Moles eat insects and earthworms in the soil. You probably have voles that can look similar to moles. Voles eat plants. Voles prefer the bark of young fruit trees as well as tender stems, foliage, flowers, seeds and fruits of grasses, sedges and many other herbaceous plants. They will make surface and underground tunnels. Cats, dogs, foxes, skunks and coyotes will eat voles as will some snakes and birds.

After a w inter spent cooped up indoors, spring cleaning can rejuvenate a household and provide a great chance to rid a home of a season’s w orth of clutter. Donating old clothes and cleaning out the garage are popular spring cleaning projects, but there are a host of additional tasks homeow ners can tackle to freshen up their homes this spring.

GET KIDS EXCITED ABOUT VOLUNTEERING Volunteering benefits people of all ages, but can be especially beneficial to children. Studies have indicated that teenagers who volunteer are less likely to become pregnant or use drugs. In addition, in a review of 60 studies on child engagement, Child Trends found that children who participate in civic engagement programs are likely to be more involved in school, to graduate from high school and to hold more positive civic attitudes than those who do not get involved in such programs. Getting kids to volunteer as they progress through adolescence may not be so difficult. In its 2014 analysis of volunteering trends, Child Trends found that 39 percent of twelfth grade students volunteered at least once a month in 2014 compared to just 27 percent of eighth graders. Older students might feel compelled to volunteer to improve their college admissions applications, but parents can take steps to get their younger children excited at the prospect of volunteering. Set a positive example. Young children want to emulate their parents, so moms and dads who volunteer may find ready and willing partners in their children. Parents can inform their children when they volunteer and why they do so before bringing kids along to join them. Don’t forget fun. Young children may be more likely to embrace volunteering if the activity is fun. Kids might not find it too fun to sit next to their parents while mom and dad balance a soup kitchen’s books, but a beach cleanup excursion or a trip to refurbish an inner city park may inspire kids to spend a lifetime giving back to their communities and helping the less fortunate. Keep it simple. Kids may not enjoy volunteering activities that consume too much time or make them feel exhausted. As kids grow older, parents can encourage them to embrace more challenging activities and/or commit more time to volunteering. But keep things simple with especially young children or older children who have never before volunteered. Try and try again. It might take some trial and error before parents find volunteering opportunities their children truly enjoy. When volunteering with children for the first time, be careful not to commit to more than one day. If children enjoy themselves, then parents can commit more time. If kids do not seem excited by an activity, parents won’t feel guilty or let the charity down by not returning. Children can benefit greatly by volunteering, and parents can get their youngsters involved at an early age.

There’s less demand, so homeowners enjoy breaks on equipment and services. An easy place to begin is at the window. Cracked and warped windows can often cause heat to escape. “Windows that produce a draft or feel cold on the inside should be replaced,” explains Richard Kasunic, a home repair expert with Window Nation, one of the country’s leading window replacement companies. Modern vinyl windows fit tightly to shield homes from wind, water infiltration and inclement weather overall. When tending to their lawns, homeowners are advised to pay attention to areas that may feature standing water. According to the World Health Organization, standing water is a breeding ground for mosquitoes, which can breed in great numbers in pools of water. Mosquitoes are known to carry diseases like malaria, West Nile virus and Zika. Furthermore, mosquitoes that bite pets can transmit heartworms, a serious problem if gone it goes undiscovered. Standing water also can be a haven for bacteria, mold and parasites that are dangerous to human health. If standing, stagnant water is a problem in your yard, remediation is necessary. Directing downspouts away from the house can remediate standing water. Changing the grading of soil so that low spots are elevated is another way to reduce instances of standing water. This may be a project that requires the assistance of a drainage professional. cott Lomber of Canandaigua has a lot to live for. In addition to his wife and two grown children, he has grandchildren he’s determined to see grow. In addition, experts say it can make a tremendous difference in prognoses. In fact, a study presented at the International Association for the Study of Lung Cancer World Conference showed widespread screening could save as many as 65,000 lives in the U.S. each year. To date, however, only a fraction of area residents who are eligible have taken advantage of the opportunity. “We don’t really know what is holding patients back,” says Dr. Ben Wandtke, president of Thompson Health’s medical staff and chair of its Population Health Management Committee. “Lung cancer is by far the leading cause of cancer deaths for both men and women,” the radiologist adds, “so it’s worth focusing on.” A patient’s primary care doctor must order low-dose lung screening after determining whether the patient meets the following criteria: • Between the ages of 55 and 77 • Has smoked “30 pack years” or more (one pack per day for 30 years or two packs per day for 15 years, for example) • Still smoking or quit fewer than 15 years ago Nationally, only 1 to 3 percent of people who meet the criteria participate in screenings. Within Thompson’s service area, it’s 15 percent. Although that’s far above the national average, Wandtke’s committee has a goal of reaching 30 percent within just two years. The reason is simple: “Early diagnosis enables a far better outcome than any treatment we can provide,” Wandtke says, citing data showing those diagnosed with Stage 1 lung cancer have a 50-percent survival rate at the end of 10 years, while those diagnosed with Stage 4 have a 10-year survival rate of less than 5 percent.

DID YOU KNOW? Doctors treating patients who have been diagnosed with cancer will eventually present pathology reports as a means of helping patients better understand their disease. Upon confirming the presence of cancer, doctors will conduct tests on their patients to determine the size and appearance of the cancer, how quickly the cancer is growing and if there is anything to suggest that the cancer has spread to healthy tissues near its point of origin. Doctors may also use these tests to determine if hormones, genetic mutations or other things in the body are influencing the cancer’s growth and development. The results of these tests will be included in the patient’s pathology report. Patients who find their reports difficult to understand or those experiencing anxiety after reading their reports should go over any questions they might have with their physicians. Looks Like Acne, But It’s Not ---

DEAR DR. ROACH: I have never seen anything in your column about sebaceous hyperplasia. I read that it is quite similar to acne and that a person could use an acne treatment to keep the condition in check. Could you give your thoughts on this? -- R.G. ANSWER: Although sebaceous hyperplasia can be mistaken for acne, sebaceous hyperplasia happens mostly to middle-aged adults (or newborns). These bumps appear most commonly on the forehead and cheeks, and have an umbilicus (a small hole) in the center. They are enlargements of the oil-producing sebaceous glands. They happen particularly in people with oily skin. Treatment can be challenging. Reducing dietary fat and using a good-quality skin-care regimen for oily skin may help. Dermatologists can treat these when they are cosmetically important, not because they cause harm in themselves. Although they can be mistaken for basal cell cancers of the skin, they are not cancerous or pre-cancerous. They are mostly treated mechanically, and by that I mean using a surgical technique like cauterization or excision. But these techniques can leave scars. Laser and phototherapy have good cosmetic results but are expensive. Isotretoin, a systemic acne drug with significant side effects, also has been shown in a small study to be effective.

DEAR DR. ROACH: I am currently taking 20 mg of Xarelto daily, and have a screening colonoscopy scheduled soon. The preparation instructions said not to stop taking blood thinners, but didn’t specifically mention Xarelto. I contacted the office and spoke with a scheduler, who said those instructions applied to all blood thinners. Previously, I was told that I would need to go off of blood thinners for five days prior to a colonoscopy. Since this was a contradiction to the instructions from the clinic, I have tried to reach out to other medical sources to see if I could get a consensus on what to do, but no one has responded to me yet. There is a strong history of clotting problems in my family. Would you weigh in on this matter? The last time I had a screening colonoscopy was around 2005, and there weren’t any issues. I realize this is a scope and not surgery, but it has me concerned enough that I am considering putting this off until I can get a satisfactory response. -- B.R. ANSWER: Most screening colonoscopies now are done with patients still on their anticoagulants, although for patients on warfarin (Coumadin), it is preferable for levels to be on the low end of the therapeutic range. This is a standard recommendation by the American Society of Gastrointestinal Endoscopy. The situation would be different for a high-risk endoscopic procedure, such as removing a large polyp. I have had to deal with this situation many times in my practice, and most often, all anticoagulants are stopped then restarted after the colonoscopy. There are some people who are at such high risk for clots that we use injection medications to prevent blood clots until the night before the procedure, but that is normally reserved for the highest risk of all. It’s a judgment call as to whether to use the injection anticoagulants to prevent clots around the colonoscopy. *** Dr. Roach regrets that he is unable to answer individual letters, but will incorporate them in the column whenever possible.
