Blood Pressure Drops When He Stands Up

And Feel Rewarded

DEAR DR. ROACH: Will you write about orthostatic hypotension? I’ve been told I have this disorder. I’m a retired 86-year-old man in fairly good medical shape. I want to regain my regular activities. My regular doctor put me on labetalol. I think he is researching this a bit himself. This apparently is not a frequent problem. I have found articles about it on the internet, but the medical language is not easy to understand. -- R.T.F. ANSWER: “Orthostatic hypotension” literally means “low blood pressure on standing upright.” It is a symptom, not a diagnosis. There are many medical conditions that can lead to the symptom of orthostatic hypotension, but many people, especially those over 65, have no discernable cause that can be found. On standing, the blood pools in our veins, and the heart and blood vessels need to rapidly adjust to prevent blood pressure from falling too much. Even a brief drop in blood pressure can lead to a sensation of lightheadedness, and to fainting if it is more severe and prolonged. Normally, blood pressure drops about five points, but the body compensates by increasing heart rate and by constricting blood vessels. As we get older, our body’s ability to adapt to rapid changes in position can diminish. Medications, especially beta blockers and antidepressants, interfere with the body’s coping mechanisms. So I am surprised that your doctor is trying labetalol, since it is both a beta and alpha blocker, and it is one of the most notorious causes of orthostatic hypotension. But it can be difficult in older people with high blood pressure to find a medicine that doesn’t make orthostatic hypotension worse. Some general advice may be of benefit. First, don’t try to get up too quickly. If lying down, sit up for a few minutes before trying to stand, to give your body time to adapt. If your doctor says it’s OK, increase salt and water intake. In your case, it might be worth finding someone, like a hypertension expert or a cardiologist, with more expertise in managing orthostatic hypotension. I assure you it is far more common than you might think.

DEAR DR. ROACH: I have been experiencing shortness of breath whenever I vacation and walk in the mountains. I was a smoker, but I quit 31 years ago. My doctor sent me for the lung capacity breathing test, and I passed with flying colors. The technician said my number was one of the highest she has seen. This concerns me. Why is it happening? -- N.L. ANSWER: I have two concerns. The first is that lung capacity is a measure of just what it sounds like -- how big the lungs are. When the technician says it’s among the biggest she’s seen, I worry that it’s too big. An elevated lung capacity can go along with emphysema, which can be related to distant smoking or can be due to a condition called alpha-1 antitrypsin deficiency. Emphysema can be diagnosed by other components of pulmonary function tests, particularly a test called the DLCO, and confirmed by X-ray or CT. However, breathing problems also might indicate heart problems and anemia, so you might need another visit. On the other hand, there is less oxygen in the thin mountain air, so some degree of shortness of breath might not be abnormal. *** Dr. Roach regrets that he is unable to answer individual letters, but will incorporate them in the column whenever possible. To the person who mentioned political correctness, insensitivity to the majority and the spirit of the season regarding wishing others a Merry Christmas. Acknowledging and respecting the beliefs of others during December is not about being “politically correct.” It is generally accepted that the spirit of the season is about love and generosity. Imposing one’s own personal beliefs on others flies in the face of this spirit. Further what one believes and how one expresses those beliefs is not subject to the “majority rules” system which only applies to elections. Don’t you just love when your doctor’s office calls or leaves a message to verify the day and time of your appointment? It’s nice of them to remind us BUT it’s always followed by, “If you don’t cancel before 24 hours, you’ll be charged a late fee,” or “if you’re 5 minutes late, your appointment will be canceled and rescheduled at a later date.” Remind them of that courtesy call next time you are seen 1 to 2 hours past your appointment time. Cook in the Kitchen with Mom French Toast Kebabs 2 eggs ¼ cup milk ¼ tsp vanilla extract ¼ tsp ground cinnamon 6 oz French bread, cut into 1-inch cubes 2 Tbsp butter Berries, powdered sugar and maple syrup

Place eggs, milk, vanilla and cinnamon in a large bowl. Add bread cubes and stir until coated. Place butter in a large skillet over medium-high heat. When butter is melted, transfer bread cubes with a slotted spoon into the pan and cook, flipping often, until browned on all sides, 2 to 3 minutes. Thread toasted cubes onto 6 (6- inch) wooden skewers alternately with berries. Dust with powdered sugar and drizzle with maple syrup.

1979 Miss USA Mary Therese Friel Remembers Growing Up In The Finger Lakes By Amanda Dudek

I

f you are looking for 1979 Miss USA Mary Therese Friel, skip London, New York City, Paris and Milan. Most days find Friel in Mendon, a town nestled in the Finger Lakes of Upstate New York. Mary and her husband, Kent, make their home in a renovated barn, complete with a glorious stained-glass window. It is somehow fitting that something so beautiful and serene now fills Friel’s home with the same radiant light it once spread over prayerful worshippers. “I love my home and this area,” said Friel. “This place is magic to me.” Friel traveled extensively upon being crowned Miss USA. “I visited so many places and have seen so much,” said Mary. “From London to Paris, Milan and the beautiful skies of Spain. I have journeyed on camelback to Saudi Arabia. I approached awe-inspiring Pompeii Mary Therese Friel as the 1979 Miss USA in a yacht, sailing with the richest man in the world.” Yet, for all the majestic sights she has seen, Friel prefers the New York countryside. “It is great to go to Brazil and walk the beaches, but I would miss a good snowfall!” she quipped before adding more seriously, “I feel that people who raise their families here – in towns like Canandaigua, Naples, Dansville, Avon – give their children the greatest gift.” She may be a bit partial, being a Pittsford native. “I loved growing up in the Pittsford and Greater Rochester area,” said Mary. “There was always so much to do! We used to go horseback riding. We also would ski, ice skate and build snowmen, especially when I would go with my family to the Adirondack Mountains. We loved watching my father plant his garden each spring.” Friel also fondly recalled another local feature which she still enjoys today. “I loved the Genesee Valley Penny Saver,” Mary said. “I would read it as a child, and it is still one of my favorite papers! I especially enjoy checking Give & Take and leafing through its pages, hunting for leads to vintage finds.” When she was crowned the 1979 Miss USA, Friel rocketed to national prominence. “I was in shock,” Mary recalled. “I didn’t go into it knowing I would win. For me, it was simply what I was meant to do.” She described her pageant beginnings as a Kodak Girl at age 11. “I later entered Miss Finger Lakes and was 1st runner up,” Mary explained. “After returning from college in Miami, I found a letter waiting for me, asking me to be in it again.” She agreed. Ten girls, including Friel, placed in Miss Finger Lakes and were sent to the State of New York pageant in the Catskills. “Another girl was already “Miss Pittsford,” so I became ‘Miss Barge’ for the Erie Canal,” said Mary. Friel’s father quickly pointed out that she had the only one syllable title, and when she was upset over her green bathing suit, her father noted how well the green would complement her hazel eyes. “He could take anything and make it a positive,” said Mary.

Even with Beauty Queen status, success hasn’t changed what is important to Friel. “God, church, family, animals, community, education – all of that was first for me growing up,” said Mary. “I felt like participating in pageants was for them because these things are a gift.” Self-identified as “down to earth,” Friel’s compassion reveals a deeply empathetic soul. “I care so much about everything,” she confided. “I love children, seniors, animals, veterans, the homeless – everyone! God made this world, and we are blessed to live in it. If we were all good to each other, we would take care of the world.” Friel’s life reflects the sincerity of her words. “We support many charitable causes,” she stated. “Through our Comfort Campaign, we annually support FoodLink, Pittsford Food Cupboard, Gary the Happy Pirate, Toys for Tots, Lollypop Farm, Salvation Army, the Red Cross, the Center for Youth through Fashion Week of Rochester, Veterans... It seems like we are always loading up our 1-ton truck to help!” Friel also operates the Mary Therese Modeling Agency’s program You Can Be... as well as its Goodwill Ambassador Program. Along with her faith and her upbringing, an encounter with Mohammed Ali greatly impacted Friel’s life. “When I met Mohammed Ali in 1980, he said, ‘Service to others is the rent we pay for our room in the hereafter.’ That really stuck with me,” said Mary. “He said when you are given the gift of fame or acknowledgement, what really matters is how you use it to help others.” Even Friel’s pets, her “children,” give back to the community. “My rat, ferret, rabbits, two dogs and two cats are all participants in Lollypop Farm’s pet therapy program,” Mary shared, adding that While she has no child of her own, she and Kent work with children all day at their modeling agency. “It is the highlight of our lives,” she said. Thought they might be most often associated with dogs, ticks can affect cats as well. According to Blue Cross for Pets, cats are most likely to encounter ticks if they live near areas with lots of deer and sheep or in regions where other wildlife, such as hedgehogs and rabbits, are present. While ticks are active throughout the year, cats are most likely to encounter them between spring and autumn. That’s due in part to the climate during these seasons, which tends to be warmer and therefore more inviting to cats, compelling them to spend more time outdoors than they do in winter. Ticks, which are egg-shaped, are big enough to spot, so cat owners should conduct a careful examination of their cats’ fur each time they come into the house after spending time in the yard. Blue Cross for Pets notes that ticks feel like small bumps on a cat’s skin. Ticks are most likely to attach themselves to particular areas of a cat’s body. These areas include the head, neck, ears, and feet. Routine brushing, especially whenever cats come inside after time spent outdoors, can help cat owners spot and remove ticks from their cats. It’s important to recognize that indoor cats are not invulnerable to ticks, as other pets and humans can track ticks indoors. Periodic inspections and brushing of indoor cats’ can protect them from ticks and the diseases ticks carry.
