DEAR DR. ROACH: I’m a 67-year-old man who developed typical atrial flutter two years ago, but I haven’t had any symptoms that I am aware of. I don’t have a history of heart problems. My heart rate was 117 bpm while experiencing atrial flutter. My cardiologist recommended an ablation, as that can be curative. He specializes in electrophysiology. The ablation was successful, and my heart rate was in the low 60s afterward. Six months ago, atrial fibrillation (AFib) showed up, but again, there weren’t any real symptoms for me. My heart rate is nice and low, and I never really feel any problems ... perhaps some fatigue. But at 67, I can’t always distinguish this from the feeling of getting older. I’m on Xarelto and a low dose of carvedilol. My blood pressure is good. Is it worth having an ablation of the pulmonary veins to try and stop AFib? Or is it better to just live with an irregular heartbeat? Apparently, it’s best to have an ablation sooner rather than later, so I’ll need to decide as soon as possible. -- D.S. ANSWER: Atrial flutter is an abnormally fast heart rhythm that starts in one of the top chambers of the heart, usually the right atrium. The heart rate in the atria is very fast, typically 240-300 bpm, which is faster than any adult heart can beat. It is common that every other impulse in the atria gets transmitted to the ventricle, so a pulse rate in the range of 120-150 bpm would be typical. Catheter ablation prevents the progression of the fast impulse to the rest of the heart, and it is successful 65% to 100% of the time in various studies. Unfortunately, studies have also shown that somewhere between 7% and 44% of those who had a successful catheter ablation for atrial flutter will later have a recurrence -- or more commonly develop AFib. This is called a chaotic arrythmia and can cause a fast heart rate, but more importantly, the lack of coordinated heart contractions in the atria can predispose people to the development of a clot in the atrium. The clot can then float downstream where it can lodge and cause damage, specifically a stroke. Each year, a percentage of 5% of people with AFib will develop a stroke without treatment. Treatment is designed to relieve symptoms and reduce stroke risk -- your cardiologist has already achieved both of these aims pretty well. The carvedilol is a beta blocker that helps keep the heart rate under control, and Xarelto is effective at reducing stroke risk. In my opinion, there isn’t a need to consider another treatment like an ablation, which has a small but real potential for serious side effects. In fact, it is not 100% effective; about 70% to 75% of patients are symptom-free a year after the procedure. Many people need to continue anticoagulants like Xarelto even after a successful catheter ablation. I recommend ablation for people who cannot tolerate medication treatment for their symptoms.

CHARRED GREEN BEANS WITH LEMON VERBENA PESTO Serves 2 to 4 Green Beans 11/2 pounds slender green beans 2 teaspoons olive oil Lemon Verbena Pesto 1 cup fresh lemon verbena leaves (substitute fresh lemon balm leaves) 2 garlic cloves 1/4 cup grated Parmesan cheese 1/4 cup pine nuts or English walnuts 1/2 cup olive oil Fine kosher or sea salt and freshly ground black pepper to taste Prepare a hot fire in your grill. Toss the beans with olive oil and place in a perforated grill basket or wok set on a baking sheet. For the Lemon Verbena Pesto, combine the lemon verbena, garlic, cheese, and nuts in a food processor and pulse to puree. Slowly add the olive oil with the processor running until the mixture thickens and emulsifies, about 1 minute. Season to taste with salt and pepper. The pesto will keep in the refrigerator for 7 to 10 days, or it may be frozen for up to 3 months. Place the grill wok or basket directly over the fire and stir-grill, tossing the beans with wooden paddles or grill spatulas until crisp-tender, about 5 to 8 minutes. Transfer the grilled beans to a large bowl and toss with about 1/4 cup of the Lemon Verbena Pesto or to taste.

Yields 64 Crust 21/2 cups all-purpose flour 3/4 cups powdered sugar 1 teaspoon baking soda 3/4 cups butter or margarine, melted Filling 3 cups pecan halves, divided 2 cups packed brown sugar 3 eggs 4 tablespoons butter or margarine, melted 2 teaspoons vanilla extract 1/4 teaspoon salt Powdered sugar, optional Preheat oven to 350 F. For crust, combine flour, powdered sugar and baking soda in a batter bowl. Add butter; mix until crumbly. Lightly press crumb mixture over bottom of stoneware bar pan; roll lightly. Bake 15 minutes; remove from oven to a cooling rack. Meanwhile for filling, reserve 1 cup of the pecans for garnish; chop remaining pecans. Combine chopped pecans, brown sugar, eggs, butter, vanilla, and salt; mix well. Pour filling over warm crust, spreading to edges of pan. Arrange remaining pecan halves over filling. Bake 17 to 19 minutes, or until filling is set in center. Remove from oven; cool completely. Sprinkle with additional powdered sugar, if desired. Cut into 32 squares using a utility knife; cut each square in half diagonally.

PLEASE Please don’t sit in your car using a handicap spot waiting for your person. It takes away from those who truly need it. You can drop them off in front of the store and then sit in the front and wait for them. You can see them come out. It’s okay as long as you stay in your car. It’s so hard for me and others truly walking disabled, especially coming out of the store after being on my feet to have to walk any distance whatsoever. And when I see people sitting in the closest handicap spots just upsets me. It’s bad enough we know that there’s people who truly don’t need a handicap spot. I know many of them who do lots of yard work very easily then have a handicap sticker.
Thank you I live in Henrietta and I want to thank the CHS Ambulance crew that helped me Saturday, January 27th around 5:00 PM. I am grateful not only for their knowledge and expertise, but how they treated me. They were both great. Thank you CHS. Thank you Thank you! On Thursday, Feb. 1, I ate lunch at a local restaurant in Clifton Springs. I had soup, sandwich, and coffee -- all delicious. When I asked for the bill, I was told it had been taken care of and I was all set. To whomever paid my bill, thank you! It was very thoughtful and much appreciated!
Restaurants When you are going to post your hours and when you may be closed for specials events on your website, please keep your site accurate. We checked the website shortly before leaving, headed to a local establishment for dinner, just to find that they were closed for a private party. Inconvenient, to say the least.

Are dogs really happy in freezng weather?
Hi I’m Canela. I’m a pitbull mixed with a labrador, and I am 1 year old and 6 months! I love to play and eat work boots. LOL.
DEAR PAW’S CORNER: While walking along the local riverway this frosty weekend, I noticed at least three people whose dogs were under-equipped for the cold. One dog wore an insulated vest but no booties; the other two dogs wore no cold protection at all. Why do so many owners ignore their dogs’ comfort and safety this way? -- Harold in New Hampshire DEAR HAROLD: Unfortunately, not every dog owner knows that it’s up to them to protect their pets from cold-weather hazards. Most dogs won’t ask for a sweater vest, and I’ve yet to meet one that didn’t hate booties. I know quite a few dogs that just won’t tolerate booties. Many owners do know their dogs’ limits in the cold, however. They check the temperature before going outside and survey the sidewalk or trail they’re on for hazards like ice chunks, salt or glazing. They limit the time spent outdoors so their dog’s core temperature doesn’t drop. They monitor their dogs to make sure they’re not shivering or in pain, and that their paws aren’t getting frostbitten. And they carefully wipe and check their dogs’ paws as soon as they get back indoors. How do you, an observer, make sure that an owner is following these guidelines? Confronting owners is not advisable. You may be able to strike up a casual conversation on the path, and ask how their dog tolerates the cold weather. But don’t preach at them. In my experience, owners who take the time to walk their dogs on a leash in freezing weather do care about their health and keep an eye on them in the cold.

Craig Houghton: Crafting Success, One Slice at a Time BY SUZIE WELLS
I
n the heart of Newark and Penfield, Salvatore’s stands as more than just a pizza place – it’s a testament to the hard work, dedication, and dreams of its owner, Penfield native Craig Houghton. At 35 years old, Craig worked his way up the Salvatore’s ranks, living the dream of owning his own shops. Craig spent six and a half years working at a fast-food restaurant through high school and college. After graduating from SUNY Cortland, Bachelor’s degree in hand, Craig’s friend happened to work for Salvatore’s corporate, introducing Craig to the world of pizza and paving the way for his future in the business. From 2011 to 2013, Craig honed his skills working with Salvatore’s founder, affectionately known as “SoccerSam,” at the Webster location. This experience set the stage for opportunity in 2013 when Sam’s son sold the Penfield store to Craig. Craig’s entrepreneurial spirit didn’t stop there; in March 2020, Craig took over the Newark location, a proud moment in his journey. Craig lives and breathes Salvatore’s, often working 50–60-hour weeks for the past 15 years. He attributes his success to a simple mantra: “You can’t be in the business if you don’t love what you do.” For Craig, owning a Salvatore’s store means being his own boss and having the freedom to shape the business according to his vision. Craig acknowledges the challenges along the way, but he remains dedicated, emphasizing that success in the restaurant industry demands hard work and a genuine love for the craft. He humbly credits his ability to balance work and family to his wife, Kate, who works at Rochester General Hospital and understands the demands of non-traditional hours. One of Craig’s favorite moments in his entrepreneurial journey was acquiring the second Salvatore’s location. Drawing a parallel to parenting, he likens it to having a second child – still exciting as with the first, but with the benefit of experience. With the analogy, he laughs that though he has three children, ages 11, 7, and 1, Craig does not intend to expand to a third location, instead focusing on his Newark and Penfield locations, appreciating the unique rewards each brings. Beyond the pizza business, Craig mostly spends time with his family, and admits the perhaps-unconventional interest in antiquing. Alongside his sister, he frequents the Avon Flea Market on Sundays. He also enjoys visiting family on Conesus Lake, cherishing the moments to recharge. Alongside his career ambitions, he remains deeply involved in his children’s activities, exemplifying the importance of balancing work and family life.

Craig highlights the unique aspect of Salvatore’s being small, mom-and-popstyle restaurants with store owners’ creative freedom while maintaining a cohesive brand identity. As Craig steers Salvatore’s through the ever-changing landscape of the restaurant industry, his goals are clear: keep both stores running successfully, increase sales, and evolve them into familyoriented spaces, moving beyond simple takeout shops to create memorable dining experiences. Despite challenges posed by the recent decline in festivals and community events Salvatore’s often contributed to preCOVID, each Salvatore’s location continues to give back to the community. Craig says he’s “just a small-town pizza guy,” but emphasizes his commitment to local sports teams and organizations by donating pizza and prizes for fundraising efforts, etc. With two decades in the restaurant business under his belt, Craig Houghton is a local entrepreneur who turned a passion for pizza into a thriving business. His journey exemplifies the resilience and determination required to succeed in the competitive world of food service, proving that with hard work, dedication, and a love for what you do, dreams of owning a place like Salvatore’s can become a reality.
