Medication Doesn’t Stop Vertigo’s Spinning ---
DEAR DR. ROACH: I have a terrible case of vertigo. I’m not sure what causes it or what I can do to prevent it from happening to me again. I was given two medications, promethazine for nausea and meclizine for vertigo, but still the problem persists. Can you offer some kind of help? -- J.G. ANSWER: Vertigo is the sensation of movement when not moving. Although it is classically described as a spinning sensation, some people feel as if they or the world is moving in other ways. There are many causes of vertigo, but we generally break them down into two groups: peripheral causes, from the vestibular nerve, which connects your brain with the organ of balance in the inner ear; and central causes, from the brain itself. Peripheral causes are much more common and generally are labelled “benign,” even though they can be quite severe. Central causes are most commonly attributed to vestibular migraine and poor blood flow to the brainstem. Only a careful exam and, occasionally, more sophisticated tests can reveal the underlying cause of the vertigo. The most common cause is benign paroxysmal positional vertigo, and I suspect that is what you were being treated for, having received a nonspecific treatment for vertigo. Although it is commonplace, I disagree with this treatment if it is used for more than a day or two, and then only in people whose symptoms are severe. Most people get relief from an office treatment called the Epley maneuver. They can continue at home with other exercises. I would suggest that you ask your doctor about these procedures, and if he or she isn’t familiar with them, it might be wise to consult with an expert. In any event, vertigo is a symptom that can arise from several distinct medical conditions. You should find out what your diagnosis is. ***
DEAR DR. ROACH: I would like your opinion of taking a vitamin B complex capsule. A friend recommended it, but the dose of one to two capsules a day makes my urine turn bright-yellow. The daily values are from 3,000 to 8,000 percent, and that seems pretty high to me. I don’t enjoy taking it, because it smells and tastes unpleasant, but I’m mostly concerned about getting too much of a “good” thing. -- R.W. ANSWER: Here’s the good news: B vitamins are necessary, and your body is able to get rid of any excess. In fact, the yellow color of your urine is exactly that, your body spilling off the B vitamins you don’t need. There are a few medical conditions that benefit from B vitamins. Here’s the not-so-good news: You almost certainly don’t need so much, and most people don’t benefit from taking vitamins at all. A healthy diet with lots of fruits and vegetables gives you most of the B vitamins your body needs. (Vitamin B-12, by contrast, is found only in animal products, which is why vegans require supplementary B-12.) If you choose to keep taking a vitamin just for general health, I would change to a brand that has lower doses and isn’t unpleasant to take. I don’t recommend stopping megadose multivitamins suddenly. This is particularly important for vitamin C, where symptoms of deficiency can show up temporarily in people who suddenly stop high doses. *** Dr. Roach regrets that he is unable to answer individual letters, but will incorporate them in the column whenever possible. The Real Secret of the Damascus Shriners
F
or many, the name “Shriners” may conjure up images of mini cars in parades and a man in a maroon fez holding a child. However, these things only hint at all the Shriners are and do. The Shriners is an offshoot fraternity of the Freemasons, or just “Masons.” This means that while all Shriners are Masons, not all Masons choose to become Shriners. Few have truly understood this incredible organization, shrouded with mystery. Monument Mo nt iin fr frontt of the “There have been many silly Damascus Shrine rumors about us,” said Bruce Katz, an officer of the Damascus Shriners. “But basically, we’re a fraternity. We’re the second oldest out of the nearly-200 Shrine Centers. In the 1870s, one of the thirteen founders, an actor named Billy Fleming, had attended a Middle Eastern-themed party in Europe, and he brought the idea of the Shriners, with a similar theme, back with him. A Cofounder, Dr. Fleming, brought the idea to Rochester.” “The Masons of today trace their origins back to the stone masons of the Middle Ages,” explained Katz. “These masons had trade secrets to protect, so they would give each other secret phrases and handshakes as they rose through the levels of trades. Our purpose within the group is to make good men better,” said Katz. During the 1920s, this purpose took on a greater meaning. The year 1916 saw the beginning of the Polio Epidemic. It was proposed to start a hospital for crippled children, but nothing had been decided. At the annual meeting in 1922, one Shriner finally demanded, “Are we going to do something for these children, or are we just going to keep blowing bubbles?” The first Shriners Hospital for Children opened the following year in Shreveport, Louisiana. “The Damascus Shriners supports our Erie, PA and Springfield, MA Children’s Hospitals,” Katz stated. Katz described a highly recognizable image symbolizing their work: a Shriner wearing a maroon fez with a black tassel and holding a little girl. “He epitomized one of our greatest ideals,” said Katz. “‘No man stands so tall as when he stoops to help a child.’” At the 22 Shriners Hospitals for Children, treatment is given without regard to the families’ ability to pay; transportation can also be provided. “Shriners hospitals focus on orthopedic surgery, orthotics (devices to correct and straighten limbs), and prosthetics,” explained Katz, who is an associate member of the board of governors at the Springfield hospital. “It is amazing to go to see these children running around with prosthetics, all colorful and decorated,” Katz said. Springfield also has specialty support for pediatric urology and cleft lip & palate; other locations treat burn and spinal problems. These not-for-profit hospitals are supported by Shriners and their generous donors. “Globally, the Shriners spend $1.5 million on our hospitals per day,” said Katz. “We consistently work to raise these funds. Our car cruise nights, which will run Friday evenings from May 31st until September 6, regularly attract 300-400 show cars and visitors, contributing over $25,000 to the hospitals last year. We are planning to add a Super Cruise Night this year.” The Damascus Shriners will also hold a Sportsmen’s Raffle on April 27th. “We will be focusing on updating our facility, too, which is available for rental.” said Katz. “Our location has a magnificent view of Irondequoit Bay. It also includes picnic tables, a pavilion and a hall with a full-service commercial kitchen and a full bar.” The chief officer of Damascus Shiners, known as the “potentate,” is Jerry Lack. Under his leadership, the Damascus Shriners are moving

forward with several initiatives, including addressing membership. “Things are different than they were 100 years ago,” Katz said. “By tradition, we don’t ask a man to join Masons,” explained Katz. “He has to ask.” The Damascus Shriners are hoping a few good men will seek them out. “I became involved when I saw retirement approaching,” shared Katz. “We do have some members who are in their 20s, as well, and we welcome these younger men.” There are related groups for ladies and children, also: the Daughters of the Nile (women), the Order of the Eastern Star (coed), the Order of DeMolay (boys), and the Triangle (girls). “On our Utica Campus, New York Masons operate retirement homes and a world-class Heart Institute. Another local Masonic group is the Lalla Rookh Grotto, the oldest functioning Grotto in existence, which focuses on dental treatment for children with mental disabilities.” The Damascus Shrine Center in Webster also houses the Masonic Service Bureau, which operates a Medical Equipment Loan Closet. “We receive several specialty items such as crutches, wheel chairs, transport chairs, walkers, and commodes, as well as cases of Depends,” said Service Bureau Executive Secretary Jan Porter. The Loan Closet serves an - Bruce Katz 8-county area, without respect to residence. Every item is cleaned, checked thoroughly for safety and disinfected. There is no charge, but some items, like wheel chairs, require a small security deposit. This is the true secret of the Damascus Shriners. Without fanfare and often without notice, they work hard to improve the lives of so many. They provide treatment and resources for children with medical needs, without regard to the families’ ability to pay. Quietly, faithfully, they continue changing lives. I was skeptical that we would ever find our keys again and I took a risk that To the person who complained about being wished “a happy holiday”. I read several objections to your comments. One said you should not say Merry Christmas as you may offend someone. Another said you are being insensitive and disrespectful. I applaud you for saying Merry Christmas. For those who wish to change the way we or the large percentage of people living in the U.S. acknowledge the spirit of the season, I say toughen up! Political correctness is insensitive to the majority of us Americans. You claim that you want to keep our dollars local, I think not!
