Poor dental hygiene is a particular concern. DEAR DR. ROACH: I have had weight-loss surgery, specifically an adjustable lap band that was revised to a gastric sleeve in 2018. I have been given and read conflicting recommendations regarding the use of NSAIDs following these procedures. I have read that taking a low dose (400 mg) is considered safe. I have also read and been told to take the children’s liquid form at an adult dosage rather than the tablets. I am a 57-year-old female. I have largely avoided taking NSAIDs for years, but I have developed osteoarthritis and find that I am taking them more often (400 mg once daily, one to two times a week at most). I do ensure that I take them with food and also take a Pepcid whenever I take ibuprofen. -- A.W. ANSWER: There are several types of weight-loss (bariatric) surgery, and the best evidence that I found among those who had a Roux-en-Y gastric bypass shows up to a 70% increase in the relative risk of developing peptic ulcers after surgery. This sounds pretty terrible, but it actually means that in an average time span of four years after surgery, 1.4% of people who didn’t take any NSAIDs got an ulcer, while 2.4% of the group that had the greatest amount of NSAID use developed an ulcer. However, the same study showed that only 0.2% of patients who underwent a sleeve gastrectomy developed an ulcer, and the use of NSAIDs did not significantly increase this number. Finally, other studies have shown that medicines like famotidine (Pepcid) reduce the risk of ulcers by about half when used long-term. NSAIDs always have risks, but for a person with a gastric sleeve, the risks of lowdose ibuprofen are modest and need to be weighed against the benefits for a person’s quality of life.

A Journey of Faith and Service: The Story of Pastor Greg Falco BY CHELSIE WELLS

reg Falco, lead pastor of Elim Life Church in Lima, embarked on his journey of faith with a deep understanding of the human condition and a fervent love for Jesus. His decision to become a pastor was grounded in the foundational belief that every person is created to know and be loved by God, but the choices we make often separate us from Him. Greg’s passion for sharing the transformative message of Jesus became the driving force behind his call to ministry. From an early age, Greg’s parents played a pivotal role in shaping his Christian identity. His mother exhibited a unique connection with God, fostering an active relationship through prayer, Bible reading, and a lifestyle marked by peace. For his pastoral role, he engages in prayerful reading, using various translations and conducting deeper studies with commentaries and tools to extract timeless truths. Personally, he embraces devotional readings on the YouVersion app, selecting studies that provide a logical progression through the Bible. Since October 2002, Greg has served in various pastoral capacities, including working with children, youth, and as a worship leader before assuming the role of lead pastor. His vision for Elim Life Church extends beyond the confines of a physical building; he envisions a community where people experience the love of Jesus and actively share it with others. Pastors, according to Greg, are spiritual guides, aiding their congregations in understanding the Bible’s application in daily life. HOW ASSISTIVE TECHNOLOGIES ARE HELPING THE DEAF Cochlear implants The NDC notes that cochlear implants are electronic devices that are surgically implanted under the skin behind the ear. Cochlear implants utilize electrodes placed in the cochlea, or inner ear, to stimulate the auditory nerve of individuals who have substantial and permanent hearing loss. The NDC reports that cochlear implants are not necessarily ideal for everyone with signiﬁcant hearing loss. That’s because no two brains are the same, and it’s difﬁcult to predict how well a given person’s brain will interpret the new auditory signals the implant is sending. In addition, the NDC notes that certain types of hearing loss cannot be remedied with cochlear implants.

An April 2023 report from Global WebIndex indicated 60 percent of the world’s population uses social media, and the daily average usage is two hours and 24 minutes. Data from Meta indicates that 83 percent of Instagram users discover new brands while using the platform. Hooray for the Medical Aftercare by Rochester Regional Health Elizabeth Browning’s poem “How Do I Love Thee” was written in 1850 as a love poem for another person. I want to share the ways I love what Rochester Regional Health (RRH) has done to help my wife Dianne (81) since a recent six-day stay at Rochester General Hospital for heart-related issues. Since coming home from the hospital, the follow-up care by RRH has been Tremendous. RRH provided a slew of medical equipment, such as a blood pressure device, oxygen meter reader, great scale commode, and two four-prong walkers for use upstairs and downstairs in our home. RRH has provided me a Telemedicine number to call in her vitals. RRH provided a 24/7 nurse contact for medical issues. There have been periodic visits of physical therapists, nurses, and a social worker. RRH has been God-sent with all the follow-up being provided for Dianne, my dear wife of 59 years. DID YOU KNOW? The American Heartworm Society advises dog owners to have their furry companions tested for heartworm annually. Such tests can typically be conducted during routine preventive care visits to the veterinarian. The AHS notes that such tests are necessary even for dogs that are taking year-round heartworm prevention medications. Tests can confirm that such programs are working or indicate if they are not effective. The American Veterinary Medical Association notes that antigen tests are the most commonly performed tests. These tests detect the presence of adult female heartworms and they are typically highly accurate. If a test is positive, veterinarians may recommend further testing, including radiographs, a blood profile and an echocardiogram. Such tests can determine a number of things about the infection, including its severity. The AVMA also notes that the additional testing can help determine the best course of treatment. Cat owners should know that it’s more difficult to detect heartworm in cats than in dogs. The AHS indicates that cats are much less likely than dogs to have adult heartworms, making the disease harder to detect in felines. Veterinarians may recommend cats receive both the antigen test and an antibody test, which detects exposure to heartworm larvae. The AVMA notes that more testing may increase the likelihood of detecting the disease in cats.

Tipping men and women who work in the food service industry is commonplace in the United States and Canada, where citizens typically tip between 15 and 20 percent when dining out at restaurants. But American and Canadian travelers about to depart for overseas destinations may want to familiarize themselves with the practices regarding gratuities before their planes touch down. Some countries automatically add gratuities onto customers’ bills. For example, the government in Dubai mandates that a 10 percent service charge be added to all hotel, restaurant and bar bills. This service charge is then divided by the staff at the establishment, making the charge the equivalent of a tip. Diners in Morocco, where the gratuity is sometimes included in the bill, might be surprised to learn that a 10 percent tip in that country is considered generous. Tips are included in the bill in Costa Rica, where travelers need not feel obligated to leave any additional gratuities. Service is often included in the bill within the United Kingdom as well, though travelers should inspect the bill before assuming gratuity was included. Visitors to Spain should not feel obligated to leave a gratuity larger than 13 percent, and even a tip in the range of 7 percent is considered acceptable.

DID YOU KNOW? Firms on the lookout for ways to manage employees’ time more efﬁciently may want to reconsider their approach to meetings. A recent study on meetings commissioned by Verizon Conferencing found that an overwhelming majority of professionals who are considered heavy meeting-goers admitted to an array of behaviors that suggest meetings aren’t as productive as employers may believe. Ninety-six percent of respondents admitted to having missed meetings in the past, while 91 percent acknowledged daydreaming during meetings. In addition, 73 percent of respondents did other work during a meeting, while 39 percent acknowledged falling asleep.

Rather than focusing on job responsibilities at each job you held, gear the résumé around what you achieved in each position. This will help identify how you performed in the position, instead of just a general retelling of what you did. It can be challenging to achieve this for jobs that don’t naturally lend themselves to numerically quantitative results (i.e., increased department sales by 15 percent), but it’s still possible to use a résumé to illustrate your achievements. Rather than state that you were responsible for providing customer service, explain that you built your reputation on conveying difﬁcult technical terms to the layperson, serving as the go-to employee for translating job jargon for outside correspondence. If you have quantitative proof of how you accomplished something, certainly add it. This can include measures of proﬁt growth, reduction of debt or increase in customer base.

Because I can’t bring my dog Jasper with me, I’m trying to decide whether to board him at a nearby kennel or hire a dog-sitter instead. I’m worried about him contracting that respiratory illness that has been affecting dogs lately. What’s your recommendation? -- Todd G., via email DEAR TODD: Both of those options are fine -- as long as you take a few minutes to investigate both the kennel and interview the dog-sitter. Schedule a tour of the kennel facility so you can see the conditions Jasper will live in and talk to the staff. Has the kennel had any dogs develop kennel cough or other transmitted viruses recently? How often do they disinfect the kennels and play areas? How much exercise do the dogs get? Likewise, interview the dog-sitter in person, or over Zoom. How often will they visit Jasper -- once or twice a day? How often will Jasper get walks and play time? Will Jasper be the only dog being walked, or will he be part of a group of dogs? Is the dog-sitter insured, in case of accidents? Once you’ve decided which caregiver to place Jasper with, be sure they have your contact information in case of emergencies, and details about Jasper’s diet, type of food and medications. Place an article of clothing that has your scent on it in Jasper’s bag so that he is more comfortable whether staying at home or sleeping at the kennel. After you return, keep an eye on Jasper to make sure he’s healthy. If he is coughing or sneezing frequently, has a lot of nasal or eye discharge, and seems lethargic or uninterested in food, take him to the vet for an evaluation.
