THE DANGERS OF BELLY FAT

Rapid heart rate persists for a decade without evaluation

Q.DEAR DR. ROACH: I’m a 75-year-old woman who, for at least 10 years now, has been experiencing a rapid heart rate. My blood pressure, however, remains normal. In fact, doctors are usually surprised at how good my blood pressure is since I am borderline obese and slightly diabetic.

During this past physical, my doctor did mention my rapid heart rate but didn’t recommend treatment or referral to a cardiologist. Should I insist on a referral to a cardiologist, or would this only bring me to more unnecessary testing? -- M.R.

ANSWER: I unfortunately hear about people (more commonly women) having a potentially serious medical condition that gets ignored for many years, and I fear this is the case with you.

In a person with a persistently fast heart rate (over 100 bpm), an electrocardiogram (EKG) is an essential first step. I suspect you may have had this done, but if you haven’t, this clearly needs to be done. Atrial fibrillation and some less-common heart rhythms need to be excluded.

What’s so bad about belly fat? Seeing belly fat as a merely cosmetic concern overlooks the effects it has on overall health.According to the Mayo Clinic, belly fat is not limited to the extra layer of padding just below the skin known as subcutaneous fat. Belly fat also includes visceral fat,which means it extends to deep inside the abdomen and surrounds internal organs. Johns Hopkins Medicine notes that visceral fat in the belly is very close to the liver, which can turn it into cholesterol. From there, the fat goes through the bloodstream and may collect along the walls of the arteries, potentially contributing to a hardening and narrowing of the arteries known as atherosclerosis. The complications of atherosclerosis depends on which arteries are affected, but they can include an assortment of diseases that affect the arteries and a greater risk for aneurysms and chronic kidney disease.

Johns Hopkins Medicine notes that visceral fat is just as harmful for the heart as high blood pressure and smoking. So while efforts to reduce belly fat may initially be embraced to improve appearance, individuals should know that reducing fat in their midsections also can reduce their risk for a host of conditions, some of which are deadly.

It’s likely that you have “sinus tachycardia,” meaning that the impulse comes from the sinus node but is persistently too fast. The heart rate is supposed to be fast if you are exercising, and a fast pulse can often come on as a response to an infection or a loss of fluids. But I think that, after 10 years, you most likely have a condition called inappropriate sinus tachycardia (IST).

Since you are writing to me, I assume that this is bothering you. Most people with IST note palpitations, but loss of endurance and decreased exercise tolerance are other common symptoms. Before getting treated, it’s important to make sure that there isn’t some longstanding cause, such as anemia or thyroid disease.

It’s also necessary to exclude the diagnosis of postural orthostatic tachycardia syndrome, which can overlap with IST and seems to be much more common now, especially after a COVID infection.

I have had some success in prescribing exercise for carefully selected patients with IST. The cardiology literature recommends beta blockers or ivabradine to reduce symptoms and improve exercise ability.

Referral to a cardiologist is appropriate given your 10 years of not being evaluated. Even if you decide against therapy, making sure that there isn’t a serious problem is very wise.

MidlakesTheatreBrings TheLittleMermaidtoLife

Since fall 2022, social studies teacher, Layna Gray, has been at the helm of Midlakes eatre, bringing her deep-rooted passion for musical theatre to the students and community. With the addition of Emily Houston in spring 2023 and Lindsay Douglass in spring 2024, the program flourishes, offering students a place to grow both as performers and individuals.

A Director’s Journey

Hailing from Pittsford, Gray was immersed in musical theatre from an early age. “We were the family that played soundtracks on long car rides and argued about who got to sing which vocal part,” she says. With a family deeply involved in performance—her brother a professional actor and her sister a competitive dancer—Gray’s love for the stage was inevitable. While acting may not have been her strongest suit, she found her passion behind the scenes, directing and shaping productions. While studying adolescent social studies education at SUNY Geneseo, Gray added a theatre minor coursework, solidifying her love for directing.

Her experience, combined with her assistant directors’ skills, has created a well-rounded team that fosters student growth both on and off the stage. Lindsay Douglass grew up in Wyoming, NY and loved performing at Alexander High School productions and assistant directing at Sodus High School before she came to Midlakes. Gray says Douglass’ passion for musicals and singing is obvious to all she connects with, and Douglass “enjoys helping students find a place they are comfortable to express a more artistic side of themselves.” English teacher, Emily Houston, brings a long-term love of theatre and expertise in character development to each production.

For Gray, the most rewarding aspect of directing is witnessing students find confidence and express themselves. She says, “It’s astonishing how some of the shyest, quietest students can come to life on stage. eatre has always been a safe space for any person to express themselves, and I take pride in helping kids reach their fullest potential.” One of her fondest memories remains a quiet moment during Mamma Mia! rehearsals, where the cast lay on the stage waiting for a tech adjustment, and then ended up singing together in a moment of true camaraderie and connection Gray treasures to this day.

With iconic numbers such as “Under the Sea,”
“Poor Unfortunate Souls,” and “Part of Your World,” audiences can expect to be swept away.

Selecting the annual production is a collaborative effort, Gray says, explaining that they consider factors like set complexity, vocal arrangements, and casting opportunities. is year’s choice, e Little Mermaid, was made with the ten graduating seniors and students’ overall enjoyment in mind. With its beloved songs, many ensemble roles, and a variety of group solos, the production provides a unique opportunity for many students to participate for the first time. It’s also a fun, family-friendly experience Gray and the team hope will engage the entire community. With iconic numbers such as “Under the Sea,” “Poor Unfortunate Souls,” and “Part of Your World,” Gray says audiences can expect to be swept away. Christine Cole serves as vocal director and Talia Holtzman is choreographer of this year’s production.

While Gray is hard-pressed to recognize only a few individuals, as there are so many parents and staff volunteers working hard to make this show incredible, she says a special thank you to Heather DeRuyter, Jodi Clemons, and Michelle Dorr for their work in the costume shop, with Mary Ann Footer’s cra smanship on the mermaid tails standing out. Bob Belcher and Brandon DeRue have also played crucial roles in set design, ensuring a visually stunning performance.

Spotlight on Seniors

While the cast is relatively young this year, Gray says, seniors Kaylynn Judson and Kyra Guest have emerged as leaders in the program. Judson reflects on her time in Midlakes eatre, cherishing the friendships and experiences it has given her. “I am so happy Midlakes eatre has played such a large role in my

life,” she says. Guest, who has student-directed the past two plays, credits the program with inspiring her future aspirations to direct high school theatre.

Since she came to Midlakes in 2019, Gray has been impressed by the Midlakes theatre program as well as the broader Western NY theatre community. “ ere’s a stereotype of small schools having struggling programs or ones that are not supported by the community, but I’ve found the opposite to be true at Midlakes.” rough participation in the Stars of Tomorrow program with the Rochester Broadway eatre League, Midlakes students gain exposure to a vibrant network of performances across the Rochester area, proving that “ e theatre community is truly thriving with these kids!” as Gray says.

As Midlakes eatre prepares to debut e Little Mermaid March 13th-15th, Gray encourages the community to embrace and support the arts. “ ere are so many incredible community and school programs in this region, and they put on amazing shows.” Whether through attending performances or engaging with local theatre programs, involvement in the arts fosters invaluable skills and lifelong memories for students.

Lemon Drizzle

Cupcakes Makes 12

Cupcakes:

1/2 cup butter

1/2 cup sugar

3 eggs

1 cup self-raising flour

Zest of 1 lemon

Drizzle:

Juice of 2 lemons

5 1/2 tablespoons sugar

LemonButtercream:

1/2 cup unsalted butter, at room temperature

2 1/4 cups powdered sugar

Juice of 1/2 lemon

Sprinkles

Lemon Zest

Preheat the oven to 350° F and get 12 cupcake liners ready. Beat the butter with the sugar until light and fluffy. Add the eggs, self-raising flour and lemon zest and mix until combined. Spoon the mix evenly into the cupcake liners and bake for 18-22 minutes until they are baked through and springy to the touch.

Mix together the lemon juice and sugar for the drizzle in a bowl. Once the cupcakes are out of the oven, carefully spoon the drizzle over the cupcakes, then leave them to cool fully on a wire rack while you make the buttercream.

Beat the butter until smooth; this can take a couple of minutes. Add the powdered sugar in two batches, beating well after each addition, then add the lemon juice and beat again.

Pipe or spread the buttercream onto your cupcakes, then sprinkle each one with some sprinkles and lemon zest.

While it’s tempting to huddle up indoors when winter hits full swing, families who embrace the great outdoors when the temperatures dip can avoid cabin fever and enjoy one another’s company along the way.

STREAMING SOURCE

Medicare card scams are on the rise. During one calendar year, Medicare had over $100 billion in fraudulent claims,and the damage cost goes up when identity theft and stolen financial information are added in.

What the Medicare thieves want is your card number, and they’ll try any number of tricks to get you to reveal it.

Scammers might contact you to offer you a “cheaper” Medicare but will need your personal information to sign you up. Don’t fall for it. They might impersonate a Medicare employee and say that your card is about to expire and you need a new one.They’ll ask for your current card number and your personal information to verify it’s really you. (Wrong: Medicare cards don’t expire.)

Scammers might call and say that your Medicare benefits have been canceled, but claim they can help you get the benefits reinstated if you give them your personal information or even a fee. (Wrong: Medicare won’t communicate with you that way, and if they need to contact you, it will be by mail.)

Thieves might try to claim that your old paper card isn’t good now and that you need a new one with a chip in it, like a credit card. (Wrong: Medicare cards are only made of paper.)

Perhaps you should hibernate at home all winter. NYS DOT (and I assume other departments) have a preset amount of salt to apply based on road conditions. Many go to shift work in the winter to cut down on overtime. Many towns have central locations for salt storage (do you also complain when you pass multiple school busses at the end of the school day?).

Remember, wet roads freeze as the temperature drops to 32F and below, creating black ice. The wind drifts snow across roads. Both require continued maintenance after the snow has quit falling to prevent accidents. If you want to take your vent to mother nature and request snow and ice falling only certain hours and days that would be awesome.

Is my rabbit depressed?

DEAR PAW’S CORNER: I took over the care of a rabbit named “Chucky” a few weeks ago, after the previous owner, my aunt, passed away. Since he got here, Chucky has spent most of his days inside his hutch. When I try to take him out, he huddles tightly against the far wall. I don’t see him eat or drink during the day, but he does come out at night after I go to bed. Is he depressed, or is it something else? -- Jim F., Chula Vista, California

DEAR JIM: Chucky does sound pretty stressed out, and depression is definitely on the table here. He also may be having trouble adjusting to his new environment, and is missing your aunt, too. A stressed and unhappy rabbit is tense, staying in a down position with their ears flattened against their body. Their nose doesn’t twitch. They don’t hop around the cage freely, and they’ll hide away if you reach for them.

The first thing to do is establish care, if you haven’t done so already. Take Chucky to a veterinarian specializing in small animals (including rabbits) for a checkup, explain the circumstances, and detail his behaviors. The vet will rule out any underlying health conditions and can give you a wealth of advice on helping him be more comfortable and easing his transition to a new owner.

It’s important to be supportive and not force interaction at this time. Keep Chucky’s cage clean and fresh, change his water and food daily, and provide him with rabbit toys and chews strewn around the cage that he can explore. Talk to him in a soothing voice while you are near his cage. As he adjusts, he will gradually respond and feel comfortable with you.
