DID YOU KNOW? According to the Insurance Institute for Highway Safety and the Highway Loss Data Institute, roughly 10 percent of all highway deaths involve large trucks. Braking capability can play a role in such deaths, as the IIHS notes that loaded tractortrailers take between 20 and 40 percent farther than cars to fully stop. That discrepancy is even greater on poorly maintained roads or roads marked by adverse conditions such as ice, rain and/ or snow. While 16 percent of the roughly 3,600 people who died in large truck crashes in the United States in 2014 were truck occupants, more than four times that many people (68 percent) were occupants of cars and other passenger vehicles. That’s because large trucks typically weigh 20 to 30 times more than passenger vehicles, meaning occupants in cars and passenger trucks are incredibly susceptible to serious injury or death should they get in accidents with tractor-trailers and other large trucks. Time of day also must be considered when examining large truck crashes and fatalities. While 17 percent of deaths in 2014 took place between the hours of noon and 3 p.m., that figure dropped to 11 percent between the typical rush hours of 6 p.m. and 9 p.m.

Anyone with diabetes can develop diabetic eye disease, though some groups are more affected than others. According to the National Institute of Diabetes and Digestive and Kidney Diseases, African Americans, American Indians and Alaska Natives, Hispanics/Latinos, Pacific Islanders, and older adults are at an elevated risk of diabetes-related vision loss or blindness. It’s also important that pregnant women with diabetes recognize the risk of eye problems. The NIDDK reports that women with diabetes can develop eye problems very quickly during pregnancy. Women who have already been diagnosed with diabetic retinopathy, the most commonly diagnosed diabetic eye disease, may have their condition worsen during pregnancy. The link between diabetic eye disease and pregnancy has to do with the changes a woman’s body undergoes when she is with child. The NIDDK notes that changes the body undergoes to support a growing baby can put stress on the blood vessels in the eyes. Damaged blood vessels can harm the retina, thereby increasing a woman’s risk for diabetic retinopathy. Individuals with diabetes who are at a heightened risk for diabetic eye disease, including pregnant women, can speak to their physicians about what they can do to protect their vision in the short- and long-term.

Data from the United States Department of Justice indicates that partner violence accounted for 20 percent of all violent crime in the U.S. in 2018. The World Health Organization defines intimate partner violence as behavior within an intimate relationship that causes physical, sexual or psychological harm. Such violence can include acts of physical aggression, sexual coercion, psychological abuse, and controlling behaviors. Violence perpetrated by both current and former spouses and partners falls under the umbrella of intimate partner violence. Domestic violence is so prevalent that the National Network to End Domestic Violence reports that it fields more than 19,000 calls on a typical day. Social Security Gets Big Benefits Bump The news is in. Our Social Security benefit for 2022 will increase by 5.9%, slightly less than the 6.1% estimate the government put out this summer. Still, that’s much greater than the 1.3% increase we saw for 2021 and the 1.6% the year before. The average monthly dollar increase will be $92, making the average benefit $1,657 per month for a single person. For a couple, a $154 increase would come to $2,753 per month. The official mail on the cost-of-living adjustment will come out in December. I’ve done my calculations for next year, and once again I shake my head. We’ve been notified already that I’ll have a rent increase. Heating fuel is likely to go up 47%. We all know what has happened at the grocery store. Medicare Part B is sure to go up, maybe a $10 per month increase. There is one worrying note, and we need to think about how it will play out for us individually. The question is: Will that increase move us into a higher tax bracket? It depends on our provisional income. That number is any income (pension, wages, dividends and more) not from Social Security, plus half the amount of Social Security. Those of us who come in under $25,000 won’t have a federal tax put on our Social Security. If that number is between $25,000 and $34,000, there will be a tax on half of our Social Security income. Over that and it goes up to a tax on 85% of our Social Security income. If your situation for 2022 is likely to put you in the category of having to pay taxes on your Social Security benefit, you can elect to have them take money out of your check each month. DEAR DR. ROACH: I read your recent column on pulmonary fibrosis. I especially appreciate knowing that there are medications known to slow the progression of lung disease, reduce exacerbations and reduce mortality. With a 20-year-old diagnosis of COPD and having been prescribed medications to reduce exacerbations, you might imagine my interest in your article. I’m wondering how pulmonary fibrosis differs from COPD and if the two medications that you mention (pirfenidone and nintedanib) might be helpful in slowing the progression of lung disease in patients such as myself. Have any studies been completed using these medications on patients with COPD? -- S.A. ANSWER: Chronic obstructive pulmonary disease -- its two main forms are emphysema and chronic bronchitis -- usually, but not invariably, is a result of long-term exposure to lung toxins, especially smoke. In the most common case of COPD, due to cigarette smoking (at least, that’s the most common in North America and Europe: cooking fires are still a common cause in less developed countries), stopping the exposure will dramatically slow down further damage. Unfortunately, there are no established treatments that can restore lung function in people with moderate to advanced COPD. Pulmonary fibrosis is, by contrast, a rare disease; about 30,000 people in the U.S. are diagnosed each year (compare that with the 9 million people in the U.S. diagnosed with chronic bronchitis last year). The exact mechanism of action of pirfenidone and nintedanib is not known, but they are not thought to be effective in COPD. Surprisingly, I did not find a published trial looking at whether these drugs might be effective. The need for new therapies to treat COPD is so great that I would have thought some researcher might have tried it, despite the long odds.

SOMEONE YOU LOVE IS LIVING WITH DEMENTIA—NOW WHAT? PART II BY NANCY SMYTH Helping caregivers find peace-of-mind. Understandably, several anxieties are commonly experienced by new residents and their families. Quality care staff will respond with compassion and flexibility to ease the transition to care while safeguarding your loved one’s pride and dignity. They should also carefully evaluate each resident’s reactions every step of the transition, developing a care plan as the move progresses. Every resident and situation is unique; however, the following guidelines can help create a successful “moving day” for you and your loved one. Don’t introduce the move too soon, or too often. Depending on the level of dementia involved, discussing the impending move may produce confusion and anxiety for your loved one when you broach the subject. Here are some additional strategies you can and should utilize to ensure the best transition possible: • Solicit your physician’s help. Many people accept the professional advice of a physician more easily than the personal opinions of family members. • Ask your physician to prescribe the move to an assisted living residence based on the individual’s health care needs. • Ask the physician to discuss the recommendation and the reasons with both you and your loved one present. If your loved one again raises objections to the move, you can refer back to the doctor’s advice. •
Consult your physician regarding appropriate medication.
• Take it step-by-step and remember, this is a unique process for each family and Elder. Consider easing the transition from your loved one’s home to a new home by introducing the facility and services in stages: • Consider bringing your loved one for “just a visit”—enjoy lunch and an activity, then go back home.
Bring familiar and personally important items from home. When your loved ones have their own rooms, this not only allows for privacy, but provides the opportunity to truly individualize one’s living space. Furniture, artwork, photographs, and personal mementos help create an atmosphere that affirms the individual’s past and present and helps reassure the resident. Please understand that change is difficult for everyone involved. It is not uncommon for family members to experience a multitude of unexpected, even conflicting emotions when the move to a facility becomes real. If you have been the primary caregiver, you may experience everything from sadness and anger to guilt and relief. These feelings are all part of a normal grieving process. This is a major change for the entire family, and even members who have not expressed interest in the planning stages may suddenly become involved once the decision is made. Not only the timing of the move, but the location of the facility, payment arrangements, Power of Attorney and Health Care Proxy issues can all come into question at this time. Staff should meet with any friends and family significant to the new resident to help facilitate appropriate, informed decisions.
• Consider a short-term respite stay to give the Elder the opportunity to make the decision about the move.
Helping the new resident adjust to staff as caregivers:
• Give positive cues. Although people living with dementia may have difficulty interpreting and communicating with words, they tend to be sensitive to emotional cues.
For some, it is appropriate for the family to arrange significant visiting time during the first few days; others find it is best to allow time for the resident to transfer dependence and trust to the new caregivers. Each move is different, and staff should collaborate with you in evaluating what appears to ease your loved one into the new surroundings.
Creating a Successful Transition: Even with careful planning, extra attention and reassurance, the combination of dementia and a major life change can temporarily produce an even greater level of anxiety and agitation for new residents. Many older people accumulate a variety of prescribed medications for age-related disorders which may contribute to panic, confusion, restlessness, and insomnia-all of which could add to the new resident’s anxiety. If your loved one seems overly anxious about the move, ask your physician to review his/her current medications and consider additional medication to ease symptoms for the initial transition period. Consider a Companion or Personal Aide:
Share with the staff. Prior to admission, each family is asked to complete a social history. From this history, staff can appreciate everyday preferences as well as introduce familiar names, places and occupations into their daily interactions. This social history can be especially helpful during the initial move to understand what will calm and comfort the new resident. At any point, families are encouraged to contact the staff with additional information or concerns. Search Engine Land, which covers digital advertising technology, reports 88 percent of consumers trust online reviews as much as personal recommendations. Unfortunately for businesses, Inc. magazine and entrepreneur Andrew Thomas found that a customer who has a negative business experience is highly likely to share that experience by leaving a bad review. On the other hand, someone who has had a positive experience is unlikely to leave a good review. Roughly 10 percent of happy customers say they were pleased by a product or service. Furthermore, Thomas indicates it takes roughly 40 positive customer experiences to undo the damage from a single negative review. Customer happiness should be the goal of businesses, with the added benefit of helping to generate more positive reviews and word-of-mouth recommendations. While that is happening, customers can think twice about online reviews.

Speak to product quality, especially when you have a very good experience with an item. Many retailers request feedback via email. Take advantage of this opportunity to offer positive feedback. Companies gather personal information from customers so they can follow up in many cases. Great customer service helps prevent bad reviews and defines companies’ brands. If you are asked for a phone number or email address, provide it. Doing so helps companies improve their products or services. Happy customers can reach out to friends and family members to share their experiences with certain companies. Posting your own review on social media also can drum up business for a company you respect. Customers should weigh their words heavily and think before posting a review. The marketing and consulting resource The Chat Shop says 31 percent of customers are interested in grocery store reviews, 60 percent are likely to read restaurant reviews and 70 percent rely on technology product reviews. Make an effort to discuss issues with a company before leaving a negative review. You may be pleasantly surprised with the outcome. There’s no need to hesitate when leaving positive reviews. Simply explain why you were pleased. Reviews can make or break a company. Both business owners and customers have a role to play in making online reviews a valuable way to learn about a company.
