Aggressive Dogs Keep Owner Away From the Dog Park

MOMENTS IN TIME The History Channel * On Jan. 15, 1870, the first recorded use of a donkey to represent the Democratic Party appears in Harper’s Weekly, drawn by political illustrator Thomas Nast. Four years later, Nash originated the use of an elephant to symbolize the Republican Party in a Harper’s Weekly cartoon. * On Jan. 16, 1938, Benny Goodman brings jazz to Carnegie Hall, a notion so outlandish at the time that Goodman himself initially laughed off the idea. The concert at the citadel of American high culture sold out weeks in advance. * On Jan. 17, 1950, 11 men steal more than $2 million ($22 million today) from the Brink’s Armored Car depot in Boston. It was the perfect crime --almost -- as the culprits weren’t caught until January 1956, just days before the statute of limitations for the theft expired. DEAR PAW’S CORNER: I’ve had to change the time of day that I visit the local dog park, because a new neighbor brings his two huge, aggressive dogs each morning at the same time as I do. They charge other humans, chase and snarl at other dogs, and make it impossible for other dogs to socialize calmly. The neighbor does nothing to control them and tells other owners that it’s their fault that his dogs are acting aggressive. To prevent injury to my dog (or myself), I now wait until after work to go. But this is inconvenient for everyone in the neighborhood. How do I address this problem? -- Dog Mom in New York DEAR DOG MOM: You’re doing the right thing by changing the time that you bring your dog to the park. It’s important to remove a dog from a situation you perceive as potentially dangerous or traumatic for them, and protecting them from harm. That’s only the first step, however. The new dogs are terrorizing other dogs within the park, making life miserable for them and their owners. It’s not fair, nor is it right. Dog parks within a city or town typically have rules of conduct posted at each entrance, but sometimes they’re vague. It’s up to each owner to properly train and socialize their dogs, and to keep them away from the park if they can’t control rough play, aggression or reactivity. Report the problem to the organization in charge of maintaining the dog park. This may be a community group or the municipal parks department. Contacting animal control is another option. Write down the dates and times that incidents have occurred. BRAIN-HEALTHY HABITS TO EMBRACE Cognitive decline is a condition that is often associated with aging, but even middle-aged people can experience memory loss or cognition issues. The Alzheimer’s Association says that more than five million Americans are living with Alzheimer’s disease and other dementias. By 2050, that number could rise to as high as 16 million people. More than 747,000 Canadians are living with Alzheimer’s or another dementia, says the Canadian Alzheimer’s Association. Although there is no definitive way to prevent dementia, living a long, vibrant life may be possible by encouraging some healthy habits for the brain. It is never too late or too early to begin health and lifestyle changes. Exercise: Becoming more active can improve brain volume, reduce risk for dementia and improve thinking and memory skills. The journal Neurology found that older people who vigorously exercise performed better on cognitive tests than others of the same age, placing them at the equivalent of 10 years younger. Increased blood flow that occurs with physical activity may help generate new neurons in the hippocampus, an area of the brain involved with learning and memory. The Harvard Medical School says aerobic exercise may help improve brain tissue by improving blood flow and reducing the chances of injury to the brain from cholesterol buildup in blood vessels. Quit smoking: The Alzheimer’s Association indicates that evidence shows smoking increases the risk of cognitive decline. Smoking can impair blood flow to the brain and cause small strokes that may damage blood vessels. Eat healthy foods: Foods that are good for the heart and blood vessels also are good for the brain. whole grains, fish-based proteins, unsaturated fats, and foods containing omega-3 fatty acids. Neurologists state that, while research on diet and cognitive function is limited, diets, such as Mediterranean and Mediterranean-DASH (Dietary Approaches to Stop Hypertension), may contribute to a lower risk of cognitive issues. Consume caffeine: Caffeine may help boost memory performance and brain health. A Journal of Nutrition study found people ages 70 and older who consumed more caffeine scored better on tests of mental function than those who consumed less caffeine. Caffeine may help improve attention span, cognitive function and feelings of well-being. Information from Psychology Today also indicates caffeine may help in the storage of dopamine, which can reduce feelings of depression and anxiety. In addition, compounds in cocoa and coffee beans may improve vascular health and help repair cellular damage due to high antioxidant levels. Work the brain: Engaging in mentally stimulating activities can create new brain connections and more backup circuits, states Dr. Joel Salinas, a neurologist at Harvard-affiliated Massachusetts General Hospital. Working the brain through puzzles, reading and participating in social situations can stimulate the release of brain-derived neurotrophic factor (BDNF), a molecule essential for repairing brain cells and creating connections between them. A good way to combine these lifestyle factors is to take an exercise class with friends, mixing the social, stimulation and exercise recommendations together. Cognitive decline can come with aging, but through healthy habits, people can reduce their risk of memory loss and dementia.

DID YOU KNOW? A landmark study conducted by researchers in the United Kingdom discovered a notable link between low to moderate alcohol consumption and breast cancer risk. According to the Million Women Study, every 10 grams (roughly 0.35 ounces) of alcohol a woman consumed per day was associated with a 12 percent increase in her risk for breast cancer. The Million Women Study focused on various areas of women’s health but included 28,000 women who had breast cancer. In addition, the National Institute on Alcohol Abuse and Alcoholism notes that more than 100 epidemiologic studies have examined a potential association between alcohol consumption and the risk for breast cancer, consistently finding that risk for the disease increases as alcohol intake increases.

Hot Flashes Persist Well Past Menopause and Thyroidectomy

--DEAR DR. ROACH: I am 81 and had a thyroidectomy in 2017. My whole thyroid gland was removed. I also had a little bit of cancer. I have started to get hot flashes again. I had originally been on thyroid medicine before and after surgery. Then I was put on levothyroxine and decided to try NP Thyroid. I was doing better on NP Thyroid, then started getting the hot flashes again. At this point, for several reasons, I would rather stay on NP Thyroid for the time being. I was wondering if there is anything I can take for hot flashes? Years ago, in my 50s, I took estrogen, which helped, but this is a different time. I have spoken with other people who are also having hot flashes again. -- Anon. ANSWER: Hot flashes are common -- about 75% of women in North America will experience them around the time of menopause. It’s not uncommon for hot flashes to go away, then return, but I personally haven’t seen them recur after 30 or so years. I’d be concerned that there may be another cause for them, which brings me to your thyroid. Elevated levels of thyroid hormone can cause sensations very similar to hot flashes. One reason that I, along with most endocrinologists, recommend against products like NP Thyroid is because most of those products come from pigs. There are two thyroid hormones: thyroxine (T4) and triiodothyronine (T3). Humans have a different ratio of T3 and T4 than pigs do, with T4 getting converted into the active form, T3. T3 is then quickly broken down, so if it’s used, it needs to be dosed twice daily. As a result, if a person takes NP Thyroid, their thyroid levels are too high during one part of the day and too low during another. This isn’t good for anyone; the “high times” not only cause hot flashes, but also predispose people to atrial fibrillation. But it’s particularly a problem in a person with a history of thyroid cancer, where we want the thyroid-stimulating hormone (TSH, the hormone in the pituitary gland that regulates the activity of the thyroid gland) to be on the lower side. If the thyroid blood levels get low in the afternoon/evening when the T3 in NP Thyroid is gone, that can theoretically increase the risk of cancer recurrence as the TSH rises in response. So, in my opinion, you should be on levothyroxine, not NP Thyroid. (I do have a handful of patients who take levothyroxine and also take T3 twice daily. Some people are unable to convert T4 to T3 efficiently.) Estrogen is the most effective treatment for hot flashes, but it increases the risk of heart disease when used by women more than 10 years away from menopause. I don’t normally prescribe it to a woman in her 80s. However, there is a new medication called fezolinetant (Veozah), which is highly effective and reduced hot flashes by 93% in a trial. My only patient on it so far has reported 100% cessation of her hot flashes. There are other rare causes of hot flashes, including tumors that secrete substances, such as carcinoid tumors, and tumors that secrete adrenalin-like hormones (pheochromocytomas).

Gratitude Finland has been rated the world’s most “happiest” country. Connection with nature, community and simplicity are three of the major factors cited as to the Finnish quality of life. However, there is an over-riding factor that they’ve learned that stands out for them and perhaps for us - gratitude. To quote: “The Nordic countries are united in their embrace of curbed aspirations for the best possible life. This mentality is famously captured in the Law of Jante -- a set of commandments believed to capture something essential about the Nordic disposition,” he explains. They include: “You’re not to think you are anything special” and “You’re not to imagine yourself better than we are.” Which, as Savolainen points out, is pretty much the opposite of America’s endless pursuit of personal and national exceptionalism. This is a great time of year to take stock of what we all have and be grateful. Amen.

Our new delivery person refuses to drop my paper off at the back of my short drive on the ramp so I can easily pick it up. I’ve posted a sign at my drive asking them to do this to no avail. They drop it in the gutter and if it rains it gets wet and if it snows the plow takes it away. Am I only person in [local town] to have this problem and is there anything that can be done to help? I’m 96 years old!

THE HEALTH BENEFITS OF DAIRY Heart health benefits: Some people avoid dairy because of perceptions that it is bad for cardiac health. A report presented at the 2018 Congress of the European Society of Cardiology that looked at 20 studies involving around 25,000 people found no association between the consumption of most dairy products (including whole fat varieties) and cardiovascular disease. The only exception was milk, but the results showed one would have to drink almost a liter of milk a day for a higher risk of cardiovascular disease. Still, the American Heart Association recommends adults choose fat-free or low-fat dairy products whenever possible. Also, a 2018 study published in the British Journal of Nutrition found men who ate plenty of fermented dairy products like yogurt and cheese had a smaller risk of coronary artery disease than men who ate less.

GREEN WAYS TO CLEAN UP SNOW Although residents of chilly climates come to expect the flakes and gathering snow mounds, some people never quite get used to the cleanup involved in keeping neighborhoods safe and accessible. In M arch 2014, North America w as covered by the third-highest amount of snow that late in the season since record keeping began in 1966, according to NOAA’s U.S. National Ice Center. Snow can begin in September and October in some northern areas and stretch on until early spring. estled on the edge of Mendon Ponds Park, Wild Wings Inc. stands as a beacon of hope for injured and non-releasable birds of prey. With both the iconic bird of prey facility and the Mendon Ponds Park Nature Center, this not-for-profit organization has an incredible team of volunteers and staff helping make a profound impact on the community. At its helm is Terry, Executive Director, whose dedication to wildlife and nature has spanned two decades. Terry’s journey with Wild Wings began 20 years ago, driven by a shared passion for animals and “my son and I wanted to volunteer with animals because of our interest in birds.” Originally from Irondequoit, now residing in Henrietta, Terry’s connection to the community has deepened over the years, committed to connecting people with the natural world to help foster a greater appreciation for it. Terry emphasizes the crucial role Wild Wings plays in wildlife conservation, reflecting that, “The animals at Wild Wings would be euthanized due to their injuries if not placed at an organization like Wild Wings. We are able to care for them, give them a second chance at life, and educate the public about their species and the environment.”

With around 40 dedicated volunteers, Wild Wings thrives on the support of its community. Daily operations involve the feeding and cleaning of animals and their enclosures, coupled with ongoing facility maintenance. The tireless efforts of volunteers ensure that the organization fulfills its mission.

For Terry, every day at Wild Wings brings new adventures. The eclectic mix of caring for animals, providing educational programs, and the dynamic nature of the work keeps him engaged.
