A 7-year-old boy had his lemonade stand shut down by a New York State health inspector last summer. This year, a bill was introduced in Albany to exempt stands run by children 16 and younger from needing a state permit. Would this be a good law?

HOROSCOPE ARIES (March 21 to April 19) An unexpected development could change the Arian’s perspective on a potential investment. Keep an open mind. Ignore the double talk and act only on the facts. TAURUS (April 20 to May 20) A surge of support helps you keep your long-standing commitment to colleagues who rely on you for guidance. Ignore any attempts to get you to ease up on your efforts. GEMINI (May 21 to June 20) Family continues to be the dominant factor, but career matters also take on new importance. You might even be able to combine elements of the two in some surprising, productive way. CANCER (June 21 to July 22) A realistic view of a workplace or personal situation helps you deal with it more constructively once you know where the truth lies. Reserve the weekend for someone special. LEO (July 23 to August 22) As much as you Leos or Leonas might be intrigued by the “sunny” prospects touted for a potential investment, be careful that you don’t allow the glare to blind you to its essential details. VIRGO (August 23 to September 22) A friend’s problem brings out the Virgo’s nurturing nature in full force. However, don’t go it alone. Allow others to pitch in and help share the responsibilities you’ve assumed. LIBRA (September 23 to October 22) A business decision might need to be put off until a colleague’s personal matter is resolved. Use this time to work on another business matter that you’ve been anxious to get to. SCORPIO (October 23 to November 21) Relationships (personal or professional) might appear to be stalled because of details that keep cropping up and that need tending to. Be patient. A path begins to clear soon. SAGITTARIUS (November 22 to December 21) A promotion could cause resentment among envious colleagues. But others recognize how hard you worked to earn it, and will be there to support you if you need them. CAPRICORN (December 22 to January 19) Handling a delicate personal matter needs both your wisdom and your warmth. Expect some setbacks, but stay with it. The outcome will more than justify your efforts. AQUARIUS (January 20 to February 18) Resist the temptation to cut corners just because time is short. Best to move ahead step by step so you don’t overlook anything that might later create time-wasting complications. PISCES (February 19 to March 20) Use the good will you recently earned with that wellreceived project to pitch your ideas for a new project. Expect some tough competition, though, from an unlikely source. BORN THIS WEEK: Your love of family extends beyond your personal life to include others to whom you generously extend your care and affection.

WHAT CAUSES DEPRESSION? Although the cause of depression remains a mystery, certain distinguishing factors are common among those who have the condition. People with clinical depression appear to have physical changes in their brains. In addition, naturally occurring brain chemicals known as neurotransmitters likely play a role in depression. The Mayo Clinic states changes in the function and effect of these neurotransmitters and how they interact with neurocircuits involved in maintaining mood stability may play a significant role in depression and its treatment. In addition to biological factors in the brain, hormones can impact rates of depression or even trigger it. These hormone shifts may be a leading reason why women have higher incidence rates of depression than men. Depression is more common in people whose blood relatives also have the condition. Therefore, those with a family history of depression may be more likely to get it than those without such a connection.

about sebaceous hyperplasia. I read that it is quite similar to acne and that a person could use an acne treatment to keep the condition in check. Could you give your thoughts on this? -- R.G. ANSWER: Although sebaceous hyperplasia can be mistaken for acne, sebaceous hyperplasia happens mostly to middle-aged adults (or newborns). These bumps appear most commonly on the forehead and cheeks, and have an umbilicus (a small hole) in the center. They are enlargements of the oil-producing sebaceous glands. They happen particularly in people with oily skin. Treatment can be challenging. Reducing dietary fat and using a good-quality skin-care regimen for oily skin may help. Dermatologists can treat these when they are cosmetically important, not because they cause harm in themselves. Although they can be mistaken for basal cell cancers of the skin, they are not cancerous or pre-cancerous. They are mostly treated mechanically, and by that I mean using a surgical technique like cauterization or excision. But these techniques can leave scars. Laser and phototherapy have good cosmetic results but are expensive. Isotretoin, a systemic acne drug with significant side effects, also has been shown in a small study to be effective. ***
DEAR DR. ROACH: I am currently taking 20 mg of Xarelto daily, and have a screening colonoscopy scheduled soon. The preparation instructions said not to stop taking blood thinners, but didn’t specifically mention Xarelto. I contacted the office and spoke with a scheduler, who said those instructions applied to all blood thinners. Previously, I was told that I would need to go off of blood thinners for five days prior to a colonoscopy. Since this was a contradiction to the instructions from the clinic, I have tried to reach out to other medical sources to see if I could get a consensus on what to do, but no one has responded to me yet. There is a strong history of clotting problems in my family. Would you weigh in on this matter? The last time I had a screening colonoscopy was around 2005, and there weren’t any issues. I realize this is a scope and not surgery, but it has me concerned enough that I am considering putting this off until I can get a satisfactory response. -- B.R. ANSWER: Most screening colonoscopies now are done with patients still on their anticoagulants, although for patients on warfarin (Coumadin), it is preferable for levels to be on the low end of the therapeutic range. This is a standard recommendation by the American Society of Gastrointestinal Endoscopy. The situation would be different for a high-risk endoscopic procedure, such as removing a large polyp. I have had to deal with this situation many times in my practice, and most often, all anticoagulants are stopped then restarted after the colonoscopy. There are some people who are at such high risk for clots that we use injection medications to prevent blood clots until the night before the procedure, but that is normally reserved for the highest risk of all. It’s a judgment call as to whether to use the injection anticoagulants to prevent clots around the colonoscopy. *** Dr. Roach regrets that he is unable to answer individual letters, but will incorporate them in the column whenever possible. DID YOU KNOW? Halitosis, or bad breath as it’s commonly known, can result from any number of factors. While food can be a culprit, some additional factors may be contributing to mouth odor as well. Poor dental hygiene is a particular concern. According to Colgate and the American Dental Association, failure to brush and floss teeth regularly leads to the accumulation of food particles in the mouth. This food can collect bacteria and begin to break down, causing a bad aroma. Dry mouth is another condition that causes bad breath. Without adequate saliva production, particles of food that may cause odor are not swept away regularly. Bad breath also may be an indication of a medical condition elsewhere in the body. Upper respiratory infections and gastrointestinal disturbances may contribute to halitosis.

Plants depend on insects to transfer pollen as they forage, and many insects are quite beneficial to have around. While some pollinate, others are predators of other pests. • Antlion: A foe of ants, they’ll help eat and control ant populations and pollinate flowers. They pose no threat to humans, either. • Big dipper firefly: These colorful insects feast on earthworms, slugs and snails during the larval stage. Fireflies add drama to evening gardens with their twinkling lights. • Garden spiders: Although some spiders can be venomous, many are quite handy to have around the garden. They’ll help control pest populations that can damage plants and crops. • Dragonflies: These arial artists that zip around the yard are consuming smaller insects that would otherwise pester plants and humans. • Blue-winged wasp: This wasp attacks the larvae of Japanese beetles, helping to control beetle populations. • Bees and butterflies: Butterflies and bees are some of the best pollinators out there, and each can add whimsy to gardens.

Stripe smut The University of Maryland Extension notes that stripe smut primarily poses a threat to Kentucky bluegrass that is older than three years. Pale green streaks that run parallel to the veins in the leaves and leaf sheaths are symptomatic of stripe smut, which tends to be noticed in spring and fall, when weather is cool. As the disease progresses, stripes turn black or a silvery gray, causing the leaf blade to shred and curl. After the blades have shred, they turn brown and die. The grass thins because stripe smut makes it vulnerable to problems like drought.

Gardeners How do you get rid of moles in your garden that are eating all your plants? - Diane S. If the animals are actually eating your plants, then they are not moles. Moles eat insects and earthworms in the soil. You probably have voles that can look similar to moles. Voles eat plants. Voles prefer the bark of young fruit trees as well as tender stems, foliage, flowers, seeds and fruits of grasses, sedges and many other herbaceous plants. They will make surface and underground tunnels. Cats, dogs, foxes, skunks and coyotes will eat voles as will some snakes and birds.

You can use live traps but it is illegal in NY to relocate the animals so you will need to kill and bury them on your property. You can use a half inch mesh to surround your garden or individual plants but you will need to bury it 6 to 10 inches or so underground and have it extend about 12 inches above ground. Voles are not much for climbing.

For the most part they are as other organic material useful as a mulch on top of the ground or as an amendment to the soil after composting. Earthworms seem attracted to coffee grounds. The aeration of the soil from worm tunnels and recycling of nutrients by the worms eating the grounds and other soil contents will benefit the plants. A Look Back… MOMENTS IN TIME The History Channel * On May 23, 1911, the New York Public Library, the largest marble structure ever built in the United States, is dedicated in New York City. It took 14 years to complete. The next day some 40,000 people passed through its doors. The collection already consisted of more than a million books. * On May 25, 1927, Robert Ludlum, the author of 21 best-selling thrillers, including the Jason Bourne spy novels, is born in New York City. Ludlum sold more than 300 million books before his death in 2001.
Good Ideas On Aging Well (NAPS)—May is Older Americans Month! Staying connected to the community is a key ingredient to healthy and successful aging. But for many older adults, staying connected can be difficult. Fortunately, older adults and their caregivers can contact the Eldercare Locator to learn about local programs and resources that can help them stay engaged and active in their communities.
* On May 26, 1959, Harvey Haddix of the Pittsburgh Pirates pitches 12 perfect innings against the Milwaukee Braves, only to lose the game on a two-run double in the 13th inning. It was the first time in major-league history that a pitcher threw more than nine perfect innings . * On May 24, 1964, a referee’s call in a soccer match between Peru and Argentina sparks a riot. More than 300 fans were killed and another 500 injured in the violent melee at National Stadium in Lima, Peru.
Programs funded through the Older Americans Act provide older adults with many ways to stay active and involved in the community. For example, exercise classes, educational programs, volunteer opportunities and other health and wellness activities are available to older adults in communities around the country. Need more help than that? The Eldercare Locator can also connect you to local resources designed to help older adults maintain their independence at home, such as transportation, in-home assistance with daily needs, home modifications and assistance with healthy meals. The Eldercare Locator is a free service of the U.S. Administration for Community Living that is administered through the National Association of Area Agencies on Aging. Contact the Eldercare Locator at (800) 677-1116, 9 a.m. to 8 p.m. ET, Monday through Friday and online at eldercare. acl.gov. Older Americans and the people who care for them can get help and advice from experts.

Thank you to Richard of R.A. Haitz Company. Your company really went above and beyond for me. Everyone was so helpful I would like to alert computer users (seniors and younger users) of a very clever SCAM going around that I fell victim to. I started having trouble accessing my e-mail – the man would say, “Hmm can’t locate that info right now.” Then the screen would teasingly say, “Here’s your e-mail!” Then it would disappear. Then pop-ups would appear from an IT with an 800 number – “Please call us, we see you’re having trouble.” I didn’t call them. Suddenly she was on the phone and had contacted me! “I can see you’re having trouble. It’s an e-mail block! We can fix it! We “clean” it and add an antivirus and protection for just $249.99!! What’s your credit card number?” Like a fool, I gave it to them. It’s so much more convenient they told me. I can have new icons with news channels and an icon just for email. My banker today said he didn’t know why this SCAM company is still in business at all. I should try the 800 # to cancel, but I doubt they would do it. I have to file

What kind of people would take advantage of a Retired United States Army Veteran? Obviously, it’s the kind of people who have no morals or conscience. The audacity and greed of this particular landlord and his wife, knowing what they have been doing for the past 6 years. These two individuals live in our very own community. You know who you are, and you both should be ashamed of yourselves. And why are landlords not held accountable for their rental property? In particular, privately owned residences with apartments or houses. They should be up to code; not every landlord complies with the law. It’s a terrible injustice when a landlord can avoid fines and penalties for providing substandard rental units. Can someone enlighten me with some legal information pertaining to this topic? #CARMA

Thompson Encouraging Eligible Smokers, Ex-Smokers to Get Low-Dose Screenings
S
cott Lomber of Canandaigua has a lot to live for. In addition to his wife and two grown children, he has grandchildren he’s determined to see grow. So when primary care physician Dr. Cynthia Teerlinck recommended he get a low-dose lung cancer screening at F.F. Thompson Hospital, the longtime smoker agreed. The screening revealed a spot on one of Lomber’s lungs and in 2018, two weeks after he quit smoking, he underwent surgery at Strong Memorial Hospital for Stage 1 lung cancer. His six-month follow-up appointment this past September yielded a clean bill of health. “To find something and to have it be Stage 1, I feel lucky because I know it could have been a lot worse,” Lomber says. “To find it in the early stages makes all the difference.” Low-dose lung Dr. Ben Wandtke & Dr. Cynthia Teerlinck cancer screening takes about five minutes, causes no discomfort and has zero co-pay. In addition, experts say it can make a tremendous difference in prognoses. In fact, a study presented at the International Association for the Study of Lung Cancer World Conference showed widespread screening could save as many as 65,000 lives in the U.S. each year. To date, however, only a fraction of area residents who are eligible have taken advantage of the opportunity. “We don’t really know what is holding patients back,” says Dr. Ben Wandtke, president of Thompson Health’s medical staff and chair of its Population Health Management Committee. “Lung cancer is by far the leading cause of cancer deaths for both men and women,” the radiologist adds, “so it’s worth focusing on.” A patient’s primary care doctor must order low-dose lung screening after determining whether the patient meets the following criteria: • Between the ages of 55 and 77 • Has smoked “30 pack years” or more (one pack per day for 30 years or two packs per day for 15 years, for example) • Still smoking or quit fewer than 15 years ago Nationally, only 1 to 3 percent of people who meet the criteria participate in screenings. Within Thompson’s service area, it’s 15 percent. Although that’s far above the national average, Wandtke’s committee has a goal of reaching 30 percent within just two years. The reason is simple: “Early diagnosis enables a far better outcome than any treatment we can provide,” Wandtke says, citing data showing those diagnosed with Stage 1 lung cancer have a 50-percent survival rate at the end of 10 years, while those diagnosed with Stage 4 have a 10-year survival rate of less than 5 percent.

Submitted by Ann Johnston Teerlinck is a member of the Population Health Management Committee as well and refers all eligible patients for the free screenings, which become an annual test. “Following recommended guidelines and more importantly, finding disease early to prevent suffering and death are the reasons to utilize this screening test,” she says. “I have lost too many patients to lung cancer over my career.” From a patient’s perspective, Lomber says he would strongly recommend the screening to anyone eligible. “If you have the opportunity to do it,” he says, “do it.” If you think you may be eligible for a free, lowdose lung cancer screening, speak to your primary care physician about a referral. Dr. Ben Wandtke and Dr. Cynthia Teerlinck are members of the UR Medicine Thompson Health Population Health Management Committee, which is focused on ensuring that local residents who are eligible for low-dose lung cancer screenings take advantage of the opportunity to catch cancer in its early stages. If you meet the criteria listed in the article, consider asking your primary care physician to refer you for a low-dose lung cancer screening at Thompson Hospital in Canandaigua. There is zero co-pay but if you want to confirm, call Thompson’s Consumer Price Line at 585-396-6194 with your insurance information. Pearl white is the color of the ribbon for lung cancer awareness. DEAR PAW’S CORNER: I read that stray dogs from foreign countries are being put up for adoption here and bringing new diseases with them. My family wants to adopt a dog, but now we’re uncertain about doing so. How can we make sure we’re getting a healthy dog? — Julie in Alexandria, Louisiana DEAR JULIE: You should certainly do some research before taking that super-cute puppy home from a shelter. Rescue shelters have gone through some dramatic changes in the past decade, and perhaps the most notable change is that dogs eligible for adoption are often shipped from a shelter in one region of the country to another region. Dogs from overseas also are being acquired by some shelters as demand for rescue dogs — particularly the “adorable” one or popular breeds — outstrips supply. There is little federal regulation — the USDA doesn’t track how many dogs are imported from other countries, for example — and states are only just beginning to tighten rules to ensure the health of dogs shipped across state lines. There are reports of newly adopted dogs developing severe illnesses, including a new strain of canine distemper virus — a highly contagious disease that closely resembles rabies. Here are some important questions you should ask: • Where did the dog come from? Was it surrendered locally, sent from another state or unknown? • Ask for copies of health records. Has the dog been given vaccinations yet? • If the dog comes from another state or country, was it quarantined after arriving (at least 48 hours in some states)? Take the dog you adopt to a veterinarian as soon as possible for a health check and to get any needed vaccines.
