BREAST CANCER TREATMENT Most patients with breast cancer have surgery to remove the cancer. Sentinel lymph node biopsy is the removal of the sentinel lymph node during surgery. The sentinel lymph node is the first lymph node to receive lymphatic drainage from a tumor. It is the first lymph node where the cancer is likely to spread. A radioactive substance and/or blue dye is injected near the tumor. The substance or dye flows through the lymph ducts to the lymph nodes. The first lymph node to receive the substance or dye is removed. A pathologist views the tissue under a microscope to look for cancer cells. After the sentinel lymph node biopsy, the surgeon removes the tumor using breast-conserving surgery or mastectomy. If cancer cells were not found in the sentinel lymph node, it may not be necessary to remove more lymph nodes. If cancer cells were found, more lymph nodes will be removed through a separate incision. This is called a lymph node dissection. Types of surgery include the following: • Breast-conserving surgery is an operation to remove the cancer and some normal tissue around it, but not the breast itself. Part of the chest wall lining may also be removed if the cancer is near it. This type of surgery may also be called lumpectomy, partial mastectomy, segmental mastectomy, quadrantectomy, or breast-sparing surgery. • Total mastectomy: Surgery to remove the whole breast that has cancer. This procedure is also called a simple mastectomy. Some of the lymph nodes under the arm may be removed and checked for cancer. This may be done at the same time as the breast surgery or after. This is done through a separate incision. • Modified radical mastectomy: Surgery to remove the whole breast that has cancer, many of the lymph nodes under the arm, the lining over the chest muscles, and sometimes, part of the chest wall muscles. Chemotherapy may be given before surgery to remove the tumor. When given before surgery, chemotherapy will shrink the tumor and reduce the amount of tissue that needs to be removed during surgery. Treatment given before surgery is called preoperative therapy or neoadjuvant therapy. Even if the doctor removes all the cancer that can be seen at the time of the surgery, some patients may be given radiation therapy, chemotherapy, or hormone therapy after surgery, to kill any cancer cells that are left. Treatment given after the surgery, to lower the risk that the cancer will come back, is called postoperative therapy or adjuvant therapy. If a patient is going to have a mastectomy, breast reconstruction (surgery to rebuild a breast’s shape after a mastectomy) may be considered. Breast reconstruction may be done at the time of the mastectomy or at some time after. The reconstructed breast may be made with the patient’s own (nonbreast) tissue or by using implants filled with saline or silicone gel. GENESEE VALLEY PENNY SAVER

One of the easiest and most effective defenses against heat loss is proper insulation. All that’s needed is a serrated blade or bread knife. Cut the batt to fit the cavity and press into place. Aim for an R-50 or a depth of 16 inches.

I love the outdoors – even laying in the snow while the family is in the warm house.

DEAR KARL: I can totally relate! Years ago, I moved across the country with a very nervous cat in a carrier next to me on the bench seat of my pickup truck. Even though I made sure he had water, food and anxiety meds prescribed by the veterinarian, it was a stressful trip for him, and he did not eat or even poop for the whole two-day trip. Once I reached our new home, he returned to his usual snuggly self. Traveling with all three cats is possible with planning and may be the least stressful option for them. If you have an extended-cab pickup, make room in the back seat for three cat carriers. Put a litter pan on the floor and cover with plastic wrap while traveling; at rest stops, open one carrier at a time and let one cat out to use the litter pan and to snuggle with you (don’t open the vehicle doors while you do this). Look for pet-friendly hotels that will let you keep the cats in the room. Airline cargo transport can quickly transport pets across long distances. But there is a risk of death for pets traveling in airline cargo. Another option is private pet transport. If you choose this service, make sure they are insured and bonded, and that they will stay in contact with you throughout the trip.

MUSIC A HUGE PROBLEM? Wow! You use such strong adjectives to describe people who post their views, words such as “attacked” and “ganging up” on the person who has a complaint about neighbor and their music. I read the post as did many others, I’m sure, and there was no such thing. You may not like or agree with the suggestions given, but that does not add up to ganging up on or attacking someone. It was just practical advice. It’s clear to see how someone’s playing music can become so distorted. I think the complaining neighbor has more problems than just the neighbor’s music playing. They seem to have such distaste for them it’s a shame. How sad. With all the woes in the world, it’s a pity to make such a big thing over music. How long has this been going on that, all of a
sudden, it is a huge problem? MUSIC TROUBLE So sorry about neighbor being bothered by loud music. How long have you been suffering? Are you the only neighbor having a problem with loud music so loud that it rattles your house? Maybe you should try to nicely explain to your neighbor how bothersome the music is to you? Have you given that a thought? Maybe do it over a cup of coffee? That always brings the tension down, just a few suggestions. Also, ear plugs can help somewhat. There are codes where you can’t play music late at night or too early in the morning. Don’t know if that is when the music is being played. It’s always good to get along with neighbors. Hopefully, you can get some much-needed relief. Kindness does go a long way. I have been blessed with
neighbors who are kind and considerate. RESPECT OTHERS WITH MUSIC The nature of character is not based on self-entitlement but of respect for other’s right to peace and quiet. This must be a consideration, especially when residing in close communities or apartment complexes. We should not have to suffer through someone else’s loud music or faulty exhaust systems, etc. Also, taste in music is variable. One person’s treasure is another person’s torment. Peace and quiet is not trivial. It is of precious value. Remember the quiet individual doesn’t annoy the loud one; it’s vice versa. So, who should move? MUSIC & GETTING ALONG If you live in an apartment complex and a neighbor is blaring their music, then you

Diabetes Not Limited to Only 2 Main Types --DEAR DR. ROACH: Last year, at 57, I was diagnosed with Type 1 LADA diabetes. Initially I was put on insulin to get my blood sugar within the normal range. Then I was just on metformin until the “honeymoon period” was over. My endocrinologist is great. I now am back on insulin (six units at night). I walk anywhere between 3 and 8 miles about three times a day. My blood pressure is excellent. I initially lost about 18 pounds after diagnosis, so my weight is good. When looking at Google, it says my life expectancy has been cut by 20 years! Yikes, I JUST retired and if I go by their gauge, I’ll be passing around age 68! Is this based on someone who has had diabetes for many years, since Type 1 is usually diagnosed much earlier? The doctor said even though my initial sugars were over 500 when diagnosed, I was very healthy and that’s why nothing was “destroyed.” My vision was off for about a month but no permanent damage was done. Any reassurance would be appreciated! -- M.W. ANSWER: Most people are familiar with Type 1 diabetes, with a typical onset in childhood and at a normal weight. It is caused by an autoimmune destruction of the insulin-making cells in the pancreas. People also know about Type 2 diabetes, usually adultonset and usually diagnosed in someone who’s overweight; it’s caused by resistance to insulin. But there are other subtypes of diabetes, including latent autoimmune diabetes in adults (LADA). In the U.S. and Canada, it accounts for a small fraction of diabetes cases, but is more common in Scandinavian countries. It has some characteristics of both Type 1 and Type 2 diabetes, but the autoimmune nature and the usual progression to insulin need makes most experts feel it is closer to Type 1 than to Type 2. A recent paper has outlined a potential new classification of diabetes types. The change in life expectancy from diabetes relates to how long you have had diabetes and how well-controlled it has been. With outstanding diabetes control (normal or near-normal blood sugar levels), the effect on overall mortality is modest, at best. In one model, the reduction in life expectancy is less than a year. However, it certainly is possible to have life expectancy drop by 20 years with very poorly controlled, longstanding diabetes. For people with diabetes, to a very large extent, their potential to live longer (and healthier) can be improved by better blood sugar control. For new-onset autoimmune diabetes (both Type 1 and LADA), early and aggressive blood sugar control with insulin may reduce the amount of autoimmune damage and improve long-term prognosis. Studies are in progress to evaluate this. *** DEAR DR. ROACH: I am a female, 16 years old. My white blood cell count is 16.6. I am suffering from so much weakness and pain in my legs, arms and shoulder area. I also have a cough and cold. Is it dangerous? What should I do for it? -- K.C. ANSWER: I don’t have enough information to help much. A white blood cell count that high suggests an infection. At your age, those symptoms are most likely a viral infection, but acute mononucleosis and even pneumonia are possible, so you need to see your doctor. *** Dr. Roach regrets that he is unable to answer individual questions, but will incorporate them in the column whenever possible. SENIOR NEWS LINE By Matilda Charles Now More Than Ever, Get Your Flu Shot A sincere wish just came true: This year's high-dose senior version of the flu shot will contain all four vaccines. In past years, the senior shot (for those over age 65) contained only three of the vaccines, two A and one B. Most years I would write a snarly note to the Centers for Disease Control and Prevention (CDC) and ask about the reason for the false economy. Of course I never got a reply, but now ... finally, we seniors will have a flu shot with all four vaccines. And what a shot it will be. The four vaccines are names we haven't heard in a while, if ever: A/Guangdong-Maonan (H1N1), A/ Hong Kong (H3N2), B/Washington and B/Phuket. To help us kick start our immune system, the senior version will contain four times the amount that younger folks get. The standard trivalent (threeway) will have two A and one B, while the standard quadrivalent (four-way) has all four. If your doctor says you should have a flu shot, don't delay. When I called my pharmacy (where I get my flu shots each year), the first appointment was many days out, and they had been swamped with requests for the shot. Yes, they will be resupplied when they run out, but no, they didn't know how quickly that would be. By getting a flu shot, should you get sick later this season, it will be a good bet that it's not the regular flu, which will save much time in getting a diagnosis. To find flu shots, call your doctor, local pharmacies, grocery stores or go to vaccinefinder.org. If you have a Medicare or insurance card, you won't have to pay anything. Dr. Schulz Answers Back-to-School Questions BY AMANDA DUDEK
A
cross Rochester and the Finger Lakes Region, children have gone back to school. For many students this means returning to the classroom, whether as a hybrid with virtual learning or fulltime. During this unprecedented back-to-school experience, families find themselves with many questions. Providing clarity is Dr. Steven Schulz of Finger Lakes Medical Associates, Rochester Regional Health Pediatrician, and member of the New York State Finger Lakes Reopening Schools Safely Task Force. Several school protocols include “masks breaks” and circumstances during which masks are not required, leading parents to question what constitutes an “exposure” should a student at their child’s school contract COVID-19. The answer is surprisingly clear. “An exposure is close contact with someone with COVID-19,” said Dr. Schulz. “This is defined as being unmasked and within 6 feet of an infected person for longer than 15 minutes. The highest rate of transmission occurs from close contact.” “Mask breaks can be done safely with adequate distance, but best practice is for students to remain masked and socially distanced whenever possible, especially indoors,” Schulz continued, noting there is a lower risk of transmission when outdoors. Dr. Schulz and the Task Force are working with Common Ground to share and implement New York State Department of Health guidelines with all schools within a 13-county area. Although some cases are anticipated, Dr. Schulz encourages families that we are in an excellent place to be returning to school. “In the Finger Lakes Region, we only have a 1% prevalence rate,” he explained. “That means there is a 99% chance that a child’s symptoms are due to another virus that is not COVID.” However, Dr. Schulz advises parents to contact their health care provider for next steps if their child does develop symptoms which are also consistent with COVID. But how can a parent determine when their child’s symptoms are likely COVID-19 rather than simply a cold, allergies or something else? “That is the hard part,” Dr. Schulz acknowledges. “COVID in children tends to be milder with symptoms ranging from runny nose, congestion, cough, shortness of breath and sore throat to nausea, vomiting, diarrhea, abdominal pain, fatigue and poor appetite. What parents should be most aware of are new, different, or worsening symptoms, particularly a fever.” What happens if your child gives a positive response during the school’s screening protocol or is sent home due to related symptoms? “Students who have these COVID symptoms must have an examination,” said Dr. Schulz. “The NYS Department of Health requires that the student meet four criteria to return to school: 1. Receive an evaluation by their health care provider, 2. Have a COVID-19 test with negative results, 3. Obtain a letter from the child’s primary health care provider, 4. All the child’s symptoms must have resolved.” Dr. Schulz again emphasizes that “kids are being sent home for a wide variety of symptoms, many of which are likely not COVID related.” However, he recommends isolating the child as much as possible within their household until their test results are received. During this time, adults and children over 2 years old should wear masks and wash hands frequently, and surfaces should be disinfected. “It is better to be safe than sorry to keep the virus under control,” he added. “Stay home when your child is sick and be honest when answering screening questions.” For the children who receive a positive result on their COVID-19 test, returning to school is a little more involved. “The health department

Dr. Schulz is satisfied with the opening safety plans of area schools, which had to be approved by the state, adding that every family is different with its own tolerance of risk. “Every district is offering remote classes,” he observed. “At the same time, there are lots of benefits for attending school in person such as socialization, learning requirements, food stability and a stable place to stay. The schools are taking every precaution and are helping families to determine which choice is best for them.” Meanwhile, Dr. Schulz encourages people to talk to their local healthcare provider about their ability to get a flu shot. “It is especially important this year,” he said emphatically. “COVID-19 and the flu have a lot of overlapping symptoms, and theoretically, you could be infected with both.” He cautions that outbreaks of COVID-19 combined with outbreaks of the flu could overwhelm the healthcare system, reminiscent of the strain COVID-19 and the flu put on NYC’s healthcare system earlier this year. “Typically, the flu season starts in November, so we encourage people to get theirs by the end of October.” Regarding a future COVID-19 vaccine, Dr. Schulz believes it is only a matter of time. “There is a lot of vigorous study going into this,” he stated. “Vaccination trials are taking place regionally, and there is continued diligence regarding efficacy and safety. How do I get rid of bishop’s weed? It invaded a bed of lily of the valley and I donít want to kill that as well. - Kathleen C. Bishop’s weed has an incredibly dense root system which is quite difficult to eradicate. You would need to remove the lily of the valley plants, then smother the entire bed for at least six growing months; a year would be more effective. The ground is frozen during December,
January, February, and March, so plants are dormant, not growing. Before replanting your lily of the valley, completely remove all current soil from the roots by soaking and using a forceful hose. Cover the area with landscape fabric or 6 layers of newspaper with overlapping edges, then top with several inches of mulch. Secure the edges with stones or branches. The newspapers will gradually disintegrate, but you would need to pull out the landscape fabric. If you don’t remove the fabric, it will
gradually break down in to smaller pieces, become mixed into the soil, and impede root growth. Watch the edges of this area during growing months, and spray any bishop’s weed plants that emerge with herbicide.
