PECAN-CRUSTED RACK OF LAM B W ITH ROSEM ARY BALSAM IC SAUCE 8 servings • Preparation time: 15 minutes • Cooking time: 30 minutes 2 American Lamb racks, frenched 2 tablespoons oil 1 teaspoon salt 3 tablespoons coarse grain mustard 2 cloves minced garlic 3/4 cup fresh bread crumbs 3/4 cup toasted and chopped pecans 4 tablespoons chopped fresh rosemary, divided 4 ounces semi-soft goat cheese, crumbled 1 cup balsamic vinegar Rub lamb w ith oil and salt. Place in a roasting pan, fat side dow n. Roast at 425 degrees F for 8 minutes; turn and roast an additional 8 minutes. Remove from oven. Combine mustard, garlic, bread crumbs, 2 tablespoons of the rosemary and cheese. Spread over lamb. Return to oven; cook 15 minutes, until crust is golden brow n. Remove from oven; cover w ith foil to keep w arm. In saucepan, combine vinegar and remaining 2 tablespoons of rosemary; simmer for 6 to 8 minutes or until reduced to half. Slice lamb into individual chops; drizzle sauce over chops. Serve w ith roasted butternut squash.

Hot Flashes Persist Well Past Menopause and Thyroidectomy
--DEAR DR. ROACH: I am 81 and had a thyroidectomy in 2017. My whole thyroid gland was removed. I also had a little bit of cancer. I have started to get hot flashes again. I had originally been on thyroid medicine before and after surgery. Then I was put on levothyroxine and decided to try NP Thyroid. I was doing better on NP Thyroid, then started getting the hot flashes again. At this point, for several reasons, I would rather stay on NP Thyroid for the time being. I was wondering if there is anything I can take for hot flashes? Years ago, in my 50s, I took estrogen, which helped, but this is a different time. I have spoken with other people who are also having hot flashes again. -- Anon. ANSWER: Hot flashes are common -- about 75% of women in North America will experience them around the time of menopause. It’s not uncommon for hot flashes to go away, then return, but I personally haven’t seen them recur after 30 or so years. I’d be concerned that there may be another cause for them, which brings me to your thyroid. Elevated levels of thyroid hormone can cause sensations very similar to hot flashes. One reason that I, along with most endocrinologists, recommend against products like NP Thyroid is because most of those products come from pigs. There are two thyroid hormones: thyroxine (T4) and triiodothyronine (T3). Humans have a different ratio of T3 and T4 than pigs do, with T4 getting converted into the active form, T3. T3 is then quickly broken down, so if it’s used, it needs to be dosed twice daily. As a result, if a person takes NP Thyroid, their thyroid levels are too high during one part of the day and too low during another. This isn’t good for anyone; the “high times” not only cause hot flashes, but also predispose people to atrial fibrillation. But it’s particularly a problem in a person with a history of thyroid cancer, where we want the thyroid-stimulating hormone (TSH, the hormone in the pituitary gland that regulates the activity of the thyroid gland) to be on the lower side. If the thyroid blood levels get low in the afternoon/evening when the T3 in NP Thyroid is gone, that can theoretically increase the risk of cancer recurrence as the TSH rises in response. So, in my opinion, you should be on levothyroxine, not NP Thyroid. (I do have a handful of patients who take levothyroxine and also take T3 twice daily. Some people are unable to convert T4 to T3 efficiently.) Estrogen is the most effective treatment for hot flashes, but it increases the risk of heart disease when used by women more than 10 years away from menopause. I don’t normally prescribe it to a woman in her 80s. However, there is a new medication called fezolinetant (Veozah), which is highly effective and reduced hot flashes by 93% in a trial. My only patient on it so far has reported 100% cessation of her hot flashes. There are other rare causes of hot flashes, including tumors that secrete substances, such as carcinoid tumors, and tumors that secrete adrenalin-like hormones (pheochromocytomas).

There’s a disconnect between saving money on energy with window replacements, according to government agencies. In fact, they found, it may take decades to recoup the cash spent on the installation of new, energy-efficient windows. The Problem The assessed value of adding 20 new energy-efficient windows in California, for example, may range from $10,220 to $18,580 based on a 2019 building assessors manual listing $511 to $929 for each window. Annual energy savings for the replacement of existing windows in a home are from $27 to $111 for dual-pane windows and $126 to $465 for single-pane windows, according to the U.S. Department of Energy. This means it may take decades to financially justify window replacements based on energy savings alone. An Answer Fortunately, there’s an alternative. Window films offer advanced energy savings and other benefits and can be quickly installed at a fraction of the cost of replacement windows, often with a return on investment in under three years. “Window films professionally installed on structurally soundsingle or double-pane windows may reduce energy consumption by as much as 30 percent, while the installation cost may be 91.5 percent less than putting in new windows,” explained Darrell Smith of the nonprofit International Window Film Association. Gratitude Finland has been rated the world’s most “happiest” country. Connection with nature, community and simplicity are three of the major factors cited as to the Finnish quality of life. However, there is an over-riding factor that they’ve learned that stands out for them and perhaps for us - gratitude. To quote: “The Nordic countries are united in their embrace of curbed aspirations for the best possible life. This mentality is famously captured in the Law of Jante -- a set of commandments believed to capture something essential about the Nordic disposition,” he explains. They include: “You’re not to think you are anything special” and “You’re not to imagine yourself better than we are.” Which, as Savolainen points out, is pretty much the opposite of America’s endless pursuit of personal and national exceptionalism. This is a great time of year to take stock of what we all have and be grateful. Amen.

Our new delivery person refuses to drop my paper off at the back of my short drive on the ramp so I can easily pick it up. I’ve posted a sign at my drive asking them to do this to no avail. They drop it in the gutter and if it rains it gets wet and if it snows the plow takes it away. Am I only person in [local town] to have this problem and is there anything that can be done to help? I’m 96 years old!
Fake service animals (dogs) Stop with all these fake service dogs! And stop bringing those fakes into the stores... it’s dirty! They stink. If you have these dogs around people, bathe them please. Don’t just spray them. Dirty and air freshener/perfume doesn’t mix well. The dirty is still there. Seems like any Dr will write off for a *pet. And you can purchase vests online. It’s become too much because you get away with it.

In its latest World Tourism Barometer, the United Nations World Tourism Organization found that 1.184 billion tourists traveled outside their countries’ borders for at least one night in 2015. Europe, Asia, the Pacific, and the Americas all recorded around a 5 percent increase in international arrivals that year. Europe was particularly popular, perhaps hedged by a weaker euro against the American dollar and other currencies. The U.S. Travel Association says that direct spending by resident and international travelers in America averaged $2.7 billion a day.

2. Connect with new people. Travel opens a person up to not only new experiences, but also new people. English poet John Donne penned the famous line, “No man is an island,” which underscores the importance of having friends and making new acquaintances. Research conducted by the University of Michigan found the act of talking with people in a friendly way can improve a person’s memory, suppress external and internal distractions, and encourage people to see things from another person’s perspective. It doesn’t hurt to broaden one’s social network, either. 3. Build confidence. Leaving one’s comfort zone can be a great way to bolster one’s self-esteem. Navigating cultural boundaries and overcoming those boundaries may be initially intimidating, but doing so can make a person more confident and more adaptable to change. 4. Develop opinions. Until a person visits a place in person, he or she only has third-party information to form opinions. Visiting a city or country for the first time can offer a more complete perspective. Travel gives people the chance to rest, explore, meet new friends, and make lasting memories.

Aggressive Dogs Keep Owner Away From the Dog Park IT PAYS TO USE CRATES WHEN TRAINING PUPPIES Many dog owners, particularly those who have never before had a dog, may hesitate to employ crates to train their puppies. That’s understandable, as on the surface it can seem cruel or inhumane to confine a living animal to a cage. However, the AKC notes that dogs are den animals that naturally seek out canine caves for security. That’s true whether dog owners provide crates for their puppies or not. So while humans may see crates as cruel, dogs very likely view their crates as secure places where they can go if they get scared or feel skittish. In fact, according to the AKC, dogs’ perception of crates is one reason why dog owners typically find it relatively easy to get their dogs to adjust to crates and ultimately enjoy them.
DEAR PAW’S CORNER: I’ve had to change the time of day that I visit the local dog park, because a new neighbor brings his two huge, aggressive dogs each morning at the same time as I do. They charge other humans, chase and snarl at other dogs, and make it impossible for other dogs to socialize calmly. The neighbor does nothing to control them and tells other owners that it’s their fault that his dogs are acting aggressive. To prevent injury to my dog (or myself), I now wait until after work to go. But this is inconvenient for everyone in the neighborhood. How do I address this problem? -- Dog Mom in New York DEAR DOG MOM: You’re doing the right thing by changing the time that you bring your dog to the park. It’s important to remove a dog from a situation you perceive as potentially dangerous or traumatic for them, and protecting them from harm. That’s only the first step, however. The new dogs are terrorizing other dogs within the park, making life miserable for them and their owners. It’s not fair, nor is it right. Dog parks within a city or town typically have rules of conduct posted at each entrance, but sometimes they’re vague. It’s up to each owner to properly train and socialize their dogs, and to keep them away from the park if they can’t control rough play, aggression or reactivity. Report the problem to the organization in charge of maintaining the dog park. This may be a community group or the municipal parks department. Contacting animal control is another option. Write down the dates and times that incidents have occurred. Terry’s Two Decades of Dedication: A Glimpse into Wild Wings, Inc. and the Passion for Nature BY SUZIE WELLS

estled on the edge of Mendon Ponds Park, Wild Wings Inc. stands as a beacon of hope for injured and non-releasable birds of prey. With both the iconic bird of prey facility and the Mendon Ponds Park Nature Center, this not-for-profit organization has an incredible team of volunteers and staff helping make a profound impact on the community. At its helm is Terry, Executive Director, whose dedication to wildlife and nature has spanned two decades. Terry’s journey with Wild Wings began 20 years ago, driven by a shared passion for animals and “my son and I wanted to volunteer with animals because of our interest in birds.” Originally from Irondequoit, now residing in Henrietta, Terry’s connection to the community has deepened over the years, committed to connecting people with the natural world to help foster a greater appreciation for it. Terry emphasizes the crucial role Wild Wings plays in wildlife conservation, reflecting that, “The animals at Wild Wings would be euthanized due to their injuries if not placed at an organization like Wild Wings. We are able to care for them, give them a second chance at life, and educate the public about their species and the environment.”
With around 40 dedicated volunteers, Wild Wings thrives on the support of its community. Daily operations involve the feeding and cleaning of animals and their enclosures, coupled with ongoing facility maintenance. The tireless efforts of volunteers ensure that the organization fulfills its mission.
For Terry, every day at Wild Wings brings new adventures. The eclectic mix of caring for animals, providing educational programs, and the dynamic nature of the work keeps him engaged. Terry’s favorite moments are those spent educating the public, especially children, witnessing the joy on their faces as they encounter animals up close.
Addressing common questions from the public, Terry underscores the importance of understanding an animal’s story. Each resident at Wild Wings has a tale of injury, whether from a car collision or gunshot wound. These creatures, permanently injured and non-releasable, live out their lives as educational ambassadors, offering invaluable lessons about the resilience of wildlife.
“Seeing the curiosity and interest sparked in them is rewarding, and hopefully, it ignites a lifelong passion for learning about nature,” Terry shares, emphasizing the transformative power of firsthand wildlife encounters. However, the journey is not without its challenges. Wild Wings operates without funding from Monroe County, relying on public support through donations, fundraisers, and fees for special events. The financial sustainability of the organization is an ongoing concern, requiring constant effort and community involvement. Wild Wings’ impact on the community is profound. The organization, true to its mission, does not charge admission, ensuring that everyone has the opportunity to learn about nature. Visitors often contribute either monetarily or by donating items from the organization’s wish list, contributing to the continuous care of the animals.
