How to Distinguish Heart Disease Symptoms From Panic Attacks ---

Take off clothes worn outside and launder them promptly, showering to wash pollen off of the body. Use an air conditioner or keep windows closed when the pollen count is high.

ROACH: How can you tell with at least a moderate degree of certainty that you might really have a heart or blood pressure problem, as opposed to just having an anxiety attack that is less serious? -- D.S. ANSWER: A panic attack is an abrupt change in mental state. They are usually unexpected, sometimes with or without identifiable triggers. Symptoms of a panic attack vary from person to person, but among the most common are feelings of heart palpitations; sweating; trembling or shaking; shortness of breath; chest pain or discomfort; dizziness; numbness or a “pins and needles” feeling; abdominal pain or nausea; feeling abnormally hot or cold; feeling detached from oneself or from reality; and a fear of dying, losing control or “going crazy.” Most of us have had some of these feelings from time to time, and some of these symptoms are identical to ones of heart disease, specifically of angina pectoris -- the classic symptom of poor blood flow to the heart. There are many conditions, both medical and psychiatric, that can cause these symptoms as well, so acquiring a moderate degree of certainty isn’t always easy. A very careful history exam helps. With chest pain due to heart disease, for example, symptoms are commonly brought on by exertion; whereas with a panic disorder, there might not be a trigger, or it may be triggered by psychological stress. Some people have highly specific triggers, such as open spaces or needles. Some people can trigger a panic attack just by worrying that they are going to have one. The time course of the symptoms can be very helpful. With a panic attack, the attack starts, and chest discomfort (for example) will then occur. In people with heart disease, chest discomfort starts, and then people will get anxious. I had a wise cardiology professor who told me to always consider heart disease even if a person’s history is suggestive of a panic attack. I generally check their blood pressure and order an electrocardiogram, blood testing (including thyroid tests) and sometimes other tests when considering the diagnosis of a panic attack. There is very effective treatment available for people with panic disorder (people with recurrent unexpected panic attacks that lead to avoidant behaviors), both through therapy and medication. But your question implies that being pretty sure of the diagnosis is wise before dismissing the possibility of cardiac disease or other medical diseases. *** Dr. Roach regrets that he is unable to answer individual questions, but will incorporate them in the column whenever possible. A TURKEY ALTERNATIVE FOR INTIMATE THANKSGIVING MEALS Serves 6 to 8 12 chicken thighs (about 3 to 4 pounds), bone in Salt and freshly ground black pepper 4 tablespoons unsalted butter 2 tablespoons olive oil 6 shallots, minced 6 cloves garlic, minced 1/2 cup apple cider vinegar 11/2 cups dry white wine 1 tablespoon tomato paste 11/2 cups chicken stock, preferably homemade 1/2 cup chopped Italian parsley Season the chicken generously with salt and pepper to taste. In a large, deep skillet (big enough to hold all chicken fairly close together; or use two skillets), melt 3 tablespoons of the butter in 1 tablespoon of the olive oil over medium-high heat. Brown the chicken for about 8 minutes, turning so both sides get nicely browned. Transfer the chicken to a deep, broad, ovenproof casserole that can go from the oven to the table. Remove the skillet from the heat and pour off most of the fat. Preheat the oven to 375 F. Place the skillet back over medium heat and add the remaining 1 tablespoon butter and 1 tablespoon olive oil. Add the shallots and garlic and cook for about 1 minute, or until they become translucent. Add the vinegar and wine, and stir up the crispy bits that have stuck to the bottom of the pan. Allow the liquids to cook down a bit, stirring occasionally, for about 8 minutes. Whisk in the tomato paste and let the sauce simmer and condense, about 5 minutes more. Then add the chicken stock, a little at a time. Keep stirring to mix well. Pour the sauce over the chicken. Bake for 35 to 45 minutes until the chicken is very tender, turning the chicken pieces halfway through, so that both sides are infused with the sauce. Remove one thigh and cut into it to make sure it is done and no red juices flow from the cut. Page 11

It typically includes cinnamon, cloves, ginger, and nutmeg. It also may include allspice. Pumpkin spice is the name given to the blend in the United States. Noah French of Geneseo BSA Troop 4070 Achieves Eagle Scout Rank BY JOHN RUTIGLIANO
N
oah French earned the Rank of Eagle Scout with Boy Scouts of America Troop 4070 of Geneseo. Noah earned his rank on September 26th and received his rank of Eagle Scout during an Eagle Court of Honor ceremony on October 30th. The rank of Eagle Scout is the highest rank available to youth in the Boy Scouts of America. Noah joins over 2.5 million Scouts who have earned the rank since 1912. The French family is an aerie of Eagles—Eagle Scouts, that is. Noah’s older brother, father, three uncles, and grandfather have all earned the rank of Eagle Scout. Noah began his Scouting journey as a Cub Scout in Geneseo Cub Pack 4072. After earning the Arrow of Light award as a Webelos Scout, Noah joined Boy Scout Troop 4070 in April 2019. Noah has camped at Letchworth State Park, Rattlesnake Hill Wildlife Management Area, Silver Lake, the Buffalo Audubon Society Nature Center in North Java, Camp Dittmer in Phelps, Camp Sam Wood in Pike, Camp Scouthaven in Freedom, Camp Merz, Camp Massawepie in the Adirondacks, and Camp Conewago near Gettysburg. Noah’s leadership experience includes serving as a member of the Senior Patrol from 2022-2023. He also served as Webmaster from 2021-2023. He is currently Assistant Senior Patrol Leader. Noah was a member of a team of older Scouts who planned meetings, campouts, service projects, and activities for an entire school year of scouting. Noah’s scouting experience continued this summer at the National Scout Jamboree in West Virginia, a ten-day long high-adventure camp run by the Boy Scouts of America. Noah was one of ten Scouts and three adult leaders from Troop 4070 attending the Jamboree. He served as Patrol Leader of the Yellow Jacket patrol while at the Jamboree. He plans to attend Sea Base in Florida next summer.

After high school, Noah plans to attend college to pursue a degree in a math, science, and/or a computer related field. As for life goals, Noah says, “In the short term, I want to take and pass multiple AP courses, win sectionals in Varsity soccer, and win World Finals for Odyssey of the Mind. Many of my goals are not easy, but I believe that all are achievable if I am persistent and use the skills and knowledge that I have gained from all my experiences in Scouts.”
For his Eagle Scout project, Noah constructed a kiosk for Temple Hill Cemetery to put signage and information about the cemetery’s history and notable members. Temple Hill Cemetery Association representative, Bettina DeBell, explained that the kiosk currently features “six residents whose stories may be new to many. They are the stories of ordinary people in extraordinary circumstances. The panel will invite visitors to wander and wonder as they visit Temple Hill Cemetery.” Noah also had an app made to start geomapping graves within the cemetery to enable them to make self-guided tours of the notable people buried there. DeBell says Noah’s kiosk enables the community to connect with the self-guided version of the spirit walk tours the cemetery has hosted since 2013.

He quips also that, “At the end of my life, I want to be the fun guy in the nursing home that has the most entertaining stories. No matter who and what I become in my life, I know that the life lessons I have gained from Scouts will be with me all along the way, helping me make choices to better the world around me.”
At Noah’s Court of Honor, DeBell declared Noah enthusiastic about preserving historic cemeteries such as Temple Hill, which serve as “outdoor museums,” “public art,” “memorial architecture,” and “community history” combined. DeBell said, “Noah’s project is a model of sustainability that truly gives back to the community.”

Grateful for huge hearts! Two weeks ago I posted a need in the “Give & Take” section for unused cards with envelopes to send to participants in our Catholic Charities of Buffalo older adult programs in Genesee, Orleans and Erie Counties. So many wanted to help and we want to say a HUGE THANK YOU! We received so many donations of cards that our storage area is at capacity! I apologize that I wasn’t able to individually respond to every call left as there were so many. Those whose cards weren’t able to be donated, please consider offering them to a skilled nursing facility or adult day program where they can be used to brighten the day of an older adult who may be spending their birthdays and holidays without those they love. The big hearts of people in our WNY community make us so proud to do the work we do to serve those in need.
Free School Supplies and Meals The budget for the [local school] District has been approved for 20232024, which includes supplies and free meals for all students. I had three children attend [local school] and had to purchase all their school supplies and meals. Now I’m a Senior Citizen on Social Security with a fixed income, and I should not have to pay for these free supplies and meals. Voters, compare your 2022 taxes with 2023 taxes. My school taxes have gone up 21%, yet Social Security only went up 3.2%. Why should Senior Citizens have to pay so much? We have already paid our share. The Superintendent and the School Board need to address this issue.
Plowing Fields Will farmers with tractors, especially big tractors, after leaving the field onto the roads, stop and move the huge chunks of mud left on the road. Some were over a foot and a half around. I saw numerous vehicles who had to maneuver around these. Oncoming traffic plus the damage to their vehicles could be serious. I went out with one other driver and cleared what we could off the driving surface. With all these home cameras, if something serious happens, I’m sure you would be responsible.
