Warning Signs Your Aging Parents Need More Help (NAPS)—Home care agencies often see a surge in calls in late December and early January. Why? Many grown children head home for the holidays and notice their aging parents need some help. Here are ways to tell: Take The Sniff Test - Breathe in that holiday hug. If you notice unclean clothes or an uncharacteristic body odor, that could signal that a parent is having trouble bathing, maintaining personal hygiene or keeping up with household chores such as laundry. Next, follow your nose to the kitchen. Open the fridge. You’ll notice pretty quickly if there’s spoiled food—but your parents may not. The sense of smell diminishes with age, expecially after 70. Many seniors can’t detect unpleasant odors that would normally tell them it’s time to toss last week’s meat loaf. If the sniff test reveals some olfactory oddities, it might not just be “normal aging.” Diminished sense of smell can be an early indicator of Alzheimer’s and Parkinson’s diseases. If you sense something isn’t quite right, help Mom or Dad to set up a doctor’s visit to see if there’s something else going on. Have A Look Around - Your childhood home may look the same as ever—but it may not work as well for older adults. As people age, their environment typically needs simple upgrades to keep them safe. You can significantly reduce fall risks by taping down throw rugs, removing extra furniture, securing electrical cords out of pathways and adding grab bars in the bathroom. Pay attention to clutter as well. Stacks of unopened mail, broken appliances or drooping houseplants may also signal a need for help. Being unable to manage everyday activities can be a symptom of depression or of cognitive impairment, which often presents like depression. If you see any of the above warning signs, you may want to arrange for your parent to see a doctor and for family members or professional caregivers to stop by and help with projects and chores. Just knowing your parents have help and resources can relieve worry, both yours and theirs. Listen Up - Is the TV or radio volume blaring? Are conversations repeated? Does Mom keep asking “what?” or just smiling and nodding instead of participating? Don’t brush it off. Difficulty following conversations may be a sign of hearing loss, cognitive impairment or both. Seniors who begin losing their hearing are at greater risk of developing dementia over time since hearing is a brain function. Sometimes, hearing aids stimulate the sounds needed for the brain to restore organization and correctly process sounds. Making Sense Of Your Senses - Talking to parents about warning signs you’ve noticed isn’t fun. But do it anyway. Let them know you want to be involved to help them stay independent for as long as possible. Even if you live far away there’s a lot you can do, such as scheduling appointments and finding help with transportation, chores and projects. Talk to nearby family members about how they can help out or set up a visit with a home care agency that can provide ongoing support. Fear of losing independence keeps a lot of seniors tight lipped.

The most likely place for cavities to develop in children is on the chewing surfaces of their back teeth, where the tiny grooves known as “pits and fissures” trap bacteria and food particles. Because toothbrush bristles don’t always effectively clean down to the bottoms of these crevices, children stand to benefit greatly from the application of an invisible plastic coating that smooths uneven chewing surfaces and seals out food and bacteria. Sealants are easy to apply and cost far less than the dental fillings they prevent.

Is Big and Tall Patient Getting Petite-Size Meds?
--DEAR DR. ROACH: I am 71 years old, stand 6 feet, 5 inches tall and weigh 325 pounds. The “average” person is half my size, yet when I get medication for various ailments (e.g., gout, pain, flu shots, etc.) I get the standard dose. I assume I have more physical mass as well as a larger blood supply, which leads me to the conclusion that I am receiving a smaller concentration of medicine than the typical patient. Am I correct? -- M.K. ANSWER: All medicines have concentrations at which they work optimally. Too low a dose and they aren’t effective, and too high a dose increases risk of toxicity. Some drugs have a very small range that’s both effective and nontoxic, and these are referred to as having a “narrow toxic to therapeutic ratio.” Others have a very large range where they are effective and nontoxic, such as most over-the-counter pain medicines and flu shots. Penicillin has a very wide toxic to therapeutic ratio, but I use that example precisely because some people will have an allergic reaction. There is no drug -- prescribed, over-the-counter or supplement -that is either perfectly safe while being effective, or perfectly effective while being safe, for every person. The concentration of the drug in the appropriate area of the body (sometimes blood, sometimes the affected organ, sometimes urine) may depend on the dose as well as your characteristics. Despite being much larger than average, your blood volume is probably only a little above the average, since the increased bone mass, muscle mass and, in most people your size, fat mass are disproportionate to the blood. Some medicines go to the blood, others to “lean body mass” and still others preferentially to fat. So while you are correct that you are getting a smaller concentration of medicine, that’s still OK most of the time. For medications that need to be dosed precisely, those with a narrow toxic to therapeutic ratio (such as some antibiotics, heart medication and chemotherapy), the dosing is based on height, weight, age and sometimes kidney and liver function. *** DEAR DR. ROACH: I am 80 years old, in good health with no cancer in the family. Up to now, I have always had regular Pap smears and mammograms. Now I am aware that there are different thoughts as to whether those tests are needed at my age. What is your advice? -- J.G. ANSWER: For Pap smears, there is very good evidence that women who have never had any problems with their Pap smears by age 65 are very unlikely to ever develop any, and I agree with the recommendation by the U.S. Preventive Services Task Force to stop them at age 65. Mammograms are more controversial, but my personal practice is to continue to recommend them for most women until about age 75. I stop recommending mammograms to women when they have an underlying medical condition that makes it unlikely that they will receive benefit from continued screening. Deciding when that is takes some experience and a lot of discussion with the patient and sometimes her family. There is no absolute age cutoff for mammograms, and getting a mammogram on a healthy 80-year-old might make sense. Getting a mammogram on a woman with incurable lung cancer, for example, does not. *** Dr. Roach regrets that he is unable to answer individual letters, but will incorporate them in the column whenever possible. Is anybody upset about a recent change to our [local restaurant] here in our [local town]? The beautiful landscaping was destroyed and the building was made to have a cold appearance. Our [local town] certainly deserved better than this monstrosity. Recently my friend and I went to lunch at a small restaurant in our [local town]. The room was crowded, and we were all very close. In came a woman with a severe bronchial infection and sat
at the table right next to us. She proceeded to cough her way through the entire lunch, disregarding all the patrons around her. Both of us now have severe colds. Ironically, we are both infectious disease nurses. Please stay home and keep your germs to yourself. To the driver of a purple vehicle: Slow down before you kill someone! After the town hall meeting addressing public water access, I left disgusted at my “neighbors” and local farmer’s negative and uninformed opinions. With climate changing, many residential wells don’t produce water like they used to. Can the farm’s water be next? Then what? Public water causes autism due
to medical waste in the lakes? Now there’s a real reason to be against it. The air we breathe is polluted so stop breathing. If you prefer your washed clothes to be gray or dingy or nasty teeth due to drinking your well water so be it. I don’t. My well is good, but I want better! I want my neighbors’ water to be better! I would gladly give up vices to help my neighbor. “Move back to the city” What a GREAT answer! It may not cost you as much if you do your homework. I am so upset that someone can hit my car and take off without reporting it or leaving a note. They had to know they hit my car because the bumper and the hatchback is dented; also, the taillight was broken. I know that accidents happen, but to just leave is totally WRONG!
Brighter Side The Memorial Day Detachment # 468 of the Marine Corps League would like to thank everyone who donated to the Toys for Tots campaign this year. The generosity of everyone who donated or provided a space for our collection boxes enabled us to distribute over 6200 toys to children who might otherwise not have received any toys for Christmas. We appreciate the agencies and individuals who helped to distribute the toys to the individuals who were in need. We look forward to working with everyone again next year. me at no charge. Having a small infant and a husband away on business, this act of kindness really meant a lot to me. We went to KRONY’S on Jan. 4th. HOW CHOCOLATE BECAME TIED TO VALENTINE’S DAY Heart-shaped boxes filled with decadent treats are coveted gifts on Valentine’s Day. Chocolate lovers typically have a favorite type of chocolate, whether it’s creamy filled truffles or chocolate pieces with fruit or nut fillings. The tradition of gifting chocolate is anything but new. Chocolate and other sweet treats have been offered for centuries as prized gifts. Even ancient Aztecs and Mayans celebrated chocolate and saw it as a hot commodity. Drinks made of cacao beans would be given as presents to people of high status. Chocolate also would be offered to the gods as a token of appreciation. Cacao beans were even used as a form of currency at one point.

I
t’s a new opportunity for Junior and Senior High School students at ByronBergen. This past fall, their High School science program garnered a $25,000 America’s Farmers Grow Rural Education Grant. On Friday, January 4, 2019, Agriculture Teacher Jeffrey Parnapy, a Cornell graduate, received a muchanticipated delivery: five modest shipping boxes containing $25,000 worth of stateof-the-art equipment to support the agriculture, biology, chemistry, and physics programs. For Parnapy, who wrote the grant proposal in collaboration with his science colleagues Michael Conine, Briana DelVecchio, Jenifer Faro, Peter Spence, and Terry Vick, it was a dream come true. “My colleagues and I are ready to implement these new tools directly into our coursework,” said Parnapy. “We have been anticipating their arrival and it’s very exciting to be able to bring a new level of technology to our students.” Jeff Parnapy could not be more pleased to see the ag classes making a comeback at Byron-Bergen. “Next fall will be only the third year in which we have offered ag classes since the 1970s,” said Parnapy. Byron-Bergen is doing a reboot of agricultural education that is happening in many other districts, as well. “Pavilion, Oakfield-Alabama; there is a resurgence.” Parnapy had taught agriculture at the secondary level for several years before taking a position teaching other science classes at Byron-Bergen in the Fall of 2000. It wasn’t until 2018, however, that he was asked to teach an Introduction to Agriculture class thanks to his certification in Agricultural Education. “I think people are realizing that we need young people with an interest in agriculture, young people who will go into this business,” said Parnapy. “Everyone needs to eat!” Parnapy went on to share why he believes agricultural education is so important for today’s youth, whether they enter the field or not. “We all must realize that each and every one of us is going to be a consumer of agricultural products. If we are going to raise smart consumers, then we have to make sure they understand the whole production-to-supply chain of agriculture. We also are all citizens as are the children we are instructing, so we have to educate ourselves and our children about agriculture and our food system so that we are all more informed when we vote.” Parnapy clarified that since those for whom we vote are responsible for making ag policy decisions for us on such issues as conservancy and Right to Farm, it is important to educate future generations about agriculture so that they will understand how these decisions will affect them.

The complete list of new equipment includes: 13 Lab Quest interfaces, 1 anemometer, 1 barometer, 12 CO2 gas sensors, 9 conductivity sensors, 12 dual range force sensors, 5 dissolved oxygen probes, 4 flow rate sensors, 10 energy sensors, 12 infrared thermometers, 12 motion detectors, 24 pH sensors, 1 sound level meter, 24 temperature probes, 4 drop counters, 12 photogates, 3 spectrophotometers, and 4 Lab Quest interface charging stations. “Just being able to have the kids handle tech that is different from what they have been exposed to before is so important,” says Parnapy. “When they graduate and further their education or training, and they are presented with a new piece of equipment, at least they are likely to have some idea of what it is, what it does and where to start.” While this is a field-specific skill, Jeff Parnapy states that it is also important for young people to learn the value of the expensive technology, that individual items can cost hundreds of dollars and were earned through the grant writing efforts and perseverance of their teachers. Therefore, the equipment should be cared for and respected. “Things do not just show up,” he says. “Kids are walking around with expensive tech (like cellphones) and sometimes, do not understand that it is costly to replace these things. We are not just teaching skills, but a mindset, as well, for how young people have to be for things to work out when they enter into a career. This includes both hard and soft skills.” Parnapy explains that teaching these skills, much like the equipment received, extend beyond Byron-Bergen’s Agriculture classes.

I knew if I was going to write a grant, a lot of people should be able to benefit from it. I needed this equipment, such as the Lab Quest II interfaces, for the Agriculture curriculum which I was going to teach, and other science classes would need to benefit, also for a grant to be approved; 7th-12th in our case. So, when I was going through and writing this grant, I made sure I talked to all the science teachers, asking what it is they could use that we could include in this grant. When I started totaling up what their students needed with what my students needed, I thought, well, I guess we will apply for $25,000!” said Parnapy. He further explained that all the equipment is multifunctional, able to measure various things such as Oxygen, CO2, wind speed and infrared. “For example,” Parnapy continued, “a 7th grade student could measure temperature and pH. An 8th grade student could work with physics and motion.” When looking to the future, Jeff Parnapy is optimistic and already sees progress and enthusiasm. “Agriculture was our most subscribed elective last year,” he enthused. “Last year, we had one section. This year, we have two. Next year, we will not only have Introduction to Agriculture but also an Animal Science class.” Parnapy describes the upcoming Animal Science elective as a “good starter” for someone who thinks they may be interested but is not yet ready to commit to a BOCES level class. “Another thing I am really excited about is that we are going to continue to build strong leaders for the future. That is why there are FFA (Future Farmers of America) chapters like the one I advise here,” says Parnapy. “We practice parliamentary procedure, employment skills, public speaking and leadership and career development. We also do agriculturally relevant fundraisers. For example, we will be having a strawberry sale in the middle of March. Students also visit Camp Oswegatchie, the State FFA Camp, to gain a deeper appreciation for nature and network with students from other FFA chapters.” This project was made possible by local farmers and America’s Farmers Grow Rural Education, sponsored by the Monsanto Fund. To address this need, DAV has published “The Veteran Advantage: DAV Guide to Hiring and Retaining Veterans with Disabilities.” This free, comprehensive guide is the result of more than four years of study about what the veteran community contributes to the workforce and how hiring veterans, especially disabled ones, can improve a company’s bottom line. The 36-page publication, which can be downloaded at http://dav.la/jobs, offers best practices and helpful tools for employers. But it goes further, helping to correct misperceptions and inspire more organizations to hire from this talent pool. Content includes statistics about veterans with disabilities, in-depth onboarding and retention strategies, a practical checklist for employers, and explanations of financial incentives, tax credits and other support. The Guide also highlights powerful testimonials of veterans thriving in successful careers, and case studies from employers who benefit from having disabled veterans in their workplace. Nationally respected companies such as First Data and USAA apply the findings within their own internal human resources departments and hiring teams.
