--DEAR DR. ROACH: What effect, if any, does alcohol consumption have on swelling in the legs, ankles and feet? I drink only when I go out for dinner or have guests, typically a cocktail and a glass of wine; no swelling in those instances. However, I noticed that after a few days on vacation, when I drank more than two glasses of wine every night, my lower legs, ankles and feet began to swell. Once I returned home and resumed no daily alcohol consumption, the swelling abated. So, is there a connection? -- B.A. ANSWER: The physiology of alcohol is complex, with potentially adverse effects on the heart, the liver and on secretion of anti-diuretic hormone. All of these can affect swelling. However, it is entirely possible, and probably more likely, that it is a combination of travel and increased sodium intake that is responsible for your most recent swelling. Even in healthy young volunteers, alcohol immediately reduces the ability of the heart to squeeze out blood. Usually the heart returns to normal after the alcohol is metabolized, but in some people, the heart dilates over time, resulting in heart failure (swelling in the feet has many causes, but heart failure is one of the biggest concerns). In the liver, longstanding alcohol use affects the liver’s ability to synthesize proteins. Reduced levels of the blood protein albumin also might cause leg swelling. Both heart and liver effects are very mild except in people who already have disease of these organs. The role of vasopressin, also called anti-diuretic hormone, is complex. Initial inhibition of ADH leads to an increase in urine production, then an increased level of the hormone, which can lead to water and salt retention and swelling. Even though there are at least three ways alcohol can lead to or worsen edema, it is likely that it’s simply swelling from sitting and standing too much, which commonly occurs in travel, combined with a greater sodium intake from eating out at restaurants. Sodium content at many restaurants is much higher than if you prepare your own food. *** DEAR DR. ROACH: I’ve been dealing with a problem for a very long time. I’d like to have someone tell me how to stop my eyes from tearing and my nose from running every time I go out in the cold weather. I’ve been given pills (Claritin and Benadryl) and sprays (Nasacort, Beconase), and although they help me breathe better, I still fill up with tears and mucus. I’d like to know if you’ve ever run across these symptoms. -- S.R.M. ANSWER: These symptoms are not only common, they are nearly universal. One of the body’s protections against cold weather is to increase fluid flow to the eyes and nose, as the cold, dry air evaporates their protective moisture. Almost everybody has had the experience of a runny nose after being out in the cold, especially when first coming into a warm room. In some people, the normal response is so strong that it becomes very annoying. It’s a type of nonallergic vasomotor rhinitis. I have had frequent success in prescribing nasal antihistamines, such as azelastine, or an anticholinergic drug like ipratropium, if the treatments you have tried haven’t worked. *** Dr. Roach regrets that he is unable to answer individual letters, but will incorporate them in the column whenever possible. SAFETY IN NUMBERS: WHY IT PAYS TO REPLACE HEADLIGHT BULBS IN PAIRS Why replace aging headlight bulbs? Few drivers include headlight bulb replacement in their vehicle maintenance routines. But old headlight bulbs can drastically compromise visibility at night, when drivers’ visual acuity is naturally reduced by as much as 70 percent. As headlight bulbs age, the effects of exposure to sunlight, ozone and pollution can significantly reduce their light output. According to Philips Automotive, a leading manufacturer of vehicle lighting components, headlight bulbs begin to project significantly less light within two to three years of their initial usage, producing dimmer light outputs that compromise the safety of drivers and their passengers. For example, new headlight bulbs capable of projecting 240-foot beams in front of a vehicle may see that projection dwindle to just 160 feet within two to three years.

DID YOU KNOW? According to the Insurance Institute for Highway Safety and the Highway Loss Data Institute, roughly 10 percent of all highway deaths involve large trucks. Braking capability can play a role in such deaths, as the IIHS notes that loaded tractortrailers take between 20 and 40 percent farther than cars to fully stop. That discrepancy is even greater on poorly maintained roads or roads marked by adverse conditions such as ice, rain and/ or snow. While 16 percent of the roughly 3,600 people who died in large truck crashes in the United States in 2014 were truck occupants, more than four times that many people (68 percent) were occupants of cars and other passenger vehicles. That’s because large trucks typically weigh 20 to 30 times more than passenger vehicles, meaning occupants in cars and passenger trucks are incredibly susceptible to serious injury or death should they get in accidents with tractor-trailers and other large trucks. Time of day also must be considered when examining large truck crashes and fatalities. While 17 percent of deaths in 2014 took place between the hours of noon and 3 p.m., that figure dropped to 11 percent between the typical rush hours of 6 p.m. and 9 p.m.

Canandaigua’s Christkindl Market: German-Inspired, Holiday Shopping By Amanda Dudek Several white tents, filled with both vendors and crafters, spread across the lawns of the Granger Homestead & Carriage Museum in Canandaigua from Friday until Sunday, November 9th, 10th and 11th. For those hoping to make a dent in their holiday shopping list, this event couldn’t be more ideal. Over 130 artisans displayed their wares at booths within each tent. Samples and specials abounded throughout this year’s Christkindl Market, as did the sheer variety of goods, from jewelry to homemade soaps to glasswork to wine to art and decor. It would be difficult not to find something for everyone at this diverse, open-air, holiday extravaganza. The first hint one has as to the size of this festive event was that parking was not on site. There were, in fact, three separate parking areas, which were several blocks away from the Granger Homestead. However, there was plenty of space and no charge for parking. Festively-decorated, off-duty school busses and drivers graciously transported visitors to the Christkindl Market, and many of these drivers were exceptionally helpful, kind and full of holiday spirit. Busses pulled up along the sidewalk next to the Admissions Tent at the Granger House property, where admission to Christkindl Market was just $5 per person and free for children under 12 years of age. Admission included access to three, large, white tents housing the vendors at their booths. Heated air was pumped into each tent to help keep guests warm as they perused the goods covering every available surface. Outside of the tents, food trucks and stands circled around chairs and tables to create an outdoor food court. Comfort foods, desserts, hot dogs, strudel, funnel cakes, kettle corn and German specialty foods were just some of the food court’s offerings. Across the lawn, near the Carriage House, children were delighted to participate in activities offered at the Elf School. Helpers, dressed fittingly as elves, were welcoming and kind as they ushered children in. The activities included crafts as well as making holiday cards for soldiers. Guests young and old alike

also were welcome to visit Santa’s House next door, take photos and say hello to Saint Nick himself. Between these attractions for the youngsters and the vendor tents the regal Granger House stood in tasteful holiday grandeur. Visitors could wait on the steps to be allowed inside as only so many guests were permitted to tour the home at a time. Upon entry, visitors were given blue shoe covers and a detailed list of all the Christmas trees and other holiday displays in the Granger House. There were over 50 trees featured in the Festival of Trees throughout the downstairs of the home. There were also wreaths and other decorative displays across fireplace mantles and up stairways. The dining room table was set in exquisite holiday finery. Each tree and display was sponsored by an area business or group, and its decorations were themed accordingly. The effect of all these decorated trees and displays within the beautifully preserved architecture of the 1816, Federal-style mansion was joyous and pleasantly nostalgic. As guests exited through the rear of the house, they passed through the back porch on which the Homestead Women’s Council was holding their Biggest Best Bake Sale, and so it truly did appear to be, with delicious-looking treats across every table. The busses returned regularly to take guests back to their cars. Guests seemed to leave with spirits buoyed by the festive atmosphere and their holiday finds. This German-inspired holiday market is well-worth a visit whether for food, gifts, history, Santa and elves or just to get out and enjoy the season. In Christkindl Market spirit, “Frohe Weihnachten!”

This event is a fundraiser for the Scouts and the proceeds go directly to individual scouts to help pay for summer camp as well as other Scouting expenses. You stole from every (local town) Boy Scout and the volunteers who help provide the Scouting program for them. It is hard for me to imagine what your motivation would be for trying to hurt a young man's chance at attending summer Scout camp-Have a change of heart and return the sign no questions asked.
