Thompson Health Spiritual Care Volunteer, Richard Kolb, Turns 90 By: Amanda Dudek His presence is immediately disarming, this pleasant, 90-year-old Chaplain’s Volunteer from Thompson Health in Canandaigua. It’s easy to see why his quiet, easy manner is so welcome to the patients he serves. Like his fellow volunteers, Richard Kolb is part of Thompson Health’s Spiritual Care Volunteer Program, developed over 20 years ago in 1996. Program Coordinator Eileen Alven has described the work of the volunteers as simply “being there on the journey,” and that certainly seems to be what Mr. Kolb is doing and has been doing for his past 11 years at Thompson. To Mr. Kolb, however, it seemed that, years ago, the volunteers had not yet connected with an important part of the patient’s journey: the patient arrival, preparation and wait time␣before␣surgery, which often occurs very early in the morning. Not surprisingly, Mr. Kolb, a U.S. Navy Vet, took action. He began arriving at Thompson’s Surgical Care Center by 4 a.m., ready not only to greet patients and their families in his supportive, unassuming way, but also available to support surgical staff, as well. Mr. Kolb states that while there are several Spiritual Care volunteers supporting patients after surgery, there were previously no volunteers to see them off on that journey. Now, there is. Just what is it Mr. Kolb does during these preoperative moments, which lead to what he describes as the only “2 to 3 minutes,” he has with each patient? “When they come in, I observe things like age and behavior,” he says. While he is a morning person, Mr. Kolb explains that most people are not, and this is something to which he is sensitive. He also explains that most people aren’t allowed to eat or drink before their procedure. “Always␣procedure,” he insists, nixing the word “surgery,” immediately. After observing a few moments, Mr. Kolb quietly approaches each patient and introduces himself, offering his assistance as well as a prayer. “Some people don’t feel they need a prayer, and that’s okay,” Mr. Kolb says, sincerely. “I tell them, ‘It’s strictly your decision,’ whatever makes them more comfortable.” Easing patient anxieties and letting them know that the hospital cares about them is paramount to Mr. Kolb. If patients do request a prayer, Mr. Kolb has prayers for each age group. These prayers are generally prayers to God for strength and peace from the outlook of a child, a young adult, an older person and so on. Sometimes, Mr. Kolb says, he looks up from the prayer to find tears in the patient’s eyes. It is reaching patients in that

Mr. Kolb Sits with Fellow Spiritual Care Volunteers moment; it is when people feel that his service has helped them, that means the most to Mr. Kolb. He doesn’t stop there, however. A former Hospital Corpsman, top of his class, Richard Kolb doesn’t do anything halfway. Knowing that what often causes the most anxiety is the unknown, he explains the pre-and-post procedure processes in detail, explaining what the patient and his or her family members can expect during their time at Thompson. He clearly demonstrates the hospital’s Call Card system in which the patient’s family is given a Call Card with a number on it. The card itself is striped with colors, each color with a different message, relaying information about stages in the procedure. When the patient’s number flashes in a certain color on a board in the waiting area, that family, and only that family, knows what is happening with their loved one. Serving others through information is just another way Richard Kolb shows patients that Thompson Health truly cares about them, being what he calls “the face of the hospital.” It is this level of service, all 8,100 hours of it to date, that earned Richard Kolb the 2015 Health Care Achievement Award in the Volunteer Category. This honor, along with a beautiful etched glass trophy, was awarded to Mr. Kolb at a beautiful luncheon held by the Rochester Business Journal. Thompson Hospital’s Administrator, the Head of Volunteers, Mr. Kolb’s Chaplain and Mr. Kolb’s daughter all were with him at this luncheon, supporting his dedication and hard work. While Mr. Kolb is pleased and refers to this award modestly as a “nice honor,” it is the times when patients return, sometimes years later, and remember him and are happy to see him, because they believe he was helpful to them during their last visit, that Richard Kolb counts as the greatest honors of all.

What is a seborrheic keratosis? Thanks to education and awareness about skin cancer, more people are taking heed of skin growths and changes and visiting their dermatologists. A spot or mole on the body can be a cause for concern. A seborrheic keratosis is one such type of skin growth that is actually benign but may cause concern due to its appearance. Seborrheic keratoses, or SKs, are one of the most common noncancerous skin growths, says The Mayo Clinic. Although anyone can get them, they are more common in middle-aged to older adults. Also known as basal cell papilloma, senile warts or barnacles, these growths form as a common sign of skin aging, according to DermNet New Zealand. The American Academy of Dermatology says a seborrheic keratosis may seem worrisome because it can look like a wart, a pre-cancerous skin growth or even skin cancer. However, despite their appearance, SKs are harmless. How do SKs form? Seborrheic means “greasy” and keratosis means “thickening of the skin.” Despite the name, SKs are not known to be caused by an abundance of sebum in the skin, nor are they limited to areas of increased oil production. The exact cause of SKs isn’t known, but some doctors feel that an overproduction of skin cells can cause a thickening of the skin in certain areas. The American Osteopathic College of Dermatology offers that SKs are most likely hereditary, and they often are found on the trunk of the body and where the face meets the scalp. Sometimes seborrheic keratoses may erupt during pregnancy, following hormone replacement therapy or as a result of other medical problems.
Appearance of SKs Some of these growths have a warty, rough surface, while others look like dabs of warm, brown candle wax on the skin. ADA says they can range in color from white to black, but most are tan or brown. The appearance of SKs may change as one ages, and it is even possible for them to fall off naturally over time. The Mayo Clinic says that, unless changes include becoming sore or bleeding without healing, there’s little need to act. SKs may form singularly, but often they appear in groups. Cancer risk? SKs have no relationship to skin cancer and do not pose a risk to one’s health. Unlike moles, they will not turn into melanoma. The superficial nature of SKs mean they can become irritated by clothing and grow. Treatment Unless a seborrheic keratosis is in an area that causes constant irritation, there is really no need to seek treatment. However, dermatologists can remove them quite easily if they are unsightly or uncomfortable. A topical anesthetic, cryosurgery or laser therapy is possible. Generally little scarring is produced, offers AOCD. Although SKs are harmless, their appearance may be similar to skin conditions that are not. Those who are unsure about changes to the skin should always seek the help of a licensed dermatologist.

HOW EVERYDAY CITIZENS CAN PROTECT LOCAL PARKS Leave nothing behind. Memories are not the only things park visitors should take with them when they leave the park. Estimates suggest that as much as 100 million pounds of garbage are generated at California’s Yosemite National Park each year. Significant damage can result if even a tiny fraction of that garbage is left behind. In addition, park officials forced to expend their limited resources on garbage pickup may not have enough resources left to address other issues, further threatening the park. Know the ABCDE’s of Melanoma --DEAR DR. ROACH: In 1976, I had a mole on my right calf about the size of a pencil eraser. A biopsy showed it to be melanoma, and they thought they had removed all of the cancer, but they did another surgery to be sure that I was cancer-free. I am a natural redhead and never sunbathed or used a tanning bed. It has been 38 years, and I am still cancer-free. I get a skin check from my dermatologist every six months. At the time of my diagnosis, my family doctor said that if the melanoma spreads inside the body, “the ball game is over.” Is that still true? I know melanoma can be deadly. -- W.K. ANSWER: Melanoma is the most commonly lethal skin cancer. Although there have been some exciting new developments in treating advanced melanoma, the survival rate for stage IV melanoma, where the cancer has spread to internal organs, is only 15 percent to 20 percent at five years. Fortunately, the survival rate for melanoma caught early, at stage I, is greater than 90 percent, which is why it’s so important to know the ABCDE’s of melanoma recognition: A: Asymmetry - one half looks different from the other half; B: Border - the border in a melanoma is often irregular, not a near-circle; C: Color - melanomas usually are not a uniform color, like most benign lesions. Some areas are light brown, others brown-black, others red or pale; D: Diameter - greater than 6 millimeters, the eraser on a standard pencil; E: Enlargement or Evolution - any change in color, shape, size or symptoms (including bleeding). Any of these should prompt a visit to your regular doctor or a dermatologist. As an internist, these are the features I look for, and I have a very low threshold for sending suspicious lesions to my colleagues in dermatology. *** DEAR DR. ROACH: My son is 40 years old. After a recent stress test, he ended up with five stents in his arteries, and also was diagnosed with diabetes. His doctor ordered him not to go back to work for three or four weeks, but he went back after three days. He told the doctor that he would be more stressed if he stayed home. His job is mostly sitting down. Since he didn’t follow the advice, how will this affect his health? -- V.B. ANSWER: We don’t normally have people stay home for long periods of time after placement of stents, so I was surprised to hear that he was “ordered” (I almost never “order,” only recommend, more or less strongly) to stay home. My experience is that after receiving life-changing diagnoses, such as diabetes or coronary artery disease, it’s best to keep some habits, such as work, the same and change others. For your son, I am sure he will be recommended a proper diet for diabetes and exercise appropriate for a person with a new diagnosis of heart disease. FRUITS & VEGETABLES MAY LOWER RISK OF PAD Pain in the lower extremities may be indicative of a condition called peripheral artery disease, or PAD. The National Heart, Lung, and Blood Institute says PAD occurs when plaque builds up in the arteries that carry blood to the limbs, head and organs, contributing to atherosclerosis, or a hardening of the arteries. Blocked blood flow and eventual limited oxygen supply to these areas can cause pain and numbness. When severe enough, PAD may lead to tissue death. Discomfort due to PAD usually occurs when a person is walking or exercising, because the muscles are not getting enough blood during these activities to meet their needs, says the American Heart Association. Those with diabetes may confuse pain with neuropathy, and the elderly may think pain from PAD is a normal sign of aging and stiffness. When undiagnosed, PAD can lead to further complications, including increased risk for heart attack, stroke, and coronary artery disease. Even amputation of a limb may be necessary. However, PAD is preventable when taking a few steps to improve diet. According to new research published by the American Heart Association, eating more fruits and vegetables may reduce the risk of developing PAD, which affects an estimated 8.5 million people in the United States alone. Researchers examined dietary data from roughly 3.7 million men and women, with an average age of 65. Approximately 6.3 percent of the subjects had PAD, and 29.2 percent indicated they ate three or more servings of fruits and vegetables each day. The discovery was those who reported eating three or more daily servings of fruits and vegetables had an 18 percent lower risk of PAD than those who reported eating less of these foods. The association between fruit and vegetable consumption and lower PAD risk remained even after accounting for age, gender, race, smoking status, and various cardiovascular risk factors. Saturated fats, trans-fats and sodium can contribute to the formation of plaques that lead to PAD. Replacing these foods with more vegetables and fruits that are naturally lower in saturated fats can help, as can increasing dietary fiber consumption. One way to incorporate these types of foods is to adhere to a Mediterranean diet, which offers high proportions of legumes, fruits, vegetables; moderate amounts of fish and dairy; and limited meat and meat products.

Luke was born on December 7, 1969 in Penn Yan, NY. After graduating from Canandaigua Academy he made his home on the High Tor outside of Naples, NY, and he wintered in Fort Myers Beach, Florida. He worked as a sawyer, carpenter, organic farmer and vendor of produce. Luke was a member of “Sons of the American Legion”, “Loyal Order of the Moose”, and ABATE (American Bikers Aimed Toward Education). Luke was predeceased by his sister, Karla Dekouski Moreau (Canandaigua); grandparents Ray and Roma Dekouski (Middlesex, NY), Paul Jaynes (Ithaca, NY) and Doris Jaynes (Penn Yan, NY); aunt Adele Dekouski (Middlesex, NY) and uncle, Karl Jaynes (Newark, NY) and his beloved dog, “Jessie”. Luke is survived by his mother, Patricia Jaynes Heckman (Canandaigua), and his father, Terry (Deb) Dekouski (Canandaigua and St. Petersburg, FL); sister Kelly Struble, nephew Tyler Struble, niece Paige Struble, and great-niece, Mia (Canandaigua); step siblings, Eric Heckman (St. Louis, MO) and Natalie Heckman Scott (Chicago, IL); aunts Janet Read and Sharon Jaynes (Canandaigua) and Devereaux (Richard) Russell (Penn Yan); uncles Rev. Ronald (Colleen) Jaynes ( Lancaster, PA) and Charles Jaynes (St. Petersburg, FL) and several cousins. Known as “Luke Duke” by his many wonderful friends, Luke will be remembered for his outrageous sense of humor, his love and loyalty to his friends and his insistence on living every day in a big way. His passions were many, especially fishing, golfing, cooking, Native American art, “Grateful Dead” music and riding his Harley. A time of gathering of friends and family will be held at Fuller Funeral Home (190 Buffalo St., Canandaigua, NY 14424) on Saturday, August 25th at 10 a.m. A memorial service will follow at 11a.m. A private committal service will then be held at Woodlawn Cemetery in Canandaigua. I can’t be calling this waste company to keep on coming back to get what is left on the lawn. About holding a child on your lap when mowing, it is a dangerous thing to do. Turn off the deck, give the kid a ride around the yard, and then put him (or her) off the tractor and away from the mower. Child safety is the business of everyone.
It’s a shame....that the "I Gotta Vent" section is twice the size of "Brighter Side." I am a woman. On Saturday morning, I went to an estate sale a few houses over from mine. Of course nothing was priced. (I find this irritating...apparently people want to sell but are too lazy to come up with a price). I asked the gentleman who was running the sale for a price on items downstairs. He rudely passed over me to assist a woman in a short dress with plenty of cleavage showing (remember, this is an estate sale, not a bar). When he could finally make time for me, I asked him for prices on an assortment of screws, nails, etc. He replied, "That's a boy thing." Really? In 2018? Women cannot buy hardware? Needless to say, I left immediately. I simply cannot fathom this man's behavior... it was rude, crude and disrespectful. This man needs to step out of his cave.
