WEBSTER-PENFIELD-ONTARIO EDITION

FRIDAY, MARCH 10, 2017

TO YOUR GOOD HEALTH By Keith Roach, M.D.

Are Dermatologists ‘Zapping’ for Dollars? DEAR DR. ROACH: I am 73 years old and lived on a boat in my 40s. Now, whenever I go to the dermatologist (two to three times each year), he “zaps” me about 20 times and burns off “cancer cells” or maybe “potential cancer cells.” Some of them have been cancer and were healed with MOHS procedure. While I am grateful for the care he gives me, I also sometimes feel that he zaps because each zap means more money. Can you please clarify this for me? -- G.B. ANSWER: I often get questions like this, where a patient has a concern that their doctor is performing unnecessary medical care for the purpose of enriching himself or herself. I always have trouble believing it, since I can’t really imagine it, but I recently read about an oncologist pleading guilty to treating with chemotherapy people who didn’t really have cancer, which shakes my faith in my profession (and colleagues). Still, I truly believe that, in your case and probably more than 99 percent of the time, the doctor is being extra cautious to treat you for the sole purpose of reducing your likelihood of developing an invasive skin cancer later on. *** DEAR DR. ROACH: My mother lives in Florida. Her major discomfort is fatigue and no energy. I have asked her doctor to please give her a vitamin B-12 injection, even though her B-12, thyroid function, vitamin D and blood count are all in the normal range. Her doctor does not believe in B-12 injections. Do you think it might help? -- J.L. ANSWER: Vitamin B-12 is necessary for proper function in many tissues, but it is critical for blood cell production and neurologic function. B-12 deficiency causes an unmistakable anemia (called megaloblastic anemia) and can cause neurologic and psychiatric disturbances even in some people with no anemia. The blood test for B-12 is quite reliable. The major cause of B-12 deficiency is an autoimmune disease that prevents the stomach from making intrinsic factor, which is necessary for efficient absorption of B-12. B-12 can be absorbed orally by people with pernicious anemia by taking very high doses, such as 1 mg. This gives as good a replacement as injection. It was common 50 years ago to give B-12 injections as a “tonic.” However, it offers no advantages for people with normal B-12 levels beyond the placebo response. As I have noted many times, the placebo response can be very powerful, and 1 mg of B-12 orally is very safe. *** DEAR DR. ROACH: I have heard that drinking water with your meal is bad for you. Is this true? And if so, why? I have a dry mouth due to cancer treatment and must have water with everything I eat. Is something bad going to happen to me? I am otherwise healthy. -- R.G. ANSWER: It’s not true at all. Water is, generally speaking, the healthiest drink. It is possible to drink too much water, but it’s very hard to do so unless you are taking medications that affect your kidneys or you have excess anti-diuretic hormone, an unusual medical condition. Keep drinking your water. *** Dr. Roach regrets that he is unable to answer individual letters, but will incorporate them in the column whenever possible. Readers may email questions to ToYourGoodHealth@med.cornell.edu. Appointment companions are a smart move Visits to a physician are just one step people make in an effort to improve their well-being. While it’s important that everyone visit their physicians at least once a year for a checkup, seniors may need to see their physicians more frequently than other age groups. When visiting the doctor, it is easy for anyone to miss important components of what the doctor is saying, and it’s just as easy to misunderstand certain instructions or medication information. Seniors who bring companions along to appointments with their physicians can reduce the risk of misunderstanding advice or diagnoses given by their doctors. Data from U.S. News and World Report states that about one-third of seniors still living on their own take a companion with them to their routine doctor’s office visits. Companions are typically spouses, but they can include children or other family members as well. Patients may find there are many advantages to bringing someone along to an appointment. And companions may want to learn more about patients’ goals at each appointment prior to going along so they can prepare and know how to help during the appointment. Listen People tend to forget at least half of what they hear in the doctor’s office, says the Archives of Internal Medicine. This tendency may be increased when patients are nervous about the potential outcome of their visits. Bringing a companion along means that both people are actively listening. Together, the information they’ve heard can combine to provide a full account of the visit.

WEBSTER-PENFIELD-ONTARIO EDITION

FRIDAY, MARCH 10, 2017

Taking notes Companions can jot down important notes about the appointment, such as dates and times for follow-up visits, medication advice and any other instructions that may be forgotten once the patient leaves the office. Companions can later translate the jargonheavy language of a physician into an understandable language the patient can fully understand. Medical history Sometimes a companion can be a useful resource, calling a doctor’s attention to a patient’s prior hospitalizations and illnesses. Companions can even call attention to any medications the patient is currently taking. Serving as an advocate Very often a companion can operate as a patient advocate, clarifying questions or getting further information out of a doctor if the patient is hesitant to ask. If the information isn’t clear, the companion can raise red flags or ask to have the instructions put in a different light. Translation Sometimes patients need companions who speak their native languages when their physicians do not. People are learning that bringing a family member or friend along to doctors’ appointments can be a smart way to make the most of doctor/patient interactions.

ST. PATRICK’S DAY FUN FOR YOUNGSTERS Parades are fun for people of all ages. Local community parades may encourage families to march, and parents should take advantage of such opportunities. But even large parades in big cities can be fun for youngsters, who can dress up in green and enjoy all the pomp and circumstance of high school bands, bagpipers, Irish dance groups, and additional participants.

MOMENTS IN TIME • The History Channel • On March 20, 1995, several packages of deadly sarin gas are set off by the Aum Shinrikyo cult in the Tokyo subway system, killing 12 people and injuring over 5,000. Sarin originally was developed in 1938 in Germany as a pesticide. SIGHTS, SOUNDS AND SYMBOLS OF ST. PATRICK’S DAY The color green is everywhere come St. Patrick’s Day, but celebrants may be surprised to learn that blue is the color that was once most affiliated with St. Patrick himself. Numerous paintings depict St. Patrick wearing blue vestments. But green is the color now most instantly associated with Ireland, which is widely referred to as the “Emerald Isle.” Ireland’s lush countryside and rolling green hills attract millions of visitors each year. The color green can be found just about everywhere on St. Patrick’s Day, even if Patrick himself was partial to blue.

We are a group of Traffic Control/Crossing Guards hired September 2016 at Victor High School. We have gotten a lot of thank yous from faculty, parents, teachers and students alike. As the weather changes it does get colder as winter sets in. At Christmas time the faculty, parents and students gave us hand warmers, hot chocolate, donuts and other gift cards to show their appreciation. Well, the group of us, about 12 guards, wish to thank the faculty and parents for showing us as a group their appreciation. We want to send to all who appreciate us to a Big Thank You! Victor Crossing Guards

A couple weeks ago my granddaughter was to leave for, college on a Sunday. Her car started to make a noise; she stopped by Zirbel’s on Saturday, they weren’t open; however,

there was a man there getting ready to leave. He took her car in for a quick fix, checked oil and put air in the tires and sent her on her way. You can’t beat customer service like that. Thank you to whomever at Zirbel’s. Last night we had a problem with our electricity and my husband called our utility provider (National Grid) who sent out crews to assess the situation. These men were not only extremely polite, but were working in frigid temperatures, until after three in the morning. They were determined to keep working until the problem was solved. Yes, perhaps they do make good money, but no amount would be worth it to me to be hanging on to the top of a pole in the freezing temps! I think that too many times we overlook the work that these men and women do. University Eye employees went the extra mile demonstrating that they are committed to serving the needs of their clients! Thank you for taking the time to resolve a difficult circumstance for our dad! Your thoughtfulness had a positive impact on our family and will be remembered!!

To those who visit local cemetery after hours. Please be considerate and visit only during posted visiting hours. We know you are grieving, and we, as neighbors, keep watch for any suspicious activity. If you are visiting after 8pm, you are not following the rules. We know who the “regulars” are, and those who may have other intentions for being back there...beyond 8pm and into the wee hours of the morning. The area is patrolled by local police, and we have been instructed to report any suspicious activity. Please be considerate, to those who have loved ones buried there, and to us neighboring the cemetery. I recently had to have an ambulance ride to the local hospital, staffed by unpaid volunteers. The dispatcher also sent a paramedic from a local town. Medicare paid for the expense of the ambulance thankfully. A few weeks later I received a bill for the paramedic service - $1700 (for

30 minutes of time). As a retired Emergency nurse I was appalled at the amount, especially since he was unable to start an IV, did a minimal history and seemed to be very unhappy to be there at 3am. The amount is in excess of my retirement income by several hundred dollars! Everyone please be aware that ‘ALS care’ may not be covered by Medicare and is an out of pocket expense whether the care is good or not.

While at a local store, a customer had a shopping cart with about 20 items sectioned into 7 clusters. I was VERY patient but being the only 2 people in the store and after you looked right at me while I was holding my 1 item the least you could have said was “want to go ahead of me?” I waited for 10 minutes while you made your returns, used your coupons and confused the cashier so much that she gave me a “help me” look. That’s when I just gently said, “do you think I can just have my 1 item checked out?” to which she yelled wait your **** turn. Moral of the story.....she was in her 70’s and I’m in my 50’s....so much for respecting your elders, huh?

BAGPIPES ARE AN IMPORTANT PART OF IRISH MUSIC TRADITION Celtic culture is rife with tradition. Traditional music is one identifying factor of the people who reside in countries awash in Celtic heritage, such as Ireland and Scotland. The strains of bagpipes can be heard during various celebrations and memorials. Bagpipes can produce uplifting and upbeat tunes or somber notes ideal for moments of reflection. Bagpipes are in a class of musical instruments known as aerophones. They produce sound through reeds from a constant reservoir of air trapped in a bag. Bagpipes are comprised of an air supply; a bag that holds the air; a chanter, which looks similar to a recorder and has finger holes to produce the melody; and one or more drones, which produce the sustained pitch. In bagpipes, the drones are the pipes placed over the musicians’ shoulders. Since the 14th century, bagpipes have appeared in European art and literature. Evidence of bagpipes in Ireland can be traced back to 1581, when John Derrick’s “The Image of Ireland” clearly illustrates a bagpiper. However, actual examples of bagpipes found before the 18th century are rare. What some music aficionados may not know is that not all bagpipes are the same, and Irish bagpipes have distinctions that set them apart from other varieties. Irish bagpipes, or uilleann pipes, differ from other types of bagpipes, including Scottish Highland pipes. Uilleann is derived from the Irishlanguage term piobai uilleann, which means, “pipes of the elbow.” A full set of uilleann pipes has a chanter, three drones and three keyed regulators. A half set will have no regulators. The pipes are tuned in the key of D, and unlike many other pipes, the reed will over blow. This gives uilleann pipes a range of two octaves. The chanter also can produce sharps and flats. In addition, regulators give them extra dimension of sound. Irish pipes have a softer sound than other bagpipes, enabling them to be played indoors. Although bagpipes have been integral to ancient Irish history, they are still alive and well today. Many bagpipers still proudly play them throughout Europe and in North America. The resonating sound of the pipes can be heard during parades and various celebrations, such as on St. Patrick’s Day.

ENJOY IRISH COFFEE THIS ST. PATRICK’S DAY St. Patrick’s Day is synonymous with many things, most notably the jovial atmosphere of the various events celebrating both the life of St. Patrick and Irish culture. New York City’s St. Patrick’s Day celebrations are known for the Big Apple’s impressive parade up Fifth Avenue, while few who have partaken of Patty’s Day festivities in Chicago can forget the green Chicago River. Celebrations may vary from city to city, but it’s a good bet that celebrants will hear some traditional Irish music, enjoy a pint of Guinness and/or join the masses wearing green wherever they end up this St. Patrick’s Day. Drinking Irish coffee is another St. Patrick’s Day tradition many revelers enjoy. Few may indulge in Irish coffee throughout the year, but this special concoction can be a great way to warm up after attending a St. Patrick’s Day parade in the often chilly March temperatures. Cocktails made with coffee date back quite far, but Irish coffee does not have to go back too far to trace its history. In the mid-20th century, chef Joe Sheridan was working in Foynes, County Limerick when a group of United States-bound travelers found themselves stranded due to inclement weather that grounded their plane. Whether he was trying to warm them up or wake them up after their ordeal, Sheridan served the passengers coffee with a little bit of Irish whiskey mixed in. When the group inquired as to what they were drinking, Sheridan reportedly told them, “that’s Irish coffee.”

Job Hunting Tips For M en And Women Over 50 Emphasize your age. BE SAFE, HEALTHY AND HAPPY IN THE WORKPLACE WHILE PREGNANT Under the Family and Medical Leave Act, women in the United States are entitled to 12 weeks maternity leave. Women can choose to use a portion of that maternity leave even before their children are born if physical limitations are making it challenging for them to continue working. Women should put their health and the safety of their unborn children ahead of their responsibilities at work. In addition, women should not hesitate to speak with their employers about their comfort levels while pregnant. If an issue arises, speak with your boss, who deserves the benefit of the doubt. In some instances, breastfeeding also is protected by law at job sites or at the office. It is best for women to familiarize themselves with breastfeeding policies ahead of time so they can make the proper accommodations. Pregnant women should discuss with their doctors which work activities are permissable and which should be avoided at particular times in the pregnancy. Every effort should be made to avoid toxic substances, and pregnant women should not lift heavy items or engage in any potentially risky physical activity. For more information about workplace rights, visit the Equal Employment Opportunity Commission at www.eeoc.gov.
