CHOCOLATE CARDAMOM TRUFFLE CAKE Serves 10 to 12 Amaretti Base 5 ounces amaretti 1/2 stick (4 tablespoons) unsalted butter 5 tablespoons sugar Cardamom Truffle Filling 2 1/3 cups heavy cream 4 to 6 green cardamom pods, lightly bruised to open them 25 ounces dark chocolate (60-70% cocoa solids), grated 1/2 stick (4 tablespoons) unsalted butter, cubed 4 tablespoons chocolate liqueur Cardamom Cocoa Sugar 3 green cardamom pods 1/4 cup sugar 1/2 cup unsweetened cocoa 1 10-inch cake pan with a removable base, base and sides lined with nonstick parchment paper To make the amaretti base, finely crush the amaretti in a food processor or put them in a plastic bag and crush with a rolling pin. Melt the butter and sugar in a pan over low heat, then stir in the amaretti crumbs. Press the mixture evenly over the base of the prepared cake pan and refrigerate for 20 minutes. To make the cardamom truffle filling, put the cream and cardamom pods in a pan and heat until almost boiling. Remove from the heat and set aside to infuse for 20 minutes. Meanwhile, put the chocolate and butter in a saucepan. Strain the infused cream through a fine strainer onto the chocolate (the cream will be warm enough to melt the chocolate). If it doesn’t melt, warm over very low heat, stirring occasionally, but do not allow to boil. Stir in the liqueur, then set aside until almost cold, but not set. With an electric mixer, beat the chocolate mixture for a couple of minutes - this will give the cake a lovely airy texture. Pour onto the amaretti base. Let cool, then cover and refrigerate for at least 6 hours, or up to 48 hours before serving. To make the cardamom cocoa sugar, remove the black seeds from the papery cardamom pods and put the seeds in a spice grinder with the sugar and cocoa. Grind to a powder. Carefully remove the cake from the pan and peel off the lining paper. Set on a chilled serving plate and sift the cardamom cocoa sugar evenly over the top. Serve immediately, cut into very thin slices.

Platelet Donations Are Perfectly Safe

--DEAR DR. ROACH: I am writing about platelet donations. As a way of giving back, I donate platelets. I was wondering if there is a time that a person should stop donating, based on age or difficulty. I am 61 years old, and the past three times I’ve donated, I felt uncomfortable in some way. Either my arm or wrist hurt, or I had a feeling of malaise during the donating process. One time, I was told that my platelet count was on the low side, but it was not any danger to my well-being. I feel well immediately afterward and do not have health issues, so my concern is whether this is somehow detrimental to my own health as I age. -- S.B. ANSWER: Platelet donation generally is a very well-tolerated procedure. The platelets are removed from the donor’s blood via a process called apheresis; a centrifuge separates the blood into four components: plasma (containing proteins), platelets, white blood cells and red blood cells. For platelet donors, just the platelets are removed, and the rest of the blood components are returned to the donor. In order to keep the blood from clotting inside the machine, sodium citrate is added to the apheresis machine. This reduces the calcium, which is needed for blood clotting. However, some people can develop symptoms of low calcium during the apheresis. This can be treated by slowing the rate of donation, or by taking some oral calcium before donating. More-severe cases might require giving intravenous calcium to the patient via another IV catheter. The Food and Drug Administration recommends that people make no more than 24 donations per year, but there is good evidence that platelet donation is safe. I congratulate you on being a communityminded citizen, and I can reassure you that you have very little short-term risk and essentially zero long-term risk from donating platelets, whatever your age. Donate as often as is comfortable, and try taking a calcium supplement before donating. *** DEAR DR. ROACH: I have taken 5 mg zolpidem (Ambien) nightly for about three years. My doctor seems to think this is acceptable. I sleep five to six hours with this aid and function normally in the morning. Recently I was told that this drug has a cumulative effect on the body. Would you verify this? Over-thecounter sleep aids have no effect, and this drug seems to be the only way I can sleep. -- D.M. ANSWER: Zolpidem and its cousins, eszopiclone (Lunesta) and zaleplon (Sonata), work for a relatively short time in the body before they are metabolized, so they do not build up in the body. They are mostly gone in the morning, and grogginess is not a problem for most people. However, people often develop a dependence on these medications; it becomes difficult to sleep without them. More importantly, they do increase the risk of falls and auto accidents, especially when used every day and in people over 65. I don’t recommend them for the long term, and I have referred my patients who can’t sleep without them to a sleep specialist, who sometimes (but not always) is able to find a better solution than daily Ambien. *** Dr. Roach regrets that he is unable to answer individual letters, but will incorporate them in the column whenever possible. FAIRPORT David Marsh, A Life of Courage Born in 1940, David Marsh contracted rheumatic fever at age six leading to many hospital stays and a life bound to a wheelchair. However, due to the compassion of the Fairport community, he was able to realize many of his dreams before his 1964 death. Recently, his sister donated many of his favorite items to the Perinton Historical Society and are on exhibit through 2018 at the Fairport Museum (18 Perrin St.), helping visitors remember and honor his short life. Rush Rounds Out Its Bicentennial Year with a Clock Dedication & Tree Lighting Ceremony By Amanda Dudek

On Saturday, December 1st, Rush, Monroe County’s southernmost, suburban community, will conclude its year-long bicentennial festivities with a Clock Dedication and Tree Lighting Ceremony set to begin at 6:30 pm. The residents of Rush, a village with a charming, rustic, small-town atmosphere, will hold its tree lighting and Christmas caroling at the gazebo in the Rush Veterans Park which celebrated its own rededication just this past September. The Rush-Henrietta Spectrum Singers of RushHenrietta High School will lead guests in singing these Christmas carols. After the caroling, visitors are invited to walk across the drive to the new clock in the Commons across from Montesano’s Pizza. At this time, this new Bicentennial Street Clock will be officially dedicated in recognition of Rush’s 200th anniversary. The Bicentennial Street Clock, manufactured by the Verdin Clock Company, is an elegant, two-sided, black-and-gold, commemorative timepiece, marking across its top the year in which the town was established. During this ceremony, a time capsule will be buried at the base of the clock, with plans to open the capsule after 50 years. The hope is that the future residents of Rush will unearth the time capsule and enjoy what will then be a window to the past. This time capsule will contain memorabilia from the town’s Bicentennial, including the bicentennial book, entitled, Town of Rush, NY - Bicentennial Commemorative History 1818-2018. Photos from this year’s events will be included as well as town newsletters

Following the clock dedication and time capsule, all attending are welcome to stop across the road and visit the Rush Creekside Inn for cookies, hot chocolate and tea, compliments of the Rush Bicentennial Committee. Adult beverages will, of course, be available for purchase from the Creekside’s bar. The Creekside Inn will also be hosting a live band that evening, and music will begin at 8 pm. The Creekside Inn will also hold an Ugly Sweater Contest that evening, and guests are encouraged to join in the fun. Please join us for the closing event of Rush’s Bicentennial year!

There has been a massive growth in fatherhood involvement over decades—fathers now spend nearly triple the amount of time with their children than fathers did in the 1960s.2 What Dads are Already Doing Recent research suggests fathers are already very involved:

Finding work after 50 Unemployment isn’t easy for anyone, regardless of their age. But unemployed men and women over 50 may find it especially difficult to find work. Whether it’s a byproduct of age-related discrimination or any of a host of additional variables, jobless older workers often struggle to find work. In a 2016 analysis of government figures, the Schwartz Center for Economic Policy Analysis at the New School estimated that the jobless rate for workers 55 and older in August of 2016, six years after the Great Recession, was nearly 9 percent. At the time, the national jobless rate hovered around 5 percent. Unemployed men and women over 50 who are struggling to find work can consider the following strategies as they look to rejoin the workforce. • Revisit your résumé. Unemployed men and women over 50 have no doubt updated their résumés to reflect their most recent professional experience. But they may need to trim some of the fat in regard to their work life 10 or more years ago. Today’s hiring managers may only be concerned with recent experience showcase their enthusiasm about working with and learning that illustrates skillsets that are relevant to today’s jobs. The Scottsville Fire Department brought out a fire truck and handed out treats for Halloween in Scottsville. I can't thank the Department enough for their generosity and the time spent volunteering within our community. They made the smiles of children in Scottsville even bigger! Today I was grocery shopping in Brockport, in a handicapped scooter, due to recent surgery. A wonderful, kind lady ahead of

Perhaps the child was ill, which is why (s)he was crying, and they were waiting that long in the store for a prescription? No, I am not this child's mother, but I have been in that position before! Also, you complain that she should teach her child how to behave, but did you ever stop to think that was exactly what she was doing? Yes, it's very inconvenient & irritating to hear a toddler scream. But by not giving in to
me at checkout put all my groceries out of the cart for me. It was so kind of her! Thank you, Sweet Lady! I have voted in suburban Western New York, Buffalo and Rochester areas, for over 30 years. The election workers have always been helpful and efficient and the polling places convenient and welcoming. I've never had to wait more than a few minutes to vote. The ballots are easy to understand and machines in good working order. I've always been confident my vote is properly counted. You read of hours long waits and malfunctioning machines in other parts of the country. Not
here. The Monroe County Board of Elections seems to be doing things right - just try their website; it's contains everything you need to know about a current election. Now if only I could convert those voters that don't agree with me! Thank you to all who helped me the evening before Thanksgiving in Livonia. I had taken a nasty fall on a sidewalk. People were covering me with their coats and praying with me while we waited for the ambulance. Thank you to Avon ambulance for being available to take me to the hospital. I went through the [car wash]. Halfway through, I heard an odd noise and I assumed it was water or brushes. I arrived home and saw an 8-inch scratch on my driver’s door. I went back told them about it. They told me they would review the video they have for every car that comes through. The next I went back and they told me they reviewed the video and the scratch was there when I entered the wash. They do not have a camera at the end of the wash, only at the beginning. Be careful at the car wash as they will deny any damage done by them. I stopped going because of this incident.
