Who’s Who Hammer Residences’ Organizational/Membership Structure Hammer Residences, Inc. is a private, non-profit, tax-exempt 501-C3 corporation incorporated in the State of Minnesota. The on-going leadership and oversight of Hammer falls to the Board of Directors to fulfill. Board members offer strategy and direction but are not involved in management or tactics. The Board in turn hires a chief executive officer (CEO) and delegates management authority to this chief executive and his/her staff. The Board elects officers, creates policies and establishes committees as it sees fit to accomplish Hammer’s mission. John Estrem is our CEO. Under John is a team of 9 executive staff members. The organizational chart on the following page shows each department, the staff member’s name in charge and his/her staff within the department. Sue Walker is the Chief Program Officer. Sue manages the 7 Program Directors. Each of the directors supervise 7-9 managers who are responsible for the operation of a house or apartment. These managers have some direct care responsibilities as well. They each have 4-15 staff, called Direct Support Professionals (DSP) that are responsible for direct care. Tom also manages the Director of the Nursing team, Brandon Eddy. Brandon supervises the four nurses who provide care to those we support.

Departments The following is a breakdown of Hammer’s various departments including the department director’s name and their general responsibilities. These departments enable us to focus our efforts on specific areas and provide the best possible services to individuals and their families.

CEO John Estrem

• Oversees all programs at Hammer and is directly responsible to the board for the collective actions of the organization

• Comprised of our group homes, apartment programs, case management, support planning, Medica and in-home services • Includes all program directors, program managers and DSPs for our homes • Responsible for providing the quality service that we continuously strive toward. Receives the support of the following departments:

Finance Department

• Monitors and maintains our operating and household budgets

• Monitors individuals’ funding sources, service agreements, etc

• Able to serve as rep payee for individuals if the families choose to have them do so • Has the ability to maintain custodian accounts for individuals to be used in addition to, or in lieu of, a bank account

Health Services Department

• Includes our in-house nursing team, as well as a contracted dietician

• Our nurses oversee individuals in the homes and monitor their health needs

• Our nurses provide care for everything from minor scrapes to psychotropic medication monitoring in addition to handling communication with physicians • Brandon provides training to new staff members in medication administration and first aid


Environmental Services Department

• Our maintenance team is responsible for maintaining our homes’ safety and aesthetics

• This team of 4 responds to all requests that come from our house managers, ranging from installing flooring to painting, fixing broken appliances and general home repair

• Hammer also contracts with a home security company to monitor our homes, as well as various plumbing, heating, and remodeling contractors Development Department

• Responsible for fundraising and maintaining the vital donor relationships that help make Hammer an industry leader

• The Development department hosts Hammer’s Reach for Ralph fund raising event each year – usually in July

• Responsible for maintaining Hammer’s computer network made up of all the houses and our office • This department’s commitment to excellence has resulted in Hammer being on the leading edge of technology, not just in our field, but in business at large • Maintains relationships with our 500+ volunteers – supporting the staff in our programs by taking individuals to games, plays, or assisting at the house as needed

Community Living

• Develops and oversees numerous activities including bowling, bingo, movies, etc
