Cologuard Test Worth Risk of False Positive ---

DEAR DR. ROACH: I am a 77-year-old man in good health. At my recent annual physical exam, my physician ordered a Cologuard kit to be sent to me to screen for colorectal disease. I have never had symptoms of disease or problems with any part of my gastrointestinal tract. I had two colonoscopies at different times several years ago, the results of which were negative. When I got the kit, I read the patient guidelines and learned that the Cologuard test could result in false positives and that there is an unspecified increase in the risk of false positives for persons over the age of 75. A positive result from the test requires a follow-up colonoscopy, which is also riskier for elderly patients. Since I am in good health with no symptoms of any disease, I decided I did not want to risk a false positive result from the test, which would require a follow-up colonoscopy. Did I make the right decision? -- E.G. ANSWER: Cologuard is a brand of test that looks at DNA in stool for evidence of cancer. Certain DNA mutations are highly suggestive of cancer. In addition, Cologuard does an immunochemical test looking for blood. Because it combines these two techniques, Cologuard is more sensitive than only a test looking for blood. Compared with colonoscopy, Cologuard is about 92% sensitive in finding cancers, meaning it will miss about 8%. With improved sensitivity comes a lack of specificity, which means you could have a false positive result. The Cologuard can say that you have an abnormality when a colonoscopy would indicate normal. For 45% of people with a positive Cologuard test, no abnormalities will be found upon colonoscopy. You are right that false positives are more prevalent in older individuals. There are two reasons that I think the Cologuard is still a reasonable choice, despite the fact that it isn’t as good as colonoscopy. The first is that I continue to recommend a screening test for healthy 77-year-old men, and although colonoscopy is my first choice, a normal Cologuard test prevents a need for colonoscopy, and a false positive would lead to the same colonoscopy you would have gotten anyway (although there probably is more anxiety after a false positive test). The second is that a person with an abnormal Cologuard and a normal colonoscopy may be at risk for an abnormality that wasn’t found on colonoscopy. A study looking at people in this category found a 25% risk of abnormality on follow-up colonoscopy, if the Cologuard was abnormal again one to two years later. *** DEAR DR. ROACH: I had lung surgery in 2014. The scar is still red and sore. I got no instructions on how to deal with it. Do you have any suggestions? -- J.S.B. ANSWER: That’s a very long time for the scar to still be red and sore, and I would encourage you to have the surgeon or your regular doctor take a look. For people with redness and soreness in a scar shortly after surgery, I recommend silicone gel (there are many available), as it has been shown to be effective. Many people recommend vitamin E oil, but well-done studies have shown this does not help and may harm the cosmetic appearance. *** Dr. Roach regrets that he is unable to answer individual questions, but will incorporate them in the column whenever possible. UR STRONG I want to give a huge shout out to the many nurses I met during my stay this past month. Each and every one of you in your own way made my stay a little more bearable. CAR ACCIDENT ANGEL I was in a bad car accident on Route 247 in Canandaigua on Friday, August 28th, and right after my car was hit I was
so scared and upset, but a very kind woman came to me and held me as I cried. She stayed right by side until the ambulance came. She was my angel, and I am forever grateful and blessed for her act of kindness. I did not get her name, but God bless you, and thank you so very much for being by my side. There are still kind people in this world, and she is definitely one of them! ABOVE AND BEYOND I want to recognize Alden Smith who owns Smith Solutions, our garbage service in Dansville. He is hands down the hardest working, most honest and respectful young man I know. He

BARE FEET After I left the store, you asked the cashier why I was allowed to be barefoot. (My son was behind you in line.) Had you asked, I would have been happy to explain US Department of Justice 28 CFR Part 36 (.201 and .302) protects a person’s right to go barefoot due to health, medical or disability reasons. It is a violation to ask why the person is barefoot, just as it would be to ask why a person is in a wheelchair. To educate: I have neuropathy, deadening the feeling in my feet. The greater my points of contact with the ground, the less likely I am to fall. If you pay attention, you will notice there is no longer a sign on any public entity stating no shirt, no shoes, no service due to this law. So, if you see me barefoot, please do not run to get the manager. I am a person with disabilities.
OUT OF SEASON My brother has 30 acres of land and has been watching a large turkey family in the fields eating grasshoppers. Something spooked them and they all flew across the road onto a certain property. This guy grabs his gun and starts shooting! My brother flipped out as he watched this. This person said he wanted a turkey for dinner after he lied about it saying the turkeys scared his horses, claiming he shot over the horses at the turkeys. My brother does not know for sure if he hit one, but if he got the hen, those babies surely won’t make it. I think these people are shooting deer, as well as rabbits and anything they can eat. What makes them special that they can shoot everything that is out of season? If I catch them doing this, I will turn them in.
OVERPOPULATION The only real way to solve the pollution and trashing of the earth’s environment is to have fewer people on the planet. Overpopulation is the source off the problems, and until people stop having more than two kids, the problems are only going to get worse. That is why there are over 8 billion people on earth. Magazines can publish good ideas and suggestions, which is fine, but in the end, it is too many people are the basic problem. I know that where I live, we had a beautiful hardware store set up business. It was so wonderful to have a place so close to home. They once told us what a difficult

Just want to express my appreciation to the two Ontario County Sheriff’s Deputies who rescued me when I carelessly locked my keys and phone in my truck when I stopped at a veggie stand on 5 and 20 on Sept. 9th. Badly Behaved Dogs Need Basic Commands DEAR PAW’S CORNER:
Every time I read an article about a badly behaved dog, the trainer says that the dog needs to relearn basic commands like sit, stay and lie down. Why would unrelated commands solve a behavior problem? -- Edward in Albany, New York DEAR EDWARD: Every dog should be trained to respond to five basic commands: come, heel, sit, stay and lie down. Dogs need to obey their owner the first time and every time a command is issued. That may sound a little harsh, but a dog that can’t be controlled is a dog in danger. Owners need to be able to recall a dog that has slipped its leash before it runs into traffic. They need to make sure their dog will walk beside them and will sit calmly on command, especially when another dog approaches. Their dog needs to stay in position when told to do so -- whether sitting or lying down. Dogs aren’t robots, though. Like us, their discipline and training will slip if not regularly reinforced. Think about how often you and I give in to temptation and take that second cookie (or third or fourth or fifth). Just as we humans sometimes need reinforcement of good habits, our dogs need regular reinforcement of basic commands. When owners spend more time working one-on-one with their dogs, they frequently report that undesirable behaviors happen less often. The time and attention that owners give can quickly blunt attention-seeking behaviors. The AKC has a good breakdown of the basic dog commands here: www.akc. org/ expert-advice/training/ and search for “5 basic commands.” Energetic young dogs can be a handful sometimes. If you are having trouble getting your dog to respond to basic commands, enlist the help of a professional trainer.

Dr. Schulz Answers Back-to-School Questions BY AMANDA DUDEK
A
cross Rochester and the Finger Lakes Region, children have gone back to school. For many students this means returning to the classroom, whether as a hybrid with virtual learning or fulltime. During this unprecedented back-to-school experience, families find themselves with many questions. Providing clarity is Dr. Steven Schulz of Finger Lakes Medical Associates, Rochester Regional Health Pediatrician, and member of the New York State Finger Lakes Reopening Schools Safely Task Force. Several school protocols include “masks breaks” and circumstances during which masks are not required, leading parents to question what constitutes an “exposure” should a student at their child’s school contract COVID-19. The answer is surprisingly clear. “An exposure is close contact with someone with COVID-19,” said Dr. Schulz. “This is defined as being unmasked and within 6 feet of an infected person for longer than 15 minutes. The highest rate of transmission occurs from close contact.” “Mask breaks can be done safely with adequate distance, but best practice is for students to remain masked and socially distanced whenever possible, especially indoors,” Schulz continued, noting there is a lower risk of transmission when outdoors. Dr. Schulz and the Task Force are working with Common Ground to share and implement New York State Department of Health guidelines with all schools within a 13-county area. Although some cases are anticipated, Dr. Schulz encourages families that we are in an excellent place to be returning to school. “In the Finger Lakes Region, we only have a 1% prevalence rate,” he explained. “That means there is a 99% chance that a child’s symptoms are due to another virus that is not COVID.” However, Dr. Schulz advises parents to contact their health care provider for next steps if their child does develop symptoms which are also consistent with COVID. But how can a parent determine when their child’s symptoms are likely COVID-19 rather than simply a cold, allergies or something else? “That is the hard part,” Dr. Schulz acknowledges. “COVID in children tends to be milder with symptoms ranging from runny nose, congestion, cough, shortness of breath and sore throat to nausea, vomiting, diarrhea, abdominal pain, fatigue and poor appetite. What parents should be most aware of are new, different, or worsening symptoms, particularly a fever.” What happens if your child gives a positive response during the school’s screening protocol or is sent home due to related symptoms? “Students who have these COVID symptoms must have an examination,” said Dr. Schulz. “The NYS Department of Health requires that the student meet four criteria to return to school: 1. Receive an evaluation by their health care provider, 2. Have a COVID-19 test with negative results, 3. Obtain a letter from the child’s primary health care provider, 4. All the child’s symptoms must have resolved.” Dr. Schulz again emphasizes that “kids are being sent home for a wide variety of symptoms, many of which are likely not COVID related.” However, he recommends isolating the child as much as possible within their household until their test results are received. During this time, adults and children over 2 years old should wear masks and wash hands frequently, and surfaces should be disinfected. “It is better to be safe than sorry to keep the virus under control,” he added. “Stay home when your child is sick and be honest when answering screening questions.” For the children who receive a positive result on their COVID-19

test, returning to school is a little more involved. “The health department will go through contact tracing and determine when it is safe for that child and others to return,” said Dr. Schulz, noting that several advances in treatment are now available. “Household members will also be quarantined and should be tested, as well.” Within those children’s schools, the local health department will take over, determine high and low risk Dr. Steven Schulz, Finger Lakes exposures, and reach out Medical Associates, Rochester Regional to individuals as necessary. Dr. Schulz is satisfied with the opening safety plans of area schools, which had to be approved by the state, adding that every family is different with its own tolerance of risk. “Every district is offering remote classes,” he observed. “At the same time, there are lots of benefits for attending school in person such as socialization, learning requirements, food stability and a stable place to stay. The schools are taking every precaution and are helping families to determine which choice is best for them.” Meanwhile, Dr. Schulz encourages people to talk to their local healthcare provider about their ability to get a flu shot. “It is especially important this year,” he said emphatically. “COVID-19 and the flu have a lot of overlapping symptoms, and theoretically, you could be infected with both.” He cautions that outbreaks of COVID-19 combined with outbreaks of the flu could overwhelm the healthcare system, reminiscent of the strain COVID-19 and the flu put on NYC’s healthcare system earlier this year. “Typically, the flu season starts in November, so we encourage people to get theirs by the end of October.” Regarding a future COVID-19 vaccine, Dr. Schulz believes it is only a matter of time. “There is a lot of vigorous study going into this,” he stated. “Vaccination trials are taking place regionally, and there is continued diligence regarding efficacy and safety. It will be a big step and help put this pandemic behind us.” Enjoy this article?
