Lab Finds Tiny Amount of Blood in Her Urine --DEAR DR. ROACH: My question is about microscopic hematuria. I see a rheumatologist for sarcoidosis. Last year I had various lab tests, including a urinalysis. That test and several since then have shown microscopic hematuria. The rheumatologist sent me to a urologist, who did scans and a scope of the bladder up through the right kidney. There’s a slight stricture of the right ureter. My biopsies were negative. Tests and scans continue to be OK except for the microscopic hematuria. I am a 60-year-old woman. Is the hematuria a cause for concern? -- K.E. ANSWER: “Hematuria” means “blood in the urine.” “Gross” hematuria means there is so much blood that it changes the urine color, while “microscopic” hematuria is recognizable only in the laboratory. Blood in the urine can come from anywhere in the urinary system, and there is a long list of possibilities. Although the cause is sometimes never found, your physician should consider urinary tract infection, kidney or bladder stones, and, in the right age group, cancer of the kidney and bladder. CT scans and cystoscopy are commonly performed tests, and the negative result in your case is reassuring. You have one more reason to have hematuria than others, and that is the sarcoidosis. Sarcoidosis is a condition that can affect any tissue in the body with abnormal clumps of cells called granulomata. (Just one is called a granuloma.) While the lungs are the most common site for sarcoidosis, it can affect the kidney, causing hematuria. Sarcoidosis also can cause kidney damage, resulting in a high creatinine, so that should be tested. Severe kidney disease from sarcoidosis may be treated with prednisone, but you should see an expert (nephrologist) to consider this diagnosis. The majority of people with small amounts of blood in the urine with no documented reason for it, including the cystoscopy and scans you had, will not have serious disease. However, it is probably still prudent to keep an eye on the kidneys and bladder periodically. *** DEAR DR. ROACH: I was just told that I have brittle veins. I have exhausted every place I can think of to find out more about this condition. What is it? What causes it? What should I do? -- T.M.G. ANSWER: There isn’t a condition called “brittle veins”: Whoever told you that was trying to explain a symptom you have. They may have meant that your capillaries (the smallest type of blood vessel) break easily. That’s a cause of easy bruising. They may have meant that you have stiff blood vessels, from calcium or cholesterol. That’s a cause of high blood pressure. They may have been trying to draw blood or put in an IV and found that your vein “blew,” meaning the needle went through the vein entirely and you had bleeding under the skin. There are also some rare blood disorders causing blood vessels to become fragile, and sometimes inflamed. These can be quite serious, but I doubt you have one of these. You would have been referred to a specialist. While I understand why medical professionals will use nonspecific terms like these, they can lead to confusion. *** Dr. Roach regrets that he is unable to answer individual questions, but will incorporate them in the column whenever possible. BREAST CANCER TREATMENT Most patients with breast cancer have surgery to remove the cancer. Sentinel lymph node biopsy is the removal of the sentinel lymph node during surgery. The sentinel lymph node is the first lymph node to receive lymphatic drainage from a tumor. It is the first lymph node where the cancer is likely to spread. A radioactive substance and/or blue dye is injected near the tumor. The substance or dye flows through the lymph ducts to the lymph nodes. The first lymph node to receive the substance or dye is removed. A pathologist views the tissue under a microscope to look for cancer cells. After the sentinel lymph node biopsy, the surgeon removes the tumor using breast-conserving surgery or mastectomy. If cancer cells were not found in the sentinel lymph node, it may not be necessary to remove more lymph nodes. If cancer cells were found, more lymph nodes will be removed through a separate incision. This is called a lymph node dissection. Types of surgery include the following: • Breast-conserving surgery is an operation to remove the cancer and some normal tissue around it, but not the breast itself. Part of the chest wall lining may also be removed if the cancer is near it. This type of surgery may also be called lumpectomy, partial mastectomy, segmental mastectomy, quadrantectomy, or breast-sparing surgery. • Total mastectomy: Surgery to remove the whole breast that has cancer. This procedure is also called a simple mastectomy. Some of the lymph nodes under the arm may be removed and checked for cancer. This may be done at the same time as the breast surgery or after. This is done through a separate incision. • Modified radical mastectomy: Surgery to remove the whole breast that has cancer, many of the lymph nodes under the arm, the lining over the chest muscles, and sometimes, part of the chest wall muscles. Chemotherapy may be given before surgery to remove the tumor. When given before surgery, chemotherapy will shrink the tumor and reduce the amount of tissue that needs to be removed during surgery. Treatment given before surgery is called preoperative therapy or neoadjuvant therapy. Even if the doctor removes all the cancer that can be seen at the time of the surgery, some patients may be given radiation therapy, chemotherapy, or hormone therapy after surgery, to kill any cancer cells that are left. Treatment given after the surgery, to lower the risk that the cancer will come back, is called postoperative therapy or adjuvant therapy. If a patient is going to have a mastectomy, breast reconstruction (surgery to rebuild a breast’s shape after a mastectomy) may be considered. Breast reconstruction may be done at the time of the mastectomy or at some time after. The reconstructed breast may be made with the patient’s own (nonbreast) tissue or by using implants filled with saline or silicone gel.

MEDICAL BED BLESSING I love my neighbors near and far! I placed a “need medical bed” in the “Give & Take,” and my phone rang off the hook (as we use to say). So many people opened their hearts to offer a bed for a needy person. When they heard that Mary Ellen had donated her bed, everyone said “Praise God for his Blessings on one in need”. MOLLY & I THANK YOU! Rarely in one’s lifetime does an individual encounter kindness of the magnitude I experienced when my precious 11-year-old lab suddenly became acutely ill. Bob and Debby Berry carried Molly out to my car and accompanied me to the emergency vet, remaining with us the entire time. Fortunately, Molly’s prognosis is good, and she has made a full recovery. When she came back, she had her arms full of various different brand C’s and tried to assist me to see which ones I could purchase without going over the amount available on my insurance card. How thoughtful! She went above and beyond what many others would do. Jamie really impressed us! Because of her diligence, I was able to obtain a different brand of vitamin C’s! INCONSIDERATE SHOPPING BEHAVIOR If you’re one of the vest-wearing, jack-booted tribe of women currently wandering the aisles of grocery stores, texting and yakking on your cell phones, blocking aisles, oblivious to all the other people attempting to shop around you, you can expect people to call you out on your entitled, self-centered behavior. Just do us all a favor and shop online. No one wants to hear about your brow sculpting appointment. PLEASE PROOFREAD I was reading a local newspaper (not the GVPS), and the article on the first page was riddled with grammatical errors. Does anyone proofread what is being

published, or is it just a free for all? This certainly takes the professionalism out of journalism and “quite frankly” diminishes the entire purpose of the article. NO DAYCAMPING I have lived here for 20 years, and every year, I pull out my camper, to read, watch movies, etc. in my yard. I cannot go anywhere else due to my work schedule. The neighborhood is more like town than country now: lots of lawn mowers going this year, loud cars, power tools, etc. This is the first year this has happened. I will most likely sell camper next year. I realize noise can be made, but we cannot enjoy peaceful use of our yard. We can even hear noise inside of our house.

It will tolerate almost any soil, but does require good drainage. It is quite drought tolerant, once established. Coreopsis in general are tough, easy-to-grow, sun loving perennials. Coreopsis, like other profuse flowering perennials, tends to decline after a few years, so if your coreopsis is several years old, this could be the problem. Foliage may become sparse and flowers fewer in number. You could try to rejuvenate it by cutting the entire plant off near ground level before bloom season in the spring, then fertilize and water well to encourage new growth. Shearing your Moonbeam

after its first flowering will encourage a new round of flowers. To keep plants vigorous, divide every 2-3 years. This can be done in the fall or the spring. Six hours of sun should be sufficient; however, you could try planting some of the divided pieces in different sunny locations. Gardening is often a matter of trial and error. Even the most experienced gardeners sometimes try more than one location to find where a plant is happiest. First follow what the experts advise about plant care, but if your plants arenít living up to expectations, experiment with location, more or less watering, various fertilizers and compost types, and different timing or approaches to pruning and dividing.

Once you've read one, delete it and get another one. After three months you can decide if you want to spend $9.99/month to keep the subscription going. • If you have someone who can deliver and assemble it, an outdoor propane grill can be a bit of summer fun in the middle of a cold winter. • Keep an eye on sales if you need an additional pot for the extra cooking you're doing. Halloween Candy Is No Treat for Pets DEAR PAW’S CORNER: Last year, my kids dumped out their trick-ortreat candy on the kitchen table to sort through. Halfway through it, they got distracted and left. I wasn’t paying attention either, but our two Golden retrievers, “Nala” and “Jetty,” certainly were. They both got onto the table and ate much of the candy -- of which most contained chocolate. For pet owners who don’t know, chocolate can be deadly to dogs. Never mind the candy wrappers that they had ingested! That led to a panicked rush to the emergency vet and an overnight stay for both our dogs. Fortunately, their size and overall good health helped keep them from suffering any serious complications. However, I still shudder when I think about might have happened. What if we lived in an area where a 24-hour emergency vet wasn’t available? What if we hadn’t noticed the wrappers and smeared chocolate on the floor under the table until the next morning? Rochester’s Pat Bono, manager of Rochester Beekeepers and owner and sole beekeeper of award-winning Seaway Trail Honey, keeps her bees along the shores of Lake Ontario, harvesting honey from them each week. She shares her insights on our region’s honey flavors. “This has been a phenomenal honey year,” says Pat. “We are in a very good part of the state for honey production thanks to a limestone belt that runs across the state. This creates sweet soil here, which is not found in the Southern Tier and Adirondack Regions.”
