MEDICATION-INDUCED DRY MOUTH Not only can the condition known as “dry mouth” (xerostomia) arise because of certain health conditions (diabetes, stroke, thrush, etc.), but it may also be a side effect of some prescription and over-the-counter medications. In fact, there are up to four hundred medications that can contribute to dry mouth. These drugs include antihistamines, decongestants, painkillers, muscle relaxants, tranquilizers, diuretics, antidepressants, and drugs that treat high blood pressure, Parkinsonís disease, and incontinence. Anti-seizure medications, immunosuppressant drugs, and channel calcium blockers may also cause “gingival overgrowth” (enlarged gum tissue). It is important to identify any medication that causes dry mouth and adjust the dosage or replace it with

massive heart attack in April. My father died of a massive heart attack at 49. I have four stents in three arteries and am on Brilinta, low-dose aspirin, blood pressure medication, etc. I have essential tremor, first noticing the tremors in middle school. The tremor is in my hands, making writing nearly impossible, and I have head bobs. I have an internal tremor that never stops. A previous practitioner prescribed a month’s trial of beta blockers several years ago. I noticed no relief with the tremor but was physically ill for the first 30 minutes of the morning in the first two weeks. Recently, I found an online blog regarding essential tremor and CBD oil. Nearly all of the bloggers found some level of relief using this treatment. If I choose to try CBD oil to assist with my tremor, is there a risk of it interfering with my heart and blood pressure medications? -- M.M. ANSWER: Cannabidiol, or CBD, is a nonpsychoactive substance found in Cannabis, as opposed to tetrahydrocannabinol, or THC, the best-known psychoactive component. CBD is typically sold as an oil and has been receiving press as a potential treatment for a wide variety of medical issues. Many or most of these claims have no data to support them. Anecdotal reports, such as most blogs, are neither reliable nor scientific (and may or may not be true). There also is an issue with proving the dosage and purity of products said to contain CBD. However, in the case of essential tremor, there is some evidence: Studies in mice have shown benefit with CBD, although a single case report showed effectiveness of THC but not CBD in essential tremor. The issue of drug interactions is a significant one. CBD inhibits two powerful pathways the body uses to detoxify drugs, the CPY3A4 and CYP2D6 systems. This is a serious issue for you. Ticagrelor (Brilinta), an anticoagulant, is metabolized by CYP3A4, as are some of the statin drugs usually prescribed to people with heart blockages. The effect of the anticoagulant could be much higher than expected, leading to bleeding risk. Other commonly used heart medications are metabolized by CYP2D6, and the effect on these medications is unpredictable. I can’t recommend CBD products given the medications you are taking, but would suggest you consider alternative treatments. You may have read about deep brain stimulation and ultrasound, two powerful and effective treatments for people whose symptoms have not responded to standard treatments. ***

DEAR DR. ROACH: This question is for my husband. How safe is drinking tea while on warfarin? He has his INR checked monthly, and for the most part it stays between 2 and 3. He watches his vitamin K intake regularly. He has read conflicting stories of how tea interacts with warfarin by making the INR levels high. -- S.J. ANSWER: Tea comes in two major types: black tea, which is fermented, and green tea, which is not. Black tea has no known interactions with warfarin (Coumadin). Green tea contains small amounts of vitamin K, which could make the INR lower and the warfarin less effective. However, the effect is likely to be small. Further, if he drinks a consistent amount per day, his dose can be adjusted to reflect his vitamin K intake. *** Dr. Roach regrets that he is unable to answer individual letters, but will incorporate them in the column whenever possible. Florence Kinney: Mrs. Claus for 30 Years! By Amanda Dudek n December 1989, Florence Kinney and her husband, Charles, had just moved from Wayne County to Webster when a local newspaper story caught her attention. Two little boys were to undergo organ transplant surgery right at Christmas. Kinney knew she had to do something for these children.

children are often also in need of clothing,” explained Kinney. “So, we added clothing to our gifts, particularly things like hoodies, socks, mittens and scarves.” Area knitters also contribute warm clothing items. “These children have next to nothing,” said Kinney. “We give them nice clothing as well as toys.”

“I asked my husband if we could buy two little toy trucks for these boys,” said Kinney. “He agreed, and we left the them anonymously at the hospital.” It was not long afterwards that Kinney read more news stories about the children who were sick and in the hospital over Christmas, about how sad they were and how they wondered if Santa would know where they were.

Florence Kinney plans to continue with SWHC because she has seen its impact firsthand. “A doctor at RGH told us it was the best medicine at Christmastime that a child could receive,” shared Kinney. “He told my husband and I that there wasn’t a medicine on their shelves that could raise the morale like what we did. It nearly brought us to tears.”

“I wanted to buy just a few presents for these children, too,” Kinney said. “Charles said, “Okay, just a few more, but this is it.” We went out shopping and bought 400 gifts.” When the Kinneys called Rochester General Hospital (RGH) with the news, they were “thrilled to pieces.” Word of this generous couple donating Christmas gifts quickly spread to other hospitals and organizations.

For 30 years, parents and children only knew the gifts came “from somebody who cares.” Only administrators knew who Mr. & Mrs. Claus really were. “It has been a wonderful experience,” said Kinney. “A lot of little kids have smiled over the years, and that makes me smile. For little ones who are so ill and have next to nothing, you change their lives and make a difference.” For Kinney, it is God’s work. “It was a calling on our lives, and this world needs more hands and feet for the Lord. That’s how I see it,” shared Kinney, who wrote about SWHC in her book, From the Heart of a Child. “SWHC is a Christ-Centered organization, and we know the Lord blesses it.” This year, the SWHC is already preparing for the holidays. “We need more elves!” exclaimed Kinney. “This requires a lot of lifting and sorting. We are talking thousands of pieces here! Anyone who would like to donate, please do. Everything is completely appreciated!”

“We were approached by a reporter for an interview, but we told him that we were not going to continue because we felt we did not have the financial means to do so,” explained Kinney.

Kinney and her husband believed that would be the end of it, but one afternoon she received an unexpected phone call from this same reporter. “He said, ‘Mrs. Kinney, I do not mean to be pestering about this, but I have one question for you. Have you and your husband prayed about this?’ I said, ‘No.’ He replied, ‘I wish you would because I think you might come up with a different answer.’” So, Charles and Florence Kinney did. “We suddenly felt this warmth from our heads to our feet. We knew this was the Lord’s calling on our lives,” explained Kinney, and Santa’s Workshop for Hospitalized Children (SWHC) was born. SWHC is celebrating its 30th anniversary this year and has given over 85,000 children a gift of love at Christmastime and beyond. Sadly, SWHC lost Charles Kinney in 2003; however, it continues to touch lives at RGH, Ronald McDonald House the Golisano Children’s Hospital Cancer Wing, the Cure Cancer Association, Webster Hope House and Williamson Community. “There is joy in a toy,” shared Kinney. “We typically bring gifts to 4,000-5,000 children a year. When we started, we were only serving a few hundred children and never dreamed we would serve this many.” Florence Kinney worked three jobs to help fund SWHC, from managing an office to being an Avon representative, even selling candy bars! “It was tough,” she recalled. The Kinneys soon realized they needed help. “We got ourselves some elves!” shared Kinney. “During those early years, we were myself, my husband, my mother and nine others, like the 12 disciples. We have seen a lot of miracles over the years, including needed items that just show up.”

Each year, Santa's Workshop for Hospitalized Children gives gifts to thousands of children who are ill.

Whomever took our wooden benches - we’d love to have them back! Our girls had a lemonade stand and they were left out - not for you to take! They go to our picnic table. If someone brought home picnic benches and said they were free on the side of the road - older but nicely varnished - it was not the case! We understand the mistake, please be kind and return them.

To the person complaining about places that offer parking to parents of small children and pregnant women: please focus on something better to take the time to complain about. “Too lazy” is not the reason the spots exist. First, congratulations that you trucked your three kids through a parking lot, walking long distances. Second, those spots exist for people who may be on bed rest but must take the kids to the store because that’s what us mothers must do. Maybe it is for the women who just gave birth Those that feel entitled four days ago and have not to demand respect and honor, when they have none even recovered enough to themselves and lack integrity, be walking long distances. As for drive up groceries and grocery delivery, get with the times! Where is our local village public works dept? They are MIA. I haven’t seen or heard anything of the trucks or big frontend loader that they overuse for small piles of brush. Brush has laid on the lawn for over 1 week, killing the grass, but if it’s put out late, the police give you warnings about fines. Poor service and high taxes. Your complaint is that a lawn was being mowed in the late evening, when you had seen the resident’s car in the driveway all day. This is just to make you aware that some people work from home. Just because their car is in their driveway doesn’t

mean that they have the whole day to tend to yard work. Furthermore, some people work from home during the day and then work 40 minutes from home in the evening. So, it is very difficult to find time to mow the lawn during “acceptable” times. My town has a noise ordinance that does not begin until 11pm; therefore, mowing the lawn at 9:30pm is within those guidelines. Please consider other possible situations before ranting to the public, or perhaps approach that neighbor to share your concern. They may be able to explain.

TIPS TO MAKE AN OLDER HOME MORE ENERGY EFFICIENT Switch to LED light bulbs. Some LED light bulbs have a lifespan of up to 10 years, and are 80 to 90 percent more efficient than incandescent bulbs. They are generally more environmentally friendly than CFLs and safer, too, since they produce less heat. The return on investment is excellent, as LED bulbs often pay for themselves through energy savings within the first year.

KEEP CRITTERS OUT OF YOUR HOUSE Human beings are not the only creatures looking for warm and cozy spots to ride out autumn and winter. Very often wild animals see homes as the perfect spots to nest and survive the cold. Fall is a great time to inspect your home top to bottom and make sure there are no points of entry for unwanted guests. The Humane Society notes that deteriorated trim and fascia boards can make great entry points for animals like squirrels, raccoons, skunks, and possums. Other animals may bed down under porches or decks or even inside chimneys. Seal any holes or repair points that are penetrable by cold animals. Just be sure to inspect the home prior to sealing or capping any potential entry points to be certain there aren’t any animals already living inside. Otherwise you can trap unwanted animals in your home. Test possible entry points by sealing them loosely with a paper towel or insulation and see if the material gets pushed out.

This easy project will reduce your household energy consumption and will save you money on monthly energy bills. A poorly insulated attic is a primary source of energy loss, forcing your AC unit or furnace to work harder than necessary. Most attics are insulated, but many, especially those in older homes, are not insulated adequately enough. Over time, insulation can settle and compact, allowing heat to escape through gaps. Topping it up is easy. Ideally, you want to aim for a depth of 16 inches of insulation for an overall R-value of 50. When existing insulation is present, simply level existing insulation to the top of the joist. Lay Roxul ComfortBatt on top of joists, running perpendicular to the first or existing layer of insulation. Ensure batts are butted against each other tightly. Fit batts closely to cross joists: cut batt if necessary (this can be done easily with a serrated blade or bread knife) and leave no space between layers. Don’t forget to insulate the attic hatch to the same level as the rest of the attic. Apply weather stripping to the edge of the hatch to reduce air leakage. Keep in mind that only IC-rated electrical fixtures can have insulation placed with zero clearance. Follow manufacturer’s instructions and local building code to insulate around the chimney. Routine doctor visits are a vital component of a healthy lifestyle. As noted by the U.S. National Library of Medicine, even men who feel healthy and live active lifestyles must make routine visits to their healthcare providers. Such visits can assess risk for future medical problems and offer men the opportunity to update vaccinations. Routine health checkups also give doctors a chance to screen for medical issues. Doctors consider a man’s age and other risk factors to determine when and how frequently he will need certain medical screenings. For example, while men between the ages of 40 and 64 are often advised to get blood pressure screenings at least once per year, those with diabetes, heart disease, kidney problems, and other conditions may need more frequent screenings. No two men have the same medical histories, which only highlights the need for men of all ages to schedule annual physical exams with their physicians. Such exams can reveal potential problems and also make for great opportunities to discuss more specific medical screenings with their physicians. The following are some general screening tests and guidelines recommended for all men between the ages of 40 and 64, courtesy of the USNLM. Prostate cancer - The USNLM notes that most men age 50 or older should discuss screening for prostate cancer with their physicians. Ethnicity and family history are some of the recognized risk factors for prostate cancer. As a result, African American men and men with a family history of prostate cancer in a first degree relative younger than 65 should discuss screenings beginning at age 45. There are pros and cons to prostate cancer screenings, and these should be part of men’s discussions with their physicians. Colorectal cancer - All men between the ages of 50 and 75 should be screened for colorectal cancer. Physicians may recommend colorectal screening for men under age 50 with a family history of colon cancer or polyps. In addition, physicians may consider screenings for men under 50 who have a history of inflammatory bowel disease. Cholesterol - The USNLM advises men to have their cholesterol levels checked every five years. Men with certain conditions, such as diabetes and heart disease, may need to be checked more often. Diabetes - Diabetes screenings are recommended every three years for men age 45 and older. Men who are overweight and younger than 45 should ask their physicians if they should be screened before they reach 45. Osteoporosis - Women are more likely to develop osteoporosis than men, but that doesn’t mean men are immune to this condition marked by a weakening of the bones due to tissue loss. Fractures after age 50, heavy alcohol use, smoking, and low body weight are some risk factors that can make men vulnerable to osteoporosis. Health screenings can catch diseases and other conditions in their early stages when they’re most treatable. Such screenings should be a vital part of men’s health routines.
