Pulmonary Fibrosis Needs Exact Diagnosis ---

DEAR DR. ROACH: My husband was recently diagnosed with mild to moderate pulmonary fibrosis. His mother died from pulmonary fibrosis. From what we have read there is no cure, only palliative care. The thought of doing nothing to try to stop the progression is totally unacceptable when there have been such successes with immunotherapy and stem cells. Would he receive more aggressive treatment going to a pulmonary fibrosis care center that has lung disease programs? -- C.D. ANSWER: There are several diseases that may cause fibrosis of the lung, so having an exact diagnosis made by an expert is critical for getting the right treatment. The most common type of pulmonary fibrosis is called “idiopathic pulmonary fibrosis” (it also has been called “usual interstitial pneumonia” and “cryptogenic fibrosing alveolitis”), and I am going to discuss that diagnosis. The cause of IPF is unknown (both “idiopathic” and “cryptogenic” mean “of unknown cause”), but cigarette smoking is a risk factor. It is usually diagnosed in people who are their 50s and 60s, although some familial cases may come on earlier. IPF can run in families. Although it is a rare disease, it seems to be on the rise. The major symptoms are cough and shortness of breath. There is no cure for IPF, but there are treatments available. Two drugs, pirfenidone and nintedanib, are approved for use in the U.S. and Canada. Both of them slow progression of lung disease, reduce exacerbations and have been proven to reduce mortality from the condition, if only slightly. I am concerned that you and your husband apparently haven’t heard of these drugs. I strongly advise you get a referral to a center for lung disease in order to get your husband the best care. I found 35 IPF trials recruiting patients in the U.S. and six in Canada. A clinical trial will help scientists understand this condition as well as determine the best current treatment. You also can find out what centers have expertise on this condition by seeing where the trials are conducted at www.clinicaltrials.gov. The most aggressive treatment isn’t always the best, but you and your husband deserve to hear about all the options. The Pulmonary Fibrosis Foundation can help you find experienced centers for treating this condition. You can find them online at www.pulmonaryfibrosis.org. *** DEAR DR. ROACH: My niece recently was diagnosed with breast cancer. Radiation was recommended. Her mother said that first she’d have to boost her immune system. Is it possible to actually do that? -- P.P.L. ANSWER: Radiation is a common treatment after breast cancer surgery. It’s done to reduce the risk of recurrence, depending on the exact type of cancer. Boosting the immune system is a new adjunct for many types of cancer. There are several ways to do this: One is to genetically modify the patient’s own immune T-cells, but many other techniques are in trials now. However, if your niece’s mother is talking about supplements, I’d be cautious. Supplements that are promoted to boost the immune system generally are ineffective at doing so. Making sure your niece has proper nutrition, gets good sleep and experiences as little stress as is manageable are ways to prevent stress on the immune system. Everyone could benefit from those deceptively simple steps, but especially a person being treated for cancer. *** Dr. Roach regrets that he is unable to answer individual questions, but will incorporate them in the column whenever possible. INEFFICIENCY I am 39 years old. I have grown up around dogs, and my dad used to adopt from (local shelter) years ago. (Local shelter) now forces you to make an appointment to even enter the building. I saw two dogs on their website that I wanted to look at, so I made an appointment that put me a week out. By the day of the appointment, the two dogs I was interested in were already adopted. The whole point of making the appointment was to view these dogs. What a completely inefficient way of conducting your operations, and don’t you dare blame it on coronavirus. I don’t appreciate having my time wasted. Am I left to assume I have to make an appointment and hope there happens to be a dog I want at that moment, or else I’m out of luck? You are making adoptions more difficult than they have to be!

ABUSING HOTELS AND MOTELS I manage a hotel in (local town) and want to vent about unacceptable behavior I’ve observed over the last 7 years. Local people, almost exclusively from (local city), will reserve a room by pooling money amongst 6-10 friends to afford the room. Once in the hotel, they play loud music, smoke, come in & out throughout the night, have parties, making excessive noise. They bring many non-guests to the swimming pool, and we’ve needed to shut down our pool on several occasions because of poor water quality. They have absolutely no consideration for our other valued guests. Here’s a suggestion to those trashing our hotels: stay at one of your friends’ houses, smoke, drink and make as much noise as you want. Don’t do it in a hotel!

IN RESPONSE TO NOSY NEIGHBOR Upon reading the post, I don’t feel that the person was nosy or spying. You also stated that the person makes the rules on his property. Well, in the times we live in, I think, in some ways, that is changing. As the Governor stated long ago, no one has the right to make anyone else sick with corona virus. They are enforcing wearing a mask, if you haven’t noticed, also social distancing. I would be disturbed if I saw neighbors, whomever, blatantly disobeying common sense things, such as social distancing, and the wearing of masks. That does not make a person nosy, just alert to what is going on in their surroundings. Disobeying such things as having a large group over your house, without social distancing, nor masks, can potentially be life threatening to the rest of us.

DEAR PAW’S CORNER: Whenever I’m videoconferencing from home, my cat insists on jumping into my lap and pawing at the screen and the keyboard. If I put him outside the room, he will meow loudly and nonstop until I let him back in. Once the call is finished, he quiets down and goes away. To my co-workers, this is just a funny distraction, but I worry that it will have a negative impression on clients. How can I calm him down? -Melanie in Pittsburgh DEAR MELANIE: This is a tough one that many cat owners have faced. And now that so many of us are exclusively working from home, it’s going to continue -- unless you take some proactive steps. Cats jump onto your keyboard because they know it gets your attention. They’re not trying to figure out how to write social media posts. They just want you to pay as much attention to them as you do to the laptop screen. And locking a cat out of the room can trigger separation anxiety or just plain frustration that they can’t get in to be with you. Try this: Before a video call, give your cat 10 to 20 minutes of undivided attention. Pet him, play with his favorite toys, scratch his belly. This may help stop or reduce the number of times he jumps into your lap. Use positive reinforcement to teach your cat to do more of what you want and less of the things you don’t want him to do. Wild Wings Educates on Wildlife at Mendon Ponds Nature Center By Amanda Dudek

M

any Greater Rochester area residents have likely heard of Wild Wings, a not-for-profit educational wildlife organization. However, many are not aware of what they do, how they are funded and the crucial role they play for area wildlife. Jessica Kozakiewicz, a Wild Wings volunteer with over 10 years of experience, fills readers in on the organization’s mission, challenges, new happenings and more. Wild Wings is not a wildlife rehabilitator, but rather, they give animals a home after rehabilitation. “We provide a home for animals which have already been rehabilitated and have been deemed non-releasable,” said Kozakiewicz. Wild Wings primarily cares for injured birds of prey which are unable to survive on their own in the wild such as eagles, hawks, falcons, vultures, and owls. “Any animal that is non-releasable is typically euthanized,” she explained. “Rather than let that happen, we give them a permanent home, and they partner with us for education.” In fact, Wild Wings’ mission is to “teach environmental stewardship through programs featuring [its] resident raptors as [its] teaching partners.” Volunteers, trainers and handlers travel with some of these birds of prey to present at schools and events. Kozakiewicz participates in training, handling and educational programs which now have another facility from which to serve the community: the Mendon Ponds Nature Center within Mendon Ponds Park. Wild Wings, currently houses and cares for 34 permanently injured non-releasable birds of prey. “These birds are of various species, both native and non-native to New York State,” shared Kozakiewicz. “We now

house injured songbirds in our new aviary, providing visitors with a new educational experience. We have also had a bobcat for the last several years.” “We just took in two new fox kits, which is new for us at Wild Wings,” shared Kozakiewicz. “We plan to have a good variety of reptiles that you would see in the park, such as a number of snakes, turtles, frogs, toads and fish species.” Wild Wings hosts educational programs and hikes, as well, where visitors can learn about animals and the environment, although things have been difficult in 2020. “COVID has certainly been challenging,” Kozakiewicz acknowledged. “It closed our facility down, and we weren’t able to have the public come through. We are also on a lease agreement for the Nature Center, and while many people believe we are funded through Monroe County or other government agencies, we are not. We are funded entirely by donations with some help from the fees of our educational programs. We had to cancel some of our larger fundraising events and lost several fundraising opportunities.” Kozakiewicz believes Wild Wings and the impact it has had and continues to have on wildlife education and conservation is too important to let fall by the wayside. “At Wild Wings, people get to see the world as they haven’t before,” she explained. “They get to know our resident birds and other animals. When people leave us, they can identify birds on these park trails and realize why some of them are helpful to humans. They

want to protect these species in the wild, preserving them in the natural world.” It appears to be working. “One of the best examples of human wildlife conservation is what has happened with our local bald eagle population,” shared Kozakiewicz. “They were almost completely extinct, in large part due to the use of the pesticide, DDT. The bald eagle populations were very low, and people realized this was a threat. They joined together, stopped widespread use of DDT, and encouraged numbers to grow. Today, there are bald eagle populations on Lake Ontario and every single Finger Lake.” It is imperative, then, that Wild Wings continues providing for these birds in need as well as for its education programs which help people appreciate them. “We are looking at different ways to be able to offer more private, individualized group settings for now,” said Kozakiewicz. “The nature center is one of our biggest expansions yet, and we are working hard to be able to offer it this fall. We plan to talk about the different trails in the park, its ecology, and which species are found within the park. Also, we are always very open to things visitors would like to see and learn about with regards to the animals we care Jessica Kozakiewicz with Kisonsen for.” “It has always been so inspiring to me to work up close with these amazing animals,” Kozakiewicz added on a more personal note. “I feel such a sense of pride that I get to share this knowledge with all our visitors, to help them to see something new and inspiring in the world around them, as well.” Like many not-for-profit organizations, however, the future of Wild Wings, its educational efforts, and the magnificent birds they care for will depend in many ways on the support of those who share its love for these incredible birds of prey, songbirds and other wildlife. “We are almost completely supported by the generosity of public donations,” affirmed Kozakiewicz. “Our supporters can be assured that, as a primarily volunteer-run organization, funds donated to us are going to our animals and furthering our mission of education, allowing it and us to continue.” “We are a local, grassroots organization built by the community for the community,” said Kozakiewicz. “Those who donate can clearly see the impact of their investment in us.” Enjoy this article? THE GROWTH OF THE SOLAR EMPLOYMENT SECTOR According to the Solar Foundation’s National Solar Jobs Census 2016, there were 260,077 solar workers working at 8,601 solar companies in the United States in 2016. The census also found that the five states with the most solar jobs in 2016 were California, Massachusetts, Texas, Nevada, and Florida. Twenty-eight percent of state solar workers in 2016 were females, making women the largest demographic in the solar industry. Latinos and Hispanics (17.2 percent), Asians or Pacific Islanders (9.1 percent), and veterans of the U.S. Armed Forces (9 percent) rounded out the top four demographics employed by the solar industry. Of the 260,077 solar industry jobs in 2016, more than 137,000 were classified as installation jobs that paid a median wage of $26 per hour. With roughly 38,000 jobs in 2016, solar manufacturing was the next biggest employer of workers in the solar industry.
