Persistent Cough Can be Tricky to Treat --DEAR DR. ROACH: I went through many tests for a cough

that lasted for 30 years. No treatments worked. The only sign my doctor could find was that my throat had scars from coughing that long. My allergy specialist had tried for years with all kinds of prescriptions, to no avail. I am now living a normal life since a doctor prescribed tramadol (50 mg) twice a day. What a relief. My wife, grown children, friends and clients are happy. -- L.G. ANSWER: I found only a single case report of tramadol being used for chronic cough, and it was effective in that patient. The authors advised further study on its use. Tramadol is a pain medication distantly related to codeine. Its abuse potential is low, but there have been overdoses and deaths due to this drug, so I don’t recommend its indiscriminate use. However, I have seen personally and received many letters from people with intractable coughing for years, so much so that I wanted to call attention to this as a possible treatment worth discussing with one’s doctor for people with longstanding cough that has resisted diagnosis or treatment. I previously mentioned taking gabapentin for laryngeal neuropathy, a condition that is often unrecognized and that may present with a prolonged cough. I received many letters telling me it was effective. Tramadol at the low dose (for an adult) you mention usually is safe.

DEAR DR. ROACH: I have a very dear relative who had

radiation seed therapy for prostate cancer in 2003 after a biopsy revealed a Gleason score of 7 (3/4). Afterward, his PSA was only 0.1 to 0.3 the first three years, and it rose only .3 more each year until 2015, when it was 2.7 recently. He is 83. He has been told he must have hormone treatment: shots, pills or both, by his VA provider, who is not a urologist and who says he should see one. He has no symptoms, he says, and he leads a very active lifestyle, with daily walking of 3 miles or more, bowling twice a week and extensive gardening and yard work. Could the rising PSA be due to normal exercise activity, including production from surviving prostate cells? -- R.A. ANSWER: Rising PSA after cancer treatment typically is due to cancer returning, not an increase in normal prostate. However, that does not mean that your friend “must” have treatment. Goals for cancer treatment in an 83-year-old man may be very different from those of a 20-year-old man. In his case, since it took about 10 years to increase by 10 times, it may be a very long time before the probable cancer would be large enough to bother him, and it requires skill and experience to determine whether treatment, which can make quality of life worse, is worthwhile. Many urologists would recommend deferring treatment until and unless he develops symptoms. I certainly would recommend that he go to see a urologist as his VA provider has recommended. He may be able to learn whether it really is the cancer returning and discuss his options. Shots and pills are options, but so is keeping a careful eye on it and not rushing to treat.

Lack of funding has been the culprit, but educators tend to agree that the arts play a crucial role in the academic and creative development of our children. Debra Sheen Crafts Chocolate Indulgence By Amanda Dudek

D

ebra Sheen has just the kind of warmth one would imagine. Kind and generous, Sheen, like her chocolate confections, has a beautiful richness about her. Her Fairport chocolate shop is lined with shelves full of all kinds of delights, both traditional and unique, including yellow and dark chocolate roses, each petal delicately molded, and a chocolate Eiffel Tower. Perhaps surprisingly, Sheen has not always been a chocolatier. “It was a gradual move into making chocolate,” said Sheen. “I’ve always like making sweets, and I even made wedding cakes for a while.” She explained that, at first, she experimented with a variety of chocolate recipes. Then, through trial and error, she began creating her own chocolate recipes. Things quickly progressed from there. “Originally, I worked out of my home,” Sheen shared. “Then, I had a tiny retail space out of which I sold my chocolate wholesale, as corporate gifts and things like that.” Sheen decided to work in the private sector, preferring to offer her luxurious treats to those around her. “I rented out a larger place in Penfield for over seven years,” said Sheen. A move to Fairport followed. “When I made my move here to Fairport two years ago, it was partially due to the much larger production space this location offers,” said Sheen, adding that the Fairport community has been very supportive. “New people stop in every week, and others come in regularly for gifts.” Sheen also received the Small Business Person of the Year Award from Alignable this year. “I love making chocolate,” explained Sheen. “You can shape it, mold it. Every day, chocolate is different, and the process has to be adjusted. Chocolate is very temperamental, and working with it is an art.” Sheen described one of the most satisfying parts of being a chocolatier is seeing people’s faces when they taste her chocolate and they love it. “That raw reaction...it’s great!” she added. That isn’t to say that the process is easy. In fact, Sheen describes it as delicate and focused. “There is only one pace with chocolate,” she explained. “Chocolate can’t be rushed. I can’t leave the chocolate while I am making it. If I do, then I have to start all over again.” She explained that larger equipment that is automated may allow for more interruptions, but chocolatiers like herself who create their chocolate in small batches with small equipment

must have a very controlled environment. “It is worth it for that small batch goodness,” said Sheen, adding that making chocolate is also a mood booster. “You can’t be in a bad mood and make chocolate!” She was recently a part of Fairport’s St. Patrick’s Day Crawl. For her, the event was a success. “It was excellent,” she said. “We were one of people’s first stops, and we had a wonderful crowd. Guests were all dressed up, ready to have fun and explore local businesses.” According to Sheen, people came in and lingered, tried her tasting items and made purchases. “Thankfully, I didn’t run out!” Her St. Patrick’s Day Crawl tasting items consisted of a rich, dark chocolate brownie topped with a soft ganache, a smooth caramel sauce lightly flavored with Irish créme and slivered almonds. This was served with a sipping chocolate, a surprisingly delicate mixture of dark chocolate, cream, butter and créme de menthe. There is a purpose behind such luxury. “When people come in, I want them to have a unique and positive experience,” stated Sheen. “I try to get people to just slow down and feel that this is their moment to simply enjoy.” Sheen recognizes that everyone is busy juggling so much, and she feels this makes it even more important to take some time to slow down. “It actually changes the chemistry of your brain in a good way!” she added. Sheen explained that her treats are also meant to be an indulgence. “I never recommend that people eat chocolate or other treats mindlessly,” she said. “It is a special treat for self, a small reward.” With Easter on its way, Debra Sheen is working on some new creations. “I am working on an ‘adult’ Easter egg using Iron Smoke Maple Bacon Bourbon,” she said, adding that she will also be using airbrushing techniques to decorate some of her eggs this year, a method she has not used before. “I am also making little bird’s nests with different grains covered in chocolate. Inside these nests sit a marshmallow bird with little candy eggs,” said Sheen, adding that the treat weighs about 4 ounces altogether. “I also make bunnies and many other traditional Easter treats,” Sheen said. One of her best selling items is her Caramel Bijou which are made with a very soft caramel center on a crunchy base, dark chocolate covering and French sea salt. MORCILLA CON SETAS (BLOOD SAUSAGE WITH MUSHROOMS, SPRING PEAS AND MINT OIL) Serves 4 1 pound morcilla blood sausage 1 pound mushrooms, sliced 2 tablespoons olive oil 4 cloves garlic, slivered Salt and pepper to taste 2 tablespoons white wine 1 cup shelled spring peas 1 tablespoon chopped fresh mint 2 tablespoons mint oil Mint Oil 1/2 cup extra-virgin olive oil 1/2 cup chopped mint Pinch of salt Cut the blood sausage into 12 slices. Sauté the mushrooms in olive oil with garlic, salt and pepper for 5 minutes. Add the wine and cook for 2 more minutes. Boil the peas for 2 minutes and drain. When mushrooms are done, remove from heat and stir in the peas. In a cast-iron skillet, sear the morcilla in a dry pan for 2 minutes per side. Arrange the slices of morcilla down the middle of a platter. Surround with mushroom/pea mixture and drizzle the plate with the mint oil. For the mint oil: In a small saucepan over medium heat, heat olive oil to warm. Remove from heat and add the mint and a small pinch of salt. Allow to rest for 1 hour. Drain through a sieve. Reserve extra mint oil for other uses, such as vinaigrette.

1 sheet puff pastry, thawed 5 ounces Brie cheese 1/2 cup Musselman’s Apple Butter 1/4 cup pecans, chopped Heat oven to 400 F. Grease mini muffin pan. Press seams of puff pastry sheet together. Cut sheet into quarters then each quarter into six pieces. Use rolling pin to flatten each piece of pastry into square shape. Place one square into each muffin cup. Put 1/2 teaspoon brie, 1 teaspoon apple butter and 1/2 teaspoon chopped pecans into each muffin cup. Bake 13-15 minutes until golden brown. Serve warm.
