Farmers Market - SHOPPING For the best selection, go to the farmers market early. The best goods go first. Popular-but-limited items may even sell out before the day is done. It’s as simple as that. Planning ahead is essential. However, you need to leave a bit of wiggle room for those strawberries you didn’t know would be at market so early, or the zucchini blossoms you’ve never tried before. Trying new things is part of the fun of going to farmers markets. Get to know the merchants. There are a lot of great stories there, if you just take the time to listen. Smile and say hello. VISUALLY IMPAIRED READER INSPIRES AUTHOR (NAPSI)—Four years ago, bestselling author Mary Jane Clark spoke at the Friends of the New Jersey Library for the Blind and Handicapped’s Fall Festival. There, she met Ottilie Lucas, a former rehabilitation teacher for people with macular degeneration and other disabilities who is visually impaired herself. After the festival, “we continued our correspondence and one thing led to another,” Lucas says of Clark. “She decided to include a character in her next book with macular degeneration.” And so was born Terri Donovan, introduced in the first book in Clark’s Wedding Cake Mysteries series, 2011’s “To Have and to Kill.” “Mary Jane asked me to give her ways to identify symptoms that a person may be experiencing macular degeneration,” Lucas says. These include blurred vision and difficulty discerning the intensity of colors—symptoms shared by Terri Donovan, who runs a bakery and is the mother of the protagonist of Clark’s series, struggling actress Piper Donovan. Lucas, who was born with retinitis pigmentosa, became a patron of the New Jersey State Library Talking Book and Braille Center (TBBC) in 1966. TBBC is among more than 100 libraries across the United States affiliated with the National Library Service for the Blind and Physically Handicapped (NLS), part of the Library of Congress.

Know the ABCDE’s of Melanoma

It’s a fact that you already know. A mammogram can find lumps in the breast long before they are found any other way. A mammorgram only takes minutes. If you are uninsured and 40-64 years old, it’s FREE. So, what’s stopping you from calling right now?

--DEAR DR. ROACH: In 1976, I had a mole on my right calf about the size of a pencil eraser. A biopsy showed it to be melanoma, and they thought they had removed all of the cancer, but they did another surgery to be sure that I was cancer-free. I am a natural redhead and never sunbathed or used a tanning bed. It has been 38 years, and I am still cancer-free. I get a skin check from my dermatologist every six months. At the time of my diagnosis, my family doctor said that if the melanoma spreads inside the body, “the ball game is over.” Is that still true? I know melanoma can be deadly. -- W.K. ANSWER: Melanoma is the most commonly lethal skin cancer. Although there have been some exciting new developments in treating advanced melanoma, the survival rate for stage IV melanoma, where the cancer has spread to internal organs, is only 15 percent to 20 percent at five years. Fortunately, the survival rate for melanoma caught early, at stage I, is greater than 90 percent, which is why it’s so important to know the ABCDE’s of melanoma recognition: A: Asymmetry - one half looks different from the other half; B: Border - the border in a melanoma is often irregular, not a near-circle; C: Color - melanomas usually are not a uniform color, like most benign lesions. Some areas are light brown, others brown-black, others red or pale; D: Diameter - greater than 6 millimeters, the eraser on a standard pencil; E: Enlargement or Evolution - any change in color, shape, size or symptoms (including bleeding). Any of these should prompt a visit to your regular doctor or a dermatologist. As an internist, these are the features I look for, and I have a very low threshold for sending suspicious lesions to my colleagues in dermatology.

*** DEAR DR. ROACH: My son is 40 years old. After a recent stress test, he ended up with five stents in his arteries, and also was diagnosed with diabetes. His doctor ordered him not to go back to work for three or four weeks, but he went back after three days. He told the doctor that he would be more stressed if he stayed home. His job is mostly sitting down. Since he didn’t follow the advice, how will this affect his health? -- V.B. ANSWER: We don’t normally have people stay home for long periods of time after placement of stents, so I was surprised to hear that he was “ordered” (I almost never “order,” only recommend, more or less strongly) to stay home. My experience is that after receiving life-changing diagnoses, such as diabetes or coronary artery disease, it’s best to keep some habits, such as work, the same and change others. For your son, I am sure he will be recommended a proper diet for diabetes and exercise appropriate for a person with a new diagnosis of heart disease. Researchers found that the most common problem reported by divers was “barodontalgia.” More popularly known as “tooth squeeze,” barodontalgia is the experience of a squeezing sensation or pain in teeth that occurs as changing air pressure forces any pockets of air in teeth (such as those found in untreated cavities) to expand and contract. Other divers reported jaw and gum pain, which was most likely caused by clenching hard on the mouthpiece, as well as the loosening of crowns. With this in mind, those planning on scuba diving are advised to visit the dentist first to help avoid “diver’s mouth syndrome.”

Your What is a seborrheic keratosis? Thanks to education and awareness about skin cancer, more people are taking heed of skin growths and changes and visiting their dermatologists. A spot or mole on the body can be a cause for concern. A seborrheic keratosis is one such type of skin growth that is actually benign but may cause concern due to its appearance. Seborrheic keratoses, or SKs, are one of the most common noncancerous skin growths, says The Mayo Clinic. Although anyone can get them, they are more common in middle-aged to older adults. Also known as basal cell papilloma, senile warts or barnacles, these growths form as a common sign of skin aging, according to DermNet New Zealand. The American Academy of Dermatology says a seborrheic keratosis may seem worrisome because it can look like a wart, a pre-cancerous skin growth or even skin cancer. However, despite their appearance, SKs are harmless. How do SKs form? Seborrheic means “greasy” and keratosis means “thickening of the skin.” Despite the name, SKs are not known to be caused by an abundance of sebum in the skin, nor are they limited to areas of increased oil production. The exact cause of SKs isn’t known, but some doctors feel that an overproduction of skin cells can cause a thickening of the skin in certain areas. The American Osteopathic College of Dermatology offers that SKs are most likely hereditary, and they often are found on the trunk of the body and where the face meets the scalp. Sometimes seborrheic keratoses may erupt during pregnancy, following hormone replacement therapy or as a result of other medical problems.

Did You Know? Though vaccines are usually associated with childhood or foreign travel, older adults may need certain vaccinations as they age. Even those who received immunizations as youngsters may need refresher doses or vaccinations tailored specifically to the over 50 crowd. Seniors are urged to talk to their doctors about certain vaccines.

Appearance of SKs Some of these growths have a warty, rough surface, while others look like dabs of warm, brown candle wax on the skin. ADA says they can range in color from white to black, but most are tan or brown. The appearance of SKs may change as one ages, and it is even possible for them to fall off naturally over time. The Mayo Clinic says that, unless changes include becoming sore or bleeding without healing, there’s little need to act. SKs may form singularly, but often they appear in groups. Cancer risk? SKs have no relationship to skin cancer and do not pose a risk to one’s health. Unlike moles, they will not turn into melanoma. The superficial nature of SKs mean they can become irritated by clothing and grow. Treatment Unless a seborrheic keratosis is in an area that causes constant irritation, there is really no need to seek treatment. However, dermatologists can remove them quite easily if they are unsightly or uncomfortable. A topical anesthetic, cryosurgery or laser therapy is possible. Generally little scarring is produced, offers AOCD. Although SKs are harmless, their appearance may be similar to skin conditions that are not. Those who are unsure about changes to the skin should always seek the help of a licensed dermatologist.

Turn off the deck, give the kid a ride around the yard, and then put him (or her) off the tractor and away from the mower. Child safety is the business of everyone.

It’s a shame....that the "I Gotta Vent" section is twice the size of "Brighter Side." I am a woman. On Saturday morning, I went to an estate sale a few houses over from mine. Of course nothing was priced. (I find this irritating...apparently people want to sell but are too lazy to come up with a price). I asked the gentleman who was running the sale for a price on items downstairs. He rudely passed over me to assist a woman in a short dress with plenty of cleavage showing (remember, this is an estate sale, not a bar). When he could finally make time for me, I asked him for prices on an assortment of screws, nails, etc. He replied, "That's a boy thing." Really? In 2018? Women cannot buy hardware? Needless to say, I left immediately. I simply cannot fathom this man's behavior... it was rude, crude and disrespectful. This man needs to step out of his cave.

EFFICIENCY AT WORK People with multiple hobbies recognize there are many great ways to spend one’s time. But even people with plenty of ways to pass their time may find themselves wasting time each day, a phenomenon that seems to be growing. In its 2014 “Wasting Time at Work” survey, Salary.com found that 89 percent of participants acknowledged wasting time at work each day. That marked a sharp increase from a year prior, when 69 percent of workers surveyed reported wasting time at work. Thirty-one percent of workers surveyed admitted wasting roughly 30 minutes per day, while another 31 percent estimated they waste around one hour each day. Perhaps the most surprising finding from the survey was that 4 percent of participants admitted they waste four or more hours, or at least half a typically eight-hour workday, at work each day.
