Is Medication Risk Worth the Benefit? DEAR DR. ROACH: I am a 76-year-old woman with diabetes. My body, as I have been told, is bad with arthritis, for which I have been prescribed Celebrex. I have been taking it for a few months now, and am definitely moving around with a lot less pain. I recently watched a program that devoted the entire hour to Celebrex. It scared me out of my wits upon hearing the side effects of the medicine. I am asking your advice about whether to stop taking the Celebrex. -- S.A. ANSWER: All medicines have risks, and in order to decide whether you should keep taking the medicine, you need to really understand what the risks are and balance them against the benefits you feel in your ability to move around more easily. In the case of Celebrex, we have a pretty good idea of what the risks are. Like all related drugs (including ibuprofen and naproxen), celecoxib (Celebrex) can damage the kidneys, so it isn’t recommended for people with kidney disease. There also can be allergic reactions, which may be related to allergies to sulfa drugs. A few people have had vision changes. However, the most concerning risk is heart disease -- both heart attacks and heart failure. These risks definitely are increased among users of celecoxib. However, the absolute risk is small: There were about two extra cases of all heart events, including heart attack and heart failure, per thousand people taking 400 mg once daily, and six extra events in people taking 400 mg twice daily. Those risks probably are higher if you are at increased risk for heart disease (which you probably are, because of your diabetes), but it still means less than 1 percent of people per year. If the medicine isn’t helping, the risk certainly isn’t worth it. However, only you can decide if the risk is worth the benefit. Drug companies get into big trouble when they try to hide the risks of their medications, and honesty demands that we admit that all drugs and supplements have risks. Only you can make that decision, but I think it’s up to us as doctors to have a frank discussion, especially about medications intended to be taken long-term. *** DEAR DR. ROACH: I am a 72-year-old female. My doctor told me four years ago that I would need a Pap test only every two years, which I have done. She told me recently that I would not need to have any more. My tests have always been negative. I am concerned, as I would rather be proactive and know early if I had cancer. What is your opinion, and what does the American Cancer Society say? -- B.L.S. ANSWER: All of the leading organizations, including the American Cancer Society, have recommended stopping screening at age 65 for women with no history of cancer and who have had adequate screening, meaning three negative Pap smears (or two negative combination Pap and HPV tests) in the past 10 years, with at least one in the past five years. The risk for cancer in this specific group is very low. *** Dr. Roach regrets that he is unable to answer individual letters, but will incorporate them in the column whenever possible. Livonia Third-Graders Make a Difference in the Lives of Homeless Animals During the month of December, four third-graders put their passion for homeless animals and their plight to find their “fur-ever” homes to work by creating a campaign in conjunction with Livonia Central School to raise awareness of and bring in donations for Susie Q Dog ResQ and Hearthside Cats. Ultimately, they wanted to give pets a good Christmas even if they didn’t have homes. Sophie Beikirch, Gabby Peri, Aubrey Rivera and Molly Rowe researched several areas to donate to before coming up with Susie Q Dog ResQ and Hearthside Cats and then looked at the organizations’ websites to see what supplies the groups needed. After brainstorming their idea, they presented their plan to Principal Whittle who ultimately approved their idea. With the support of third-grade teacher Mrs. Willow Johnston, the girls worked hard on creating signs and making announcements to fellow students in the elementary school. Parents helped by posting the details of the drive on social media sites. Bins were put out in the school to accept donations of dog and cat food, cat litter, toys, blankets and towels, leashes and halters. They were able

to collect enough donations to fill the backs of two vehicles! Of course, without the support of their fellow students, teachers, administration and parents this would not have been possible. It was a true display of the meaning of small community support. Prevent stairway injuries Stairs are a necessity in many homes. While stairs serve a practical purpose, they also have the potential to be dangerous. Staircase and stairway accidents constitute the second leading cause of accidental injury, second only to motor vehicle accidents. Each year, there are 12,000 stairway accident deaths. The Canadian Mortgage and Housing Corporation states that a large portion of Canadians who visit hospitals after a fall on or from stairs or steps in their homes are seniors (men and women age 65 years or older). However, many stairway falls can be prevented with some simple planning. · Install sturdy handrails. A common contributor to stairway falls is a failure to use handrails. Sturdy handrails can decrease fall rates. Even if a person slips, he or she may be able to prevent the fall by grabbing on to the handrails and regaining some of his or her balance. · Rely on highlighters or friction strips. According to a study titled, “Safety on stairs: Influence of a tread edge highlighter and its position,” by Richard J. Foster, fall risk may be reduced by using a highlighter positioned flush with the tread edge. This helps make stairs more visible, particularly for those with impaired vision. · Improve lighting on staircases. Failure to see steps may contribute to falls as well. Staircases should be well lit so that residents and guests know where to step. Subtle

This year, I was diagnosed with a form of leukemia. I have encountered many wonderful service providers along this journey that have been so kind to me during this time. I have been treated better than I ever have in my life. While I really appreciate people’s caring nature and friendly ways, I challenge this mindset at the same time. I shouldn’t be treated any differently because I have a disease. We are all fighting a battle of some sort in our lives. Your battle may not be my battle, but it’s important whatever it may be. How long does it take to repair the bridges? This work has been going on for months and still isn’t totally completed yet. It seems that there is a mini crew taking their time on this job. Traffic is backed up due to lane closures all along this route and sometimes no work is even taking place at all. It sure seems like this small crew of workers is taking their sweet time with little regard to the motorists trying to get to work or home after work. Everyone knows we can avoid offensive sites, but there is one thing I don’t understand. A site I used to enjoy has so many beautiful photos of flowers and my greatest love, animals. I was browsing baby animals and the most gruesome photos were included in this pin. It only took one to get me weeping and turning off my computer, although there were many more. Cruelty is not normal. We all know

Did you know? According to the United States Department of Labor, single men and women are having an easier time finding work in the slow economy than their married counterparts. A survey conducted by the Labor Department found that single men and women had lost roughly 5 million jobs during the financial crisis. However, the same survey revealed that single men and women, in spite of the slow job recovery across the country, regained about 90 percent of those jobs. Married people have not fared nearly as well. Since the onset of the financial crisis, married people lost more jobs (6 million) than single people and have not been nearly as successful regaining those jobs, recouping only about 22 percent. The disparity could be due to a host of factors, but economists feel married people, many of whom live in dual income households, have had the freedom to be more selective when choosing their next jobs, a freedom not afforded to single men and women.
