DID YOU KNOW? According to the National Institute of Mental Health, panic disorder is a type of anxiety disorder characterized by recurrent, unexpected panic attacks. Panic attacks occur suddenly and are periods of intense fear that may include palpitations, pounding heartbeat and/or accelerated heart rate. People in the midst of a panic attack may also experience shortness of breath or feel as though they are choking or being smothered. Feelings of impending doom may also occur during a panic attack. Anxiety disorders, including panic disorders, may be caused by the interaction of certain genetic and environmental factors. These factors include behavioral inhibition in childhood, exposure to stressful life events in childhood and adulthood, anxiety disorders in close biological relatives and a parental history of mental disorders. Anxiety disorders are generally treated with psychotherapy or medication, or a combination of both.

Could Abdominal Migraine Cause Cyclic Vomiting?
--DEAR DR. ROACH: My son started having gastrointestinal problems in high school. Approximately three years ago, they became so intense that he couldn’t work. Last May, he finally had surgery for gastric reflux. This helped with the persistent vomiting, but he is still experiencing intractable nausea. After a year of tests and trials of every anti-nausea medication known, he finally was told that he has cyclic vomiting syndrome. He also was told, basically, “We don’t know how to treat it.” He currently is on a trial of a tricyclic antidepressant, but wasn’t given much hope that it would help. Could you suggest any possible treatment? He is only 28, and I can’t imagine him living with this and not being able to work or participate in life forever after. -- M.S. ANSWER: Cyclic vomiting syndrome is best described in children, but is certainly described in adults. It is called “cyclic” because there are periods of multiple vomiting episodes (lasting from one day to a week) interspersed with periods of completely normal health. Children have 12 cycles per year on average, while adults have four. CVS has been associated with migraine headaches, and treatment for migraines is sometimes successful in treating CVS, so it’s possible CVS represents a variant of abdominal migraine. CVS is a difficult diagnosis to make, as it can be easily confused with other causes of recurrent vomiting. Both children and adults often go a long time with incorrect diagnoses, sometimes getting surgeries, like your son did. Treatment should be administered by a clinician experienced in this condition, and unfortunately, that doesn’t seem to be the case with your son. In one series, 3 in 17 adults with CVS had a complete response to tricyclic antidepressants, but 13 had at least some response. So I would hold out more than just a little hope for some benefit. Other medications that have been tried include sumatriptan (normally used for migraine), ketorolac (a powerful anti-inflammatory medicine) and prochlorperazine (usually used for nausea, and I’m sure your son would have tried it). I would strongly suggest you contact the Cyclic Vomiting Syndrome Association to get more information and suggestions for an expert in your area, at www.cvsaonline.org. *** DEAR DR. ROACH: I have noticed that the medical profession uses a number of standard words in different ways. For example, “negative” is usually good, and “positive” often is bad news. -- N.M.G. ANSWER: It’s true that we in medicine use words differently than they are used conversationally. “Negative” and “positive” are examples: We would like the biopsy to be negative for cancer, and if the HIV test is positive, that’s not good. Doctors also use the word “complaint” to describe the patient’s chief concern; it’s not that we think people are being annoying. *** DEAR DR. ROACH: Can skim milk or oat bran cause gas? -- J.R. ANSWER: Both certainly can cause gas. Skim milk contains the milk sugar lactose, to which many people are intolerant. In mild cases, it causes some gas; severe cases can cause diarrhea. Oat bran is a good fiber source, and all good sources of fiber can cause gas, especially if taken in amounts that are greater than the person is used to. *** Dr. Roach regrets that he is unable to answer individual letters, but will incorporate them in the column whenever possible. lenn Galbraith will discuss the works of his great-great grandfather, Albert Roland Stone at 7:00PM, Tuesday, January 15 at the Fairport Public Library. Albert R. Stone was Rochester’s first newspaper staff photographer. Speaker Glenn Galbraith will also relay Albert’s message to his granddaughter, Helen, to save the images and discuss the inspiration behind ROC Archive’s mission statement to preserve image media. ROC Archive is a regional archive dedicated to the collection, preservation and accessibility of the region’s audiovisual media as an artistic and historical record of our culture and heritage. The region includes New York State, Connecticut, New Jersey and Pennsylvania. The archive focuses on newsreels, home movies, audio recordings, video recordings and all other audiovisual tools and media, digital and analog, created to capture a region’s culture. Glenn Galbraith holds a Certificate in Film and Media Preservation from the L. Jeffrey Selznick School of Film Preservation at the George Eastman Museum and a B.S. in Photography/Gallery Management from The College at Brockport, State University of New York. He completed his internship in the Schuyler C. Townson Research Library at the Rochester Museum and Science Center working with the Albert R. Stone Negative Collection. In addition to his current work at the Livonia Area Preservation & Historical Society, he is Curator of the Stone Family Collection; consisting of glass plate negatives, photo prints, books, scrapbooks, newspaper clippings and other artifacts related to his photographic lineage; he is the great-great-grandson of Albert R. Stone. Glenn is an avid audiovisual media collector and the founder of ROC Archive Initiative, a multi-state audiovisual regional archive and has been in media production since 1995. This program discussing Albert R. Stone’s work is sponsored by the Fairport Public Library and co-sponsored by the Perinton Historical Society. Reservations are required. Take Steps To Improve Your Retirement Confidence (NAPS)—A recent study by the Insured Retirement Institute found that only 25 percent of baby boomers believe they will have enough money in retirement and just 28 percent believe they did a good job financially preparing for their retirement years. This dismal view of their retirement prospects by so many aging Americans is concerning, but what is even more sobering is the study found that boomers are actually less satisfied with their finances than they were as recently as five years ago. In 2012, 41 percent of baby boomers believed they were doing a good job of financial preparation. This decline in retirement confidence has taken place in spite of healthy market conditions and rising interest rates for most retirees’ investments. Fortunately, the news doesn’t have to be so bad if seniors allow this news to serve as a wake-up call for them to take action. Here are five steps that experts say boomers can take to revisit their retirement plans now and fix things that have gone awry: 1. Reassess priorities. Take stock of your available resources and prioritize your needs vs. your wants, based on where your life is now and what is most important to you in the years ahead. 2. Raise cash. Consider options that may be available to you for boosting your savings, such as a part-time job or perhaps selling off some possessions you no longer need. 3. Generate interest. Re-evaluate where your money is sitting today and consider moving as much as you can out of cash and into interest-bearing accounts, which are paying depositors more than they have in several years. 4. Look for hidden assets. Inventory all your assets that might create cash flow, including those you may have overlooked in the past, such as a life insurance policy. A life insurance policy is your personal property and, if you no longer need or can afford it, you may be able to sell it for an immediate cash payment. 5. Leverage your house. If you own your home, consider a reverse mortgage or a home equity line of credit as a way to free up cash that you can use to pay off health care bills or other expenses. Of these five steps, the one that could produce the most immediate cash flow may be the sale of a life insurance policy, known as a life settlement. Most consumers who qualify for a life settlement are 70 or older and own a life insurance policy with a benefit of at least $100,000.
