualify for Lifeline. One way you do that is if you (or anyone in your household) receive benefits from Supplemental Security Income, Temporary Assistance for Needy Families, veterans pensions, Supplemental Nutrition Assistance Program, Medicaid and others, or through having verifiable low income. (You’ll need paperwork to prove it.) You can sign up in a number of ways, such as by mail or online at nv.fcc. gov/lifeline. Scroll through the information on that page. Call 800-2349473 for more information or to ask that an application be sent to you by mail. But there’s more. In addition, if you qualify for Lifeline, you likely also qualify for an even bigger assistance: the Affordable Connectivity Program. ACP is new and can give you a $30 monthly discount on your internet. Once you’re in the Lifeline program, call your internet provider and enroll in ACP with them. There’s no need to fill out another application. There are hundreds of internet providers that are part of the program, including some big names like Xfinity, AT&T and Spectrum. Melt the butter in a large skillet over medium heat. Add the apples, turn the heat to high and sauté until they begin to brown on the edges, about 5 minutes. Keeping Our Promise: More than a Mission Part II BY AMANDA DUDEK

K

eeping Our Promise (KOP), the most comprehensive resettlement program for Afghan, Iraqi and Kurdish interpreters and support personnel in the United States today, is a Rochester-based program which assists with initial visa applications under the Special Immigrant Visa (SIV) Program. KOP Executive Director Ellen Smith and KOP Assistant Executive Director Laurel Prothero continue the struggle to not only bring these individuals and families to safety, but also to help them resettle with a solid foundation on which to successfully rebuild their lives. After all the families have endured, it cannot be easy. The work they did, even being associated with the United States or the former Afghan government, put them in danger. When American troops were withdrawn, the more progressive government fell. “The pace at which everything happened was very quick,” said Laurel. “The families didn’t have time to adjust or make other arrangements.” Ellen agreed. “All the sudden, the president left,” she explained. “Those associated with his government knew they had to try to get out.” Muzafar* worked with the Afghan Supreme Court, creating a docket system and working for women’s and children’s legal rights in Afghanistan. For this, of course, he would be targeted by the Taliban. His wife was educated, as well. She went to dental school in Turkey and came back as a dentist. If they had stayed, he probably would have lost his life, both because of his job with the Afghan government and because he was fighting the very things the Taliban supported. KOP helped bring Muzafar and his wife to safety in the United States. KOP first connected with Vares* in 2019. On the day Kabul fell, Vares was still at work. “He messaged me and said, ‘I am being left behind,’” said Ellen. “I told him, ‘I need all of your American contacts immediately.’ I called this retired army colonel, and I said, ‘You don’t know me, and this might sound crazy, but I need a letter of recommendation to get this man out of the country.’” Vares had worked with the United States, then for Mercy Corps before going to work for the Afghan government. He had 17 ex-military people supporting him, and it was clear how much his service meant to them. Thanks to KOP, Vares and his family have been welcomed to safety. Dalir* was a TV anchor. When KOP brought him safely to the United States, his goal was to continue to work in media. He took a job indirectly related to media at first and continued to work towards his dream. “He was very focused; one of the most persistent people I’ve met in a lot of ways, and he just really made it happen,” said Laurel. Now, Dalir is working for a local TV station. “His coworkers seem to love him, and he is very passionate about his work,” she continued. “It is amazing to see someone’s dream come true, to see how the supports we provide set them up to achieve.” Unfortunately, there are still thousands of individuals and families waiting to leave Afghanistan. Tareq* worked for an army captain who was from south of Rochester. Tareq was a journalist and was present during an airport bombing. The army captain called Ellen. “I had to get clothing descriptions for everyone in his family because I had to describe everything to identify them,” she recalled. “I even had them wave a brightly colored piece of clothing.” Unfortunately, Tareq’s passport was expired, and his father and youngest child did not have a passport. Now, finally, Tareq’s family all have their passports so they can travel to Qatar. They are still waiting in Afghanistan.

“Our Afghan families are stuck,” said Ellen. One Afghan gentleman told KOP, “It is really bad, worse than you can imagine. My family is fine, but the Taliban is now ordering young women to marry their fighters.” “I found the news article,” confirmed Ellen. “What do you say to that?” KOP plans to continue its efforts. “We rely on private donors to do what we are doing,” said Ellen. “The goal is to give these people a foundation through caring circles, a phone, a computer, eventually Wheels to Work. The issues are complicated. We can’t turn back time, but we can help these families.” “Our next steps will continue to be helping these people to come to this resettlement project in Rochester, creating continued support through community and policy, and to continue to bring the Afghans into our volunteering process,” added Laurel. There are many ways to support KOP, from helping with donation pickups to fixing furniture to participating in Caring Circles to donating funds or necessary home furnishings. KOP also encourages readers to call their Congress members, asking them to support the Afghan Adjustment Act which would allow certain Afghan evacuees to apply for permanent status after one year of being paroled into the U.S. Supporting these evacuees is the least we can do. “They are good people,” said Ellen. “They are coming in with skill sets and ready to work. They are running the clock all the way around, trying to rebuild their lives and take care of their families.”

STUDY SHOWS GIFT CARDS CAN GIVE A BOOST TO SMALL BUSINESSES (NAPSI)—If you own or run any of America’s nearly 31 million small businesses, recent research reveals gift cards can be a real gift to your bottom line. According to a 2019 Small Business Gift Card Study commissioned by First Data, now Fiserv, gift card programs help small business owners bring in more sales, attract new customers, and build relationships with their customers. Small Businesses, Big Benefits Not only do gift cards benefit a business, consumers expect them to be available at their favorite stores. In fact, 74 percent of consumers surveyed said they regularly buy gift cards from small businesses. And for business owners, offering gift cards can: • Grow sales—Nearly 80 percent of consumers said if they have a gift card valued at 88 cents, they will visit the store to redeem it • Attract customers—90 percent of consumers who receive a gift card from a small business they have never visited said they would shop at that business and return in the future

(NAPSI) — K–12 education has gone through a seismic shift in the wake of COVID-19. Parents are finding themselves thrust into homeschooling, and educators, into virtual schooling. EdChoice, a national nonprofit, nonpartisan organization, teamed up with Morning Consult to release results of a recent poll to find out how it has affected families and teachers. They found that parents are clearly anxious about sending their kids back to school; 66 percent said they were concerned their children will be exposed to the virus in school. The majority (68%) are also concerned their child is missing important instruction time. The silver lining to so many states having closed down brick and mortar schools: A solid majority of parents said they felt at least somewhat prepared for the transition to e-learning and distance learning. Parents are slightly more likely to say they felt very or somewhat prepared (70%) than were teachers (67 percent). There is only one type of school choice program that allows families to receive funds into a flexible spending account to customize an education for their child. They are called education savings accounts, or ESAs. And according to EdChoice’s Public Opinion Tracker, 67 percent of Americans support them. Many pet owners would never intentionally harm their companion animals. In fact, pet owners often go above and beyond to ensure their animals are well cared for and content. However, many well-meaning people may be inadvertently harming their pets with something they provide each and every day: food. Obesity is on the rise among pets. The Association for Pet Obesity Prevention says that 54 percent of dogs and 59 percent of cats in the United States were categorized as overweight or obese in 2016. That marks an increase from 52.5 percent and 58.3 percent, respectively, from four years ago. The majority of veterinarians think that pet obesity is a significant problem and nearly half of pet owners admit that their vets have discussed a pet’s ideal weight with them during pet health visits.

DEAR PAW’S CORNER: Yesterday, our beloved corgi mix, Shemp, was laid to rest. I feel so guilty. He passed from heatstroke after being out in the backyard for just one hour. I never realized that just the heat outdoors could be deadly to dogs. I always was conscientious about dangerous situations; I never once left Shemp in a hot car or walked him on hot sidewalks. Yet a few minutes of inattention, and my Shemp was suddenly in great distress. I rushed him to the veterinarian, but he just didn’t make it. Please warn your readers to keep their dogs inside on hot days and to monitor them closely for signs of heat stress. -- Devastated in Houston Dear Devastated: I am so sorry for the loss of your beloved Shemp. Thank you for warning other readers of the dangers of summer heat. Being outdoors during the extreme temperatures that we’ve experienced this summer, often passing 100 degrees in many states, is dangerous for dogs and other pets. Here are a few ways to keep them safe from heat injury. -- Walk dogs before sunrise and after sunset. Under a hot sun, sidewalks can quickly burn a dog’s paws, and heat and humidity can take a toll on their health. -- Do not leave pets outside unsupervised. Even if they’re just in the backyard, you might not notice their distress until it’s too late. -- Provide a shelter from the sun. A shelter with open sides and a solid top allows airflow and protects from the sun when your dog is outside. -- Make water available all day, everywhere. Fortunately, a leading health organization is working to help these individuals reduce their risk of developing such dangerous conditions.

Atrial fibrillation (AFib) is defined as a quivering or irregular heartbeat. Often, strokes that occur for no known cause can be linked to AFib, which a person may not even be aware they have. The American Heart Association (AHA)—the world’s leading voluntary health organization dedicated to being a relentless force for a world of longer, healthier lives—wants all people living with AFib to understand how they can reduce their risk of cardiac complications. “AFib can have symptoms such as an irregular-feeling heartbeat, shortness of breath and light-headedness or it can have no symptoms at all. Even if you are not experiencing symptoms of AFib, your stroke risk is still increased five-fold,” says Patrick Ellinor, M.D., Ph.D., cardiologist at Massachusetts General Hospital. Health care providers score a patient’s risk of a stroke caused by AFib by taking into account such factors as age, sex, and pre-existing conditions including hypertension, diabetes, vascular disease, and prior stroke or ischemic attack. “If you suspect you have AFib, meet with your doctor. Livingston Murals Not only do I enjoy seeing all the new murals when driving into work, but love the uniqueness behind each design and that they are all painted by local aspiring muralists. I hope to stop by them all sometime soon.

Pedestrian Crosswalks I think it’s time for some driver education. Drivers don’t seem to know that pedestrians have the right of way at these crosswalks. I watched a little boy on [local road] trying to cross at a marked crosswalk at this intersection. Nobody stopped to let him cross. I was waiting to make a right turn onto [local road], waiting until this little boy safely crossed. The driver behind me was honking angrily. Finally, an adult male came to his rescue and made sure he crossed safely. I see this law being broken all the time. Disrespect I do not understand the disrespect in today’s world. My husband was not a Veteran, but he was always very patriotic. He passed away a few years ago. He loved pinwheels, seeing them blowing in the wind. I found a large 16” red, white, and blue pinwheel with white polka dots, which was 36” tall and placed it at his grave for the last two years. It did not cost a lot of money but was worth millions in memories to my family. It disappeared! There was no windstorm to pull it 10” out of the ground and blow it away. And nothing else in the whole cemetery was disturbed. So, I know someone took it! I hope every time you look at it you remember where you got it and how you got it. Such disrespect stealing from a grave! We all share the Canal! We always love seeing other boats enjoying the Canal when we kayak from [local town] to [local town] and back, but we would appreciate it if power boaters slowed down when passing smaller boats and when approaching wildlife, like groups of ducks and geese, and especially those with young. What’s the rush? Slow down, enjoy the scenery, and you’ll be surprised at all of the amazing things you’ll see. We all share the Canal. Please be respectful.

Festivals are back! I love seeing all the festivals and events happening again! Of course, I understand the risk of covid-19 is still here and we should take precautions. Even if I don’t go to the events I love that we are getting back to fun this summer! From the Oatka Festival in LeRoy, to the Dansville Balloon Festival to the West Sparta OldFashioned Day and everything in-between! Disc Golf Course A few years back, the town I live in installed in a disc golf course and I cannot tell you how much we enjoy it and use it. Thank you to everyone that had a part in that. I know my family appreciates it!
