Trim the fat from the chops or blade steaks. Trim the meat from bones, and cut into small cubes. Dust pork w ith cumin and oregano. Heat oil in skillet over medium-high heat. Add onions and pork and sauté until cooked through, about 8 minutes. Add tomatoes and heat. Cook linguine in boiling w ater about 2 minutes or as package directs. UNDERSTANDING DEMENTIA A KEY TO COMPASSIONATE CARE Alzheimer’s disease is one of the more prominent forms of dementia, but there are many additional types of dementia that also can cause both physical and cognitive alterations. Understanding the complexity of dementia can be beneficial to both dementia sufferers and their caregivers. Dementia is a general term used to define a decline in mental ability severe enough that it can interfere with daily life, offers the Alzheimer’s Association. Alzheimer’s disease is the most common type of dementia. Dementia is not a disease but a term used to describe a wide range of symptoms. The words “senility” and “dementia” often are incorrectly used interchangeably. However, serious mental decline is not a normal part of aging.

DEAR DR. ROACH: I have never seen anything in your column

about sebaceous hyperplasia. I read that it is quite similar to acne and that a person could use an acne treatment to keep the condition in check. Could you give your thoughts on this? -- R.G. ANSWER: Although sebaceous hyperplasia can be mistaken for acne, sebaceous hyperplasia happens mostly to middle-aged adults (or newborns). These bumps appear most commonly on the forehead and cheeks, and have an umbilicus (a small hole) in the center. They are enlargements of the oil-producing sebaceous glands. They happen particularly in people with oily skin. Treatment can be challenging. Reducing dietary fat and using a good-quality skin-care regimen for oily skin may help. Dermatologists can treat these when they are cosmetically important, not because they cause harm in themselves. Although they can be mistaken for basal cell cancers of the skin, they are not cancerous or pre-cancerous. They are mostly treated mechanically, and by that I mean using a surgical technique like cauterization or excision. But these techniques can leave scars. Laser and phototherapy have good cosmetic results but are expensive. Isotretoin, a systemic acne drug with significant side effects, also has been shown in a small study to be effective.

DEAR DR. ROACH: I am currently taking 20 mg of Xarelto daily, and have a screening colonoscopy scheduled soon. The preparation instructions said not to stop taking blood thinners, but didn’t specifically mention Xarelto. I contacted the office and spoke with a scheduler, who said those instructions applied to all blood thinners. Previously, I was told that I would need to go off of blood thinners for five days prior to a colonoscopy. Since this was a contradiction to the instructions from the clinic, I have tried to reach out to other medical sources to see if I could get a consensus on what to do, but no one has responded to me yet. There is a strong history of clotting problems in my family. Would you weigh in on this matter? The last time I had a screening colonoscopy was around 2005, and there weren’t any issues. I realize this is a scope and not surgery, but it has me concerned enough that I am considering putting this off until I can get a satisfactory response. -- B.R. ANSWER: Most screening colonoscopies now are done with patients still on their anticoagulants, although for patients on warfarin (Coumadin), it is preferable for levels to be on the low end of the therapeutic range. This is a standard recommendation by the American Society of Gastrointestinal Endoscopy. The situation would be different for a high-risk endoscopic procedure, such as removing a large polyp. I have had to deal with this situation many times in my practice, and most often, all anticoagulants are stopped then restarted after the colonoscopy. There are some people who are at such high risk for clots that we use injection medications to prevent blood clots until the night before the procedure, but that is normally reserved for the highest risk of all. It’s a judgment call as to whether to use the injection anticoagulants to prevent clots around the colonoscopy. *** Dr. Roach regrets that he is unable to answer individual letters, but will incorporate them in the column whenever possible. What’s your credit card number?” Like a fool, I gave it to them. It’s so much more convenient they told me. I can have new icons with news channels and an icon just for email. My family wants to adopt a dog, but now we’re uncertain about doing so. How can we make sure we’re getting a healthy dog? — Julie in Alexandria, Louisiana DEAR JULIE: You should certainly do some research before taking that super-cute puppy home from a shelter. Rescue shelters have gone through some dramatic changes in the past decade, and perhaps the most notable change is that dogs eligible for adoption are often shipped from a shelter in one region of the country to another region. Dogs from overseas also are being acquired by some shelters as demand for rescue dogs — particularly the “adorable” one or popular breeds — outstrips supply. There is little federal regulation — the USDA doesn’t track how many dogs are imported from other countries, for example — and states are only just beginning to tighten rules to ensure the health of dogs shipped across state lines. There are reports of newly adopted dogs developing severe illnesses, including a new strain of canine distemper virus — a highly contagious disease that closely resembles rabies. Here are some important questions you should ask: • Where did the dog come from? Was it surrendered locally, sent from another state or unknown? • Ask for copies of health records. Has the dog been given vaccinations yet? • If the dog comes from another state or country, was it quarantined after arriving (at least 48 hours in some states)?

ational Nurses Week is celebrated annually from May 6, also known as National Nurses Day, through May 12, the birthday of Florence Nightingale, the founder of modern nursing. This week is a time for everyone – individuals, employers, other health care professionals, community leaders, and nurses – to recognize the enormous contributions and positive impact of America’s 4 million registered nurses. The theme for National Nurses Week 2019 is “4 Million Reasons to Celebrate.” HomeCare & Hospice is a local non-profit organization that knows the impact and the difference that their nursing staff makes in the community every day. “I am grateful for every single member of our staff, but also especially proud of our nurses who deliver compassion and comfort with every visit, every touch. Our nursing team reflects the very core of our mission. Our communities are well served because of the nursing excellence demonstrated every day at HomeCare & Hospice,” says Carol Mahoney, CEO of HomeCare & Hospice. One of the most recognized representatives of the HomeCare & Hospice nursing team is Ronda Ehman, Senior RN Care Coordinator for the organization, who will celebrate her 30th anniversary as a nurse with the HomeCare & Hospice this coming Fall. “Ronda has been with us through so much of our growth and change. She has been a steadfast, caring and patient teacher of others who have followed her path in home care and hospice. It has been my privilege to work with Ronda these many years and to see all the wonderful things she has done to enhance the care and comfort of our patients. She is a treasure,” Carol Mahoney continues. Mrs. Ehman was with the agency when hospice services started in 1991. Over her years in nursing, she has seen the many advancements in medical science. “50 years ago, when I went to school, there was no cure for lymphoma. Now there are treatments and options for a multitude of conditions,” Mrs. Ehman says, adding that a hospice is a different way of looking at the nursing profession. “As a nurse, you are trained to help people get better. However, as a hospice nurse, someone comes to you with a poor prognosis, and instead you are able to help make their final days better and more comfortable. That is the best part of my job as a nurse.” Nikki Wiseman, Director of Patient Care Quality appreciates all that nurses do. “Our nursing staff are amazing in what they do daily. They go above and beyond to keep HomeCare & Hospice patients in their homes or other places of residence safe, symptom-free, and comfortable. They work as a team to cover all patient needs in the very rugged rural hills of the Southern Tier and Western NY. These nurses travel great distances and sometimes go deep into the woods on dirt roads to get to patient homes or cabins. They make visits day or night to make sure that patients have the care they need, even if it is just to comfort a
