Praluent Injections Significantly Decrease LDL Level
DEAR DR. ROACH: I am a 66-year-old male who was diagnosed about 15 years ago with mild coronary artery disease, with some blockages in my peripheral arteries. I was taking the maximum recommended dosage of pravastatin to maintain a low cholesterol, but I experienced complete ruptures of both Achilles tendons. These incidences occurred five years apart, but after the second rupture, my doctor discontinued the pravastatin. I then started Praluent injections twice a month to control my cholesterol, which has been incredibly effective -- maybe even too effective -- and is the purpose of my writing to you. With Praluent injections, my LDL level is about 26, and my HDL is 100. When my primary doctor’s assistant recently called to inform me of my bloodwork results, she said she has never seen anyone with such a low LDL level -- and she sees a lot of bloodwork! My primary care doctor doesn’t like it that low, but two different cardiologists I have seen think that the lower it is, the better. What do you think? -- J.W.
ANSWER: The first issue is whether statins increase the risk of an Achilles tendon rupture, and I can’t answer that with certainty, because there is some evidence it does. (Some studies have reported anywhere from an 80% relative risk increase to triple the risk, but the risk is still very, very low. Even if they triple the risk, the risk would be only three people per 10,000.) The studies that have shown there is risk suggest that the risk is only in the first year of taking statins, but other studies have shown no risk at all from statins.
The second issue is your very low LDL level, and the data for LDL levels is firmly on the side of the cardiologists you spoke with. The lower the LDL, the lower the risk of heart disease, and levels under 30 are not at all unusual in a person taking alirocumab (Praluent) or the other drug in that class (called PCSK9 inhibitors), evolocumab (Repatha). Your doctor’s assistant will likely be seeing more people with LDL levels like yours. Do remember that a healthy, mostly plant-based diet and regular exercise still have benefits in people with LDL levels as low as yours -- and not just in heart disease, but in reducing cancer risk, too.

DEAR MADDY: While ea activity tends to drop during the colder months, chillier weather isn’t a guarantee that they’ll go away. A number of factors could keep them around, conceivably, all year. ings like a constantly warm environment in which to live (your house) or a higher than usual ea population (this can vary from year to year).
To really get rid of Isaiah’s eas, you’ll need to treat him and tackle possible ea problems inside the house.
Treat Isaiah with a ea treatment, available at the pet store or from the vet. Perfumed soaps aren’t good alternatives; they can dry and irritate his skin.
Wash or replace Isaiah’s bedding, and wash any furniture upholstery, blankets or other items where he likes to lounge for long periods.
Treat the carpet with a ea preventative, available at the pet store or online. Put Isaiah into a safe room and sprinkle the treatment into the carpet as directed, then vacuum it up. e treatment will take care of any live eas or eggs hiding in the carpet.
If your cat spends time outdoors, consider making him an indoor cat. is will reduce the chance of him contracting eas outside and bringing them into the house.

Also known as stage IV breast cancer, metastatic breast cancer is the most advanced stage of the disease. Patients diagnosed with breast cancer also should know that improved treatments may further their chances of surviving a diagnosis, even a diagnosis of metastatic breast cancer. In fact, the American Cancer Society notes that survival rates are based on women who were diagnosed and treated at least five years earlier. In the time since those survival rates were documented, treatments could have advanced even further, potentially improving the five-year survival rates for metastatic breast cancer.

Women have some control in regard to their risk for breast cancer
No one, regardless of their age, occupation, skin color, or socioeconomic status, is immune to cancer. In fact, individuals might be hard pressed to say no one in their family and/or circle of friends has been diagnosed with cancer at some point. According to the National Cancer Institute, there were 18.1 million new cancer cases across the globe in 2018. Among women, when excluding non-melanoma skin cancer, no cancer was more prevalent globally than breast cancer. e World Cancer Research Fund reports that, in 2018, breast cancer accounted for 25.4 percent of all new cancer diagnoses in women. at gure is nearly three times as high as the percentage of cases of colorectal cancer, which accounted for the second most new cancer cases diagnosed in women in 2018.
It’s understandable to be fearful of such gures, which can make a breast cancer diagnosis seem almost inevitable. However, the Centers for Disease Control and Prevention notes that not all risk factors for breast cancer are set in stone. ough age and family history, two known risk factors for breast cancer, may be beyond a woman’s control, she still can exercise some control over other risk factors.

Carrying around extra weight can make moving around more di cult, so the O ce on Women’s Health urges larger women to start slowly if it’s been awhile since they exercised. In addition, aging women can speak with their physicians for advice about exercise regimens they should or need not avoid.

Do I Need a New Coat is Year?
With cold weather here, we’re all trying to nd ways to stay warm both indoors and when we go out. Are you set up for winter? is is the time to be sure.
Here is what I’ve done so far this year.
Slippers that you wear around the house: Have yours stretched out over the years? Is there any chance you could walk right out of them? is might be the year you need to invest in new indoor footwear for safety. Slippers are jotted down on my own list because yes, I do need new ones, and I’m going for slippers with a solid bottom.
Indoor vest: I’ve had one for years that adds a layer of warmth when the house is a bit chilly when worn over a sweatshirt or a long sleeve wa e shirt. It’s still ne.
Gloves: I’ve discovered that my insulated gloves that have kept my hands warm for years now have worn through the seam on the thumb, creating a 1-inch hole and letting in very cold air. I discovered how that happened: It’s from holding the steering wheel in the car. I’ll be pulling out my needle and thread to make that repair.
Hat, scarf, boots: check.
Winter coat with hood: And this is where making a major purchase is i y. Do I need a new winter coat this year ... or not?
And the answer is no. ree winters into Covid, it’s still out there. Despite the new omicron booster, there are experts who say we’ll have another Covid surge this winter with rising cases and hospitalization numbers, mostly due to human behavior and dwindling interest in getting a booster.
So no ... once again, I don’t need a new winter coat. I still won’t be going out as much as I used to long ago.

October is Anti-Bullying Month
P
arents go to great lengths to protect their children.
Keeping a watchful eye is a great way to protect kids when they're around the house, but parents may need to look for more subtle signs to determine if their children are being mistreated when they leave home.
The U.S. Department of Health and Human Services notes that roughly 20 percent of students between the ages of 12 and 18 experience bullying nationwide. Parents might once have written off bullying as part of growing up, but research has long since indicated that bullying can be very harmful to youngsters. The DHHS notes that research indicates that persistent bullying can lead to or exacerbate feelings of isolation, rejection, exclusion, and despair. Children who are persistently bullied also may experience new or worsening feelings of anxiety and depression.
Parents can play a vital role in preventing bullying. Much of that role involves parents educating themselves about bullying, including what it is and what it's not and what are some warning signs that a child is involved in bullying.
What is bullying?
The DHHS website Stopbullying.gov defines bullying as unwanted, aggressive behavior among school-aged children that involves a real or perceived imbalance of power. Bullying behavior is repeated or has the potential to be repeated over time. The imbalance of power associated with bullying involves the children who bully using some semblance of power they have over the children they're bullying to harm or control those youngsters. Their power may be physical strength, access to embarrassing information or popularity.
What isn't bullying?
Various types of aggressive behavior have the potential to be harmful, but they do not fall under the umbrella of bullying. For example, Stopbullying.gov notes that children between the ages of three and five are learning how to coexist with one another, including how to share and cooperate. Children in these age groups may be aggressive if they don't get what they want, but their actions in such instances do not constitute bullying. More information about potentially harmful, non-bullying behaviors is available at Stopbullying.gov.
Educate Yourself and Your Children
What are some signs a child is being bullied?
The DHHS notes that not all children who are being bullied exhibit warning signs. In addition, some signs might be more subtle than others. But some potential indicators that a child is being bullied include:
• Unexplainable injuries
• Lost or destroyed clothing, books, electronics, or jewelry
• Frequent headaches or stomach aches, feeling sick or faking illness
• Changes in eating habits, like suddenly skipping meals or binge eating. For example, children who are being bullied may come home from school hungry because they did not eat lunch.
• Difficulty sleeping or frequent nightmares
• Declining grades, loss of interest in schoolwork, or not wanting to go to school
• Sudden loss of friends or avoidance of social situations
• Feelings of helplessness or decreased self esteem
• Self-destructive behaviors such as running away from home, harming themselves, or talking about suicide
What are some signs a child is bullying other children?
Parents also should be on the lookout for signs their children are bullying other youngsters. Such signs include:
• Kids get into physical or verbal altercations
• Children have friends who bully others
• Increasingly aggressive behavior
• Frequent trips to the principal's office or to detention
• Kids have extra money or new belongings but cannot explain how they got the cash or items
• Kids blame others for their problems
• An unwillingness to accept responsibility for their actions
• Kids are competitive and worry about their reputation or popularity
Parents have a vital role to play in preventing bullying so all youngsters feel safe and sound inside and outside of school.

So, what was the message? Schallert advised businesses to think bigger. Local shoppers are important, but growing your audience beyond the immediate area can be a game changer. He asked the attendees to consider “what makes you special”?
He shared that many owners he’s worked with are humble and not immediately comfortable promoting themselves as “the best at” or “the number one” of something. But that might be just what it takes to get travelers to stop by and try their goods and services.
What will it take for businesses to get the word out? Schallert said it’s a combination of savvy marketing, print ads, social media, mailings, yers, and, of course, word of mouth. But he also said not to overthink it. e resounding message was to pick something that makes you stand out and start promoting. Will
we see Livingston County as the new home of the biggest and best for new and exciting services? Time will tell. ose items undoubtedly already exist here; it might just take the right marketing idea to get the word out.

Offer practical support. Cancer affects the body in a number of ways. Energy levels may wane and certain symptoms may arise. Side effects from treatments also can make it difficult to continue with daily tasks. So an offer to help with tasks associated with daily living, such as cooking meals, gardening, washing clothes, or cleaning up around the house, can be practical and much appreciated. Approach the individual and ask questions in pointed ways. Rather than, “What can I do to help?”, which may result in an answer of, “Nothing,” figure out a way to pitch in and then ask if that would be acceptable. This may be, “Would you like me to run to the supermarket for you today?”

What are some examples of PA technology? Sensors are a prime example of PA technology that helps make farms more efficient and productive. Sensors serve various functions by helping farmers gather data on the availability of water in soil, the level of compaction in soil, leaf temperature, insect and disease infestation, and other areas. Weather modeling is another component of PA that can help farms be more costeffective and efficient. Whereas in years past many farms would need to manually assess certain variables to determine when to harvest, weather modeling technology has enabled some farmers to generate remote readings, saving time and money.

Clayton “Clay” Wallace Harris October 5, 2022
Dansville- Clayton “Clay” Wallace Harris, passed away Wednesday morning October 5, 2022, at Noyes Memorial Hospital after a long ght with Pulmonary Fibrosis.
He was born in Montour Falls, on July 16, 1942, a son of the late Clayton and Eleanor (Feeley) Harris. He was also predeceased by a granddaughter, Trinity Nichole Harris; Mother-in-law, Margaret Knopp; Father-in-law, Phillip Knopp; and brother-in-law, Robert H. Knopp. On February 19, 1966, he was married to the former Mary Knopp who survives.
Clay taught mathematics at the Dansville Central School District for 35 years. He was known as a dedicated, yet tough teacher. After his retirement he served on the School Board for an additional 12 years. Clay was a member of the group that enacted the Taylor Law for New York State in 1967, which helped unions negotiate fair wages while also keeping educators from striking. Clay was a union representative for CSEA, and a board member for the United Way. Clay was part of the Dansville Retired Teachers Association, and on the Dansville Public Library Board. He was a long-time member of the Daniel Goho Post #87 of the American Legion in Dansville. He served as the President of the Board of Directors, Finance O cer, and on the Firing Squad. He served in the Army National Guard, retiring at the Rank of Command Sergeant Major. He was activated to active duty on three separate occasions: the Corning Flood, and both the Attica and Elmira prison riots. In his free time, he enjoyed reading, watching the Bu alo Bills, and clearing his mind cutting and stacking rewood.
In addition to his wife of 56 years, Mary, are their children, Patrick (Lisah) Harris, Leah (Roger) O’Toole; a brother, Philip (Christine) Harris; grandsons, Timothy R. and Jacob C. O’Toole; and sister-inlaw, Judith Knopp.
A memorial Mass of Christian burial was held on Wednesday October 12, 2022, 10:00 am at St. Mary’s Church of Dansville. Interment with full military honors followed Mass in Holy Cross Cemetery Dansville. Arrangements are by the Hindle Funeral Home, Inc., 271 Main St., Dansville. My father, my dad, and Grandpa, Jon Walter Van Derhoof, was born on July 22, 1938, in North Hornell, New York, to his mom and dad, omas W. and Martha Edmond Van Derhoof.
He was also raised by his, as well as my own, Godparents, Lynn and Elizabeth Pickard, of Dansville, who introduced him to his love of ying at a young age. He attended school for public education and graduated in the Dansville High School Class of 1956. He then enlisted on October 30, 1959 and served in the United States Air Force and was commissioned as a Captain on October 1, 1961. He met Barbie Sue Passmore, my mom, in Texas while serving in the military. ey married and had two children, Elizabeth “Li n” Ann, and Eric Scott Van Derhoof. After leaving the service, he attended Syracuse University and received his Bachelor of Arts Degree in 1971. He worked for the XEROX Corporation,the Railroad,the United States Postal Service, and most recently for Seneca Army Depot. But the love of his life was ying. He served as a ight instructor for many years, training other private pilots and helping them obtain their license to y on many types of airplanes,helicopters,and gliders. He was also awarded accolades and recognized by the FAA for over 50 and 60 years of ight without incidence.
He was blessed with a second wife and family when he married Jacqueline “Jackie”Fournier.Together they established Sunny Grove Manor to take care of many special grannies in their later years together.
My dad loved his dogs, such as his pooch Tucker, reading, photography, shing and hunting, and working out at the gym. He was so proud that he could do 100 crunches and run 20 minutes on the treadmill at the age of 80 years plus.My dad loved America and was proud to be a descendant of many great Americans going back to the Colonial times,as well as e Revolutionary and Civil Wars.
He was a member of the Dansville Pilots Assoc., e American Legion, e Geriatric Pilots Assoc., e Quiet Birdmen, e Finger Lakes Soaring Club, and St. Mary’s Catholic Church of Dansville.
Jon W. Van Derhoof was preceded in death by the immediate family members, follows, both of his parents, his brother Tommy, his sister Gretchen, his son Eric, and his dear wife Jackie.
He is survived by his daughter, Li n Molter and Michael Molter of Midland,TX; his granddaughter,Nicole Elise McBride and her husband Jon McBride of El Paso; and a grandson Joshua Hunter Molter of Lubbock Texas, soon-to-be wed to a ancée, Lauren Deubler, plus graduating with his doctorate from Texas Tech Medical School in Lubbock. He was looking forward so much to these life celebrations ahead.
He shall be very much missed. I thank you for your kindness to him in any way during his last years, months, and days.
A Mass of Christian Burial was celebrated on Friday, October 7, 2022 at St. Mary’s Catholic Church in Dansville. Following Mass, committal prayers with full military honors took place at Greenmount Cemetery in Dansville.

Keep in mind that emotions may change on a dime, and some emotions may be directed at support systems. While it can feel hurtful, remember the real reason for any outburst is the disease.

Dawn Chabowski Didas,72 of Dansville,NY passed away on October 4, 2022 surrounded by her loving family. Predeceased by her parents, Leo and Beverly Chabowski of Dansville, aunt, Norma Lemen and brothers-in-law, Joe and Robert Didas.
Dawn was born on April 4th, 1950 in Dansville, NY. She attended both Catholic school and Dansville High School, graduating in 1968. Dawn served her community as a valued employee at the Livingston County Court and as the owner of Dawn’s Flowers & Fancy in Dansville. Dawn was an avid gardener who was known for her lovely garden as well as her beautiful smile and infectious laugh. She also had a passion for sharing her family history. With an open heart, Dawn valued her family above all else.
Dawn is survived by her loving husband of 39 years, Ron Didas of Dansville; beloved son, Jay and daughter-in-law, Julie and adored grandsons, Jack and Jace of Naples,NY; brother,Lee and sister-in-law,Margaret and niece,Olivia of Brooklyn; sister-in-law, Dawn (Doug) Sterner of Dansville; brother-inlaw, Leo (Ruth) Didas of Wayland; sister-in-law, Shirly Hayes; sister-in-law, Darlene (Phil) Chissom of Bath; aunt, Barbara Weaver and cousin, Kelly of Dansville and many cousins and friends.

Estrogen, progestin, or both may be given to replace the estrogen no longer made by the ovaries in postmenopausal women or women who have had their ovaries removed. Last Friday I was walking to my car in a suburban store parking lot. Unbeknownst to me, I was followed from the store by someone. I was warned of this by a kind young lady who noticed this person following me and pulled her van alongside me until I got in my car safely. We live in a village home in and among many other homes on a well-traveled street. Occasionally, my wife and I have things we no longer use. These items are too good to simply discard, so we put them in front of our home with a painted, wooden sign stating "Free Stuff" attached to the nearby tree. We are happy that people find uses for these items. We are not happy when they also take our painted signs which are securely nailed or bungeed to the tree. Why would anyone do that? Maybe I'm too literal. Maybe I should keep it simple and discard this stuff. Otherwise, along with the signs, someday I'll hear a chainsaw cutting down the tree, as well. Wow, this country is in serious trouble.
Handicap Parking
The abuse of the handicap parking system is beyond the pale! The number of people pulling into a handicap spot, hopping out of their vehicle and fast walking into a building or then upstairs is wrong. Sometimes, that sort of person hops out of their car with a nametag on, showing they're working. Most of these people are not elderly. Then, an elderly person with a cane or someone who has difficulty walking must walk an extreme distance because there's no close handicap spot for them. It's selfish and greedy. These people know they don't need that spot. You know who you are.

BREAST CANCER TREATMENT
Hormone therapy is a cancer treatment that removes hormones or blocks their action and stops cancer cells from growing. Hormones are substances made by glands in the body and circulated in the bloodstream. Some hormones can cause certain cancers to grow. If tests show that the cancer cells have places where hormones can attach (receptors), drugs, surgery, or radiation therapy is used to reduce the production of hormones or block them from working. The hormone estrogen, which makes some breast cancers grow, is made mainly by the ovaries. Treatment to stop the ovaries from making estrogen is called ovarian ablation.
Hormone therapy with tamoxifen is often given to patients with early localized breast cancer that can be removed by surgery and those with metastatic breast cancer (cancer that has spread to other parts of the body). Hormone therapy with tamoxifen or estrogens can act on cells all over the body and may increase the chance of developing endometrial cancer. Women taking tamoxifen should have a pelvic exam every year to look for any signs of cancer. Any vaginal bleeding, other than menstrual bleeding, should be reported to a doctor as soon as possible.
Hormone therapy with a luteinizing hormone-releasing hormone (LHRH) agonist is given to some premenopausal women who have just been diagnosed with hormone receptor positive breast cancer. LHRH agonists decrease the body’s estrogen and progesterone.
Hormone therapy with an aromatase inhibitor is given to some postmenopausal women who have hormone receptor positive breast cancer. Aromatase inhibitors decrease the body’s estrogen by blocking an enzyme called aromatase from turning androgen into estrogen. Anastrozole, letrozole, and exemestane are types of aromatase inhibitors. For the treatment of early localized breast cancer that can be removed by surgery, certain aromatase inhibitors may be used as adjuvant therapy insteadoftamoxifenorafter2to3yearsoftamoxifenuse.Forthetreatment of metastatic breast cancer, aromatase inhibitors are being tested in clinical trials to compare them to hormone therapy with tamoxifen. Other types of hormone therapy include megestrol acetate or antiestrogen therapy such as fulvestrant.
