Assistant Program Manager

Assistant program managers provide support and supervision in the homes. These individual also assist the program manager with various administrative duties.

CEO

Chief Executive Officer

Currently this is John Estrem. John has an open door policy and is always interested in hearing from families.

Christ For People

Christ For People

Christ for People is a church group that meets on Tuesday evenings at the Wayzata Free Church. This group used to be known as AMR and for many of our individuals and staff the name stuck. This group is a great socializing event which many of individuals enjoy on a weekly basis.

Goals Formal

Formal Goals

DSPs are the household staff members that provide the supervision and support for the individuals we serve. Formal goals are areas that have been identified by an individual’s team as important skill-building areas. These formal goals are documented regularly and success rates are tracked to show an individual’s progress over time. These goals evolve and serve as a way to build skills and independence.

Goals Informal

Informal Goals

Informal goals are areas that have been identified as important, but do not require the data tracking of a formal goal. Often these are areas that were formal goals in the past or are skills that an individual already has and we want to reinforce. The informal goals are still monitored and tracked by staff, just not as intensely as a formal goal.


GRH

Group Residential Housing

State-funded income supplement program that pays for room and board costs for low-income adults who have been placed in a licensed or registered setting with which a county human service agency has negotiated a monthly rate. This program pays the difference between the individual’s income less personal needs and the negotiated rate. *Note: Does not apply to apartment programs.

HRC

Human Rights Committee

Hammer’s Human Rights Committee oversees the use of psychotropic medications for our individuals. This committee is comprised of program directors, family members, community members, a pharmacist and a nurse. The goal of this committee is to ensure that individuals are taking the least amount of medications possible and that the medications they take are safe and productive.

ICF/DD

Intermediate Care facility/ Developmental Disability

These homes are 6- bed group homes. Hammer has 8 ICF/DD homes. These homes are funded differently than our typical waivered homes and therefore are overseen differently as well. They are reviewed on an annual basis by the Minnesota Department of Health.

ISP

Individual Service Plan

ISPs are county-mandated forms that are prepared by the social worker. These forms contain information about an individual’s capabilities and support needs. They also contain historical and medical information.

LLC

Limited Liability Company

LLC is a flexible form of enterprise that blends elements of partnership and corporate structures

Program Director

Program Directors supervise the program managers while also serving on committees, managing licensing and various quality improvement projects.

PM

Program Manager

The Program Manager is the supervisor of the home. They are responsible for overall operations in the homes.

PMIT

Program Manager In Training

Program Managers in training are staff members who are in the process of learning the role and duties of a program manager. These individuals assist with meeting preparation as well as administrative duties.

QOL

Quality of Life Fund

Hammer’s Quality of Life Fund is a pool of donated money that is available for those we support. This fund is intended to help them take part in activities and lifeenriching experiences. This fund helps ensure that money is not an obstacle for individuals to enjoy full lives.

Reach

Reach For Resources

Reach for Resources is a local company that organizes activities and outings for individuals with developmental disabilities. They organize regular basketball, softball and bowling leagues which many of people look forward to each year.

Referrals are used for all medical appointments. These documents

Utilization Review

REIF’s are forms that we use to keep all pertinent information about those we support in one location. These forms contain vital information such as birth dates, guardianship info, hospital preference, day program info, etc. Risk Management Plans are county- mandated forms that inform team members about an individual’s risks and capabilities in a number of areas. These forms are constantly evolving and are kept up to date by our managers Federal money paid to individuals on a monthly basis based on need. Recipients must be elderly or disabled and have insufficient RSDI credits available to meet their basic needs as set forth by the Federal Poverty Guidelines. Hammer has a team of support staff who are hired to work shifts at various houses when they are needed. These individuals are trained the same as all of our staff members. The only difference is that these people work at numerous sites instead of just one. The utilization review is a bi-annual audit of Hammer’s documentation as it relates to those we support. This audit is carried out by an external agency for our ICF homes. For our waivered homes, we complete an internal audit. These audits serve as an opportunity for our homes to get outside feedback


and ensure we are completing our documentation properly. Waiver

Waivered homes make up the majority of our group homes sites. These 4-bed group homes are overseen by county licensors.
