What to know about hospice at home e philosophy behind hospice is that people should be able to live as fully and as comfortably as possible through the last days of their lives, surrounded by loved ones and friends. Hospice treats the symptoms of illness rather than trying to be curative. Hospice also does not try to postpone nor accelerate death.

e American Cancer Society says hospice care is offered by a team of professionals who work together to manage distress, symptoms and spiritual issues. Services also are offered to family members, including access to counselors and bereavement experts. Hospice workers help family members make decisions and plan care. Hospice is a form of care, rather than a place. Saying a person is going into hospice means he or she will begin to receive this type of palliative care. While hospice can take place in a center that only offers this type of service, a hospice team can work in a variety of settings, including one’s home. Many people receive hospice care at home. VNS Health provides care in spaces patients call home and says that hospice care can include administration of medications, arrangement of equipment (like beds and oxygen) and visits from a hospice care team. A hospice team may include nurses, social workers, physicians, therapists, and even clergy who come to the home to offer support and care. Home hospice care begins with a team member visiting with a terminally ill person and his or her family. During a visit, needs will be addressed and the hospice team can arrange for desired services.

Members of the hospice staff will visit regularly to check up and make sure that any symptoms are under control.

Families are urged to do their homework to find the best program for their needs.

Here’s How It Works: Sudoku puzzles are formatted as a 9x9 grid, broken down into nine 3x3 boxes. To solve a sudoku, the numbers 1 through 9 must fill each row, column and box. Each number can appear only once in each row, column and box. You can figure out the order in which the numbers will appear by using the numeric clues already provided in the boxes. The more numbers you name, the easier it gets to solve the puzzle!

Injections

DEAR DR. ROACH: I’m a 67-yearold healthy male, but I do take Humira for rheumatoid arthritis (RA). It does a wonderful job for me with no side effects. It doessuchagoodjobthatIcanperiodically extend the time between injections, such as one every three weeks instead of the recommended dose every two weeks. I can even go as long as six months between injections without noticing much of the RA symptoms.

My rheumatologist is aware of this and encourages me to use the lesser amount if I still get relief. My quandary is that I was recently talking to my gastroenterologist about Humira, and he said that they use a very similar drug for gastrointestinal issues. But apparently, you aren’t supposed to stop taking it consecutively because the body will become immune to the drug, and the drug can lose its effectiveness. Naturally, I don’t want to lose the great relief I am lucky to be getting, but I also know it’s a strong drug. It seems that the less I take of it, the better. -- J.C. ANSWER:Ishareyourgastroenterologist’s concern. He may be thinking of a similar medication, vedolizumab (Entyvio). When this drug is stopped in someone with Crohn’s disease, it will cause a flare-up in about half of the cases within six months. In a third of them, the medication will no longer be effective.

The situation is similar with adalimumab (Humira), as there is the possibility of a relapse if you stop it entirely. Taking it every three weeks led to a flare-up in 36% of people who had their disease well-controlled for years. The researchers were unable to identify any way to predict who would flare up. Restarting Humira regained control for over half of the study participants -- but not everybody.

I do understand that these are powerful medicines with side effects. Even if you aren’t noticing any, many people will, and there can be very serious side effects to many organs. Fortunately, these are uncommon, and in most cases, it is far better to stay on the medicine, as long as you are being monitored.

As a medical student, I remember seeing what RA looked like before we had effective medications such as methotrexate and TNF inhibitors like Humira. Years of inflammation caused terrible hand deformities, mostly in the women I saw. Since you aren’t having any side effects, and it is working well, I don’t recommend the three-week dosingand certainly not six months between doses.

DR. ROACH WRITES: A recent column on hair loss and thyroid disease prompted many readers to write me with their experiences. One reader noted that increasing protein in their diet seemed to slow down hair loss and even cause some regrowth. Another reader recommended biotin. I often recommend this as a trial, despite a lack of good evidence that it works. Biotin is a B-type vitamin that is very safe, although it can interfere with a lab assay when it comes to thyroid levels and other hormone levels. Note that both hair loss and thyroid disease are very common, and it is not always the case that thyroid disease causes hair loss.

Dr. Roach regrets that he is unable to answer individual questions, but will incorporate them in the column whenever possible. Readers may email questions to ToYourGoodHealth@med.cornell.edu. © 2024 North America Synd., Inc.

A FUN BOOK TO READ TO CHILDREN THIS HOLIDAY SEASON

“Santa Claus and the Three Bears,” by Maria Modugno: This twist on the popular tale “Goldilocks and the Three Bears” finds three unsuspecting bears encountering a familiar fellow in a red suit upon returning home after a nighttime walk.

LIST

Tickets: Perhaps nothing sparks sports fans’ passion more strongly than seeing their favorite team in person, and that’s even more true in a post-pandemic sports landscape. Popular sports like football, baseball and basketball have traditionally sold lots of tickets, but even sports like college volleyball and Major League Soccer have set attendance records since fans were allowed to return to stadiums after the removal of pandemicrelated restrictions. The demand for tickets to live sporting events has spiked as a result, and Team Marketing Report noted National Football League tickets increased by nearly 9 percent in 2023 and Major League Baseball tickets increased by 3.5 percent in that same year. Such increases have priced some fans out of the market, which is why tickets can make for such a great gift this holiday season. The gift of attending a game in person is something a loved one won’t soon forget.

Here’s A Tip

* “Digital picture books are very easy to make these days, and they make great gi s. But here’s a way we use our digital photos to keep the kids engaged at our family gatherings. Each family prints out a selection of photos. We let the kids make their own books using half sheets of paper, glue and markers. ey design frames, etc. A er they have several pages done, we tie them together and make a cover of heavy card-stock. It’s a great take-home cra , a special souvenir for visitors, and it gets them talking about family moments.” -- R.E. in Alabama

* Want to mix it up a bit with your traditional chocolate chip cookies? Try rolling them in different types of chopped nuts or sprinkles. Even crushed pretzels are really good. Or change the flavor of the chip. Add minced dried fruit, quick oats or other seeds for a change of taste. Melt chocolate chips and dip one side in it, then let dry on waxed paper. Endless possibilities!

Drivers can thank the military for the introduction of sport utility vehicles to the mainstream market. Predecessors to modern SUVs date back to military models from the late 1930s, and four-wheel-drive station wagons and other vehicles that were introduced in the late 1940s. However, many car enthusiasts feel the 1984 Jeep Cherokee was the first SUV in the modern style. In 2021, the Environmental Protection Agency reported that SUVs made up 44.67 percent of all light-duty vehicles produced that year, compared to 25.69 percent of market share for sedans and wagons. In fact, as of 2023, many major automakers had announced plans to phase out many of their popular sedans. General Motors dropped nearly all passenger cars from its North American portfolio. Dodge dropped the Challenger and Charger; Acura removed the ILX model; the Chrysler 300 is no more; and Nissan no longer offers the Maxima. These are just some of the effects of the popularity of SUVs and also part of a larger shift toward the production of electric vehicles.
