Smoking, Symptoms Biggest Factors in COPD Prognosis

DEAR DR. ROACH: I was diagnosed with COPD a year and a half ago. I seem to be unable to get answers from my pulmonary doctor, as he is always busy. How fast does COPD progress? I’m 78 years old, I’m in reasonably good health, and I exercise regularly. I have chronic bronchitis with a slight amount of emphysema. I had been coughing a little during the day, had mucus in the morning and coughed at night for two to three hours after bedtime -- to the extent of having to use a rescue inhaler to settle down and get back to sleep. After he put me on Trelegy, all of those symptoms disappeared, and I feel perfectly normal except for shortness of breath. My other question is, what is the life expectancy of a person diagnosed with COPD? I’ve read answers to this all over the map, from two or three years after diagnosis to 15 or more years. I would presume that it depends on other factors such as physical condition, exercise, weight, etc. I would like some kind of guideline for someone in my condition. -- T.J. ANSWER: There are two major types of chronic obstructive pulmonary disease: chronic bronchitis, where the primary problem is productive cough; and emphysema, which is destruction of the small airways. Shortness of breath is a common feature of all types of COPD. The most prevalent cause is smoking. I can’t answer your questions on rate of progression and overall prognosis without much more information, but it is certainly a good sign that many of your symptoms are well-controlled on treatment. The single biggest factor in rate of progression of COPD is whether you are a smoker, but not being able to exercise, being male and being very underweight are additional risk factors for faster progression. In someone with COPD, the decline over months to years can cause symptoms to occur first with exercise, and then when at rest. Once people have severe symptoms at rest, the prognosis becomes quite poor. No medication can reverse lung damage. Inhaled steroids reduce inflammation and can slow progression somewhat. Inhaled bronchodilators open airways and make breathing easier. Inhaled anticholinergics reduce secretions. Trelegy contains all of these medicines and is appropriate for people with severe COPD or those who do not have good symptom control with a less intensive regimen.

Started many years ago to help low-income individuals with their phone costs, it’s been expanded to include internet at home, offering a $9.25 monthly discount. That $9.25 savings doesn’t sound like much, but if you’re like me, nowadays, you’re converting every dollar saved into food. That $9.25, for example, could turn into a loaf of bread and a gallon of milk, or even a pound of ground beef, a package of hamburgers buns and most of a can of beans. You’ll need to qualify for Lifeline. One way you do that is if you (or anyone in your household) receive benefits from Supplemental Security Income, Temporary Assistance for Needy Families, veterans pensions, Supplemental Nutrition Assistance Program, Medicaid and others, or through having verifiable low income. (You’ll need paperwork to prove it.) You can sign up in a number of ways, such as by mail or online at nv.fcc. gov/lifeline. Scroll through the information on that page. Call 800-2349473 for more information or to ask that an application be sent to you by mail. But there’s more. In addition, if you qualify for Lifeline, you likely also qualify for an even bigger assistance: the Affordable Connectivity Program. According to the U.S. Consumer Products Safety Commission, thousands of people are injured by fireworks in the United States each year. Fireworks purchased on the black market may be especially dangerous, as they likely were not subjected to government regulations and inspections.

While summer might be the season of fireworks, it’s important that people who intend to incorporate fireworks into their summer festivities keep safety in mind at all times. The following are a handful of ways to ensure this summer is both fun and safe. •

Attend a municipal firework display rather than hosting your own. Professionals follow certain safety protocols that private citizens are unfamiliar with, and there is often a considerable distance between audiences and professional fireworks displays.

Exercise caution if fireworks are legal where you live. Maintain a safe distance from fireworks at all times and never allow children to handle fireworks.

Read and strictly adhere to manufacturer’s instructions.

Light only one firework at a time, and never attempt to re-ignite a firework that doesn’t light the first time around.

Do not carry fireworks in your pocket.

Do not shoot fireworks out of metal or glass containers.

Always wear safety glasses when lighting fireworks, and keep water or a fire extinguisher around for emergencies.

Pedestrian Crosswalks I think it’s time for some driver education. Drivers don’t seem to know that pedestrians have the right of way at these crosswalks. I watched a little boy on [local road] trying to cross at a marked crosswalk at this intersection. Nobody stopped to let him cross. I was waiting to make a right turn onto [local road], waiting until this little boy safely crossed. The driver behind me was honking angrily. Finally, an adult male came to his rescue and made sure he crossed safely. I see this law being broken all the time. Disrespect I do not understand the disrespect in today’s world. My husband was not a Veteran, but he was always very patriotic. He passed away a few years ago. He loved pinwheels, seeing them blowing in the wind. I found a large 16” red, white, and blue pinwheel with white polka dots, which was 36” tall and placed it at his grave for the last two years. It did not cost a lot of money but was worth millions in memories to my family. It disappeared! There was no windstorm to pull it 10” out of the ground and blow it away. And nothing else in the whole cemetery was disturbed. So, I know someone took it! I hope every time you look at it you remember where you got it and how you got it. Such disrespect stealing from a grave! We all share the Canal! We always love seeing other boats enjoying the Canal when we kayak from [local town] to [local town] and back, but we would appreciate it if power boaters slowed down when passing smaller boats and when approaching wildlife, like groups of ducks and geese, and especially those with young. What’s the rush? Slow down, enjoy the scenery, and you’ll be surprised at all of the amazing things you’ll see. We all share the Canal. Please be respectful.

Festivals are back! I love seeing all the festivals and events happening again! Of course, I understand the risk of covid-19 is still here and we should take precautions. Even if I don’t go to the events I love that we are getting back to fun this summer! From the Oatka Festival in LeRoy, to the Dansville Balloon Festival to the West Sparta OldFashioned Day and everything in-between! Disc Golf Course A few years back, the town I live in installed in a disc golf course and I cannot tell you how much we enjoy it and use it. Thank you to everyone that had a part in that. I know my family appreciates it!

Summer Heat Can be Deadly to Pets DEAR PAW’S CORNER: Yesterday, our beloved corgi mix, Shemp, was laid to rest. I feel so guilty. He passed from heatstroke after being out in the backyard for just one hour. I never realized that just the heat outdoors could be deadly to dogs. I always was conscientious about dangerous situations; I never once left Shemp in a hot car or walked him on hot sidewalks. Yet a few minutes of inattention, and my Shemp was suddenly in great distress. I rushed him to the veterinarian, but he just didn’t make it. Please warn your readers to keep their dogs inside on hot days and to monitor them closely for signs of heat stress. -- Devastated in Houston Dear Devastated: I am so sorry for the loss of your beloved Shemp. Thank you for warning other readers of the dangers of summer heat. Being outdoors during the extreme temperatures that we’ve experienced this summer, often passing 100 degrees in many states, is dangerous for dogs and other pets. Here are a few ways to keep them safe from heat injury. -- Walk dogs before sunrise and after sunset. Under a hot sun, sidewalks can quickly burn a dog’s paws, and heat and humidity can take a toll on their health. -- Do not leave pets outside unsupervised. Even if they’re just in the backyard, you might not notice their distress until it’s too late. -- Provide a shelter from the sun. A shelter with open sides and a solid top allows airflow and protects from the sun when your dog is outside. -- Make water available all day, everywhere. Rochester Hope for Pets Receives $8,900 Spay & Neuter Grant from BISSELL Pet Foundation USED BY PERMISSION OF KRISTA BROWN, COORDINATOR, ROCHESTER HOPE FOR PETS

R

ochester Hope for Pets has been awarded a $8,900 grant from BISSELL Pet Foundation for spay and neuter procedures at Rochester Community Animal Clinic. This dedicated grant will help reduce the homeless pet population by preventing unwanted litters of stray cats and dogs in the community. With thousands of pets abandoned each year, proactive spay and neuter procedures address the root cause of animal overpopulation by preventing unwanted births, freeing up shelter space for pets in need, and saving pets from potential abandonment or euthanasia. BISSELL Pet Foundation awards numerous grants every year to shelters across the country, including Rochester Hope for Pets, to help protect and save animal lives. In 2021, more than $1.3 million was provided through grants to shelters for spay/neuter procedures. “With the recent overcrowding crisis shelters are facing throughout the country, supporting our partner’s preventative spay and neuter efforts has become even more critical to impacting the lives of both homeless and owned pets,” said Cathy Bissell, founder of BISSELL Pet Foundation. “We also strongly encourage all pet owners to be responsible and have their pets sterilized to do their part to end pet homelessness.” The funds from this grant will be distributed to individuals based on application approval through their Spay-It-Forward program, through TNVR Clinics, and medical training clinics held at Rochester Community Animal Clinic. Serves 4 24 baby parsnips, trimmed, or 8 regular parsnips, root ends trimmed and peeled 3 tablespoons extra-virgin olive oil Kosher salt and freshly ground black pepper 1/4 cup pure maple syrup 2 tablespoons sherry vinegar 1/2 teaspoon red pepper flakes 2 tablespoons Earth Balance butter stick 1 shallot, minced 2 garlic cloves, minced 1/2 pound chanterelles, wiped clean, quartered if large Finely chopped fresh flat-leaf parsley, for garnish Flaked sea salt

To prepare the parsnips, preheat the oven to 400 F. If using larger parsnips, peel and quarter them lengthwise so the pieces will be uniform in size and cook more evenly. Put the parsnips in a large mixing bowl, drizzle with 2 tablespoons of the oil, season with kosher salt and black pepper, and toss to coat evenly. Spread the parsnips out in a single layer on a large baking sheet and roast for about 20 minutes, shaking the pan from time to time, until tender and slightly charred. Set aside. (The roasted parsnips can be prepared a couple hours in advance, covered and held at room temperature. Reheat before serving.) Meanwhile, prepare the glaze. Combine the maple syrup, vinegar and red pepper flakes in a small saucepan and cook over medium-low heat, swirling the pan around occasionally, until the mixture is reduced and syrupy, about 10 minutes. Cover and keep warm. To prepare the mushrooms, put a large sauté pan over medium heat and add the remaining 1 tablespoon oil and the butter substitute. When the butter substitute has melted, add the shallot and sauté until soft but not browned, about 1 minute. Add the garlic, tossing to combine. Add the mushrooms and sauté, stirring frequently, until they lose their moisture, soften and begin to brown, about 5 minutes. Remove from the heat. To serve, crisscross the parsnips on a platter, overlapping them just slightly. Drizzle with the maple glaze and top with the mushrooms.
