Here’s A Tip
By JoAnn Derson
* “I have added an additional shower curtain bar in my tub. I hang up my towels and washcloths there to air dry, and they are out of sight behind the shower curtain. It’s very convenient.” -- A reader
* “I have small candy dishes out in my home, and I like to keep them filled with goodies throughout the year, but now, when I have little children over, I put them up high and let parents know that they and the children are welcome to have as much as they like. e parents appreciate this, as the children sometimes like to overindulge.” -- D.A. in Rhode Island
* Unexpected company on the way? row clutter in a laundry basket and stash. Clear the sink of dirty dishes; take out the garbage; clean the faucets, sink and mirror in the bathroom most likely to be used; and, finally, sweep or vacuum the floors. is should not take long, but these moves will maximize the feeling of cleanliness in your house. OBITUARIES
e American Cancer Society says hospice care is offered by a team of professionals who work together to manage distress, symptoms and spiritual issues. Services also are offered to family members, including access to counselors and bereavement experts. Hospice workers help family members make decisions and plan care.
Hospice is a form of care, rather than a place. Saying a person is going into hospice means he or she will begin to receive this type of palliative care. While hospice can take place in a center that only offers this type of service, a hospice team can work in a variety of settings, including one’s home.
Many people receive hospice care at home. VNS Health provides care in spaces patients call home and says that hospice care can include administration of medications, arrangement of equipment (like beds and oxygen) and visits from a hospice care team. A hospice team may include nurses, social workers, physicians, therapists, and even clergy who come to the home to offer support and care.
Home hospice care begins with a team member visiting with a terminally ill person and his or her family. During a visit, needs will be addressed and the hospice team can arrange for desired services.
Members of the hospice staff will visit regularly to check up and make sure that any symptoms are under control.
Families are urged to do their homework to find the best program for their needs.
The sixth grade seemed to please him from the beginning: he went through a brief Egyptian Period that baffled me — he tried to walk flat a great deal, sticking one arm in front of him and one in back of him, putting one foot behind the other. He declared Egyptians walked that way; I saidif theydidIdidn’tseehow they got anything done, but Jem said they accomplished more than the Americans ever did, they invented toilet paper and perpetual embalming, and asked where would we be today if they hadn’t? Atticus toldmetodeletetheadjectives andI’dhavethefacts.
Harper Lee, To Kill a Mockingbird, 1960 hospice at home e philosophy behind hospice is that people should be able to live as fully and as comfortably as possible through the last days of their lives, surrounded by loved ones and friends. Hospice treats the symptoms of illness rather than trying to be curative. Hospice also does not try to postpone nor accelerate death.

Writing a Sympathy Card
An authentic handwritten message can mean a lot to someone dealing with grief. It shows that you’ve sat down and thought about it as opposed to buying a pre-written card. Sharing fond memories and sincere feelings is a respectful way to remember the passing of a loved one and to offer condolences to their friends and family. If you’re able to offer support, then definitely mention this in your sympathy card. ose grieving will o en have lots to do and will appreciate any help you may be able to offer even if they don’t take you up on it.
RISK FACTOR FOR AFIB
Existing heart conditions: Heart valve problems; hypertrophic cardiomyopathy, which is marked by a thickening of the walls of the heart chamber; acute coronary syndrome, an umbrella term used to refer to conditions in which blood supplies to the heart are suddenly blocked; and a history of heart attack are just some of the heart conditions that increase a person’s risk for AFib. Anyone with a personal or family history of these and other heart conditions is urged to discuss AFib with their physicians.

Christmas Spirit
Is it just me, or did Christmas decorating start early this year? It feels like the lights went up even before the last leaf hit the ground, but I'm all good with it. There's something so happy about the Christmas lights on the houses, the 'Tis the Season section in the paper, and seeing all the hometown events being scheduled. It might be early, but I can’t help but smile when I see our little towns getting into the holiday spirit.
Political Signs
Now that Election Day has passed, can people please remove their signs? There’s still too much political hate and some can’t control themselves. It’s time to move on.
Pepper Spray for unleashed dogs?
I live in [local county] and have horses, dogs and cats. Most people are considerate and follow the rules when using the public parks. Last week when riding, a loose dog lunged up, biting me and then biting at my horse. The situation was chaotic as the dog owner rushed at my horse while yelling (at their dog) who was nipping/barking at the horse. Off leash dogs do not need to be the problem they are. Follow the rules. Leash your dog. It’s that simple.

RENOVATIONS THAT HELP SELL HOMES
Minor bathroom remodel: Remove dated wall coverings, replace fixtures, consider reglazing or replacing an old tub, and swap old shower doors for fast fixes in a bathroom. If there’s more room in the budget, replacing a tub, tile surround, floor, toilet, sink, vanity, and fixtures can cost roughly $10,500,says HGTV.You’ll recoup an average of $10,700 at resale, making a minor bathroom remodel a potentially worthy investment.
DID YOU KNOW?
Formal dining rooms were once a must-have in homes, but that popularity has waned in recent decades. Dining rooms were the spaces to host holiday dinners or family birthday parties. Slowly but surely, dining rooms became obsolete in modern homes that favored open-concept layouts. And in those homes that had formal dining areas, the need for home office spaces or playrooms for children saw many dining rooms repurposed. According to the National Association of Home Builders, 43 percent of millennials prefer the look and feel of an open-concept layout and casual dinner parties. However, even though dining rooms may no longer be de rigueur, designers are noticing an increase in clients requesting formal dining spaces, according to a report in Southern Living. Perhaps it’s a matter of open-concept fatigue. After spending a year or more during the height of the pandemic at home with few to no barriers between rooms, homeowners may be looking for homes that carve out niches for specific tasks (and privacy) once more. It may not be long until homes come back to including more traditional layouts with separate dining spaces to make hosting a little more intimate.

Home buyers must confront a host of variables in the best of times, but in recent years buyers have been forced to contend with a real estate market marked by a significant increase in home prices. According to the Federal Reserve Bank of St. Louis, the median home sales price in the third quarter of 2023 was $431,000. at marks a significant increase since the start of 2020, when the median sales price was $329,000. at 31 percent increase in a little less than four full years can be traced to a host of variables, not the least of which is the global COVID-19 pandemic, which officially earned that designation in early March 2020. But the rise in home prices is not as recent a phenomenon as some may think. In fact, data from the Federal Reserve Bank of St. Louis indicates the median home sales price rose by 576 percent between 1980 and the third quarter of 2023.

Tickets: Perhaps nothing sparks sports fans’ passion more strongly than seeing their favorite team in person, and that’s even more true in a post-pandemic sports landscape. Popular sports like football, baseball and basketball have traditionally sold lots of tickets, but even sports like college volleyball and Major League Soccer have set attendance records since fans were allowed to return to stadiums after the removal of pandemicrelated restrictions. The demand for tickets to live sporting events has spiked as a result, and Team Marketing Report noted National Football League tickets increased by nearly 9 percent in 2023 and Major League Baseball tickets increased by 3.5 percent in that same year. Such increases have priced some fans out of the market, which is why tickets can make for such a great gift this holiday season. The gift of attending a game in person is something a loved one won’t soon forget. A TRADITION THAT MAKE KWANZAA SPECIAL
Retail sales during the 2022 holiday season inched ever closer to crossing the trillion dollar mark. Though total retail sales during the November-December holiday season fell short of National Retail Federation forecasts, the group still reported that sales totaled $936.3 billion in that period in 2022. Though a host of variables, including inflation, affect how much consumers spend during a given holiday season,it’s not outside the realm of possibility that 2023 could be the year shoppers cross the trillion dollar threshold. That would require an increase of a little less than $64 billion in spending compared to 2022. Such an increase is not unheard of, even in a single year, as NRF calculations based on data from the U.S. Census Bureau indicate holiday season spending between 2020 and 2021 increased by just under $106 billion.

A TRADITION ASSOCIATED WITH CHANUKAH CELEBRATIONS
Playing dreidel: There are various stories about the origins of the dreidel, but historians say the first mention in Jewish writings did not occur until the 18th century. The four sides of the dreidel are marked with different letters, which in German are G “ganz” (all), H “halb” (half), N “nischt” (nothing) and S “schict” (put). This dictates whether the player who spun the dreidel gets all, half or nothing in the collective coin pot, or has to put in his own. Yiddish is quite similar to German, and the letters on the dreidel also correspond to the first letters of the Yiddish phrase “nes gadol haya sham,” which translates to “a great miracle happened here,” a reference to the Chanukah miracle.

HOSTING THE HOLIDAYS WHEN TIMES ARE HECTIC
Keep it simple. Instead of a four-course dinner with gourmet fare, consider something small and easy. A gathering that’s not geared around trying to impress can ensure hosts have time to kick back and relax with their guests - something everyone can use during this busy time of year. Choose a theme, like hot cocoa and cookies or cocktails and charcuterie, so you won’t have as many tasks to tackle. Asking everyone to bring a dish also reduces hosts’ workload.

HOW TO HONOR VARIOUS FAITH DURING A HOLIDAY PARTY
Let guests chip in: Extended family members and friends from different religious backgrounds may be proud of their faith and enthusiastic about the chance to share it with the people they love most. A holiday gathering is the ideal time to embrace this spirit. Encourage guests from different religious backgrounds to bring something that reminds them of their own religious celebrations. That can be a particular food or beverage, a playlist of holiday songs or a small gift such as an ornament to incorporate into holiday decor. Guests can then share as much as they would like to about their faith, including any holiday traditions that they typically engage in.

Starting A Pet Sitting Business
DEAR PAW’S CORNER: I’m 16 and love pets -- I have a dog, cat and an iguana already -- and I want to start my own pet sitting business. What is the best way to start? How do I get customers? Do I need a license or something? -- Jeannie H., Pittsburgh
DEAR ZACK: Pet sitting can be a great business to get into. Every pet owner needs someone to watch their pet from time to time: long workdays, vacations, illness and so on. However, it’s a business that carries a lot of responsibility. Being reliable is very important and is essential to building your reputation as a sitter that pet owners can trust.
I recommend a trial period first, with a couple of neighbors or your parents’ friends. is will give you an idea of what the job entails, how long it takes to care for each pet, and can open your eyes to opportunities. For example, a neighbor who works from home may not need a sitter, but their dog could use an extra walk in the a ernoon.
At the same time, study up on the requirements for running a business. Entrepreneurship courses are o en available for free through your local library or at the Small Business Association (where you can also find a business mentor).
Most states don’t have a licensing requirement for pet sitters, but you can get a certification through Pet Sitters International (petsit.com), or the National Association of Professional Pet Sitters (petsitters.org). ese organizations offer a wealth of information -- including how to find clients. Most of all, have fun! You’re getting into this business because you love pets, and that will be a big factor in your success.

Skin cancer is the most common type of cancer worldwide. In the United States, one in five people will develop skin cancer by the age of 70, and more than two people die of skin cancer in the U.S. every hour, says the Skin Cancer Foundation. Roughly 90 percent of non-melanoma skin cancers are associated with exposure to UV radiation from the sun. Melanoma also is inextricably linked to UV exposure. In the United Kingdom, nearly nine in 10 cases of melanoma can be prevented by staying safe in the sun.The American Cancer Society estimates about 100,000 new melanomas will be diagnosed in 2024, when roughly 8,290 people are expected to die of melanoma. The number of people getting skin cancer in Canada also has risen at a fairly constant rate over the last 30 years. One in 73 Canadian women and one in 59 Canadian men will develop the disease, according to Canada.ca. Early detection of skin cancer can improve prognosis.

One way is with a non-contact tonometry test, which is a puff of air squirted at each eye. With applanation tonometry, the doctor first administers a mild numbing agent to the eye, and then uses a pressure-sensitive instrument to gently tap on the surface of the eye to get a pressure reading.A rebound tonometry test requires a handheld device that makes brief contact to the eye and doesn’t require local anesthesia.

A cataract is a clouding of the naturally transparent lens of the eye that can occur as one ages. Natural proteins in the lens clump together and can cloud the eye. The National Eye Institute says most cataracts develop with age, as more than half of all Americans age 80 or older either have cataracts or have had surgery to remove them. But cataracts can occur for other reasons, such as after an eye injury or after surgery for another vision problem.TheAmericanAcademy of Opthalmology says cataracts can make it seem like a person is looking through a foggy or dusty car windshield. Everything is hazy, blurry or less colorful. Additional vision changes associated with cataracts include extra light sensitivity; seeing bright colors as

Blood Pressure Spikes Could Denote Labile Hypertension
DEAR DR. ROACH: I’m 85 years old and in good health for my age. I am an active nonsmoker, eat healthy, practice tai chi, and enjoy having many friends. I have no reason to experience extreme anxiety. Yet, for months, I’ve had unexplained blood pressure spikes.
The highest I had was 243/127 mm Hg, and this is when I called local EMTs who took me to the emergency room. I was given meds to lower my blood pressure, two electrocardigorams, and an X-ray. I was discharged after three hours and was told that my heart seemed fine.
I am now taking losartan and propranolol. I tried triamterene after consulting with a cardiologist, but I couldn’t tolerate the side effects. I occasionally use lorazepam. Today a blood pressure reading showed 197/100 mm Hg. Just the thought of checking my blood pressure spikes the reading. How long can I go on with these blood pressure levels? Can these readings be normal for some people? -- S.O. ANSWER: No, these readings are not normal. Since you say that your blood pressure “spikes,” I think you mean that most of the time, your blood pressure is normal.
The biggest concern is a condition where the body intermittently secretes substances that raise the blood pressure. This may be caused by a tumor called a pheochromocytoma. They are exceedingly rare but must be looked for in a case like yours, with sudden and dramatic spikes in your blood pressure. Your regular doctor or cardiologist will know how to look for a pheochromocytoma.
If you have a spike, a blood test will be dramatically elevated for epinephrine and norepinephrine. But if your blood pressure is normal at the time of the visit, a 24-hour urine test is done.
There are other rare causes, including blockages in the arteries to the kidneys and certain drug use, such as monoamine oxidase inhibitors (very rarely used for depression) in combination with some foods. Cocaine use is another cause in a person on a beta blocker, like the propranolol you are on.
Even more likely is a condition called labile hypertension, where emotional stress leads to an increase in blood pressure levels. This is more likely for you because of the last thing you said: Just the thought of checking your blood pressure may cause it to spike.
Managing labile hypertension should be done by an expert. There are experts in hypertension who know how to manage very unusual cases of hypertension, whether they’re generalists, nephrologists or cardiologists. Your doctor may have already considered this diagnosis because lorazepam is often used for spikes, sometimes in combination with shortacting blood pressure medicines, like labetalol or clonidine.
Dr. Roach regrets that he is unable to answer individual questions, but will incorporate them in the column whenever possible. Readers may email questions to
