Gardeners How do you get rid of moles in your garden that are eating all your plants? - Diane S. If the animals are actually eating your plants, then they are not moles. Moles eat insects and earthworms in the soil. You probably have voles that can look similar to moles. Voles eat plants. Voles prefer the bark of young fruit trees as well as tender stems, foliage, flowers, seeds and fruits of grasses, sedges and many other herbaceous plants. They will make surface and underground tunnels. Cats, dogs, foxes, skunks and coyotes will eat voles as will some snakes and birds.

about sebaceous hyperplasia. I read that it is quite similar to acne and that a person could use an acne treatment to keep the condition in check. Could you give your thoughts on this? -- R.G. ANSWER: Although sebaceous hyperplasia can be mistaken for acne, sebaceous hyperplasia happens mostly to middle-aged adults (or newborns). These bumps appear most commonly on the forehead and cheeks, and have an umbilicus (a small hole) in the center. They are enlargements of the oil-producing sebaceous glands. They happen particularly in people with oily skin. Treatment can be challenging. Reducing dietary fat and using a good-quality skin-care regimen for oily skin may help. Dermatologists can treat these when they are cosmetically important, not because they cause harm in themselves. Although they can be mistaken for basal cell cancers of the skin, they are not cancerous or pre-cancerous. They are mostly treated mechanically, and by that I mean using a surgical technique like cauterization or excision. But these techniques can leave scars. Laser and phototherapy have good cosmetic results but are expensive. Isotretoin, a systemic acne drug with significant side effects, also has been shown in a small study to be effective. ***

DEAR DR. ROACH: I am currently taking 20 mg of Xarelto daily, and have a screening colonoscopy scheduled soon. The preparation instructions said not to stop taking blood thinners, but didn’t specifically mention Xarelto. I contacted the office and spoke with a scheduler, who said those instructions applied to all blood thinners. Previously, I was told that I would need to go off of blood thinners for five days prior to a colonoscopy. Since this was a contradiction to the instructions from the clinic, I have tried to reach out to other medical sources to see if I could get a consensus on what to do, but no one has responded to me yet. There is a strong history of clotting problems in my family. Would you weigh in on this matter? The last time I had a screening colonoscopy was around 2005, and there weren’t any issues. I realize this is a scope and not surgery, but it has me concerned enough that I am considering putting this off until I can get a satisfactory response. -- B.R. ANSWER: Most screening colonoscopies now are done with patients still on their anticoagulants, although for patients on warfarin (Coumadin), it is preferable for levels to be on the low end of the therapeutic range. This is a standard recommendation by the American Society of Gastrointestinal Endoscopy. The situation would be different for a high-risk endoscopic procedure, such as removing a large polyp. I have had to deal with this situation many times in my practice, and most often, all anticoagulants are stopped then restarted after the colonoscopy. There are some people who are at such high risk for clots that we use injection medications to prevent blood clots until the night before the procedure, but that is normally reserved for the highest risk of all. It’s a judgment call as to whether to use the injection anticoagulants to prevent clots around the colonoscopy. *** Dr. Roach regrets that he is unable to answer individual letters, but will incorporate them in the column whenever possible. Then pop-ups would appear from an IT with an 800 number – “Please call us, we see you’re having trouble.” I didn’t call them. Suddenly she was on the phone and had contacted me! “I can see you’re having trouble. It’s an e-mail block! We can fix it! We “clean” it and add an antivirus and protection for just $249.99!! What’s your credit card number?” Like a fool, I gave it to them. It’s so much more convenient they told me. I can have new icons with news channels and an icon just for email. My banker today said he didn’t know why this SCAM company is still in business at all. I should try the 800 # to cancel, but I doubt they would do it. I have to file

Thompson Encouraging Eligible Smokers, Ex-Smokers to Get Low-Dose Screenings

S

cott Lomber of Canandaigua has a lot to live for. In addition to his wife and two grown children, he has grandchildren he’s determined to see grow. So when primary care physician Dr. Cynthia Teerlinck recommended he get a low-dose lung cancer screening at F.F. Thompson Hospital, the longtime smoker agreed. The screening revealed a spot on one of Lomber’s lungs and in 2018, two weeks after he quit smoking, he underwent surgery at Strong Memorial Hospital for Stage 1 lung cancer. His six-month follow-up appointment this past September yielded a clean bill of health. “To find something and to have it be Stage 1, I feel lucky because I know it could have been a lot worse,” Lomber says. “To find it in the early stages makes all the difference.” Low-dose lung Dr. Ben Wandtke & Dr. Cynthia Teerlinck cancer screening takes about five minutes, causes no discomfort and has zero co-pay. In addition, experts say it can make a tremendous difference in prognoses. In fact, a study presented at the International Association for the Study of Lung Cancer World Conference showed widespread screening could save as many as 65,000 lives in the U.S. each year. To date, however, only a fraction of area residents who are eligible have taken advantage of the opportunity. “We don’t really know what is holding patients back,” says Dr. Ben Wandtke, president of Thompson Health’s medical staff and chair of its Population Health Management Committee. “Lung cancer is by far the leading cause of cancer deaths for both men and women,” the radiologist adds, “so it’s worth focusing on.” A patient’s primary care doctor must order low-dose lung screening after determining whether the patient meets the following criteria: • Between the ages of 55 and 77 • Has smoked “30 pack years” or more (one pack per day for 30 years or two packs per day for 15 years, for example) • Still smoking or quit fewer than 15 years ago Nationally, only 1 to 3 percent of people who meet the criteria participate in screenings. Within Thompson’s service area, it’s 15 percent. Although that’s far above the national average, Wandtke’s committee has a goal of reaching 30 percent within just two years. The reason is simple: “Early diagnosis enables a far better outcome than any treatment we can provide,” Wandtke says, citing data showing those diagnosed with Stage 1 lung cancer have a 50-percent survival rate at the end of 10 years, while those diagnosed with Stage 4 have a 10-year survival rate of less than 5 percent.
