DEAR PAW’S CORNER: In regard to your recent article on a cat’s constant vomiting: I had the same issue with my cat. Stella was vomiting once or twice a day for several weeks. I couldn’t figure out why; there was no issue with new rugs or furniture. When they sprayed the lawns in our complex, I made sure not to track anything into the house. And Stella was an indoor cat, so she wasn’t being exposed to other cats. I got her in to see her vet, who ran tests and blood work and found nothing seriously wrong. The vet said that Stella might have GERD (acid reflux), and to give her one-quarter of a 10 mg famotidine pill each day. I cut each pill into fourths and give it to her in the morning with a little treat. It works perfectly; Stella no longer vomits repeatedly and has been doing well for the past two years. She gets a check-up every year at the vet and comes through with flying colors. Perhaps the owner could run this possibility by her vet and see what they think! -- Sande G., Webster, New York

Acid reflux is one possibility, but as Sande noted, it’s important to rule out more serious conditions. Be sure to ask plenty of questions at the vet’s office. Over-the-counter medications are a possibility, but keep in mind that formulations for humans contain larger dosages than a cat needs and may contain ingredients that are not great for your pets. Allison Fowler: A Symphony of Kindness and Positivity BY SUZIE WELLS

A

llison Fowler embodies the essence of kindness and positivity, a testament to the remarkable person she is. A Honeoye FallsLima native, Allison holds a Bachelor’s in music education from SUNY Potsdam and a Master’s in music education from Boston University. She treasures her journey, even with its unexpected paths, and she now lends her expertise to her husband Shaun’s business, Friendly Neighborhood Bookkeeping, while also running her own tax service, Friendly Neighborhood Tax Service. Involved with Avon’s New Horizon Church, the Ugandan Water Project, and other communitybased projects, Allison loves small town life. She says, “There isn’t a ton of privacy, but being in a place where people are willing to give and receive help is very beautiful.” Now an accomplished musician, Allison explains her early influences: “I wanted to play piano since I was a tiny girl because my Grandma Vione played beautifully. She would play Chopin, Joplin, or just hymns that she’d ‘fancy up.’ I could listen to her all day. Once I started playing when I was ten, I was just as hard to get to practice as other kids, so I’m grateful to my mom for keeping on my case about it.” This early joy in music was enriched by experiences like performing at Lincoln Center with the HFL Jazz band. She says the ineffable energy of her high school band teacher, Mark Borden, inspired her to pursue music education; she says, “I couldn’t imagine doing anything else.” A few years ago, when Lyme disease and an autoimmune disorder left her feeling very depressed, Allison dragged herself out to see one of her favorites, Trombone Shorty, at the Jazz Festival. Allison and Ella en route to She explains that while Trombone Shorty concert. there, she “felt the cloud of depression just lift off of me,” and she vowed to get back into making music and particularly perform uplifting music and bringing a little of the joy she felt that day to listeners. With expertise in clarinet, alto sax, and reasonable experience with flute, trombone, euphonium, trumpet, horn, and percussion, Allison plays bass in the Rochester area band, Random Accents, who play “a random mix of songs from Motown through current pop hits.” She says the band has such fun playing together, and she and Shaun, who learned to run sound for the band, feel “like we’re having a party every time we have a gig.” Allison loves the friendly, supportive, fun vibes at local venues and encourages folks to check out some of the incredible live music around town.

Allison says she doesn’t feel qualified to offer general life advice (though this writer disagrees), but shares some values her household tries to live by: 1. We’re a team: we all work together to create the life we want. 2. Stay curious: you don’t know everything, there’s always more to learn, and asking questions shows people you value them. 3. We’re problem solvers: we have control over nothing but our own responses to the world around us. So, we show up, try to respond gracefully, and leave things better than we found them! 4. Choose kindness: you already have something in common with everyone – we’re all humans. You don’t have to agree with someone on everything to be kind and have a great conversation. 5. Strangers are friends you haven’t met yet: most people are too shy to start the conversation, so if you want to make a friend take it upon yourself. And try not to leave anyone out. She also says, “Making your selfworth dependent on your career, relationships, finances, or other external sources will ultimately disappoint. I think coming at the parts of your life from this place of having value just because you’re human allows you to make choices for the benefit of your self and your family.” Such sage words from an incredible human being. May we all strive to be as kind, compassionate, and confident as Allison. DEAR DR. ROACH: I am a 69-year-old who used to walk up 75 to 90 flights of steps several days a week as exercise. After a steering wheel airbag went off and damaged my heart, I am afraid my ability to do that stopped. After years of medication, I am now on a pacemaker, which definitely helps. Could the exercise have weakened my heart to such a degree that it could be viewed as overdoing it, therefore exacerbating my condition? -- Anon. ANSWER: In general, exercise does not harm the heart. On the contrary, exercise is almost always good for the heart. However, there are exceptions. In healthy people, there is an optimal amount of exercise, and extreme levels of exercise (on the order of running more than 10 miles a day every day) begin to decrease the benefit that a person gets from exercise. (This remains controversial, but I am convinced from the data.) In people with severe heart disease (the kind with blockages in the arteries), overdoing exercise can lead to a heart attack, but this is an exception. For every person who gets harmed by overexercising, there must be a hundred who don’t get adequate exercise. The fact that you have improved a lot on the pacemaker suggests that at least part of your problem wasn’t with the pump function of your heart, nor with the arteries leading to your heart. The problem lies with the electrical supply. Once a pacemaker is in, that problem is immediately alleviated, but the heart may still be deconditioned (“out of shape”). Regular exercise will help the problem. Your cardiologist can better answer the question for you and also give you advice on how to build up your exercise tolerance over time. ***

BREAST CANCER TREATMENT Most patients with breast cancer have surgery to remove the cancer. Sentinel lymph node biopsy is the removal of the sentinel lymph node during surgery. The sentinel lymph node is the first lymph node to receive lymphatic drainage from a tumor. It is the first lymph node where the cancer is likely to spread. A radioactive substance and/or blue dye is injected near the tumor. The substance or dye flows through the lymph ducts to the lymph nodes. The first lymph node to receive the substance or dye is removed. A pathologist views the tissue under a microscope to look for cancer cells. After the sentinel lymph node biopsy, the surgeon removes the tumor using breast-conserving surgery or mastectomy. If cancer cells were not found in the sentinel lymph node, it may not be necessary to remove more lymph nodes. If cancer cells were found, more lymph nodes will be removed through a separate incision. This is called a lymph node dissection. Types of surgery include the following: • Breast-conserving surgery is an operation to remove the cancer and some normal tissue around it, but not the breast itself. Part of the chest wall lining may also be removed if the cancer is near it. This type of surgery may also be called lumpectomy, partial mastectomy, segmental mastectomy, quadrantectomy, or breast-sparing surgery. • Total mastectomy: Surgery to remove the whole breast that has cancer. This procedure is also called a simple mastectomy. Some of the lymph nodes under the arm may be removed and checked for cancer. This may be done at the same time as the breast surgery or after. This is done through a separate incision. • Modified radical mastectomy: Surgery to remove the whole breast that has cancer, many of the lymph nodes under the arm, the lining over the chest muscles, and sometimes, part of the chest wall muscles. Chemotherapy may be given before surgery to remove the tumor. When given before surgery, chemotherapy will shrink the tumor and reduce the amount of tissue that needs to be removed during surgery. Treatment given before surgery is called preoperative therapy or neoadjuvant therapy. Even if the doctor removes all the cancer that can be seen at the time of the surgery, some patients may be given radiation therapy, chemotherapy, or hormone therapy after surgery, to kill any cancer cells that are left. Treatment given after the surgery, to lower the risk that the cancer will come back, is called postoperative therapy or adjuvant therapy. If a patient is going to have a mastectomy, breast reconstruction (surgery to rebuild a breast’s shape after a mastectomy) may be considered. Breast reconstruction may be done at the time of the mastectomy or at some time after. The reconstructed breast may be made with the patient’s own (nonbreast) tissue or by using implants filled with saline or silicone gel. Page 19

Breast Cancer

FAQ The World Health Organization reports that roughly 2.3 million women were diagnosed with breast cancer in 2020. By the end of that year, there were nearly eight million women alive who had been diagnosed with the disease in the previous half decade. A breast cancer diagnosis inevitably leads to questions about the disease. The bulk of those questions undoubtedly are asked by the millions of women who are diagnosed with breast cancer. But millions more individuals, including friends and family members of recently diagnosed women, may have their own questions. Women can discuss the specifics of their diagnosis with their physicians. In the meantime, the following are some frequently asked questions and answers that can help anyone better understand this potentially deadly disease.

What is breast cancer? Cancer is a disease marked by the abnormal growth of cells that invade healthy cells in the body. Breast cancer is a form of the disease that begins in the cells of the breast. The National Breast Cancer Foundation (NBCF) notes that the cancer can then invade surrounding tissues or spread to other areas of the body.

Can exercise help to reduce my breast cancer risk? The NBCF notes that exercise strengthens the immune system and women who commit to as little as three hours of physical activity per week can begin to reduce their risk for breast cancer. However, even routine exercise does not completely eliminate a woman’s risk of developing breast cancer.

Is there a link between diet and breast cancer?

Is there a link between oral contraceptives and breast cancer? The NBCF reports that women who have been using birth control pills for more than five years are at an increased risk of developing breast cancer. However, the organization notes that risk is very small because modern birth control pills contain low amounts of hormones.

Can breastfeeding reduce breast cancer risk? Breastfeeding and breast cancer are linked, though the NBCF notes that the role breastfeeding plays in lowering cancer risk depends on how long a woman breastfeeds. The World Cancer Research Fund International notes that evidence indicates that the greater number of months women continue breastfeeding, the greater the protection they have against breast cancer.

Is there a connection between stress & breast cancer? The NBCF notes that researchers have found that traumatic events and losses can alter how the immune system functions, which can provide an opportunity for cancer cells to establish themselves within a person’s body. The NBCF urges women to identify ways to keep their stress levels in check. Breast cancer education can be a valuable asset as women seek to reduce their risk for the disease.

The organization Susan G. Komen®, a nonprofit source of funding for the fight against breast cancer, reports that studies have shown eating fruits and vegetables may be linked to a lower risk for breast cancer, while consuming alcohol is linked to an increased risk for the disease. In addition, the NBCF reports that a high-fat diet increases breast cancer risk because fat triggers estrogen production that can fuel tumor growth.

PROTECTIVE FACTORS FOR BREAST CANCER The ovaries make most of the estrogen that is made by the body. Treatments that stop or lower the amount of estrogen made by the ovaries include surgery to remove the ovaries, radiation therapy, or taking certain drugs. This is called ovarian ablation. Premenopausal women who have a high risk of breast cancer due to certain changes in the BRCA1 and BRCA2 genes may choose to have a risk-reducing oophorectomy (the removal of both ovaries when there are no signs of cancer). This decreases the amount of estrogen made by the body and lowers the risk of breast cancer. Riskreducing oophorectomy also lowers the risk of breast cancer in normal premenopausal women and in women with an increased risk of breast cancer due to radiation to the chest. However, it is very important to have a cancer risk assessment and counseling before making this decision. The sudden drop in estrogen levels may cause the symptoms of menopause to begin. These include hot flashes, trouble sleeping, anxiety, and depression. Long-term effects include decreased sex drive, vaginal dryness, and decreased bone density.
