Pain in the lower extremities may be indicative of a condition called peripheral artery disease, or PAD. The National Heart, Lung, and Blood Institute says PAD occurs when plaque builds up in the arteries that carry blood to the limbs, head and organs, contributing to atherosclerosis, or a hardening of the arteries. Blocked blood flow and eventual limited oxygen supply to these areas can cause pain and numbness. When severe enough, PAD may lead to tissue death. Discomfort due to PAD usually occurs when a person is walking or exercising, because the muscles are not getting enough blood during these activities to meet their needs, says the American Heart Association. Those with diabetes may confuse pain with neuropathy, and the elderly may think pain from PAD is a normal sign of aging and stiffness. When undiagnosed, PAD can lead to further complications, including increased risk for heart attack, stroke, and coronary artery disease. Even amputation of a limb may be necessary. However, PAD is preventable when taking a few steps to improve diet. According to new research published by the American Heart Association, eating more fruits and vegetables may reduce the risk of developing PAD, which affects an estimated 8.5 million people in the United States alone. Researchers examined dietary data from roughly 3.7 million men and women, with an average age of 65. Approximately 6.3 percent of the subjects had PAD, and 29.2 percent indicated they ate three or more servings of fruits and vegetables each day. The discovery was those who reported eating three or more daily servings of fruits and vegetables had an 18 percent lower risk of PAD than those who reported eating less of these foods. The association between fruit and vegetable consumption and lower PAD risk remained even after accounting for age, gender, race, smoking status, and various cardiovascular risk factors. Saturated fats, trans-fats and sodium can contribute to the formation of plaques that lead to PAD. Replacing these foods with more vegetables and fruits that are naturally lower in saturated fats can help, as can increasing dietary fiber consumption. One way to incorporate these types of foods is to adhere to a Mediterranean diet, which offers high proportions of legumes, fruits, vegetables; moderate amounts of fish and dairy; and limited meat and meat products.

The following recipe for “Garbanzo-Carrot Hummus with Grilled Yogurt Flatbread” from James Campbell Caruso’s “España: Explore the Flavors of Spain” (Gibbs Smith) includes some Moroccan flavors that give this easy-toprepare recipe a truly unique taste.

Garbanzo-Carrot Hummus with Grilled Yogurt Flatbread Makes 2 cups 2 medium carrots, peeled and chopped Salt 1 cup cooked garbanzo beans, drained 4 teaspoons chopped cilantro plus 1 teaspoon for garnish 2 tablespoons chopped red onion 21/2 teaspoons ground cumin 11/2 teaspoons ground coriander seeds 2 tablespoons lemon juice 4 teaspoons olive oil 2 teaspoons chile flakes 2 teaspoons Moroccan Spice Blend (see below) Coarse salt and freshly ground black pepper 1 batch Yogurt Flatbread (see below) In a medium saucepan, combine the carrots with 2 quarts water and 2 teaspoons salt. Bring the mixture to a boil then reduce the heat and simmer for 8 to 10 minutes, until the carrots are tender. Remove the pan from the heat and allow the carrots to drain and cool in a colander. Combine carrots and remaining ingredients, except for Yogurt Flatbread, in the work bowl of a food processor and puree until smooth. Season to taste with salt and pepper and garnish with the remaining cilantro. Serve with fresh, hot Yogurt Flatbread cut in wedges.

Moroccan Spice Blend Makes about 2 tablespoons In a small resealable glass or plastic container, combine 1 teaspoon each of: ground cumin, ground turmeric, saffron threads, ground coriander, ground cinnamon, ground smoked paprika, finely grated lemon peel.

Yogurt Flatbread Serves 4 1 cup all-purpose flour 1/4 teaspoon baking powder 1 teaspoon coarse salt 21/2 cups plain yogurt Olive oil Sift the flour, baking powder and salt into the work bowl of a stand mixer fitted with the dough hook. Add the yogurt and mix on low speed for 2 minutes. Cover the work bowl and allow the dough to rest at room temperature for 30 minutes. Preheat a gas or charcoal grill to medium. Scrape the dough from the work bowl and turn it out onto a lightly floured surface. Roll the dough into a long log and divide it into 12 equal pieces. Roll each piece into a ball and use a rolling pin or tortilla press to flatten it into a 1/4-inch-thick tortilla shape. Brush each “tortilla” lightly with olive oil. Grill each for about 40 seconds then turn and cook another 40 seconds.

--DEAR DR. ROACH: I am a 65-year-old male who has developed a lack of sensation in the lateral front half of my right thigh. The numbness involves strictly the skin. There is no loss of strength, function or balance in my leg. My gait is normal, although I find that if I go on a long walk, the skin will begin to “tingle.” When I went to an orthopedic surgeon, he suspected that the condition could be caused by a disc problem. Although an X-ray did show some stenosis, it was not definitive. I suspect I caused the problem by years of sleeping on my right side in a curled-up position, resulting in entrapment of the nerve. The orthopedist, however, says he generally sees this condition only in obese people, which I am not. How likely is it that my condition is of spinal origin rather than an entrapped nerve? If the condition can resolve itself gradually by avoiding activities that contribute to the problem, what does “gradually” mean? Do stretching exercises help relieve the entrapment? -- J.C.B. ANSWER: What you are describing is meralgia paresthetica, the compression of the lateral femoral cutaneous nerve, which can be compressed as it passes under the inguinal ligament, exactly as you described. Your orthopedist is correct that this condition is more common in the overweight or obese; however, I have seen it in both people who are losing weight and people of normal, stable weight. It is more common in conjunction with diabetes, and it has been reported after long-distance walking and cycling. Because of the area involved in your numbness, it is very likely to be meralgia paresthetica and not spinal in origin. It does usually resolve, but it does so over the course of months, typically. Stretching would seem to make sense, but as far as I know, it hasn’t been shown to work. If it isn’t getting better, an injection into the nerve usually is effective. This is commonly done by an anesthesiologist or a painmanagement specialist. *** DEAR DR. ROACH: Can you please give me some information on Elhers-Danlos syndrome? My daughter and two granddaughters have it. -- E.E.H. ANSWER: Ehlers-Danlos syndrome is actually a group of uncommon genetic disorders, with very stretchy skin, flexible joints and tissue fragility as common features. There are six different kinds of Ehlers-Danlos, and each is different in regard to what tissues and organs it most affects. Most are caused by mutations in genes for collagen and related structural proteins. They can be inherited or not (in which case they are new mutations). Flexible joints (often but wrongly called “double-jointed”) and excessively stretchy skin are almost universal in EDS. The excess joint movement can cause dislocations and chronic joint pain resembling fibromyalgia. Other commonly associated conditions include prolapse of the mitral valve, hernias and, in the vascular type of EDS, aneurisms. The prognosis in EDS depends to a great extent on the subtype. UNDERSTANDING DEMENTIA Dementia presents itself through various symptoms, and memory loss alone is not enough to lead to a dementia diagnosis. Dementia can affect thinking and social abilities, but the Mayo Clinic notes that some dementias may be reversible. The following are some common symptoms of dementia: • Trouble communicating or finding words. • Difficulty completing complex tasks. • Challenges with planning and organization. • Episodes of confusion and disorientation. • Memory loss, which is often noticed by a third party. • Personality changes that can include agitation, anxiety, inappropriate behavior and even hallucinations. Apart from Alzheimer’s disease, which is a progressive disorder most common in people age 65 and older, there are other types of dementia. The second most common is called “vascular dementia.” This results from damage to vessels that supply blood to the brain. This damage can be the result of stroke, smoking and other blood vessel conditions. Brain imaging can often detect blood vessel problems implicated in vascular dementia. Dementia with Lewy bodies, or DLB, is another dementia that laymen may mistake for Alzheimer’s disease. The Alzheimer’s Association notes that people with DLB often have not only memory loss and cognitive problems common in Alzheimer’s, but they also display initial or early symptoms such as sleep disturbances, well-formed visual hallucinations, slowness, gait imbalance or other Parkinsonian movement features, which can lead to misdiagnosis.

How seniors can preserve their brains Physical activity and proper diet and nutrition can help people age 50 and older maintain their physical health. But there are also ways aging men and women can preserve brain health in an effort to prevent or delay the cognitive decline that affects millions of seniors across the globe. It’s easy to overlook the importance of keeping the brain healthy. However, a decline in brain function can result in poor concentration, memory loss and a host of other issues. Sometimes, by the time symptoms present themselves, it may be too late to reverse any damage. Research suggests that a combination of nutrition and mental, social and physical activities may have a greater impact with regard to maintaining and improving brain health than any single activity. Harvard Medical School also states that volunteering, caring for others and pursuing hobbies may benefit the brains of older adults.

• Start exercising the brain early on. A study published in 2012 in the British Medical Journal examined cognitive function in people ages 45 to 70. Researchers found evidence of cognitive decline in the 45year-old participants as well as the older participants. It’s never too early to put a brain health plan into motion. • Read more books. Reading can open individuals up to new vocabulary and scenarios that promote a stronger brain and recall

• Keep a close-knit group of friends. Regular conversation and social interaction is a key component of any brain health wellness plan. Slowing cognitive decline and promoting greater brain health should be a priority for adults of all ages.

HOW AGING ADULTS CAN MAINTAIN THEIR MENTAL ACUITY Aging is associated with or linked to a host of mental and physical side effects. For example, many adults expect their vision to deteriorate as they grow older. Such a side effect can be combatted with routine eye examinations that may indicate a need for a stronger eyeglass prescription, a relatively simple solution that won’t impact adults’ daily lives much at all. While physical side effects like diminished vision might not strike much fear in the hearts of aging men and women, those same people may be concerned and/or frightened by the notion of age-related cognitive decline. Some immediately associate such decline with Alzheimer’s disease, an irreversible, progressive brain disorder that gradually destroys memory and cognitive skills, ultimately compromising a person’s ability to perform even the simplest of tasks. But age-related cognitive decline is not always symptomatic of Alzheimer’s disease. Learning about Alzheimer’s and how to maintain mental acuity can help aging men and women better understand the changes their brains might be undergoing as they near or pass retirement age.
