Gratitude Finland has been rated the world’s most “happiest” country. Connection with nature, community and simplicity are three of the major factors cited as to the Finnish quality of life. However, there is an over-riding factor that they’ve learned that stands out for them and perhaps for us - gratitude. To quote: “The Nordic countries are united in their embrace of curbed aspirations for the best possible life. This mentality is famously captured in the Law of Jante -- a set of commandments believed to capture something essential about the Nordic disposition,” he explains. They include: “You’re not to think you are anything special” and “You’re not to imagine yourself better than we are.” Which, as Savolainen points out, is pretty much the opposite of America’s endless pursuit of personal and national exceptionalism. This is a great time of year to take stock of what we all have and be grateful. Amen.

[Local Paid Newspaper] Delivery Service I’ve been a subscriber to [local paid newspaper] for 70 plus years. Our new delivery person refuses to drop my paper off at the back of my short drive on the ramp so I can easily pick it up. I’ve posted a sign at my drive asking them to do this to no avail. They drop it in the gutter and if it rains it gets wet and if it snows the plow takes it away. Am I only person in [local town] to have this problem and is there anything that can be done to help? I’m 96 years old!

Fake service animals (dogs) Stop with all these fake service dogs! And stop bringing those fakes into the stores... it’s dirty! They stink. If you have these dogs around people, bathe them please. Don’t just spray them. Dirty and air freshener/perfume doesn’t mix well. The dirty is still there. Seems like any Dr will write off for a *pet. And you can purchase vests online. It’s become too much because you get away with it.

DEAR PAW’S CORNER: I’ve had to change the time of day that I visit the local dog park, because a new neighbor brings his two huge, aggressive dogs each morning at the same time as I do. They charge other humans, chase and snarl at other dogs, and make it impossible for other dogs to socialize calmly. The neighbor does nothing to control them and tells other owners that it’s their fault that his dogs are acting aggressive. To prevent injury to my dog (or myself), I now wait until after work to go. But this is inconvenient for everyone in the neighborhood. How do I address this problem? -- Dog Mom in New York DEAR DOG MOM: You’re doing the right thing by changing the time that you bring your dog to the park. It’s important to remove a dog from a situation you perceive as potentially dangerous or traumatic for them, and protecting them from harm. That’s only the first step, however. The new dogs are terrorizing other dogs within the park, making life miserable for them and their owners. It’s not fair, nor is it right. Dog parks within a city or town typically have rules of conduct posted at each entrance, but sometimes they’re vague. It’s up to each owner to properly train and socialize their dogs, and to keep them away from the park if they can’t control rough play, aggression or reactivity. Report the problem to the organization in charge of maintaining the dog park. This may be a community group or the municipal parks department. Contacting animal control is another option. Write down the dates and times that incidents have occurred. Leisure activities may be widely viewed as fun ways to fill up free time, but the benefits of leisure activities extend beyond beating boredom. A 2011 analysis published in the journal BBA Molecular Basis of Disease found that leisure activities have a positive impact on cognitive function and dementia. The analysis, conducted by researchers with the Aging Research Center in Stockholm who examined various studies regarding the relationship between certain activities and cognitive function, defined leisure activity as the voluntary use of free time for activities outside the home. After retirement, leisure time constitutes a large part of many retirees’ lives, and finding ways to fill that time is more beneficial than merely avoiding boredom. The researchers behind the study concluded that the existing research is insufficient to draw any firm conclusions regarding the effects of certain types of leisure activities on the risk for dementia and cognitive decline, though they did note that multi-domain cognitive training has the potential to improve cognitive function in healthy older adults and slow decline in affected individuals. A multi-domain approach to cognitive training involves memory, reasoning, problem-solving, and map reading, among other activities. Aging adults who embrace activities that require the use of such skills may find that they’re not only finding stimulating ways to fill their free time, but increasing their chances of long-term cognitive health as well.

Hot Flashes Persist Well Past Menopause and Thyroidectomy

--DEAR DR. ROACH: I am 81 and had a thyroidectomy in 2017. My whole thyroid gland was removed. I also had a little bit of cancer. I have started to get hot flashes again. I had originally been on thyroid medicine before and after surgery. Then I was put on levothyroxine and decided to try NP Thyroid. I was doing better on NP Thyroid, then started getting the hot flashes again. At this point, for several reasons, I would rather stay on NP Thyroid for the time being. I was wondering if there is anything I can take for hot flashes? Years ago, in my 50s, I took estrogen, which helped, but this is a different time. I have spoken with other people who are also having hot flashes again. -- Anon. ANSWER: Hot flashes are common -- about 75% of women in North America will experience them around the time of menopause. It’s not uncommon for hot flashes to go away, then return, but I personally haven’t seen them recur after 30 or so years. I’d be concerned that there may be another cause for them, which brings me to your thyroid. Elevated levels of thyroid hormone can cause sensations very similar to hot flashes. One reason that I, along with most endocrinologists, recommend against products like NP Thyroid is because most of those products come from pigs. There are two thyroid hormones: thyroxine (T4) and triiodothyronine (T3). Humans have a different ratio of T3 and T4 than pigs do, with T4 getting converted into the active form, T3. T3 is then quickly broken down, so if it’s used, it needs to be dosed twice daily. As a result, if a person takes NP Thyroid, their thyroid levels are too high during one part of the day and too low during another. This isn’t good for anyone; the “high times” not only cause hot flashes, but also predispose people to atrial fibrillation. But it’s particularly a problem in a person with a history of thyroid cancer, where we want the thyroid-stimulating hormone (TSH, the hormone in the pituitary gland that regulates the activity of the thyroid gland) to be on the lower side. If the thyroid blood levels get low in the afternoon/evening when the T3 in NP Thyroid is gone, that can theoretically increase the risk of cancer recurrence as the TSH rises in response. So, in my opinion, you should be on levothyroxine, not NP Thyroid. (I do have a handful of patients who take levothyroxine and also take T3 twice daily. Some people are unable to convert T4 to T3 efficiently.) Estrogen is the most effective treatment for hot flashes, but it increases the risk of heart disease when used by women more than 10 years away from menopause. I don’t normally prescribe it to a woman in her 80s. However, there is a new medication called fezolinetant (Veozah), which is highly effective and reduced hot flashes by 93% in a trial. My only patient on it so far has reported 100% cessation of her hot flashes. There are other rare causes of hot flashes, including tumors that secrete substances, such as carcinoid tumors, and tumors that secrete adrenalin-like hormones (pheochromocytomas).

THE DANGERS OF BELLY FAT What’s so bad about belly fat? Seeing belly fat as a merely cosmetic concern overlooks the effects it has on overall health. According to the Mayo Clinic, belly fat is not limited to the extra layer of padding just below the skin known as subcutaneous fat. Belly fat also includes visceral fat, which means it extends to deep inside the abdomen and surrounds internal organs. Johns Hopkins Medicine notes that visceral fat in the belly is very close to the liver, which can turn it into cholesterol. From there, the fat goes through the bloodstream and may collect along the walls of the arteries, potentially contributing to a hardening and narrowing of the arteries known as atherosclerosis. The complications of atherosclerosis depends on which arteries are affected, but they can include an assortment of diseases that affect the arteries and a greater risk for aneurysms and chronic kidney disease. Johns Hopkins Medicine notes that visceral fat is just as harmful for the heart as high blood pressure and smoking. So while efforts to reduce belly fat may initially be embraced to improve appearance, individuals should know that reducing fat in their midsections also can reduce their risk for a host of conditions, some of which are deadly.

GROWTH OF THE SOLAR EMPLOYMENT SECTOR According to the Solar Foundation’s National Solar Jobs Census 2016, there were 260,077 solar workers working at 8,601 solar companies in the United States in 2016. The census also found that the five states with the most solar jobs in 2016 were California, Massachusetts, Texas, Nevada, and Florida. Twenty-eight percent of state solar workers in 2016 were females, making women the largest demographic in the solar industry. Latinos and Hispanics (17.2 percent), Asians or Pacific Islanders (9.1 percent), and veterans of the U.S. Armed Forces (9 percent) rounded out the top four demographics employed by the solar industry. Of the 260,077 solar industry jobs in 2016, more than 137,000 were classified as installation jobs that paid a median wage of $26 per hour. With roughly 38,000 jobs in 2016, solar manufacturing was the next biggest employer of workers in the solar industry.

SHARED HABITS OF ORGANIZED PEOPLE The beginning of the year is an ideal time to get organized and set oneself up for productive months ahead. It’s also a good opportunity to take inventory of seldom-used items and clear out as much clutter as possible. The amount of stuff the average person owns can affect how easy or difficult it is for that person to get organized. In 2014, professional organizer Regina Lark estimated the average American home contains 300,000 items. And it’s not just adults who love to collect and keep things, as kids contribute to the problem, too. British research, as published in The Telegraph, indicates the average 10-year-old owns 238 toys, playing with just 12 in a typical day. All of these belongings contribute to disorganization and lost time. A study by Esure Home Insurance found looking for lost items accounts for 10 minutes of wasted time each day. Getting organized can take perseverance, but it’s possible.
