COPD Lung Function Can’t be Restored

DEAR DR. ROACH: I read your recent column on pulmonary fibrosis. I especially appreciate knowing that there are medications known to slow the progression of lung disease, reduce exacerbations and reduce mortality. With a 20-year-old diagnosis of COPD and having been prescribed medications to reduce exacerbations, you might imagine my interest in your article. I’m wondering how pulmonary fibrosis differs from COPD and if the two medications that you mention (pirfenidone and nintedanib) might be helpful in slowing the progression of lung disease in patients such as myself. Have any studies been completed using these medications on patients with COPD? -- S.A. ANSWER: Chronic obstructive pulmonary disease -- its two main forms are emphysema and chronic bronchitis -- usually, but not invariably, is a result of long-term exposure to lung toxins, especially smoke. In the most common case of COPD, due to cigarette smoking (at least, that’s the most common in North America and Europe: cooking fires are still a common cause in less developed countries), stopping the exposure will dramatically slow down further damage. Unfortunately, there are no established treatments that can restore lung function in people with moderate to advanced COPD. Pulmonary fibrosis is, by contrast, a rare disease; about 30,000 people in the U.S. are diagnosed each year (compare that with the 9 million people in the U.S. diagnosed with chronic bronchitis last year). The exact mechanism of action of pirfenidone and nintedanib is not known, but they are not thought to be effective in COPD. Surprisingly, I did not find a published trial looking at whether these drugs might be effective. The need for new therapies to treat COPD is so great that I would have thought some researcher might have tried it, despite the long odds.

SPIRITUAL CHRISTMAS TRADITIONS Share the story of Christmas The Gospels of Mark and Luke offer differing accounts of the birth of Jesus Christ. Both indicate that Jesus was born to Mary, who was engaged to Joseph, a carpenter. Mary became pregnant through immaculate conception, as she was a virgin when visited by an angel who informed her that she was to carry God’s son. At the time of Christ’s birth, all Jewish people had to be counted by Roman soldiers for tax purposes. That required people to return to their places of birth. As a result, Mary and Joseph set out on an arduous journey to Bethlehem. CASA-Trinity Opens Community Connection Center in Dansville BY AMY GESSI

ASA-Trinity, Inc. (Council on Alcohol and Substance Abuse of Livingston County) has opened a Community Connection Center at 134 Main Street in Dansville. On Friday, November 5th, the Community Connection Center held its Open House. “It went really well,” said CASA-Trinity, Inc. Director of Community Based Services Brittany Jimenez, noting that a wide variety of people and representatives of several local organizations attended. Jimenez was also pleased to see several active program members as well as some of their family members present. “It was a good turnout and stayed fairly packed,” she said. “We received a warm welcome. Our referring partners were very excited for us, and our consumers (active members) were really happy to have an alternative option for continuing their recovery and wellness journey once they are finished with treatment.” Recovery options can be slim. “Steuben and Livingston Counties really lack recovery services outside of some NA meetings, some of which are well-established while others are not. However, there has not been a single location to which people can go,” explained Jimenez. “People in recovery have had This is such community support but nothing an exciting this consistent and organized.” Jimenez further mentioned a addition to trend of increased focus on the Dansville recovery to round out prevention and treatment. “We have added a community! lot of different components,” she - Brittany Jimenez said, noting not only meetings, but also fitness, arts, and music. “We are opening up a variety of pathways to recovery, offering a safe space to connect and participate in healthy activities.” This new Dansville space will be the location for AIR (Adventures in Recovery) Program and Main Street Station Youth Clubhouse. This center will be a safe space for adults and kids to enjoy and have access to community support meetings, art and music, alternative and holistic wellness activities, a library with both recovery and leisure literature, a gym with both independent and group fitness options and more! The Community Connection Center will be open Monday–Friday from 10am-7pm. The AIR Adventures in Recovery Program is a community of sober individuals dedicated to providing connection, The Community Connection Center’s Open House peer support, and on Friday, November 5th, 2021, was a great success! healthy fun outlets for those in recovery, their family members, and those in the community who choose to live sober. We seek to empower those in recovery to build their community, discover their inner strength, and find their recovery path. Social Security Gets Big Benefits Bump The news is in. Our Social Security benefit for 2022 will increase by 5.9%, slightly less than the 6.1% estimate the government put out this summer. Still, that’s much greater than the 1.3% increase we saw for 2021 and the 1.6% the year before. The average monthly dollar increase will be $92, making the average benefit $1,657 per month for a single person. For a couple, a $154 increase would come to $2,753 per month. The official mail on the cost-of-living adjustment will come out in December. I’ve done my calculations for next year, and once again I shake my head. We’ve been notified already that I’ll have a rent increase. Heating fuel is likely to go up 47%. We all know what has happened at the grocery store. Medicare Part B is sure to go up, maybe a $10 per month increase. There is one worrying note, and we need to think about how it will play out for us individually. The question is: Will that increase move us into a higher tax bracket? It depends on our provisional income. That number is any income (pension, wages, dividends and more) not from Social Security, plus half the amount of Social Security. Those of us who come in under $25,000 won’t have a federal tax put on our Social Security. If that number is between $25,000 and $34,000, there will be a tax on half of our Social Security income. Over that and it goes up to a tax on 85% of our Social Security income. If your situation for 2022 is likely to put you in the category of having to pay taxes on your Social Security benefit, you can elect to have them take money out of your check each month. Originally from Pennsylvania, she grew up in the Philadelphia area, then later moved to Allentown as an adult. In 2006 she relocated to New York, first to Cohocton, then later settled in Honeoye in 2008, where she lived in a lovely home overlooking a spectacular view of Honeoye Lake. Kathleen was a talented artist, known to many in the area for her expressive paintings, which were usually of mixed media and alive with vivid color, texture, and movement. She had a unique style that revealed her own imagination, vision, and truth. She participated in art events such as Artizann’s “Artists in Action on the Front Porch,” the Naples Open Studio Trail, and the former Springwater Fiddlers Fair. She often said that she was born to paint, proven by her legacy of so many beautiful paintings. At this time, no formal services will take place.

May at some point need to cover weekend shifts. • Pay – $55,000-$65,000 annually to start depending on experience. • This position takes a role in the supervision of warehouse operations, providing direction to warehouse associates and handling numerous projects and operational initiatives. Continuous improvement, safety, ongoing training, and driving efficiency in operations are all critical for this role and its success. Requirements: • Must be able to efficiently operate a sit-down forklift. • Education – Bachelor’s degree in Business Administration or related field preferred but not required. • Experience – 3 to 5 years of supervisory experience with 1 to 2 years’ related experience and/or training in Warehouse Management.
