Whether you gave your time, talent or financial gifts, we are always grateful for your support and friendship. In this 2016 Annual Gratitude Report, you will see a snapshot of the results that we accomplished together for the individuals we serve. You will read about the impact of technology used to streamline processes, and how it saved valuable time and money. You will read about the Hammer culture of caring and our 24/7 nursing team that helps to make our high-quality of service possible for a lifetime. You will see how our advocacy efforts have made great strides so the voices of the people we support are heard. In 2016, we were voted a Top 150 Workplace for the seventh year in a row by the Star Tribune. We are honored and humbled to have the caring and talented staff that make up our organization. Giving YOU CREATED A SAFE AND COMFORTABLE HOME You gave medical care, winter coats, new shoes and even cash for a night at the movies to individuals who did not earn enough to provide for themselves. You replaced appliances, furnishings and flooring to help create a comfortable home for those who could not afford it on their own. You made sure that Hammer is home through the end of life; providing added features like grab bars, elevators, ramps and roll-in showers for those who are aging.

164 Advocates now receive Action Alerts from Hammer’s Advocacy and Public Affairs Committee. 96 are new, a 240% increase from 2015.

20% of the new advocates signed on at voter registration tables hosted by Hammer’s Public Affairs members. 39 voters were also

registered.

68 Hammer self-advocates, staff and volunteers went to the state

Capitol in 2016 to rally for disability rights in 2016!

17 Action alerts were sent out to Hammer advocates via Hammer’s Engage database in 2016 with an average open rate of 57%.

24/7 Nursing In 2016, instead of rehiring two new nurses we created a full-time nursing coordinator position. Going forward, the nurses are able to spend more time in the homes with the people we support. A standard medication packaging system was implemented across Hammer’s homes and apartments in 2016. Partnered with the training department to redesign the Intro to Health Med 101 Class, utilizing iPads for the training.
