DEAR PAW’S CORNER: Flea prevention treatments can be expensive, so I’ve been soaking orange peels in rubbing alcohol and spraying it on my pets and around the house. It seems to work, but even though I can’t see a single flea on my Rottweiler-pit bull mix “Debbie,” she is still scratching fiercely around her ears until they bleed. I tried ear mite drops because I think that’s the problem, but she just runs away from me. I can’t afford the vet right now. How can I solve the ear mite problem? -- Melissa, via email

DEAR MELISSA: Stop using the rubbing alcohol on your pets immediately. It dries out a dog’s skin and causes irritation, and isopropyl alcohol can even poison a pet. The oil from the orange peel can be an irritant, too. Put the ear mite treatment on pause. It’s not improving the condition and is causing Debbie distress. It’s time to visit the vet. I know it’s expensive, but if money is an issue, tell the vet’s office when you call for an appointment. Low-cost vet clinics also may be available in your area. You can find them by searching online, asking the vet’s office or checking with the local pet-supply store. But Debbie needs to see a vet right now, because she’s suffering. To continue flea prevention without breaking the bank, consider treating your home with a product called diatomaceous earth (about $10-$14 for a 10-pound bag). Sprinkle it into the carpet and leave it for 48 hours, then vacuum it up. Reapply about once a month. You can even rub it into Debbie’s fur to kill fleas -- but only after her skin problem has been dealt with, because DE will irritate open wounds.

Some homeowners may wait to pick up a rake until all of the trees on their properties are bare. Chalkboards are either missing or completely retrofitted in modern learning environments. In the 1800s, slate blackboards were the new technology, replacing handheld tablets of wood or slate. Dr. Schulz Answers Back-to-School Questions BY AMANDA DUDEK

A

cross Rochester and the Finger Lakes Region, children have gone back to school. For many students this means returning to the classroom, whether as a hybrid with virtual learning or fulltime. During this unprecedented back-to-school experience, families find themselves with many questions. Providing clarity is Dr. Steven Schulz of Finger Lakes Medical Associates, Rochester Regional Health Pediatrician, and member of the New York State Finger Lakes Reopening Schools Safely Task Force. Several school protocols include “masks breaks” and circumstances during which masks are not required, leading parents to question what constitutes an “exposure” should a student at their child’s school contract COVID-19. The answer is surprisingly clear. “An exposure is close contact with someone with COVID-19,” said Dr. Schulz. “This is defined as being unmasked and within 6 feet of an infected person for longer than 15 minutes. The highest rate of transmission occurs from close contact.” “Mask breaks can be done safely with adequate distance, but best practice is for students to remain masked and socially distanced whenever possible, especially indoors,” Schulz continued, noting there is a lower risk of transmission when outdoors. Dr. Schulz and the Task Force are working with Common Ground to share and implement New York State Department of Health guidelines with all schools within a 13-county area. Although some cases are anticipated, Dr. Schulz encourages families that we are in an excellent place to be returning to school. “In the Finger Lakes Region, we only have a 1% prevalence rate,” he explained. “That means there is a 99% chance that a child’s symptoms are due to another virus that is not COVID.” However, Dr. Schulz advises parents to contact their health care provider for next steps if their child does develop symptoms which are also consistent with COVID. But how can a parent determine when their child’s symptoms are likely COVID-19 rather than simply a cold, allergies or something else? “That is the hard part,” Dr. Schulz acknowledges. “COVID in children tends to be milder with symptoms ranging from runny nose, congestion, cough, shortness of breath and sore throat to nausea, vomiting, diarrhea, abdominal pain, fatigue and poor appetite. What parents should be most aware of are new, different, or worsening symptoms, particularly a fever.” What happens if your child gives a positive response during the school’s screening protocol or is sent home due to related symptoms? “Students who have these COVID symptoms must have an examination,” said Dr. Schulz. “The NYS Department of Health requires that the student meet four criteria to return to school: 1. Receive an evaluation by their health care provider, 2. Have a COVID-19 test with negative results, 3. Obtain a letter from the child’s primary health care provider, 4. All the child’s symptoms must have resolved.” Dr. Schulz again emphasizes that “kids are being sent home for a wide variety of symptoms, many of which are likely not COVID related.” However, he recommends isolating the child as much as possible within their household until their test results are received. During this time, adults and children over 2 years old should wear masks and wash hands frequently, and surfaces should be disinfected. “It is better to be safe than sorry to keep the virus under control,” he added. “Stay home when your child is sick and be honest when answering screening questions.” For the children who receive a positive result on their COVID-19 test, returning to school is a little more involved. “The health department

Dr. Schulz is satisfied with the opening safety plans of area schools, which had to be approved by the state, adding that every family is different with its own tolerance of risk. “Every district is offering remote classes,” he observed. “At the same time, there are lots of benefits for attending school in person such as socialization, learning requirements, food stability and a stable place to stay. The schools are taking every precaution and are helping families to determine which choice is best for them.” Meanwhile, Dr. Schulz encourages people to talk to their local healthcare provider about their ability to get a flu shot. “It is especially important this year,” he said emphatically. “COVID-19 and the flu have a lot of overlapping symptoms, and theoretically, you could be infected with both.” He cautions that outbreaks of COVID-19 combined with outbreaks of the flu could overwhelm the healthcare system, reminiscent of the strain COVID-19 and the flu put on NYC’s healthcare system earlier this year. “Typically, the flu season starts in November, so we encourage people to get theirs by the end of October.” Regarding a future COVID-19 vaccine, Dr. Schulz believes it is only a matter of time. “There is a lot of vigorous study going into this,” he stated. “Vaccination trials are taking place regionally, and there is continued diligence regarding efficacy and safety. His first words were, “Now I guess I need to do a random act of kindness for a perfect stranger!” Yes, he plans to pay your good deed forward. In this chaotic and divided world,

I want to thank Stephani (hope name is spelled correctly) for assisting me with my banking transaction. In a world filled with so many stresses and sorrows, it was a delight conversing with Stephani. What a laugh we shared about her ring. We laughed so hard she actually turned red. BE GONE! To the people who attacked the person who complained about their neighbor’s loud music, shame on you for ganging up on someone who respects the town ordinance. Do you even know what kind of music that neighbor was playing? Do you just assume it’s the kind of music you like? Such ignorance to assume all music has the same effect on everyone! If your music is loud enough to rock your neighbor’s house, you are intruding! Period. If I want free concerts, I’d live next to a stadium. You were completely irresponsible to “suggest” that the original complainer move their residence in order to get some peace in their life. I suggest that you people find out who you all are, move into the same neighborhood, and play

your personal outdoor concerts against each other. MUSIC RULES One cannot exact “rules” that dictate peace and quiet. Next, would they like to get rid of children playing in a neighborhood? Perhaps they would have a problem with dogs barking. A lot of things are of precious value. One of them is the fact that for those who live in a neighborhood, they cannot monitor what people do, especially when we live where we have certain freedoms. If a person wants peace and quiet all the time, then go to a library. Problem solved! Maybe cars and motorcycles zooming by the road is infringing on their “peace and quiet.” Maybe they don’t like neighbors working on their house and making noise with a hammer or drill. The point

is you have to be realistic when it comes to noise, and it’s give and take, especially when it comes to being neighbors. NO RESPECT When did it become okay to enter a person’s private property and relieve yourself? I live outside of town surrounded by fields. Fields are perfect places to stop in those times of dire need. I have a son, and I remember those times that just seemed to come out of nowhere. “I have to go now!” is shouted from the back seat. You find the nearest, safest place to pull over. When did a person’s paved driveway become the acceptable place to do this? This hasn’t always been commonplace, just in the last year. First time was when a child was allowed to relieve himself right onto my driveway as cars passed by. The second time was

Orman September 30, 2020 AVON - ROCHESTER: He is survived by his children, Phyllis (Don) Vetter of MD, David Orman (Nancy Sullivan) of MD, Mark Orman of Staten Island, NY, Thomas (Xuan) Orman of CA; step-children, Robert (Helen) Hawkes, James ( Jennifer) Hawkes, Dawn (Gene) Chmiel; sister, Barbara Nostrant; brothers, James ( Joan) Orman, Albert Orman; several nieces and nephews. He was predeceased by his son, Mason Orman; wife, JoAnn Hawkes. Frank loved his country, was a Corporal in the US Army, and spent much of his career in the Defense contracting industry. He was a jack-of-all-trades and loved mechanical engineering, working on cars, and woodworking. Calling hours were held Wednesday, October 7, 2020 from 10 - 11 AM at the Stephenson-Dougherty Funeral Home, 350 Genesee St., Avon. Following calling hours there was a brief service. Interment was held at Avon Cemetery, Avon. In lieu of flowers Memorials may be made to the American Cancer Society.

Irv joined the US Air Force and served active duty from 1954-1958 and in the reserves from 1958-1962. He was honorably discharged and was always proud of his military service. He enjoyed time with family and friends the most. Irv was loved by so many. Everyone who met him would say he was such a kind and caring person. He always had a smile on his face. He was a huge Buffalo Bills fan and loved watching the football games. He also enjoyed baking muffins for his friends at Country Lane. Irv was known to some as “Irfie.” We will miss him very much.

SENIOR NEWS LINE By Matilda Charles Scammers' Tricks Are No Treat They never give up. Scammers are still taking every opportunity to steal our identity and separate us from our money, and the list keeps growing. Here are a few of the scams out there now: Seniors are getting calls from people who claim to be employees of the Department of Justice. Reports to the National Elder Fraud Hotline say that these scammers pretend to be investigators, and what they want is your personal information. If you get a call like this, don't be concerned that it's an authentic government call. It isn't. Just hang up. Maybe you'll get a call wanting to sell you an air purifier. They'll claim it will clean any coronavirus out of the air in your home. Maybe they'll try to get you to put it on a credit card, or they'll claim it's Medicare approved and all you need to do is give them your Medicare number. Don't fall for either of these tricks. Scammers will try these same tactics with miracle cures for COVID, masks that are "guaranteed" to protect you, face shields, vitamin supplements, free COVID tests, low-cost prescription drugs and much more. Sometimes scammers pretending to be from the government will try to get your personal information by saying they want to send you a stimulus check, or maybe they claim to need Census 2020 information. Sometimes it's about an expired warranty on your car or lower interest rates. Just hang up. Don't press 1 or any other number to have your number taken off their list. All that does is give them another opportunity to convince you to give up personal information. If someone you don't know leaves a message about any of these topics, ignore it. Remember that we seniors are the preferred target of these scammers. Don't fall for their tricks.

Gluten-Free Foods Aren’t a Sales Gimmick ---

DEAR DR. ROACH: I have no health issues, but I’m a cheapskate. I see products labeled as “grain-free” or as “gluten-free,” but they always cost more than the regular product. Are there any health benefits to using these, or are they marketing gimmicks? -- L.C. ANSWER: They are not marketing gimmicks at all. People with celiac disease have a sensitivity to a protein in gluten, called gliadin. Even small amounts of gluten can cause long-lasting damage to the intestines, and many people with celiac disease will be unable to properly absorb nutrients if they are regularly exposed to gluten. This can manifest with severe symptoms, such as weight loss, diarrhea and severe metabolic disturbance. Or it can trigger much milder ones, such as mild abdominal discomfort after eating, skin changes, anemia or joint pains. Because the symptoms of celiac disease are so varied, a physician must be fairly convinced it’s celiac before making the diagnosis. Biopsy is the gold standard for certainty, but blood testing -- if done while consuming a diet containing gluten -- is suggestive. I strongly recommend getting a diagnosis before going on a strict gluten-free diet if you suspect celiac disease. For people with diagnosed celiac disease, strict adherence to a gluten-free diet is essential, and the increasing availability of glutenfree foods has made the lives of people with celiac disease better. Many grains are gluten-free naturally. There are people with symptoms from gluten-containing foods but who do not have celiac disease. The term is “non-celiac gluten sensitivity.” Some people with this condition are sensitive to components of the food other than the gluten, although there are some people who are sensitive to gluten but do not have celiac disease. For people without NCGS and without celiac disease, avoiding gluten is not necessary. A diet with many different whole grains has been proven to reduce heart disease risk compared with a more meat-based diet. *** DEAR DR. ROACH: Please discuss the problem of the thirst mechanism as it relates to the elderly. I’m 70 years old and in almost perfect physical health. I bicycle every day -- 75 miles each week. Even with this enormous physical exercise, I am never thirsty all day and night. Also, I do not perspire. I need to force myself to drink water. Why don’t I feel thirsty? -- M.U. ANSWER: Thirst is a powerful motivation. In people with an intact thirst mechanism, the desire for water can be overpowering; it’s one of the body’s main ways of regulating salt and volume in the body. You are exactly right that as people age, the thirst mechanism becomes less powerful, and that tends to leave older people with a slightly higher amount of salt in the body compared with younger people. Remembering to drink water, especially on hot days, reduces the risk of dehydration. However, people who drink continually may never trigger the thirst response. There are many social cues to drink, and people who are not losing fluid through exertion and sweat may never trigger a strong thirst response. Drinking too much is as dangerous as not drinking enough: If thirst is really impaired, you need to use other mechanisms to monitor fluid status. For example, needing to urinate every several hours (at least) is one way of staying in the right zone. *** Dr. Roach regrets that he is unable to answer individual questions, but will incorporate them in the column whenever possible.
