Could Abdominal Migraine Cause Cyclic Vomiting?
--DEAR DR. ROACH: My son started having gastrointestinal problems in high school. Approximately three years ago, they became so intense that he couldn’t work. Last May, he finally had surgery for gastric reflux. This helped with the persistent vomiting, but he is still experiencing intractable nausea. After a year of tests and trials of every anti-nausea medication known, he finally was told that he has cyclic vomiting syndrome. He also was told, basically, “We don’t know how to treat it.” He currently is on a trial of a tricyclic antidepressant, but wasn’t given much hope that it would help. Could you suggest any possible treatment? He is only 28, and I can’t imagine him living with this and not being able to work or participate in life forever after. -- M.S. ANSWER: Cyclic vomiting syndrome is best described in children, but is certainly described in adults. It is called “cyclic” because there are periods of multiple vomiting episodes (lasting from one day to a week) interspersed with periods of completely normal health. Children have 12 cycles per year on average, while adults have four. CVS has been associated with migraine headaches, and treatment for migraines is sometimes successful in treating CVS, so it’s possible CVS represents a variant of abdominal migraine. CVS is a difficult diagnosis to make, as it can be easily confused with other causes of recurrent vomiting. Both children and adults often go a long time with incorrect diagnoses, sometimes getting surgeries, like your son did. Treatment should be administered by a clinician experienced in this condition, and unfortunately, that doesn’t seem to be the case with your son. In one series, 3 in 17 adults with CVS had a complete response to tricyclic antidepressants, but 13 had at least some response. So I would hold out more than just a little hope for some benefit. Other medications that have been tried include sumatriptan (normally used for migraine), ketorolac (a powerful anti-inflammatory medicine) and prochlorperazine (usually used for nausea, and I’m sure your son would have tried it). I would strongly suggest you contact the Cyclic Vomiting Syndrome Association to get more information and suggestions for an expert in your area, at www.cvsaonline.org. *** DEAR DR. ROACH: I have noticed that the medical profession uses a number of standard words in different ways. For example, “negative” is usually good, and “positive” often is bad news. -- N.M.G. ANSWER: It’s true that we in medicine use words differently than they are used conversationally. “Negative” and “positive” are examples: We would like the biopsy to be negative for cancer, and if the HIV test is positive, that’s not good. Doctors also use the word “complaint” to describe the patient’s chief concern; it’s not that we think people are being annoying. *** DEAR DR. ROACH: Can skim milk or oat bran cause gas? -- J.R. ANSWER: Both certainly can cause gas. Skim milk contains the milk sugar lactose, to which many people are intolerant. In mild cases, it causes some gas; severe cases can cause diarrhea. Oat bran is a good fiber source, and all good sources of fiber can cause gas, especially if taken in amounts that are greater than the person is used to. *** Dr. Roach regrets that he is unable to answer individual letters, but will incorporate them in the column whenever possible. The number of people who smoke is on the decline, as a 2018 report from the Centers for Disease Control and Prevention noted that the percentage of adults in the United States who smoke cigarettes declined from roughly 21 percent in 2005 to 15.5 percent in 2016. That decline has no doubt helped reduce overall cancer death rates, though the figures regarding smoking-related deaths are still staggering. According to the CDC, smoking accounts for more than 480,000 deaths each year in the United States. The 2018 report from the CDC indicates that men (17.5 percent) are more likely than women (13.5 percent) to smoke cigarettes. In addition, the report notes a correlation between education and the likelihood of smoking cigarettes. While smoking was lowest among people who had graduate degrees, with only five out of every 100 such adults being smokers, smoking rates were highest (40.6 percent) among adults with graduate education degree certificates, or GEDs. While people who had no high school diploma were less likely to smoke cigarettes than GED recipients, the CDC report found that smoking rates declined the more education people received.

Take Steps To Improve Your Retirement Confidence (NAPS)—A recent study by the Insured Retirement Institute found that only 25 percent of baby boomers believe they will have enough money in retirement and just 28 percent believe they did a good job financially preparing for their retirement years. This dismal view of their retirement prospects by so many aging Americans is concerning, but what is even more sobering is the study found that boomers are actually less satisfied with their finances than they were as recently as five years ago. In 2012, 41 percent of baby boomers believed they were doing a good job of financial preparation. This decline in retirement confidence has taken place in spite of healthy market conditions and rising interest rates for most retirees’ investments. Fortunately, the news doesn’t have to be so bad if seniors allow this news to serve as a wake-up call for them to take action. Here are five steps that experts say boomers can take to revisit their retirement plans now and fix things that have gone awry: 1. Reassess priorities. Take stock of your available resources and prioritize your needs vs. your wants, based on where your life is now and what is most important to you in the years ahead. 2. Raise cash. Consider options that may be available to you for boosting your savings, such as a part-time job or perhaps selling off some possessions you no longer need. 3. Generate interest. Re-evaluate where your money is sitting today and consider moving as much as you can out of cash and into interest-bearing accounts, which are paying depositors more than they have in several years. 4. Look for hidden assets. Inventory all your assets that might create cash flow, including those you may have overlooked in the past, such as a life insurance policy. A life insurance policy is your personal property and, if you no longer need or can afford it, you may be able to sell it for an immediate cash payment. 5. Leverage your house. If you own your home, consider a reverse mortgage or a home equity line of credit as a way to free up cash that you can use to pay off health care bills or other expenses. Of these five steps, the one that could produce the most immediate cash flow may be the sale of a life insurance policy, known as a life settlement. Most consumers who qualify for a life settlement are 70 or older and own a life insurance policy with a benefit of at least $100,000. Rush-Henrietta Honored for Instructional Technology Efforts Submitted by Heather Campo, Expanded and Edited by Amanda Dudek ush-Henrietta Honored for Instructional Technology Efforts District Administration magazine has chosen the Rush-Henrietta Central School District as one of its honorees in the 2018 Schools of TechXcellence program. This competitive national program recognizes school districts for outstanding technology initiatives. Rush-Henrietta is being recognized for the use of capstone projects in measuring student learning related to using digital tools. The projects are based on the International Society for Technology in Education (ISTE) standards and have been established for third, sixth and ninth grades. For their projects, students select a topic and then complete a series of tasks related to it. These tasks take several weeks and include both online and print research. Students use a variety of digital tools to create a presentation at the end of their projects. Created by District Administration magazine, and sponsored by HP and Intel, TechXcellence recognizes schools that have implemented innovative technology programs that contribute meaningfully to student or operational success. The December 2018 honorees reflect programs that demonstrate effective and replicable success. “Over the past five years, Rush-Henrietta CSD has worked toward the seamless integration of technology devices in every classroom that allows students access to the internet to obtain information, communicate and collaborate with others, and create products of their learning,” says Brad Malone, Rush-Henrietta’s director of information and communication technology. “This has included a 1:1 computing initiative which provides each student with a district-issued Chromebook.” Malone explains that in order to ensure the devices were leading to increases in learning and achievement, the district adopted the International Society for Technology in Education (ISTE) Standards for Students. Rush-Henrietta faculty created a five-year technology plan that started with the 2014-15 school year. As a result of this plan, each student in grades K-12 has a district-issued Chromebook which students in grades 4-12 are able to bring home with them after school. After the adoption of the ISTE Standards, the district focused on the areas of curriculum writing and professional development as well as implementing specific assessments called ISTE Capstones (given currently in grades 3, 6 and 9) that allow all students to demonstrate progress towards the ISTE Standards. The Rush-Henrietta capstone projects then began in the 2017-18 school year with third graders and showed that 79 percent of students rated proficient on skills necessary for meeting the ISTE standards. These include citing sources, communicating clearly and concisely, providing details about the main idea of a text, organizing information, evaluating sources, and leveraging digital tools. The results were used by library specialists and technology coaches to work with teachers on how to reinforce and improve key skills among students. “The capstones are designed to be rigorous assessments of information and technology skills that cannot be taught and mastered in one year,” says Malone. Moving forward, a significant portion of Rush-Henrietta’s technology vision is to make the previous efforts more viable and self-sustaining. RushHenrietta plans to also add additional focus on instructional shifts for all teachers to maximize the potential of the devices in their classrooms. As Malone shares, “Given the demographics of the district, we also plan to focus on ways in which equity for all students, specifically English Language Learners, can be promoted.” Locally, Rush-Henrietta is unique in its focused approach of measuring progress toward the ISTE standards district-wide in order to ensure all students have mastered the technology and information literacy skills necessary to be college and career ready. With the district’s move to 1:1 devices for students, effective use of technology for meeting learning standards is a key goal. Not only did nearly 80 percent of students achieve proficiency on the ISTE standards, but they also made a notable improvement on the latest ELA state standard test results, showing that the skills being taught have a positive impact on overall learning. However, using technology as a tool for academic success is just one way in which technology is valuable to students. While Malone is clear that emerging technologies must be used to facilitate how teachers and students interact and engage with the content and the world around them, he describes the benefits of technology education as reaching far beyond the classroom. “To reach today’s learners, teachers must move beyond traditional teaching methods and shift to more authentic, real-world experiences, ensuring students are producers instead of consumers of knowledge,” Malone explains. “In-depth technology integration includes creativity, innovation, communication, collaboration, critical thinking, and problem-solving.” Technology also helps students become more thorough, collaborative and global learners. Says Malone, “Students are able to find information on demand regardless of the topic. They can collaborate with peers and experts all over the world as well as create work intended to be viewed and reviewed

Class using Chromebooks by others instead of for their and their teachers’ eyes only.” While these technological benefits seem ideal, the question is, of course, Is it working? According to Brad Malone, the answer is yes. “If you were to walk into a classroom at Rush-Henrietta, you may see students researching a topic of their choice using databases. In another classroom you might see students collaborating on a science experiment by sharing lab data in realtime. Other places, you may see students determining which form of media to use to present their learning based on their message and audience. In third, sixth, and ninth grade you would see our students completing their ISTE Capstone Projects which assess an expansive array of digital age skills.” Malone goes on to explain that, looking toward the future, Rush-Henrietta would like to expand the current uses of technology into every classroom. Its faculty are also excited to be designing their Senior High ISTE Capstone Project which would be a truly culminating experience for their students over the course of their entire education at Rush-Henrietta.
