CONSERVE FUEL DURING YOUR DAILY COMMUTE Keep tires properly inflated. Properly inflated tires reduce drag on the engine. Engine drag makes the vehicle work hard to move, and that wastes fuel. But the type of tires a vehicle has also can affect how efficiently it uses fuel. According to the Transportation Research Board, tires that reduce rolling resistance by 10 percent can improve gas mileage by as much as 2 percent. While that figure might seem insignificant, that 2 percent savings can add up to a substantial amount of money for commuters who spend 50 minutes every day getting to and from work.

STRAWBERRY-GLAZED FRENCH TOAST WITH SWEETENED SOUR CREAM Serves 4 • • • • • • • • • • 1.
8 1-inch-thick bread slices, cut from a country or peasant loaf 2 cups half-and-half 4 egg yolks 3 tablespoons light brown sugar 1 tablespoon vanilla extract, plus 1/2 teaspoon 1/2 teaspoon cinnamon 3 tablespoons unsalted butter 1/2 cup sour cream 1 tablespoon granulated sugar 1/3 cup strawberry jam or preserves (see note 2) Arrange a rack at center position and preheat the oven to 400 F. Place the bread slices on a baking sheet and bake until dry and very lightly browned, about 8 minutes per side. Watch carefully so that the bread does not burn. Remove the bread from the oven and reduce the oven temperature to 200 F. 2. In a medium bowl, whisk together the half-and-half, egg yolks, brown sugar, 1 tablespoon vanilla, and cinnamon. Pour the mixture into a shallow pan (a 9-by-13-inch glass baking dish works well). Add the GENESEE VALLEY PENNY SAVER

toasted bread slices and soak them 4 minutes per side. Remove to a large plate or platter. 3. Place a large, heavy frying pan over low to medium heat. Add about 2 teaspoons of the butter, or enough to coat the bottom of the pan lightly. When melted, add enough bread slices to fit comfortably in a single layer. Cook slowly until the slices are golden brown and crisp on both sides, about 4 minutes per side. Remove to a baking sheet and place in the warm oven. Repeat, adding more butter to the pan as needed until all the bread slices have been sautéed. 4. In a small bowl, stir together the sour cream, granulated sugar and remaining 1/2 teaspoon vanilla. 5. When ready to serve, spread each toast with a thin coating of strawberry jam and top with a dollop of sweetened sour cream. Note 1: The best bread for this dish is an unsliced loaf of good-quality peasant or country bread, preferably one without an extra-hard crust. One that is rectangular, rather than round, is more convenient, but either will do. Cut off the ends of the loaf, and reserve for another use. Then slice the bread into 1-inch-thick slices. If your loaf is large and the slices seem large, cut them in half. Note 2: You can try other jams, preserves or marmalades. Cherry, raspberry or peach preserves and orange marmalade are other possibilities.

DEAR DR. ROACH: I was diagnosed last year with a Hollenhorst plaque. I have a “blue cloud” in the corner of my left eye at all times. My doctors’ only instructions were to notify them of any changes. I am scared. Am I going to have a stroke in the near future? When I asked one of my doctors, he said to “just keep an eye on it.” -- D.D.M. ANSWER: A Hollenhorst plaque is a cholesterol crystal that is seen in the blood vessels of the retina. In most cases, the cholesterol plaque had broken off from the carotid artery, the main blood vessel providing blood to the brain. While I understand your concern about its effect on your stroke risk, it seems to be fairly small -- in one study of 130 patients with Hollenhorst plaques, none of them had a stroke in the two years or so that they were followed in the study. However, all of the patients studied had some degree of blockages in the carotid artery, and your doctor should consider looking for blockages in yours if he hasn’t already. In most cases, carotid blockages are treated with medication, especially a statin medication. Aspirin might be appropriate for some people, but that is a discussion you should be having with an expert, such as your internist or cardiologist. Warfarin should not be used to treat cholesterol plaques. The manufacturers of warfarin recommend stopping warfarin if a cholesterol plaque is found, as warfarin can increase the risk of further events. However, there might be some situations in which anticoagulants must be used, such as in the case of a pulmonary embolism.

AFib can be managed, helping you avoid stroke and live a longer, healthier life,” adds Ellinor. The AHA offers a free online community for those diagnosed with AFib, My Afib Experience. that the enamel on the back of my upper teeth, especially the front, is wearing away. He asked me if I experience acid reflux. I said I don’t think so, because isn’t that something I’d notice? He then asked if I feel refreshed after waking up from eight hours of sleep. (I’m a new mother, and babies are next-level exhausting.) He said to try an experiment: Take a Pepcid at night and if I start to feel more refreshed, then I’m probably having acid reflux that prevents me from going into deep, refreshing sleep. Do you recommend this? -- H.C. ANSWER: I admire your dentist for noticing the enamel loss and considering the diagnosis of gastroesophageal reflux disease. Dental enamel can be lost in an acid environment, so it’s a reasonable thought. However, it is normally severe acid reflux accompanied by frequent acid taste in the mouth that leads to tooth damage. There certainly are cases of asymptomatic GERD, but I think tooth damage is unlikely without further symptoms. Repeated vomiting, especially in people with a history of an eating disorder, is a much more common cause of enamel loss than GERD, and that may be a possibility. I would not recommend a medication trial because there are no reliable symptoms. (I agree with you that poor sleep in a new mom is not always due to reflux.) If GERD is really a concern, a diagnostic test, such as a 24-hour pH monitor or an endoscopy, would be my preference before considering a long-term course of treatment. *** Dr. Roach regrets that he is unable to answer individual letters, but will incorporate them in the column whenever possible. CONTROL TERMITES Few things elicit fear in the minds of homeowners like termites. Termites are voracious and can turn wood to pulp wherever they take up residence. Termites have felled massive trees, but they also can bore through the wood in homes, wreaking havoc as they go. Ants and termites can look similar, so homeowners who suspect they have a termite infestation should learn to distinguish one from the other. A close look at termites can make it easy to identify them. Unlike ants, termites have no “waist;” their bodies are more rectangular. A termite also has straight, beaded antenna, while an ant’s antennae are bent or elbowed. Termite wings are equal in size, uniform in shape and much longer than their bodies. Ants have a reddish hue, while termites are creamy white.

Prevention is always preferable to having to treat termites after they are established. Termite Web, a site devoted to termite information, states that treating home foundations and surrounding soil with termite spray is often the best course of action to stop subterranean termites from taking hold. Baiting termites outside with wood that is tainted with slow-acting insecticide can eliminate an entire colony in one to four months. Termite control methods may need to be repeated. A multipronged approach using different chemicals may be necessary to kill existing insects and repel further infestations. Associates Find Hardship, Handshakes and Hugs on Mission Trip By Anne Johnston
T
wo UR Medicine Thompson Health staff members recently returned from a weeklong mission trip to Haiti and the Dominican Republic through the organization Restore Haiti. The trip was led by Physician Assistant John Oliphant of Victor. A board member for Restore Haiti as well as director of its medical/ dental operation, Oliphant works in Thompson’s Occupational Medicine and Associate Health Departments. He was joined by Registered Nurse Mary Ellen Fiorille of Honeoye – who works in Associate Health at Thompson – as well as a dentist, a physician, and five other physician assistants from other healthcare organizations. The team taught classes for Haitian high school students, covering topics including first aid, nutrition, sanitation, hygiene and oral health. They also led a session designed to focus on empowerment for girls and worked with local physicians to see several medical patients as well as provide dental care to other local residents. “Our team was welcomed and supported with genuine gratitude that was apparent in the way the communities we served in Ase-apitres, Haiti and Pedernales, Dominican Republic responded to our presence,” Fiorille said. “It was particularly gratifying to witness the empowerment demonstrated by the high school students from the Commune of Ase-a-pitres, where our team presented educational classes for first aid, nutrition, hygiene, empowerment for girls and dental care with the assistance of several student translators.” “These students were also eager and committed to translating and assisting in every possible way to the greatly-needed dental and medical services our team provided to many people along with the few affiliated staff and physicians at the local ‘Centro de Salud de Ansapit’ hospital,” she added. Both Oliphant and Fiorille said one of the most powerful aspects of the trip was when the team visited a refugee camp for Haitians who had been expelled from the Dominican Republic. “The living conditions were very stark with no toilet facilities, inadequate shelter and meager school facilities for the children, but despite that we were greeted with joyful singing, beautiful smiling children and lots of handshakes and hugs. Our team left oral health supplies for everyone in the community and a pledge to do what we can to tell their story and to Mary Ellen Fiorille and John Oliphant find ways to improve their on their mission trip. situation,” Oliphant said. Without this diagnosis and treatment plan, she said, the young man would otherwise continue to be without the care necessary for his survival. A week prior to the mission trip, Oliphant gave an invited presentation before the Canandaigua Medical Society on “ShortTerm Medical Missions Done Well: What Every Volunteer Should Understand.” This was based on his ongoing research into what healthcare providers and leaders from low- and middle-income countries want those visiting medical and dental teams to understand before their visits. Highlights included the importance of coordinating the mission with local healthcare leaders, getting Ministry of Health approval for the mission and submitting all providers’ credentials for review and approval prior to seeing patients.

While Oliphant has been to Haiti six times and has also taken two medical system evaluation consulting trips to Liberia, this was Fiorille’s first international mission. “It was something I had never anticipated, but I was blessed that the opportunity presented itself,” she said. Approximately two-thirds of Fiorille’s cost was funded by a Nursing Practice Council scholarship from Thompson’s medical staff – something which left her “encouraged, inspired and very grateful.” Oliphant said there will likely be another trip to Haiti in the spring or summer of 2020. Oliphant and Fiorille weren’t the only Canandaigua medical professionals who were jet setting for a noble cause. Jacquie Dobbertin, a registered nurse with Highland Hospital Radiation Oncology at the Sands Cancer Center in Canandaigua, recently joined a group of volunteers with an organization called CanGO to help in the South American country of Guyana. CanGO stands for Canada-Guyana Outreach Mission. The organization provides health care to the under-privileged population of Guyana. On her trip, the Honeoye resident traveled to posts only accessible by water via the Maruka River. She delivered care in Santa Rosa, the Kumaka District, Manawarin, Karaburi and Waramuri. Her team saw over 500 patients for problems such as wound care, diabetes, hypertension, burns, heart disease, mental health, headaches, infections, worms and parasitic infections. Dobbertin said her - Mary Ellen Fiorille, RN nursing skills were essential in educating people about lifestyle influences to improving their health. She added that access to health care can require people in Guyana to take a six- to 10-hour boat and/or car ride, which is expensive and nearly impossible for most of the remote villagers.
