TO YOUR GOOD HEALTH By Keith Roach, M.D.

Smoking, Symptoms Biggest Factors in COPD Prognosis

Serving Our Community for over 20 years. Discover the Human Touch in Hearing Technology

---

Accepting New Patients.

DEAR DR. ROACH: I was diagnosed with COPD a year and a half ago. I seem to be unable to get answers from my pulmonary doctor, as he is always busy. How fast does COPD progress? I’m 78 years old, I’m in reasonably good health, and I exercise regularly. I have chronic bronchitis with a slight amount of emphysema. I had been coughing a little during the day, had mucus in the morning and coughed at night for two to three hours after bedtime -- to the extent of having to use a rescue inhaler to settle down and get back to sleep. After he put me on Trelegy, all of those symptoms disappeared, and I feel perfectly normal except for shortness of breath. My other question is, what is the life expectancy of a person diagnosed with COPD? I’ve read answers to this all over the map, from two or three years after diagnosis to 15 or more years. I would presume that it depends on other factors such as physical condition, exercise, weight, etc. I would like some kind of guideline for someone in my condition. -- T.J. ANSWER: There are two major types of chronic obstructive pulmonary disease: chronic bronchitis, where the primary problem is productive cough; and emphysema, which is destruction of the small airways. Shortness of breath is a common feature of all types of COPD. The most prevalent cause is smoking. I can’t answer your questions on rate of progression and overall prognosis without much more information, but it is certainly a good sign that many of your symptoms are well-controlled on treatment. The single biggest factor in rate of progression of COPD is whether you are a smoker, but not being able to exercise, being male and being very underweight are additional risk factors for faster progression. In someone with COPD, the decline over months to years can cause symptoms to occur first with exercise, and then when at rest. Once people have severe symptoms at rest, the prognosis becomes quite poor. No medication can reverse lung damage. Inhaled steroids reduce inflammation and can slow progression somewhat. Inhaled bronchodilators open airways and make breathing easier. Inhaled anticholinergics reduce secretions. Trelegy contains all of these medicines and is appropriate for people with severe COPD or those who do not have good symptom control with a less intensive regimen.

HOW TO CONSERVE ENERGY THROUGHOUT SUMMER Install ceiling fans. Ceiling fans can improve cooling efficiency in a home. According to the DOE, ceiling fans allow men and women who use air conditioners to raise the thermostats on their AC units about 4 F without adversely affecting comfort levels. Men and women who live in temperate climates may find that ceiling fans are enough to keep rooms cool without the need for air conditioners.

Homeowners often choose to save money and learn valuable skills by doing their own home improvement projects. These minor or major renovations are made much more successful by having the right tools for the job. Investing in good tools can be smart, but those tools are only effective if they are accessible and in good repair. Homeowners who emphasize taking care of their tools can make DIY projects that much easier and more rewarding.

Fu n Fac t What is the meaning of ‘take a penny/leave a penny?’ The words ‘take a penny, leave a penny’ refer to the tray, dish, cup, or other container found in local stores, gas stations, and other places, where customers can leave or take coins for cash transactions, as required. ‘Take a Penny/Leave a Penny' can also be applied to a bigger picture, with 'penny' representing kind deeds and words. A person 'leaving a penny' is spreading good vibes all around, while someone 'taking a penny' is benefiting from another's goodwill. GENESEE VALLEY PENNY SAVER

They work well initially; then the deer get used to

SENIOR NEWS By Matilda Charles Discounts on Phone and Internet Have you heard of Lifeline? Started many years ago to help low-income individuals with their phone costs, it’s been expanded to include internet at home, offering a $9.25 monthly discount. That $9.25 savings doesn’t sound like much, but if you’re like me, nowadays, you’re converting every dollar saved into food. That $9.25, for example, could turn into a loaf of bread and a gallon of milk, or even a pound of ground beef, a package of hamburgers buns and most of a can of beans. You’ll need to qualify for Lifeline. One way you do that is if you (or anyone in your household) receive benefits from Supplemental Security Income, Temporary Assistance for Needy Families, veterans pensions, Supplemental Nutrition Assistance Program, Medicaid and others, or through having verifiable low income. (You’ll need paperwork to prove it.) You can sign up in a number of ways, such as by mail or online at nv.fcc. gov/lifeline. Scroll through the information on that page. Call 800-2349473 for more information or to ask that an application be sent to you by mail. But there’s more. In addition, if you qualify for Lifeline, you likely also qualify for an even bigger assistance: the Affordable Connectivity Program. ACP is new and can give you a $30 monthly discount on your internet. Once you’re in the Lifeline program, call your internet provider and enroll in ACP with them. There’s no need to fill out another application. There are hundreds of internet providers that are part of the program, including some big names like Xfinity, AT&T and Spectrum. In some cases, that $30 gets you internet without you having to pay anything additional. In other words, free internet. Get the details from your internet provider. For a list of providers by state, go to www.fcc.gov/affordable-connectivity-program-providers. Get more information on the Affordable Connectivity Program at www.fcc.gov/acp or call 877-384-2575. BANISH MOSQUITOES FROM YOUR YARD Inspect property for hidden pockets of water. While bird baths, pet bowls and kiddie pools are easy to find, homeowners may be unknowingly hosting mosquito breeding grounds elsewhere on their properties. For example, tarps used to cover pools, automobiles or grills that are not tightly secured can fold up, creating pockets where rain water can collect and give mosquitoes somewhere to breed. Mosquitoes also may use gutters to breed, so homeowners should routinely inspect and clean their gutters during spring and summer.

RAVES Big Thank You A big thank you to the civic minded person who gave money for a new American flag! We are looking good at Little Lakes Community Center! Thanks to the person who donated money for our new flag and the landscape volunteers who spread 75 bags of mulch and cleaned up the flower beds! Thanks for the dog bags! Thank you to the person that put dog poop bags on my porch with a note. I must’ve missed one while walking more than one dog! Sorry if my pup ruined your green grass. I know how much you love dogs! Livingston Murals Not only do I enjoy seeing all the new murals when driving into work, but love the uniqueness behind each design and that they are all painted by local aspiring muralists. I hope to stop by them all sometime soon.

Pedestrian Crosswalks I think it’s time for some driver education. Drivers don’t seem to know that pedestrians have the right of way at these crosswalks. I watched a little boy on [local road] trying to cross at a marked crosswalk at this intersection. Nobody stopped to let him cross. I was waiting to make a right turn onto [local road], waiting until this little boy safely crossed. The driver behind me was honking angrily. Finally, an adult male came to his rescue and made sure he crossed safely. I see this law being broken all the time. Disrespect I do not understand the disrespect in today’s world. My husband was not a Veteran, but he was always very patriotic. He passed away a few years ago. He loved pinwheels, seeing them blowing in the wind. I found a large 16” red, white, and blue pinwheel with white polka dots, which was 36” tall and placed it at his grave for the last two years. It did not cost a lot of money but was worth millions in memories to my family. It disappeared! There was no windstorm to pull it 10” out of the ground and blow it away. And nothing else in the whole cemetery was disturbed. So, I know someone took it! I hope every time you look at it you remember where you got it and how you got it. Such disrespect stealing from a grave! We all share the Canal! We always love seeing other boats enjoying the Canal when we kayak from [local town] to [local town] and back, but we would appreciate it if power boaters slowed down when passing smaller boats and when approaching wildlife, like groups of ducks and geese, and especially those with young. What’s the rush? Slow down, enjoy the scenery, and you’ll be surprised at all of the amazing things you’ll see. We all share the Canal. Please be respectful.

Festivals are back! I love seeing all the festivals and events happening again! Of course, I understand the risk of covid-19 is still here and we should take precautions. Even if I don’t go to the events I love that we are getting back to fun this summer! From the Oatka Festival in LeRoy, to the Dansville Balloon Festival to the West Sparta OldFashioned Day and everything in-between! Disc Golf Course A few years back, the town I live in installed in a disc golf course and I cannot tell you how much we enjoy it and use it. Thank you to everyone that had a part in that. I know my family appreciates it!

Protect Your M obile Devices - and Your Identity (Family Features) M obile devices have become an indispensable tool for many people - and the thought of losing one of those devices, or having it stolen, is disco ncerting, to say the least. But it happens every day. It’s estimated that lost or stolen phones alone could cost U.S. users $30 billion in 2012. W hen a phone or tablet goes missing, so does a lot of personal information. And that can lead to m ore than just some inconvenience - it can lead to a stolen identity. The Federal Trade Commission estimates that as many as 9 million Americans have their identities s tolen each year. W hile identity thieves still resort to old-fashioned methods of information gathering, like go ing through trash and stealing w allets and purses, they can also get the personal information you store on your mobile device.

“I join with our County veteran population in thanking Jason for his support and advocacy for veterans’ issues,” stated Board of Supervisors Chairman David LeFeber. “He’s done amazing work during his time here. We’re sad to see him go, but wish him all the best in his future endeavors.”

ivingston County Veteran Services Director Jason Skinner served his last day today after more than six years leading the County’s Veteran Services department. Skinner first began working with the County’s veterans in this role in September of 2015. During his time at the department’s helm, he’s worked tirelessly to improve the lives of area veterans by fighting for access to benefits, supporting the funding of life-saving mental health resources, and acting as an advocate for veterans as they interact with the Department of Veterans Affairs (VA). Among his many accomplishments, Skinner said he’s most proud of the Veteran Services department’s paper-to-digital transition. The office is now 100 percent paperless, with all files being scanned and digitally uploaded directly to the VA, resulting in more efficient and accessible records. Technology upgrades have also given the office the ability to perform virtual appeals hearings directly from the Veterans Services office in Mount Morris. Skinner credits County leadership for his ability to accomplish so much while serving as director. “I am honored to have served the County in this role and would like to share credit with the Livingston County Board of Supervisors, County Administrator Ian Coyle, and Probation Director Lynne Mignemi for their continued support,” he said. “The military instills a sense of responsibility and prepares veterans to become stewards of their community. Public service is a great career path for veterans that want to continue serving honorably, especially with the support of a great employer like Livingston County.” Skinner worked to connect housebound veterans with the VA’s Home Based Primary Care team while director. The team provides patients experiencing complex, chronic, progressively disabling diseases with comprehensive, long-term home care. Skinner said this service is life-changing for veterans in need of it, especially in a rural community like Livingston County. He was also instrumental in the creation of the Livingston County Veteran’s Monument, a more than 8,000 square foot memorial in the shape of an American flag, dedicated to the veterans of Livingston County. The monument features 255 red metal poppies, hand crafted by local veterans, which adorn the red stripes of the flag. Skinner helped negotiate increased funding for the Joseph P. Dwyer Peer-to-Peer Veterans’ Support Program with state government during his time as director. His direct advocacy for life-saving peer-topeer support resources resulted in 150 percent more revenue allocated to each county Veteran Service Agency in New York.

Skinner is a veteran of the U.S. Army, which he joined in 2003. He served with the 3rd Squadron of the 17th Cavalry in Iraq, achieving the rank of corporal before his honorable discharge from the armed services. Skinner was recently inducted into the New York State Senate Veteran’s Hall of Fame for his efforts on behalf of the state’s veteran population. Beyond his work with the County, Skinner also serves as a New York State County Service Officer Association President, National Association of Counties Veterans and Military Families Vice Chairman, member of the National Association of County Veterans Service Officers, and Finger Lakes Veterans Advocacy Council President. Livingston County has named Deputy Director of Veteran Services David Terry as the department’s Interim Director. Skinner offered words of high praise for Terry’s work with Veteran Services. “Without David Terry none of these achievements would have been possible,” said Skinner. “In many cases, David was the backbone of the office’s operations.” For more information on the Livingston County Veteran’s Services department, please visit https://www.livingstoncounty. us/275/Veterans-Services. PAW’S CORNER By Sam Mazzotta

AUTO SECTION

Summer Heat Can be Deadly to Pets DEAR PAW’S CORNER: Yesterday, our beloved corgi mix, Shemp, was laid to rest. I feel so guilty. He passed from heatstroke after being out in the backyard for just one hour. I never realized that just the heat outdoors could be deadly to dogs. I always was conscientious about dangerous situations; I never once left Shemp in a hot car or walked him on hot sidewalks. Yet a few minutes of inattention, and my Shemp was suddenly in great distress. I rushed him to the veterinarian, but he just didn’t make it. Please warn your readers to keep their dogs inside on hot days and to monitor them closely for signs of heat stress. -- Devastated in Houston Dear Devastated: I am so sorry for the loss of your beloved Shemp. Thank you for warning other readers of the dangers of summer heat. Being outdoors during the extreme temperatures that we’ve experienced this summer, often passing 100 degrees in many states, is dangerous for dogs and other pets. Here are a few ways to keep them safe from heat injury. -- Walk dogs before sunrise and after sunset. Under a hot sun, sidewalks can quickly burn a dog’s paws, and heat and humidity can take a toll on their health. -- Do not leave pets outside unsupervised. Even if they’re just in the backyard, you might not notice their distress until it’s too late. -- Provide a shelter from the sun. A shelter with open sides and a solid top allows airflow and protects from the sun when your dog is outside. -- Make water available all day, everywhere. Carry water on walks, keep their indoor bowl full and provide a water bowl outside. (c) 2022 King Features Synd., Inc. AUTOMOTIVE

IMPROVE ALERTNESS DURING THE WORKDAY Avoid high-fat foods at lunchtime. Foods that are high in fat should always be avoided thanks to their connection to a host of health problems. Such foods also negatively affect energy levels when consumed in the middle of the day. The University of Rochester Medical Center notes that the body digests and absorbs high-fat foods very slowly. That means workers who eat highfat foods for lunch won’t get the afternoon energy boost that lowfat, healthy lunches will provide.
