TO YOUR GOOD HEALTH By Keith Roach, M.D.
Diabetes Not Limited to Only 2 Main Types --DEAR DR. ROACH: Last year, at 57, I was diagnosed with Type 1 LADA diabetes. Initially I was put on insulin to get my blood sugar within the normal range. Then I was just on metformin until the “honeymoon period” was over. My endocrinologist is great. I now am back on insulin (six units at night). I walk anywhere between 3 and 8 miles about three times a day. My blood pressure is excellent. I initially lost about 18 pounds after diagnosis, so my weight is good. When looking at Google, it says my life expectancy has been cut by 20 years! Yikes, I JUST retired and if I go by their gauge, I’ll be passing around age 68! Is this based on someone who has had diabetes for many years, since Type 1 is usually diagnosed much earlier? The doctor said even though my initial sugars were over 500 when diagnosed, I was very healthy and that’s why nothing was “destroyed.” My vision was off for about a month but no permanent damage was done. Any reassurance would be appreciated! -- M.W. ANSWER: Most people are familiar with Type 1 diabetes, with a typical onset in childhood and at a normal weight. It is caused by an autoimmune destruction of the insulin-making cells in the pancreas. People also know about Type 2 diabetes, usually adultonset and usually diagnosed in someone who’s overweight; it’s caused by resistance to insulin. But there are other subtypes of diabetes, including latent autoimmune diabetes in adults (LADA). In the U.S. and Canada, it accounts for a small fraction of diabetes cases, but is more common in Scandinavian countries. It has some characteristics of both Type 1 and Type 2 diabetes, but the autoimmune nature and the usual progression to insulin need makes most experts feel it is closer to Type 1 than to Type 2. A recent paper has outlined a potential new classification of diabetes types. The change in life expectancy from diabetes relates to how long you have had diabetes and how well-controlled it has been. With outstanding diabetes control (normal or near-normal blood sugar levels), the effect on overall mortality is modest, at best. In one model, the reduction in life expectancy is less than a year. However, it certainly is possible to have life expectancy drop by 20 years with very poorly controlled, longstanding diabetes. For people with diabetes, to a very large extent, their potential to live longer (and healthier) can be improved by better blood sugar control. For new-onset autoimmune diabetes (both Type 1 and LADA), early and aggressive blood sugar control with insulin may reduce the amount of autoimmune damage and improve long-term prognosis. Studies are in progress to evaluate this. *** DEAR DR. ROACH: I am a female, 16 years old. My white blood cell count is 16.6. I am suffering from so much weakness and pain in my legs, arms and shoulder area. I also have a cough and cold. Is it dangerous? What should I do for it? -- K.C. ANSWER: I don’t have enough information to help much. A white blood cell count that high suggests an infection. At your age, those symptoms are most likely a viral infection, but acute mononucleosis and even pneumonia are possible, so you need to see your doctor. *** Dr. Roach regrets that he is unable to answer individual questions, but will incorporate them in the column whenever possible. PROTECTIVE FACTORS FOR BREAST CANCER Decreasing the length of time a woman’s breast tissue is exposed to estrogen may help prevent breast cancer. Exposure to estrogen is reduced in the following ways: • Early pregnancy: Estrogen levels are lower during pregnancy. Women who have a full-term pregnancy before age 20 have a lower risk of breast cancer than women who have not had children or who give birth to their first child after age 35. • Breast-feeding: Estrogen levels may remain lower while a woman is breast-feeding. Women who breastfed have a lower risk of breast cancer than women who have had children but did not breastfeed.

DID YOU KNOW? According to Let’s Move!, a comprehensive initiative launched by First Lady Michelle Obama to combat childhood obesity, doctors are concerned that the rise of obesity in children and teens may lead to problems such as heart disease, type 2 diabetes, asthma, and sleep apnea as they age. Children who are obese may have high cholesterol, high blood pressure and an abnormal glucose tolerance, increasing their risk factors for cardiovascular disease. But poor physical health is not the only concern parents should have if their children are obese. Overweight and obese children are often targets of social discrimination, which can negatively affect their selfesteem and may even hinder their academic and social functioning. Parents concerned about their children’s weight should consult their children’s physicians and recognize that a combination of daily exercise and a healthy diet is the most effective way to help kids lose weight and maintain a healthy weight into adulthood.

Rake leaves as they start to fall more heavily. Once leaves begin to fall more heavily, rake them up and add them to compost piles. The resource GardeningKnowHow.com notes composting leaves creates a dark, rich and organic matter that can add nutrients to garden soil and loosen compacted earth. Leaving leaves on the lawn once they start to fall in great numbers makes it hard for grass blades to breathe, and the leaves can block moisture from reaching the soil, which needs water to maintain strong roots. In addition, potentially harmful pathogens can breed on damp leaves left on a lawn, and such bacteria can cause significant damage to the turf over time.

MAKE FALL CLEAN-UP ECO-FRIENDLY Rake leaves sensibly Power blowers and mulchers make fast work of leaves, but at the expense of the environment. Gas-powered devices discharge fuel exhaust into the air, contributing to greenhouse gas emissions. Instead, reap the benefits of peace and quiet as well as exercise from manual raking. While raking, do not feel the need to remove every leaf. A certain amount of organic matter can be beneficial to the soil and even insulate the lawn somewhat. A leftover leaf here or there also may provide protection for insects and small animals. For example, many native bees spend the winter in tunnels in the ground, cracks in mortar, holes in dead wood, or within hollow stems, and they need lawn and garden litter to survive.

Long-Distance Move With Cats in Pickup DEAR PAW’S CORNER: I have three rescue cats. They’re the only family I’ve got. I’m planning to relocate to a rural area that is several days’ drive away. I’ll be driving a pickup truck. I feel that making my cats travel with me wouldn’t be humane. Is there an agency available that could humanely and safely transport my cats? -Karl in Avon, New York DEAR KARL: I can totally relate! Years ago, I moved across the country with a very nervous cat in a carrier next to me on the bench seat of my pickup truck. Even though I made sure he had water, food and anxiety meds prescribed by the veterinarian, it was a stressful trip for him, and he did not eat or even poop for the whole two-day trip. Once I reached our new home, he returned to his usual snuggly self. Traveling with all three cats is possible with planning and may be the least stressful option for them. If you have an extended-cab pickup, make room in the back seat for three cat carriers. Put a litter pan on the floor and cover with plastic wrap while traveling; at rest stops, open one carrier at a time and let one cat out to use the litter pan and to snuggle with you (don’t open the vehicle doors while you do this). Look for pet-friendly hotels that will let you keep the cats in the room. Airline cargo transport can quickly transport pets across long distances. But there is a risk of death for pets traveling in airline cargo. Another option is private pet transport. If you choose this service, make sure they are insured and bonded, and that they will stay in contact with you throughout the trip.

Dansville Phone: 585-335-8537 A hidden fire burns perpetually upon the hearth of the world.... In autumn this great conflagration becomes especially manifest. Then the flame that is slowly and mysteriously consuming every green thing bursts into vivid radiance. Every blade of grass and every leaf in the woodlands is cast into the great oven of Nature; and the bright colours of their fading are literally the flames of their consuming. The golden harvest-fields are glowing in the heart of the furnace.... By this autumn fire God every year purges the floor of nature. All effete substances that have served their purpose in the old form are burnt up. Everywhere God makes sweet and clean the earth with fire.

OCTOBER 2020 For the Children School is back in session! Ms Mary Ellen is planning events in which families can be physically distant but socially involved. Preschoolers can sign up for 1000 Books Before Kindergarten or continue reporting on their reading goals at the Library, or families can contact Ms Mary Ellen directly for additional 1000B4K information. More Facebook stories with Ms Mary Ellen are in the planning. And we know Halloween is on the way! We invite parents to stop in with their children to enjoy a “costume moment” with us. Fall is here, change is in the air, and our Children’s Room is currently open from 10 a.m. to 5 p.m., Monday through Friday. TAKE NOTE: if all remains the same, in about two weeks, the Library will expand its modified services by adding Saturday hours. We know your schedule may not make a visit between 10-5 easy. We’re anxious to provide you with opportunities to stop in when your schedule permits. Library Open for Business Our services and hours are still limited, but we are listening to you, and we find that more people are comfortable coming into the Dansville Public Library in person. Please note: you do have choices! We will continue to provide pick up service and bring materials to your vehicle. We will provide home delivery at your request. Because our doors are open, we do ask that you sanitize your hands on the way inside, always wear your mask covering both nose and mouth, and physically distance when you are inside. We welcome returns in the book drop 24 hours a day. We are here to help you place holds, or to teach you how to place holds yourself when you return home. Two internet computers are available, and you can sign up for small meeting rooms for one or two people by calling us at 585-335-6720. Please call us!
Check Outs & Returns & Due Dates Overdue charges will eventually start appearing if you keep materials past their automatic renewal dates. During the early days of COVID-19, the Pioneer Library System adjusted due dates on items stranded in homes to September 30. That date has come and gone! Do you have pre-COVID checkouts? Please bring them back as soon as you can. We know some materials came from other libraries and we would like to send them “home.” And we would welcome our own library’s materials so we can check them out to a new borrower. Thank you for your help! Fore the Library Golf Tournament We’re so pleased to announce the success of Dansville Lions Club’s “Fore” the Library Golf Tournament! Held Saturday, September 19 at Brae Burn Golf Course. We know that the Lions’ donation to the library will be more than $3000! We want to express our thanks to all Lions and especially to committee members Dan Beiswenger, Ashley Standish, Dave Mapes, Dan Edmond, and Dansville Library rep to the committee, Mary Ann Holden. We’re also grateful for staffers Mary Ellen Marena and Lacey Keagy who were behind the scenes preparing the mailing lists and labels. The New Story Stroll in October Ms Mary Ellen is already working on the next Library Lawn Story Stroll expected to appear in a couple weeks. It’s a well-known folk tale, about a very famous little girl and her adventures with a four-legged forest-dwelling animal. Can you guess her name? Keep watching for her!

SENIOR NEWS LINE By Matilda Charles Now More Than Ever, Get Your Flu Shot A sincere wish just came true: This year's high-dose senior version of the flu shot will contain all four vaccines. In past years, the senior shot (for those over age 65) contained only three of the vaccines, two A and one B. Most years I would write a snarly note to the Centers for Disease Control and Prevention (CDC) and ask about the reason for the false economy. Of course I never got a reply, but now ... finally, we seniors will have a flu shot with all four vaccines. And what a shot it will be. The four vaccines are names we haven't heard in a while, if ever: A/Guangdong-Maonan (H1N1), A/ Hong Kong (H3N2), B/Washington and B/Phuket. To help us kick start our immune system, the senior version will contain four times the amount that younger folks get. The standard trivalent (threeway) will have two A and one B, while the standard quadrivalent (four-way) has all four. If your doctor says you should have a flu shot, don't delay. When I called my pharmacy (where I get my flu shots each year), the first appointment was many days out, and they had been swamped with requests for the shot. Yes, they will be resupplied when they run out, but no, they didn't know how quickly that would be. By getting a flu shot, should you get sick later this season, it will be a good bet that it's not the regular flu, which will save much time in getting a diagnosis. To find flu shots, call your doctor, local pharmacies, grocery stores or go to vaccinefinder.org. If you have a Medicare or insurance card, you won't have to pay anything. If you want to know more, check the CDC website: www.cdc.gov/ flu/season/faq-flu-season-2020-2021.htm.

People of Action Give Dansville’s Greenmount Cemetery a Little TLC BY AMANDA DUDEK
D
ansville’s Greenmount Cemetery had fallen into disrepair. The grass and other plants were overgrown, and its paths and gravesites were not well kept. Those visiting there, sad to see the state of their loved one’s final resting place, did not know where to turn for help. A few reached out on social media, sharing their experiences and this need with the community. Little did they know their words would inspire one of their own neighbors to take action. Clyde Green lives in Dansville. He attends church there, playing the drums in its praise and worship band and organizing the church’s youth group, along with other efforts. When Green saw the posts about Greenmount Cemetery, he knew he had to do something. “We have a heart and a love for people and for our community,” he explained. “I saw these posts, and no one was taking any initiative. I felt led to reach out to the community and see if we could come together in unity to make a change and clean it up.” The response he received was amazing. “It was so overwhelming because a lot of people have loved ones buried up there, as well,” Green recalled. “People started jumping on board, showing up or making donations since they couldn’t be there. It really came together very well.” Green, along with a good size group of volunteers went up to Greenmount Cemetery, and with focus, hard work and determination, they transformed a large part of the grounds. “We put a really big dent in the landscaping and cleanup part of it,” shared Green. “Now, it looks very good because volunteers took action to make change.” And not just volunteers – those who donated also played a crucial role in making the cleanup a success. “Not only did people show up with push mowers, riding mowers, weed eaters and other equipment to donate their time and effort,” said Green. “But also, those who could not attend donated items like bags of ice, cases of water, Gatorade, money for food, sheet pizzas from Tops and 100 cheeseburgers, 100 nuggets and 6 bags of ice from McDonalds!”
Greenmount Cemetery once again has neatly mowed lawns thanks to People of Action!
and we hear about it, I will get ahold of the community once again to see if anyone is willing to come together and meet that need,” Green explained. He hopes people will take advantage of serving others when it is simply a matter of showing up. “Our lives are so busy with everything that is going on,” he said. “People want to step up and contribute, but they may not have that balance of time and ideas. They would like to do something, but they don’t know what. If something is put into place where someone else take the initiative and handles the logistics, all they have to do is jump in behind a good cause.” While he led the efforts, Green is quick to point out that many deserve recognition for the cleanup’s success. “Without a team, I could not do all of this,” he said. “I would like to thank my wife, Casey Green, my parents, Clyde Green Sr. and Barbara Green, Pastors Badru and Regina Brewer, Judy Ball, Angela Winans, Dansville Strong and Dansville Staying Strong, groups that deserve recognition, as well.” In speaking of his team, Green was reminded of their fundraiser for friend Heather who is battling stage 4 breast cancer. “There, too, the community came together in unity, buying hot dogs and donating cash,” he said. “It blows me away, and it is so beautiful to see that. My faith is the reason I am doing this, and I truly have a heart for people. I just have that genuine love to help people and encourage them to do better.” Green has also launched a new youth group in Dansville. “I want to give the youth in our community something positive to do,” he said. “A place where they can put their positive ideas for serving the community into action.” Meetings start every Saturday at 10am at 6 West Liberty Street in Dansville.
Clyde Green (not pictured) and People of Action volunteers gather to serve their community at Dansville’s Greenmount Cemetery!
For Green, serving the community brings its own satisfaction. “I am very grateful and humbled by the experience,” he said. “To see the community come together, even if they couldn’t be there, was beautiful. We received so much positive feedback. It was very rewarding. Both the people in our community and those who showed up to do the work asked if we were going to come up again and make this a regular thing.” The answer, of course, is yes. “They are willing,” Green said. “The desire is there to step up and take action, and I highly appreciate that.” This group of hardworking volunteers gave themselves a name: People of Action. While they do not have any immediate projects planned, People of Action hope to be ready as needed. “As a need arises in our community

How do I get rid of bishop’s weed? It invaded a bed of lily of the valley and I donít want to kill that as well. - Kathleen C. Bishop’s weed has an incredibly dense root system which is quite difficult to eradicate. You would need to remove the lily of the valley plants, then smother the entire bed for at least six growing months; a year would be more effective. The ground is frozen during December,
January, February, and March, so plants are dormant, not growing. Before replanting your lily of the valley, completely remove all current soil from the roots by soaking and using a forceful hose. Cover the area with landscape fabric or 6 layers of newspaper with overlapping edges, then top with several inches of mulch. Secure the edges with stones or branches. The newspapers will gradually disintegrate, but you would need to pull out the landscape fabric. If you don’t remove the fabric, it will
gradually break down in to smaller pieces, become mixed into the soil, and impede root growth. Watch the edges of this area during growing months, and spray any bishop’s weed plants that emerge with herbicide.

A VERY SUCCESSFUL 2020 SUMMER GAZEBO CONCERT SERIES! The Riga Recreation Department just concluded its very successful 2020 Summer Gazebo Concert Series at the Village of Churchville Gazebo. In the tine of this health crisis, we were able to provide the community with some muchneeded relief. Many, many thanks to the following organizations that made all of this possible in these times of uncertainty: NYS Senator Michael Ranzenhofer’s Office, the Town of Riga, the Village of Churchville, the Churchville Lion’s Club and the following musical performers: Curbside Pickup, Blue Sky, Johnny Bauer, 8 Days a Week,
and Generations. -DaveThomas, Riga Recreation Supervisor ABOVE AND BEYOND! I want to give a huge thank you to all delivery people for making sure my parcels and mail arrive. A special thank you goes to our local FedEx. I order from Chewy frequently, and some of the items are excessively heavy. I have a pet that has allergies, and I am finding it difficult right now finding unscented products along with special animal food in our local stores. FedEx has no idea why I order what I do, but I wanted to let them know that they do an excellent job, and I am very pleased with their work ethic. THANK YOU I would like to
thank all of the wonderful friends that have helped us through the passing of our son JP. Thank you to all who came to the wake as JP would have loved this. Also, thank you to all who have stopped by to give condolences, brought food, flowers and cards, and donations to GG’s future education and for checking on us. Thank you to the Ontario County Sheriff’s Department for their help. We can’t get by without you. Thank you to Pastor Colin Pritchard for staying with Jessica the night JP passed away and for the wonderful words at the wake. We are so thankful for you coming. We feel blessed to know that JP was loved by so many. We miss you JP. Love ya all, Margo,
MUSIC A HUGE PROBLEM? Wow! You use such strong adjectives to describe people who post their views, words such as “attacked” and “ganging up” on the person who has a complaint about neighbor and their music. I read the post as did many others, I’m sure, and there was no such thing. You may not like or agree with the suggestions given, but that does not add up to ganging up on or attacking someone. It was just practical advice. It’s clear to see how someone’s playing music can become so distorted. I think the complaining neighbor has more problems than just the neighbor’s music playing. They seem to have such distaste for them it’s a shame. How sad. With all the woes in the world, it’s a pity to make such a big thing over music. How long has this been going on that, all of a
sudden, it is a huge problem? MUSIC TROUBLE So sorry about neighbor being bothered by loud music. How long have you been suffering? Are you the only neighbor having a problem with loud music so loud that it rattles your house? Maybe you should try to nicely explain to your neighbor how bothersome the music is to you? Have you given that a thought? Maybe do it over a cup of coffee? That always brings the tension down, just a few suggestions. Also, ear plugs can help somewhat. There are codes where you can’t play music late at night or too early in the morning. Don’t know if that is when the music is being played. It’s always good to get along with neighbors. Hopefully, you can get some much-needed relief. Kindness does go a long way. I have been blessed with
neighbors who are kind and considerate. RESPECT OTHERS WITH MUSIC The nature of character is not based on self-entitlement but of respect for other’s right to peace and quiet. This must be a consideration, especially when residing in close communities or apartment complexes. We should not have to suffer through someone else’s loud music or faulty exhaust systems, etc. Also, taste in music is variable. One person’s treasure is another person’s torment. Peace and quiet is not trivial. It is of precious value. Remember the quiet individual doesn’t annoy the loud one; it’s vice versa. So, who should move? MUSIC & GETTING ALONG If you live in an apartment complex and a neighbor is blaring their music, then you

Bobby, Jessica, Anthony Emme and GG MY HEARTFELT THANKS I want to publicly thank the concerned motorist in the gray pickup truck who encountered me on North Main Street in the early morning hours on Monday, September 21st. He had purchased for me a reflective vest to wear as I exercise on the street’s shoulder. I was not aware that I was difficult to see since I wear a light-colored jacket and thought it was visible when headlights struck it. Thank you for your kindness and your personal concern for my safety. I was surprised and do feel safer wearing it!
should speak to management about it. It’s a different story if you live in a neighborhood with a lot of space in between homes and properties. Everyone has the right to enjoy their life, but unfortunately, we will not find a place where there is total peace and quiet. The bigger picture is getting along with one another, especially your neighbors. I know of someone who has a neighbor who does nothing but curse, I would take music, any day, over that.

Most patients with breast cancer have surgery to remove the cancer. Sentinel lymph node biopsy is the removal of the sentinel lymph node during surgery. The sentinel lymph node is the first lymph node to receive lymphatic drainage from a tumor. It is the first lymph node where the cancer is likely to spread. A radioactive substance and/or blue dye is injected near the tumor. The substance or dye flows through the lymph ducts to the lymph nodes. The first lymph node to receive the substance or dye is removed. A pathologist views the tissue under a microscope to look for cancer cells. After the sentinel lymph node biopsy, the surgeon removes the tumor using breast-conserving surgery or mastectomy. If cancer cells were not found in the sentinel lymph node, it may not be necessary to remove more lymph nodes. If cancer cells were found, more lymph nodes will be removed through a separate incision. This is called a lymph node dissection. Types of surgery include the following: • Breast-conserving surgery is an operation to remove the cancer and some normal tissue around it, but not the breast itself. Part of the chest wall lining may also be removed if the cancer is near it. This type of surgery may also be called lumpectomy, partial mastectomy, segmental mastectomy, quadrantectomy, or breast-sparing surgery. • Total mastectomy: Surgery to remove the whole breast that has cancer. This procedure is also called a simple mastectomy. Some of the lymph nodes under the arm may be removed and checked for cancer. This may be done at the same time as the breast surgery or after. This is done through a separate incision. • Modified radical mastectomy: Surgery to remove the whole breast that has cancer, many of the lymph nodes under the arm, the lining over the chest muscles, and sometimes, part of the chest wall muscles. Chemotherapy may be given before surgery to remove the tumor. When given before surgery, chemotherapy will shrink the tumor and reduce the amount of tissue that needs to be removed during surgery. Treatment given before surgery is called preoperative therapy or neoadjuvant therapy. Even if the doctor removes all the cancer that can be seen at the time of the surgery, some patients may be given radiation therapy, chemotherapy, or hormone therapy after surgery, to kill any cancer cells that are left. Treatment given after the surgery, to lower the risk that the cancer will come back, is called postoperative therapy or adjuvant therapy. If a patient is going to have a mastectomy, breast reconstruction (surgery to rebuild a breast’s shape after a mastectomy) may be considered. Breast reconstruction may be done at the time of the mastectomy or at some time after. The reconstructed breast may be made with the patient’s own (nonbreast) tissue or by using implants filled with saline or silicone gel.
