I

f you don’t know where you are going, any road will lead you there. The above is a famous paraphrase of a conversation between Alice and the Cheshire Cat in Alice in Wonderland. Knowing where we are going is important for us as individuals, but it is also important for organizations like Hammer Residences. For a few years, regulations and funding formulas were changing so rapidly and often without warning, that the best we could do was simply keep up. Recently there has been a slight slowing in the pace of change, and so the Board of Directors of Hammer decided it was time to set a direction for the next few years. We spend about a year creating a Strategic Framework for Hammer. This framework sets “guardrails” for our work, while remaining flexible in order to respond to any unexpected changes.

Once the Board established the framework, the staff started working on goals and implementation teams for the next two years.

The goals are in three areas: Advocacy, Workforce, and the Future of Services. The teams are working hard on reaching and measuring progress toward these goals. One thing remains clear: Our central mission of helping people to experience life to its fullest remains intact. Hammer has been doing this for 96 years and will continue to stay focused on the people we support and our workforce. This issue of Discoveries looks to the future, while celebrating the “now.” We are grateful you are on the journey with us and are a part of the Hammer family. Blessings,

For the Love of Family By Karen Trygstad, Training and Education Coordinator

Other sports are just sports. But baseball is love; and baseball brought Mitchell, who is supported by Hammer, and me back together. I grew up knowing Mitch – our extended families are related to each other. Years ago, there were family dinners and visits for lunch. As a little girl, I would come along when my parents would meet Mitch and his father, Cy, at Cecil’s Karen as a young girl. Deli in St. Paul. But as I grew up, family members didn’t get together as often, and those relationships faded away. Fast-forward 40 years. I started working part-time in the Training department at Hammer. One afternoon, in the lobby of Hammer’s central office, I saw Brian wearing a baseball uniform. Of course, a conversation started about our shared love of baseball, about how my husband and two 4 | Hammer Discoveries

sons played growing up, enjoyed playing the game through college, and my sons are now on Town Ball teams.

Brian and I bonded rather quickly over our shared love of the game and our mutual connections. From those passing conversations in the hallways, a yearly event dubbed the “Hammer Holiday Homerun” was born. At our first event, which pairs local Wayzata baseball players with Hammer athletes, fate made its way into our life. This is how Mitch came back into my life. I recognized him right away. I had to tell him who I was because he wouldn’t know me. I was a little girl the last time we were together. But I asked if he remembered the extended family dinners around the big table at the yellow house. He did! Did he remember the pastrami sandwiches at Cecil’s? Of course! Mitch started to cry. I have since learned, time-and-time again, how very important family is to him.

Mitch and Karen have been friends since they were young and reunited after Karen started working at Hammer.

That was seven years ago. Since then, Mitch has met my husband and come to watch my sons play baseball. My 85-year-old mother and I have met Mitch for coffee at Caribou. I’ve brought along old family photos and we’ve reminisced. His memory is amazing. He’s reminded me of details about extended family that I’d long forgotten. He’s also come to the office with treats and to share a cup of coffee. We’ve recently begun having a “virtual” cup of coffee almost every morning. He calls the office promptly at 7:30 a.m. when I arrive and we check in with each other. He loves to hear stories about my mom, my husband, and my boys. It’s a truly lovely way to start each day, with my coffee in hand. Because of Mitch, I’ve reached out and reconnected with some Mitch on the phone with of those family members I had lost touch Karen each morning to with. And just like family does, I’ve gone check in and wish each other a good day. along on a couple of doctor appointments with him recently to be a supportive friend. My family life growing up was fractured and disconnected. Mitch’s love of family is fiercely strong and loyal. Who would have guessed what was supposed to be a 12-week position at Hammer would turn into a full-time job and a connection to my past? As fate comes into our lives, the things you least expect can unfold in the most wonderful and magical of ways. Hammer Discoveries | 5

HAMMER’S STRATEGIC FRAMEWORK By Barbara Brandt, Director of Communication

A

lvina Hammer started Hammer school in 1923 and started from her belief that individuals, of all abilities, should live a full life. Since that time, Hammer Residences has become a widely-known disability service provider in the western metro area. It is not surprising that there have been many changes since Alvina Hammer’s school and that business model, but the mission, core values of care, and compassion still stand true today. It takes people with passion for Hammer’s mission, along with the business acumen, to be able to stay relevant in times of change. Hammer has evolved over the years, moving from our Hammer campus on Highway 12 in Wayzata, into homes in communities, as well as providing in-home services to individuals who live in their own home with families. Looking at the next generation of innovative services, the Hammer Board of Directors took a big step back before they moved forward. They came to realize that while it has always worked to lead from the heart, in the next generation of services, we may also need a framework to evaluate opportunities in a more methodical and planful way. To start, a Strategy Committee was formed to talk through how the Board would approach new opportunities. The conclusion

was that the Board needed some structure for decisionmaking. The Strategic Framework seed had been planted.

Where it all began Hammer’s Board of Directors felt they needed this framework to evaluate new opportunities. What would happen if an innovative idea came along, like Hammer Travel did back in 2009, and we were not sure if it was worth the risk? How would we be able to measure those risks versus the opportunities and make an informed decision to move forward? The Board decided to take the next steps to develop a framework.

The question That conversation led to the awareness that the group needed this framework to use as “guiderails” to measure the opportunity and risk. From there a task force was developed and a consultant was hired to help us gather information to develop and move forward on the framework. The consultant gathered data from different sources: key stakeholders, donors, surveys from employees, focus groups, and 12-15 interviews held with constituents. That data was put into categories and the beginning of a framework was formed. This was presented at a Board Retreat, and the real work began.

What is the Strategic Framework? The strategic framework is a guideline to evaluate opportunities, to make sure they align with the mission of Hammer, and are financially feasible, applicable, and sustainable for Hammer as an organization. There were three main goals that were initiated through these conversations:

ADVOCACY Ensure that policy-makers create legislation, rules, and funding formulas that recognize the needs of people with developmental disabilities.

CULTURE OF OUR WORKPLACE Build and maintain a culture that enables Hammer to attract, select, and retain a highly qualified and diverse staff to meet program and service needs.

NEXT GENERATION OF SERVICES Create and pilot the next generation of Hammer residential programs and services that offer quality options to the people and families that we serve, fit with the new rate methodology, and use our funding efficiently and effectively. Within each of the three goals there are underlying strategies and 12-18-month milestones to help measure progress and achieve goals. The purpose of each goal is to align the work of staff, with the expectations of the Board around the key priorities, giving deliverables to show progress. The framework also helps staff to prioritize their work without the board oversight and gives common language between staff and board.

Why now? Within the disability service industry, there are potential risks and opportunities and as an organization Hammer needs to be ready for them and be intentional on how to take the next steps.

“It is good business as well as good practice to be aware of the risks and opportunities as they emerge, and have a tool to measure new opportunities for growth” says Carol Curoe, Hammer Board President. “With the volatility of what is happening in the disability services industry along with the changes in the rate structure, as an organization we need to be strategic and intentional,” says Hugh Kirsch, Hammer Board Member. The strategic framework is a useful tool to help make decisions in a planful way.

Where do we go from here? The Strategic Framework is completed, and teams of staff members have been formed and are currently working on their goals. Key staff members have reported on milestones at two Board meetings. The lead team members will be reporting progress to the Board on a quarterly basis. Next steps: a Strategy Committee has been formed, to provide guidance to the staff members who have ideas to work on to bring them into fruition and present them to the Board of Directors in a formal presentation using the framework to explain their ideas. WALKING THROUGH LIFE TOGETHER By Hannah Thibodeau

V

irginia was having a stressful day working at Dayton’s. It was busy and the retail industry was not something she was passionate about. That’s when Lisbeth Armstrong, former Chief Program Officer at Hammer, came up to chat.

She has worked at multiple group homes at Hammer and even helped open our Jersey home, where she currently works full-time Virginia noted that both she and Hammer have evolved quite a bit since she started. One aspect remains the same though, her favorite part of the job being the relationships she has created along the way. She has a great bond with each of the men at Jersey and also keeps in touch with other individuals she has worked with throughout the years by picking up shifts at homes and apartments outside of Jersey. At Jersey, the group enjoys going shopping, listening to music and even grilling out all year round. She helps out with the men’s routines and encourages them to be as independent as possible.

The two had known each other for many years but hadn’t seen one another for a while. As they caught up, Virginia expressed that she was having a bad day at work. It was then that Lisbeth shared that Hammer had an opening for a Direct Support Professional position and suggested that Virginia apply. The rest is history.

“Every day you look at what the needs are and get them to help themselves and believe in themselves,” Virginia says. “You don’t want to just do things for them. I’d rather we walk side-by-side through life.” That mindset is how Virginia approaches each day of work and is one of the many reasons she has been a successful Hammer staple for over 30 years. She has been there for the individuals she

supports on a smaller scale such as just going grocery shopping, to bigger life events such as siblings getting married or difficult times, such as a parent’s death. It’s the relationships that count to Virginia. For Virginia, her life as a direct support professional isn’t a job, rather an opportunity to live life with the individuals she supports and help them along the way.

Virginia had experience with Hammer in the past, as she interned in the dormitories a few years prior. Her father, Tom Carpenter, was also on the Board of Directors when she was young, as her sister was born with a disability, so he was passionate about providing services to individuals with disabilities. Thirty years later, Virginia is a seasoned professional at Hammer. She has learned a 8 | Hammer Discoveries

WHEN YOU BLEED PURPLE By Ann Martinka, Customer Relations at Hammer Travel

O

ne of Hammer Travel’s most popular trips is the MN Vikings Training Camp. This trip tends to fill up months in advance. The reason it is so popular is the experience created by Rick Spielman, Logan Johnson, and the Vikings staff, coaches and players. This year, besides getting to be on the field, each individual was given a backpack with a football to use for autographs. Bill is a die-hard Vikings fan and his family knew this was a trip he would enjoy and be able to talk about for a long time. In order for this trip to work for Bill, a one-to-one Hammer Travel staff needed to be secured to be able to support Bill on this trip. It was fun for the staff to watch Bill as he met and talked with the players. Bill was ‘almost’ speechless when meeting Adam Thielen and Kirk Cousins. He still had plenty to say about the Vikings team and let Rick Spielman in on his thoughts. I’m sure that Rick is still pondering his suggestions! Bill has decided to have his signed football put into a case to sit in his room, but he is most proud of his backpack and it has a strategic place of honor on his wheelchair. Often time our Vikings Camp travelers will call Hammer Travel both before the trip and after to talk about the team, give predictions and relay good old-fashioned Minnesota style armchair quarterbacking. Once at the field, they are not afraid to give their advice to the players as to what to focus on to have a winning season. One of the Vikings' favorite moments was when Chris told defensive linemen Everson Griffen and Danielle Hunter to “Go get that Rogers this year!” Everson responded with a yell of agreement and picked up Chris for a big bear hug. Chris is on his fifth Vikings Training Camp trip with Hammer Travel.

Each year he reports that it gets better and better! He gets excited for the next years' camp about one week into the season and calls to put his name on the list for next year. Per Logan Johnson, “From our end, we love hosting Hammer Travel at Training Camp each year. The attitude of each traveler is truly contagious for everyone they interact with— staff, coaches, and players. We can always count on this group to bring the excitement and offer advice to our players on things to focus on this year.”

BUILDING STRENGTH FOR OUR FUTURE By Cate Saracen Peters, Director of Workforce Development

A

s we entered the house, sounds of conversation and laughter filled the stairway. Brittney Severson, Program Manager at Emery, (home to four young adults), was connecting with her team of Direct Support Professionals gathered at the dining room table. All were engaged in sharing their dedicated commitment and primary focus of supporting these four young lives.

At the time, Brittney had recently graduated from Hammer’s leadership program and was actively seeking training support for her staff members. Leading strong teams, a class focus in the program, was one that Brittney clearly had a passion for. She continually encouraged her team to “lead from where they are,” sharing their experiences and learning from one another. “Here are just a few components of the program that I valued during my learning experience,” Brittney says: •

“Hammer’s investment in developing the talents of both new-in-role managers as well as those looking to gain leadership skills and reach for growth opportunities. It is a setting where everyone is comfortable and can make connections with those in a similar career path.”

“The program implements various learning styles to help us succeed: classroom, discussion, small group, onsite, mentorship, assignments, etc.”

“The content is focused on over-arching topics that are relevant to our role while still providing examples, as well as deep dive thought provoking questions and scenarios that are specific to our sites and unique experiences.”

“Training enlisted multiple instructors who are skilled in their area of expertise.”

“I have been strengthened as a leader especially by the classes in the manager training program related to conflict resolution. Since my training I have taken on the role of coach which has allowed my teams to resolve conflict themselves, learning to grow as a team in a more independent manner. My new approach champions open and honest conversation between the staff members first, while still being a support system if needed. It has helped to strengthen relationships and saves valuable time, energy, and resources resolving conflict.”

When we consider the critical importance of retention and internal recruitment for leadership roles, Hammer’s company culture of collaborative support is at the core of our success. One program cannot be credited with providing the training and support our emerging leaders are receiving. Experienced managers and directors alike are making the commitment to recognize talent while helping to sharpen skills through on-the-job training. Abby Baggentoss’s career path to Program Manager is just one example. She shares how walking beside her employees as others did for her has been pivotal and influential toward building a strong relationship with her team: Abby shares: “When I first started as a manager, my staff team was going through a lot of changes. Considering all the different personalities and trying to rally a team into working together can be a challenge. After the training program, it really helped me to be able to focus on the strengths of each of my staff and help them succeed. It’s awesome to see growth in each of them and get them excited about becoming managers themselves! We have all really grown this past year and not only the staff–I’ve seen it in all the individuals we suppoer and that’s what this job is about.” The importance of time invested continues beyond the classroom where one-on-one training is provided with new managers at their program site. With the power of the relationship being foundational to personal and professional growth, our Leader Mentoring program rounds out the new leader’s learning experience. All participants are matched up with an experienced leader from within the organization. This connection is a true mentoring relationship where trust is built over time and provides a solid starting place for next steps in their leadership journey.

Abdul Qayyumi, a recent graduate of the program shares: “I have greatly appreciated the opportunity and support provided during the program and now as a new Program Manager. This is even more exciting when you have a mentor that you can always reach out to for suggestions. It does pave the way to achievements.” Recently after teaching a class, as Lisa Barker and I walked away, we were struck by the familiar hum of engaged, enthusiastic conversation. It was the new emerging leaders utilizing their strengths, while looking forward to educating and influencing others.

OH, HOME NOW By Kim Keprios, Sister of MK

“O

h, home now” my brother Mike proclaims… this is music to my ears. Not that I don’t love him coming to my home for slumber parties and family gatherings, but because he really does feel that he is home at “Broadway.” Thanks to dedicated, patient, fun and engaging staff who support Mike and his roommates, Mike is the happiest I have ever seen him since his years at our family home, growing up together with his four chatty siblings and parents. Staff with Hammer have invested in getting to really know Mike, what he likes and what he simply doesn’t want to do, his favorite topics to talk about — Go Twins (a sad ending to a great season) music of all kinds, especially Country Western, shopping for jeans, and eating. Each team member has a gift. Carmen spoils him with her cooking talents. Sylvester bursts into Broadway with his positive energy, banter, and his beautiful singing voice. Special outings with Jeff, Allie’s weekend support where she shares good laughs with “MK,” and Sam’s efforts to keep interesting opportunities available like music therapy and massage therapy for all the guys in their home and ensure staff are well informed about the guys they support. All these thoughtful and creative acts make Mike feel cared for, valued, respected, and safe. I also have great respect and appreciation for the Hammer

Leadership Team. They stay true to the values and visions of founder Alvina Hammer and continue to seek innovative approaches to ensure they meet the evolving needs and dreams of the people they support. As a family member and a longterm professional advocate, I believe Hammer is exceptional in the inclusion and partnership they create with families— this speaks volumes and makes a difference in the quality of services and supports provided. On behalf of Mike (aka MK) and the rest of the Keprios family, we are deeply grateful to be a part of the “Hammer Family” and the wonderful team who gives their all — indeed, there is no place like home!

y real name is Jasmine, but I prefer Jazzy, especially today. I just moved into my very own apartment for the first time. My new life is just beginning thanks to all of the wonderful people who donated to Hammer’s Quality of Life Fund. I’ve lived at Hammer for eighteen years. I’m always bright and smiley (when I want to be). While growing up at Hammer I’ve been able to experience life to its fullest. I recently graduated from Wayzata High School and because of your gifts, I’ve been able to participate in lots of school activities, to take swim lessons, and go to Camp Friendship.

F

or the first time in her adult life, Wendy, 65, is going to the dentist every six months. “My dentist is nice, he listens to me, and when I had a cavity, he gave me laughing gas so it didn’t hurt,” said Wendy. Wendy’s regular dental visits are only possible because of a grant from the Delta Dental Foundation of Minnesota. She has limited financial resources, and Medical Assistance (MA), Minnesota’s Medicaid program, only provides funds for Wendy to visit the dentist once per year and no longer covers the administration of nitrous oxide to help with the pain and stress of a visit. Because of Delta Dental, Wendy is smiling more and takes great pride in her electric toothbrush and her ability to take care of her teeth all by herself. “What started with funds for electric toothbrushes, has become a game changer in the lives of those living at Hammer,” according to Brandon Eddy, Hammer’s Director of Nursing. Hammer was recently awarded a fifth grant from the Foundation for $40,000 to pay for second dental visits, oral health tools, and needed dental work to those served by Hammer, who would otherwise not have the ability to pay.

During the most recent grant period, there were a total — Wendy of 74 requests for financial assistance from 44 individuals at a total cost of $60,147. What is not covered by the Delta Dental grant is provided by generous donors to Hammer’s Quality of Life Fund. For some people, using donated funds means catching up on years, if not decades, of poor dental hygiene. For others, regular check-ups and cleaning have significantly reduced the number of cavities, down 38 percent in just one year.

udy Mason first heard about Hammer through her brother, Bob Ziegeweid. Bob had been matched as a one-on-one volunteer with John at Zealand through Hammer’s Make a Friend, Be a Friend program. Judy enjoyed hearing his stories about the fun he and John had together, doing things they both enjoyed.

But once they started to get to know each other, the walls crumbled and Brittni’s personality shined through. She really enjoys doing arts and crafts, and going out to eat with Judy. One of Judy’s favorite memories was going to see the Lego II movie. Neither she nor Brittni had seen the Lego movie, so they weren’t exactly sure what the plot was in the movie. Instead they became interested in the audience members, particularly the young male teens sitting near them. The teens kept getting up to get more and more popcorn, candy, and soda, to the amazement of Judy and Brittni. They couldn’t believe anyone could eat that much popcorn! They also couldn’t stop laughing about it. Judy would like others to know that volunteering with Brittni has had an impact on her. “Having a friend like Brittni puts life into perspective. You learn to appreciate the little things in the moment.” She also wants those who are considering becoming a one- onone volunteer to know that, “When you are volunteering at Hammer you are surrounded by goodness and love. THE DOSE HEALTH MEDISET CHANGES EVERYTHING By Brandon Eddy, Director of Health Services

Medication passing is difficult. Change is good. Those two statements have led to the most impactful change in the way we are passing medications at eight Hammer programs. Hammer staff administer approximately 1,300,000 individual pills a year. The risk for error is high due to the fact that many pills are being administered. Direct Support Professionals are becoming harder and harder to find. Using staff time effectively to ensure the individuals Hammer supports are living their lives as independently as they are able, is the number one priority of the Health Services department. Over my three years as Director of Nursing, staff members have expressed concern about the amount of time spent punching out pills and documenting these medication passes. This supports the statement, “medication passing is difficult.” Enter Dose Health and their Dose Flip electronic pill organizer, MediSet. Born from the desire to tackle the hidden epidemic of medication non-adherence, Dose Health has created the best electronic MediSet on the market. Ebony dispensing her evening meds.

In April, Alaina Gallagher, Dose Health Vice President of Sales, approached Sean Henderson, our tech guru at Hammer, with a simple question… “Can this be used in your group homes?” With that simple question, the pilot program at our Jordan group home was born. We started using the Dose Flip Medisets to administer the majority of the medications in mid-May. Within days, the staff were thrilled at the ease of use and the amount of time saved. The old medication passing process went like this:

Log into the computer Pull out 6-15 medications Match each label on the computer Punch each medication into a medication cup Triple check the medications in the cup Administer the medications to the correct person Document on the computer and medication cards

The Dose Flip has completely changed how we administer medications. Our staff spend a fraction of the time they used on preparing and giving medication which in turn allows them to spend more time with the individuals they support. The Dose Flip also allows the individuals to dispense their own medication, which they were unable to do before we started using the Dose Flip system. The Dose Health MediSet has dramatically freed up a lot of extra time for staff to do other things in the morning. It's given the individuals much more control in taking their own medications. There also haven’t been med errors since we started Dose Flip, which is huge!

Staff members would do that for four individuals, three times a day. This process would take anywhere from 45-75 minutes per medication passing time, which means roughly three to four hours per day.

It now takes staff approximately ten minutes to administer medications each passing time, which ends up being roughly thirty minutes per day. Change is good! With this extra time, staff are able to focus more on each individual. With the success of this pilot program, Hammer has expanded the use of the MediSet to seven more group homes across Hammer. Our next question is, can we use Dose Flip to increase individual independence across Hammer?

WOW, I’M ACTUALLY REALLY ENJOYING THIS! By Katie Pease, Community Life Coordinator

A

t Hammer, the Community Life department focuses on providing opportunities for community engagement, personal wellness, fitness, and nutrition for all individuals supported by our organization. At the core of our work is the heart of Hammer’s mission to help those in our care design lives that are full and happy. Having a place to belong, a place to be yourself, a place with friends and support—all of these things contribute to overall happiness.

It is all good and well to have positive intentions with the planning and execution of activities and events; however, it takes a great deal of courage for someone to try something new. To better illustrate this, I would like to share a little bit about an individual named Jill and some of her experiences with Community Life this summer.

Jill is supported through Hammer’s Avana apartment program. I have known Jill for several years, but over this past summer especially, I had the pleasure of seeing her at more activities and events. Jill has a friendly personality and a smile that lights up a room. During Hammer Bowling this summer, Jill was faced with disappointments when it came to her personal scores. One of the staff members at her apartment program noticed this and made it a point to encourage Jill to continue trying and to not associate her personal worth with bowling scores. The staff member reminded Jill that bowling could be viewed as a fun opportunity for socialization and exercise and that scores don’t really matter. The consistency with this staff’s encouraging words saw Jill back week-after-week throughout the entire summer of Hammer Bowling and toward the end of the season, Jill got her first strike. Another moment highlighting Jill’s courage came during our Steamboat Minnehaha event. Jill was unsure if she was going to follow through with joining the Hammer group for this event, but ultimately ended up coming along. During the trip, she socialized with several people she knew and then made her way up to the front to try her hand at captaining the boat. Donning the captain’s hat and grabbing the wheel firmly with both hands, the biggest grin began to grow on Jill’s face. The happiness of that moment was palpable, and then it got even better as Jill exclaimed, “Wow, I’m actually really enjoying this!” This summer was a fun one at Hammer. Here’s to more strikes at bowling, smiles on boat trips, and reminders of just how brave a person can be when faced with the unknown.

Make a Gift Directly from your IRA to Hammer. If you are 70 ½ or older, you can give a gift directly from your IRA to support Hammer Residences. While you will not receive an income-tax deduction, you will not pay taxes on any distributions made directly to Hammer. A gift from your IRA also reduces your taxable income, which could help reduce your Medicare premiums and the amount of Social Security benefits that are taxable. Contact your IRA plan administrator to request that your qualified charitable distribution is made directly to Hammer Residences today.
