The country is already deeply in debt, and Medicare is paying for this? Unconscionable! Not to mention the pain and suffering of my mother, who is now at risk of a secondary infection. I was sickened by the entire ordeal. -- J.B. ANSWER: I agree with you completely that often patients with terminal diseases receive unnecessary care. There have been studies clearly documenting this. However, the studies don’t answer why, in a particular case, a physician performed these treatments, which add only pain, anxiety and expense. I suppose it is possible that the motivation is simple greed; however, I still have enough faith in my colleagues that I think that is a very unusual reason; I think it’s far more likely that a specialist just doesn’t see the big picture. There’s an old expression that when all you have is a hammer, the whole world looks like a nail, and I think some specialists see a problem and fix it without realizing that the problem they are fixing isn’t likely to ever cause symptoms. For example, primary-care doctors order unnecessary tests, especially screening tests. I see mammograms ordered for women with advanced colon or ovarian cancer who are in palliative care, and this makes no sense. I have heard some physicians mention that insurance companies monitor the frequency of “quality indicators,” such as mammograms, which affect their ratings and reimbursement. That’s an example of a well-meaning system motivating wrong behavior. I should emphasize that not all care for terminal patients is useless. If the goal is to improve quality of life or reduce suffering, then I am all for it, after a consideration of the costs (pain and inconvenience as well as dollar costs). *** DEAR DR. ROACH: I have heard of yeast infections, but I don’t know what causes them. How do they affect the body, and what can be done to get rid of them? -- C.R. ANSWER: Certain yeasts, but especially the Candida species, are found on our skin, mucus membranes and GI tract. They normally live in balance with the 100 trillion or so bacteria we carry around. However, yeast can cause disease that ranges from fairly mild, like thrush of the mouth or vagina, to life-threatening, like a bloodborne, widely disseminated invasive infection. Candida infection of mucus membranes is usually caused by changes in our bacteria, especially after the use of antibiotics. The antibiotics kill the bacteria they are supposed to (hopefully), but they also may kill the healthy bacteria that assist us in digestion (leading to diarrhea or worse), and this allows the other bacteria and yeast to grow. Some people with genetic faults in their immune system are predisposed to chronic candida infections. These are uncommon but can be severe, and may require treatment by specialists, such as infectious disease doctors and immunologists. In its latest World Tourism Barometer, the United Nations World Tourism Organization found that 1.184 billion tourists traveled outside their countries’ borders for at least one night in 2015. Europe, Asia, the Pacific, and the Americas all recorded around a 5 percent increase in international arrivals that year. Europe was particularly popular, perhaps hedged by a weaker euro against the American dollar and other currencies. The U.S. Travel Association says that direct spending by resident and international travelers in America averaged $2.7 billion a day. Millennials may be leading the travel-enamored pack. The United Nations estimates that 20 percent of all international tourists, or nearly 200 million travelers, are young people, and that the millennial demographic generates more than $180 billion in annual tourism revenue. The U.N. also reports that millennials are more interested than older generations in traveling abroad as much as possible. Infrequent travelers or those who have never traveled may not understand why heading to parts unknown is so appealing to so many people. The following are just a handful of reasons why travel is so enticing. 1. Engage the mind. Staying mentally active over the course of a lifetime promotes long-term health. Navigating unfamiliar places or reading a foreign language while sitting in an international country can engage the brain and get synapses firing. The Mayo Clinic reports that higher cognitive activity endows the brain with a greater ability to fend off brain pathologies, such as disease and dementias. 2. Connect with new people. Travel opens a person up to not only new experiences, but also new people. English poet John Donne penned the famous line, “No man is an island,” which underscores the importance of having friends and making new acquaintances. Research conducted by the University of Michigan found the act of talking with people in a friendly way can improve a person’s memory, suppress external and internal distractions, and encourage people to see things from another person’s perspective. It doesn’t hurt to broaden one’s social network, either. 3. Build confidence. Leaving one’s comfort zone can be a great way to bolster one’s self-esteem. Navigating cultural boundaries and overcoming those boundaries may be initially intimidating, but doing so can make a person more confident and more adaptable to change. 4. Develop opinions. Until a person visits a place in person, he or she only has third-party information to form opinions. Visiting a city or country for the first time can offer a more complete perspective. Travel gives people the chance to rest, explore, meet new friends, and make lasting memories.

Renowned Ballerina Aesha Ash Encourages Byron-Bergen Students to Dream Bigger By Christine Klem Ballet star Aesha Ash told the hundreds of students at Byron-Bergen Elementary that when she was growing up in Rochester, “there were no princesses or fairies that looked like me. There were no magical creatures, unicorns or swans that looked like me.” When she dreamed of being a ballerina, she was told that there were no black ballerinas, and that she would never succeed. She dreamed anyway, and she did succeed. She was accepted to the legendary School of American Ballet where she was chosen to join the New York City Ballet when she was 18 – one of the first black dancers in the corps. How did this world-famous dancer and winner of the National Women’s History Museum’s 2016 Women Making History Award, come to be in Byron-Bergen? Fourth-grade teacher Alyson Tardy heard about Ash’s Swan Dreams Project, which was founded in 2011. This project uses powerful imagery to counter negative stereotypes of race and socio-economic background and inspire children to dream bigger. She thought that Ash’s message would be a great tie-in to the school’s character education program. She invited Ash and coordinated the special visit. Students surprised their visitor with a hallway lined with artwork featuring swans of many shapes and colors. “The art is so beautiful,” said Ash. “I’m honored that the kids welcomed me this way.” At the crowded assembly, Ash shared her story with attentive students, beginning with her passion for dance and her determination to become a ballerina. She explained how she was part of Rochester’s Urban Suburban program, and constantly faced questions and misunderstanding from peers about her background and city neighborhood. Her family was not rich; she told

the story of borrowing toe shoes for her first tryout. Even when Ash became a professional ballet dancer, she felt a sense of not belonging, of being different. “All my life, I’ve fought to change perceptions and dispel myths – for myself, my family and my Rochester community,” she said. “The more that people told me that I couldn’t do something, the more I wanted to do it.” Ash’s professional dance career lasted 13 years. It included eight years with the NYC Ballet along with performing in the Bejart Ballet in Switzerland and Alonzo King’s Lines Ballet, and with many other companies as a freelance artist. She and her family currently live in California where she is working to make the Swan Dreams Project an afterschool program. She hopes to open a studio where she can teach ballet to children who are not able to afford lessons. The Grinch is back again this Christmas in our well-to-do town south of Rochester. There are no Christmas lights along our Main Street, only dark dreary winter pennants. Several years ago, which was the last year our illuminated Christmas lanterns were put up, we noticed that there was
something wrong with them. Upon further inspection, many if not most of the lights inside them were burned out. The lanterns were still intact. Someone was either too lazy to replace the bulbs or it was planned to get rid of them. At least they gave a nice warm glow to our Main Street. All of the surrounding hamlets have Christmas lights except us. They have the Christmas spirit! Sadly dark winter pennants are a poor substitute for lighted decorations. Drivers-If you're making a left hand turn, turn into the LEFT lane!!

HOST A SPORTS VIEWING PARTY TO REMEMBER Hundreds of millions of people across the globe enjoy sports, and many are fond of gathering with fellow fans to watch their favorite teams play, whether in-person or on television. Viewing parties are a great way to watch the big game with fellow fans, and such gatherings don’t have to require all that much effort on the part of hosts. Make it a potluck affair. Potluck affairs make perfect sense for sports viewing parties, so don’t be surprised if guests offer to whip up some homemade guacamole or a buffet-style dish for the big party. Encourage this, asking guests to share what they’re bringing with other invitees so there are no duplicate offerings. Vary the food and beverage menu. Chicken wings and beer might seem like go-to foods and beverages for the big game, but even sports bars are increasingly recognizing sports fans’ desire for healthy foods. It’s still acceptable to serve staples like wings, nachos, chips, and pretzels, but try to provide some healthy fare like celery and salad as well. A 4K ultra-HD television can be an affordable option depending on the size of the television, and such devices provide incredibly crisp pictures that can make sports fans feel as though they’re inside the stadium cheering on their favorite players. Purchase a sound system. Today’s televisions are sleek and thin. While that saves space and provides more aesthetic appeal than the bulky televisions of yesteryear, thin designs leave little room for sound tech. As a result, it can be hard to hear what’s going on in the game without a sound system. Sound systems are relatively inexpensive; just make sure to purchase one that is compatible with your television. Sports viewing parties tend to be easygoing affairs that don’t require the level of planning of more formal gatherings.

DID YOU KNOW? People with red hair are often assumed to be of Irish descent. Although not everyone with copper tresses can trace their lineage to the Emerald Isle, a deeper look at genealogy may, in fact, point out Irish origins somewhere along the way. According to Eupedia, a website that investigates European ancestry and genetics, Ireland has the highest per capita percentage of redheads in the world. The rates hover anywhere from 10 to 30 percent. In order to have red hair, a person must possess the MC1R gene, which is recessive and only occurs in about 2 percent of the world’s population, says the National Institutes of Health. Both parents must pass on a copy of the gene in order for their offspring to be born with red hair. Red hair also tends to skip generations, so if you are a redhead, your children may not be born with red hair, but your grandchildren may. The rarest combination is a person with red hair and blue eyes, which also are a recessive trait. Other cultures that historically have red hair are Scandinavian nations, like Norway. It is believed that since ancient Vikings took Irish slaves to Norway centuries ago, their genetics intermingled. Keep in mind that redheads should stay out of the sun. Research says the pigment that gives hair the red color can make redheads more susceptible to melanoma than even fair-skinned blonds.

EFFICIENCY AT WORK People with multiple hobbies recognize there are many great ways to spend one’s time. But even people with plenty of ways to pass their time may find themselves wasting time each day, a phenomenon that seems to be growing. In its 2014 “Wasting Time at Work” survey, Salary.com found that 89 percent of participants acknowledged wasting time at work each day. That marked a sharp increase from a year prior, when 69 percent of workers surveyed reported wasting time at work. Thirty-one percent of workers surveyed admitted wasting roughly 30 minutes per day, while another 31 percent estimated they waste around one hour each day. Perhaps the most surprising finding from the survey was that 4 percent of participants admitted they waste four or more hours, or at least half a typically eight-hour workday, at work each day. Some people might be wasting time out of sheer boredom, but many may be able to trace all those squandered minutes and hours to inefficiency.
