No Cure, Just Treatment for Rare Skin Disorder --DEAR DR. ROACH: My girlfriend has necrobiosis
Excerpt from
lipoidica. She suffers from this on her shins, as did her mother. She is not diabetic. We cannot find good information on how to rid her of this. The information we find is not about cures, but treatments that mask the symptom, such as steroids. We cannot even find a doctor that specifically treats this condition. Can you help? -- M.B. ANSWER: Periodically I get letters referencing a condition like this one that I know absolutely nothing about. Fortunately, I have the time to do a lot of reading, and I talked to several people with the condition. Here is what I found. Necrobiosis lipoidica is a rare skin disorder, usually but not always associated with diabetes. It occurs mostly in young adults (the average is 25 years old when associated with diabetes, 46 years if not), and is more common in women. It may also be associated with thyroid disorders and celiac disease. The cause is unknown. The diagnosis is made by skin biopsy. The appearance is usually of a large, variably colored and elevated patch of skin, most commonly on the shins, as your girlfriend’s is, but it can spread to other parts of the body, especially skin that has been scraped or damaged. Because necrobiosis lipoidica can occasionally transform to skin cancer, it needs careful surveillance. There is no cure for necrobiosis lipoidica. However, there are treatments that can reduce the inflammation. Steroids, either topical or injected, are the usual first choice. Topical steroids are more effective if they are given as ointment and covered with an air- and water-tight dressing. If topical steroids are not as effective, there are many other therapies that have been tried, including: the medication tacrolimus, which decreases the immune response; ultraviolet light A; antimalarial drugs (also used for autoimmune diseases); and others. The disease normally progresses slowly, then stabilizes, but it can spontaneously resolve in some people. Necrobiosis lipoidica may also ulcerate, at which point a wound care specialist may be of tremendous value. It may not be easy to find an expert dermatologist for this condition. I would start with a support group, as you can get expertise from others who have been living with this condition. I found one large group on Facebook. I read some tips on makeup and temporary tattoos for cosmetic improvement. I found two clinical trials recruiting subjects, one in Boston and one in Pennsylvania. by John Thomas At this interview in Haran, the Lord said to Abram, “I will make of thee A GREAT NATION, and I will bless thee and make thy name great: and then shalt be a blessing: and I will bless them that bless thee, and curse him that curseth thee: and in thee shall all the families of the earth be blessed” (Gen. 12:2-3). Alluding to this promise, the apostle says, that in making it,” the gospel was preached to Abraham” – the glad tidings of blessedness to the nations, when Abraham and his descendants should be great, and renowned throughout the earth. Abraham believed this gospel promisorily announced to him by the Lord God. Nor was his faith inoperative. It was a living, moving, faith; a faith through which he obtained a good report. By the influence of that faith, which embraces the thing hoped for, it is testified that, Abraham “when he was called to go out into a country which he should after receive for an inheritance, obeyed; and he went out, not knowing whither he went. For he looked for the city having foundations, whose architect and builder is God” (Heb. 11:8, 10). He turned his back on Babel, and with Sarai; and his nephew, Lot, and all his substance, he left his father’s house, crossed the Euphrates and the Jordan, and entered the land of Canaan, still traveling onward until he arrived at Sichem, in the plain of Moreh. Having come thus fur into the country, the Lord appeared again to Abram to let him know that he was in the land He intended to show him: and added this remarkable promise, saying, “Unto thy SEED will I give this land”(Gen. 12:7). Let us pause here in the biography of Abram, and consider this promise. Here was a country, lying between the Euphrates and the Mediterranean, in which were Abram and off his house, with his flocks and herds, and which was in the actual possession of warlike tribes, living in cities walked up to heaven; concerning this country, the Lord, to whom heaven and earth belong, said to Abram, “I will give it to thy Seed, when as yet he had no child. But it is particularly interesting to know, who is intended by Abram’s Seed in this promise? Is it the “great nation” spoken of in the former promise; or, is it some particular personage to whom the land of Canaan is here promised as an inheritance? I shall offer no opinion upon the subject, but let the apostle to the Gentiles answer the question. In writing to the disciples in Galatin about the inheritance, he says, “The promises were made to Abraham and to his Seed. God saith not, And to seeds as of many persons; but as of one person, as it is written, And unto thy Seed, which is Christ” (Gal. 3:16). The apostle here tells that the Land of Canaan was promised to the Christ when God said to Abram, “Unto thy Seed will I give this land.” Let the reader, then, bear this in mind as one of the first principles of the gospel of the kingdom. Saying Goodbye to Pandemic Stress 7 Tips for Better Self-Care in 2021 BY AMANDA DUDEK

no exaggeration to say that 2020 was a challenging year for many people. In fact, with its isolation, stress and anxiety, it is not a stretch to say it was a traumatic experience. Whether you were on the front lines as an essential worker, struggling with the isolation of extended quarantining, or somewhere in between, many of us have started this new year burdened with incredible emotional and physical fatigue. Now is the perfect time to focus on self-care with its oh-so-necessary renewal and restoration. Give yourself permission to take a break, refocus, and recharge your mind and body. Boost your wellness by incorporating something new (perhaps one of your favorite tips!) into your regular self-care routine.
1. Meditate: Meditation is an excellent means of reducing stress and restoring healthy function in the body. Practiced for thousands of years, studies show that meditation can be associated with the improvement of a variety of issues such as strengthening cognitive function, protecting against heart disease, stimulating the immune system, and reducing blood pressure as well as improving metabolism and breathing. Consider spending at least 15 minutes each day practicing mindful meditation, deep breathing, or yoga to help destress your brain.
2. Take advantage of Telemedicine: Whether you have a case of the winter blues or simply aren’t feeling like yourself, telemedicine can be a great way to see your doctor quickly and safely. The Office of the National Coordinator for Health Information Technology says telehealth, which is a broader scope of remote healthcare services than telemedicine, can utilize everything from videoconferencing, the internet, store-and-forward imaging, streaming media, and terrestrial and wireless phone communications. Also, according to the American Medical Association, nearly 75 percent of all doctor, urgent care and emergency room visits can likely be handled safely and effectively over the phone or via video. Those experiencing an emergency, COVID-19 testing and treatment, or other serious symptoms and concerns, are encouraged to seek in-person treatment right away. Want to give it a try? 3. Journal: Sometimes, committing your experiences and worries to paper can help you to leave them in the past and move forward. Journaling can also slow your racing thoughts, allowing you to identify and better manage concrete issues. Writing can also encourage out-of-the-box thinking and creative problem solving. Studies have also shown that reading can develop neural networks in the brain that can help readers understand more complex thought. It can also improve your vocabulary, even as you get older. Consider spending even 5-10 minutes each day reading. Don’t forget, it’s absolutely okay to quit and choose another book. If you are feeling particularly ambitious, start a virtual book club! 6. Don’t “break up” with nature: When many recreational options closed temporarily during the pandemic, people spent much more time outdoors. As indoor recreational facilities reopen, it is important not to leave nature behind. The National Institutes of Health and Harvard Medical School say that, in addition to providing physical benefits, simply spending time in nature in any form can improve mental outlook, boost creativity, elevate mood due to natural light, improve concentration, and reduce stress. It may even help you sleep better. As indoor recreational facilities reopen, consider maintaining favorite outdoor activities, even simply dining outside. 7. Stay Safely Social: While it is important to protect your physical health and continue to follow masking and social distancing guidelines, it is also important to protect your mental health by finding safe ways to maintain social connections. There are many safe options to help keep isolation at bay. Consider scheduling video chats regularly, either once or twice per week. Try arranging for a large group chat on holidays. Maintain a connection with your elderly and other vulnerable loved ones by putting together care packages of supplies and treats. Deliver them in person so you can see the smiles, even if only through a window. Send uplifting messages in personalized cards to loved ones at home and in long-term care facilities. Don’t forget to let potentially isolated friends and family know they are cared for and loved. Cat Frees Crickets, Gecko Goes Hungry DEAR PAW’S CORNER: I have a leopard gecko named Lenetta, and I wouldn’t trade her for the world. There’s a problem, though. I also have a cat, and that cat always finds a way into the room where Lenetta is kept, and spends his time chasing the crickets through the plastic “kritter keeper.” Sometimes it gets flipped over and the crickets escape. Then I have nothing to feed to Lenetta, and there are loose crickets all over the house. How do I get this cat to stop obeying his instinct to chase these bugs? Help, before any more crickets get loose! -- Josiah from New York DEAR JOSIAH: Ah, that’s the thing: It’s a cat’s instinct to chase small, hopping things. Cats think it’s awesome. Rather than try and teach your cat not to chase crickets -- a tough job that will require long-term commitment, daily training sessions and possibly some genetic manipulation to remove that hunting instinct -make it tougher for the kritter keeper to get turned over. Solution 1: Duct tape. One piece on each side of the lid, being careful not to cover the air vents. If you use a cricket shaker or something similar, take out the shaker tube and secure the lid. Solution 2: Camouflage. Make the cricket cage invisible. Put it on a shelf that the cat can’t reach. Cover it with a thin cloth (that air can get through) or place it behind other objects. Out of sight, out of mind. Solution 3: Keep the door to Lenetta’s room closed. This may be more difficult because your cat clearly has figured out how to get in. Think about other ways to discourage his entry. Good luck!

STRAWBERRY-GLAZED FRENCH TOAST WITH SWEETENED SOUR CREAM Serves 4 • • • • • • • • • • 1.
8 1-inch-thick bread slices, cut from a country or peasant loaf 2 cups half-and-half 4 egg yolks 3 tablespoons light brown sugar 1 tablespoon vanilla extract, plus 1/2 teaspoon 1/2 teaspoon cinnamon 3 tablespoons unsalted butter 1/2 cup sour cream 1 tablespoon granulated sugar 1/3 cup strawberry jam or preserves (see note 2) Arrange a rack at center position and preheat the oven to 400 F. Place the bread slices on a baking sheet and bake until dry and very lightly browned, about 8 minutes per side. Watch carefully so that the bread does not burn. Remove the bread from the oven and reduce the oven temperature to 200 F. 2. In a medium bowl, whisk together the half-and-half, egg yolks, brown sugar, 1 tablespoon vanilla, and cinnamon. Pour the mixture into a shallow pan (a 9-by-13-inch glass baking dish works well). Add the GENESEE VALLEY PENNY SAVER

toasted bread slices and soak them 4 minutes per side. Remove to a large plate or platter. 3. Place a large, heavy frying pan over low to medium heat. Add about 2 teaspoons of the butter, or enough to coat the bottom of the pan lightly. When melted, add enough bread slices to fit comfortably in a single layer. Cook slowly until the slices are golden brown and crisp on both sides, about 4 minutes per side. Remove to a baking sheet and place in the warm oven. Repeat, adding more butter to the pan as needed until all the bread slices have been sautéed. 4. In a small bowl, stir together the sour cream, granulated sugar and remaining 1/2 teaspoon vanilla. 5. When ready to serve, spread each toast with a thin coating of strawberry jam and top with a dollop of sweetened sour cream. Note 1: The best bread for this dish is an unsliced loaf of good-quality peasant or country bread, preferably one without an extra-hard crust. One that is rectangular, rather than round, is more convenient, but either will do. Cut off the ends of the loaf, and reserve for another use. Then slice the bread into 1-inch-thick slices. If your loaf is large and the slices seem large, cut them in half. Note 2: You can try other jams, preserves or marmalades. Cherry, raspberry or peach preserves and orange marmalade are other possibilities.

Now, COVID-19 has brought this hypothetical threat to life, and a report by the U.S. Pharmacopeia (USP), an independent, nonprofit, scientific organization dedicated to improving public health through public standards and related programs that help ensure the quality, safety and benefit of medicines and foods, and the MIT Center for Collective Intelligence can help. Called “Trust or Consequences 2040: Will Innovations in Health and Medicine Deliver?” identifies what may happen in the future and explores the role that trust will play in helping people live longer and healthier. It identifies likely drivers of change, such as big data, artificial intelligence (AI), gene editing, risks to health systems and medicine supply chains and opportunities to improve global health. Four scenarios surfaced: Scaling the tried and true: Rolling crises spur effective global collaboration to address health concerns. Medical advances based on big data and AI are implemented gradually. The focus is on baseline healthcare for all. Dangerous uncertainty: Problems with big data and AI lead to devastating healthcare failures. Unequal access to innovation means only the rich receive advanced treatments; people turn to folk medicines, science-based medicine is questioned. A world of difference: The success of big data and AI in healthcare leads to rapid advances in personalized medicine and prevention. Not everyone can access this and disparities between and within countries perpetuate a “haves” versus “have nots” dynamic. Solving tomorrow’s problems: Smart innovation is widely distributed. Big data and AI create inexpensive diagnostic tools. Diseases become more predictable and healthcare emphasizes prevention. New treatments and technological advances curb costs. The report also suggests how public quality standards for medical products could help promote trust in health and medicine in the future—as they have for 200 years. “Many of the challenging issues we face in healthcare are recurring and persistent. ‘Trust or Consequences 2040’ points out that we must plan for the future we want, or we risk failure and an erosion of trust. Standards offer a proven approach to building trust and ensuring as many people as possible benefit from promising innovations in health and medicine,” said Ronald T. Piervincenzi, Ph.D., CEO of USP. STUDY SHOWS GIFT CARDS CAN GIVE A BOOST TO SMALL BUSINESSES (NAPSI)—If you own or run any of America’s nearly 31 million small businesses, recent research reveals gift cards can be a real gift to your bottom line. According to a 2019 Small Business Gift Card Study commissioned by First Data, now Fiserv, gift card programs help small business owners bring in more sales, attract new customers, and build relationships with their customers. Small Businesses, Big Benefits Not only do gift cards benefit a business, consumers expect them to be available at their favorite stores. In fact, 74 percent of consumers surveyed said they regularly buy gift cards from small businesses. And for business owners, offering gift cards can: • Grow sales—Nearly 80 percent of consumers said if they have a gift card valued at 88 cents, they will visit the store to redeem it • Attract customers—90 percent of consumers who receive a gift card from a small business they have never visited said they would shop at that business and return in the future • Build loyalty—56 percent of consumers who shop at small businesses join their loyalty or frequent shopper programs. Of consumers that join loyalty programs at small businesses, 73 percent said gift cards are a preferred way for their loyalty to be rewarded. Restaurants, Coffee Shops, and Salons When asked about the ideal gift card to give someone, or receive and use themselves, consumers agreed on their top three: casual dining restaurants; coffee shops; personal services (i.e., hair or nail salon). Other popular businesses from which to give or receive gift cards included supermarket/ grocery stores, entertainment/movie theaters, and quick-serve restaurants. Employee Incentives Gift cards are often a popular choice for employers who use them to say thank you or reward a job well done. For 90 percent of consumers, they’ve not only received a gift card from their employer, but view it as an incentive to reward their hard work. Additional Findings • When purchasing gift cards, 60 percent of consumers prefer plastic cards or paper certificates, but interest in digital gift cards is growing. • 58 percent of small business owners said plastic gift cards cost the business owner less to manage than paper certificates. Step 1: Determine eligibility and schedule an appointment. Vaccines are available at pharmacies, hospitals and through local health departments statewide – please contact your provider of choice to schedule your vaccine appointment. Additionally, there is a network of State-run sites distributing vaccines statewide – to determine eligibility and schedule appointments at New York State-run vaccination sites only, use the Am I Eligible app. You can also call the New York State COVID-19 Vaccination Hotline at 1-833-NYS-4-VAX (1-833-697-4829). Due to extremely limited supply from the federal government, it may be 14 or more weeks before an appointment is available.
Step 2: Complete the Vaccine Form. Once you have a confirmed appointment, you must complete the New York State COVID-19 Vaccine Form. UNDERSTANDING LIFE INSURANCE When choosing coverage, you will no doubt be asked if you prefer term insurance or permanent insurance. Term insurance is the least expensive life insurance, and such policies only last for a predetermined number of years. Men and women may purchase life insurance policies if they only want life insurance until they retire or until their children reach adulthood. Permanent insurance is more expensive and will last from the moment you purchase the policy until your death. Many people choose permanent life insurance policies so the money their beneficiaries receive upon their death can be used to pay estate taxes. In addition, there is an investment component to permanent insurance policies, as a portion of the premiums on such policies is invested (policies will spell out how the money is invested) and allowed to grow tax-free so long as the policy is open. Term insurance only provides protection with no investments.
Geneseo: At age 87 years Jane passed peacefully on January 14, 2021 at Strong Memorial Hospital. She was born on November 26, 1933 in Troy, NY to the late Harry and Minnie (Fox) Pitman. In addition to her parents she was predeceased by her husband, Richard Reilly and brother, Eugene Pitman. Survived by children, Diane (Charles) Zammerilla of McDonald, PA, and David Reilly of Conesus; grandchildren, Chris Zammerilla, Matt Zammerilla, Stephanie Reilly, and Aiden Reilly; twin sister, Joan Murphy; nieces, nephews, and close friends. Jane graduated from Mechanicsville High School in 1951 and later earned her degree from SUNY Geneseo. She was a registered nurse for many years, working at Strong Memorial Hospital, Livingston County as a health nurse, SUNY Geneseo, and Groveland Correctional. Jane was involved in several groups including her craft group, Young at Heart, and Garden Club. She was a parishioner at St. Mary’s Catholic Church, Geneseo, NY. Family and friends called on Tuesday, January 19th, from 3-5pm at the Kevin W. Dougherty Funeral Home Inc., 21 Big Tree Street, Livonia, NY. Edward Eugene Opich Jr., 63, passed away in his sleep in the early hours of Saturday, January 16, 2021. Born September 25, 1957, in Lawton, Oklahoma, he moved around a lot until his 30s, then settled down in Conesus, New York. He was a skilled craftsman and handyman. He is a former World’s Strongest Man trainee and Granite City, Illinois state discus record holder, breaking the record in 1975 and currently still holding the 33rd place all time. He was a beloved father, grandfather and best friend. He is survived by his best friend and mother of his children, Carol Fong; eldest son, Travis Opich; his daughter, Veronica Opich; his youngest son, Edward Opich III; and four grandchildren, Autumn Opich, Keren Opich, Nathaniel Opich, and Hellen Opich.

Joanna Henrietta Margaret Kirk January 12, 2021 Nunda: Joanna Henrietta Margaret Kirk, our dear Jo, died peacefully in her sleep on Tuesday evening, January 12, 2021 at Strong Memorial Hospital in Rochester NY, from complications resulting from cancer treatment and surgery. She was 57 years old. She is predeceased by her mother, Margaret Craig Kirk and her stepmother, Elsa Kirk-Boon. Jo is survived by her husband of 33 years, Charles (Chuck) Strickland in Nunda, NY; her father, Robin S. Kirk and spouse, Ina residing in the Netherlands; her brother, Robert Kirk and spouse, Tracy; and sons, Matthew and Oliver, residing in the UK; her sister, Georgina Kirk Becerra and daughter, Lauren residing in Texas; her sister, Mathilde Kirk and spouse, Daan van Kempen and children, Alex and Elsa residing in Belgium; and her brother, Eddie Kirk residing in the Netherlands. Jo was born in 1963 in Newcastle upon Tyne, UK, and grew up in NW England in Lancashire and Cumbria. Always academically driven, she completed her MA in jurisprudence (law) at Hertford College, Oxford University in 1984, and went on to accomplish a post graduate diploma in jurisprudence at the College of Europe in Belgium in 1985. In 1985 Jo’s adventuring spirit then took her to Colombo, Sri Lanka where she taught at an international school. There she met Chuck, who was in the USG foreign service managing agricultural projects. They married in Sri Lanka in 1988 and traveled together to a long-term USG posting in Nepal, followed by a Washington D.C. assignment, before moving to their farm in Nunda, NY where they have resided since 1995.

Jo was a full-time professor at SUNY Geneseo teaching in the faculty of the Sociology and Political Science departments. Her service at the college began in 1997 as editor and principal writer for college advancement publications. Jo’s love for teaching drew her into the classroom where she was in her element interacting with the bright, motivated students of SUNY Geneseo, and she received awards for her dedicated teaching. Jo also had a passion for Yoga, which she studied for many years and taught regular classes in both Nunda and Geneseo, NY. Jo embarked on so many adventures and projects throughout her life, there are too many to recall within the limits of an obituary. She was a bright, warm, loving soul who drew people together, and she will be much missed by students, friends and family all over the world. There will be no funeral service. A memorial event will be held in late 2021 after resolution of the Coronavirus pandemic.
