YOU'RE INVITED!
In the wake of the COVID-19 outbreak that upended life as we knew it, a silver lining has emerged for the broader home and community-based services (HCBS) sector. This pandemic has proven a catalyst for personal care and health care innovation, which strengthens the potential for in-home care to thrive. In spite of the many challenges facing home care — including outdated, low reimbursement, the workforce shortage crisis, and burdensome government policies — the future is brighter than it has ever been. In the Medicare arena, providers are engaged in the Review Choice Demonstration (RCD) while preparing for an update to the Outcome and Assessment Information Set (OASIS-E) and the nationwide expansion of the Home Health ValueBased Purchasing (HHVBP) model, all less than two years since the Medicare payment system was reformed. Medicaid recently received a rare influx of government funds, including $1.1 billion from the federal government for recruiting and retaining direct care workers and a $15
minimum wage mandated by state law to strengthen the health care sector. Even so, the health care labor shortage continues to disproportionately impact Medicaid providers, including private pay and licensed-only providers that contract with Statewide Medicaid Managed Care plans. Whether your organization provides Medicare, Medicaid, or private pay home care services, or if you supply home care products or services, HCAF invites you to attend HomeCareCon, our 33rd Annual Conference & Trade Show, this summer. This conference aims to bring together visionaries, leaders, and innovators with the insight and experience to help front-line providers achieve economic prosperity and large-scale successes in this new era of home care. Plan your HomeCareCon experience and register today to help bring back The Roaring Twenties in Orlando this July 19-22!

RCD Tips to Make Sure You "Know Your Onions" and Get All Your “Mazuma”
Sara A. Bazzigaluppi, Esq., Ronald W. Chapman Sr., Esq., and Jonathan Meltz, Esq., Chapman Law Group
Join Palmetto GBA representatives for a presentation on several key areas of the Medicare Review Choice Demonstration (RCD) program that providers will face. The speakers will cover RCD basics, tips for navigating each selection, and review the most frequently asked questions from home care providers. The trio will also tackle common issues and take your questions.
In this all-encompassing session, our health care attorneys will cover some of the more pressing matters in compliance, licensing, regulatory, health care fraud, and criminal law: • Impairment Programs: Florida’s impaired practitioner programs (IPN/PRN) serve as “alternatives to discipline” for Practice Act violations, but they carry high burdens for both employee and employer. Attendees will learn how to identify impairment and know when reporting is required, the pros/ cons for reporting, and what kinds of exposure to liability there are for impairment cases. You’ll also learn the drawbacks of the current impairment program system, as well as alternative solutions that nurses and administrators can take with impairment issues.
Michael Bibbins, Elizabeth Brogdon & Belinda Marin, Palmetto GBA
• Background Checks, Exclusions, Exemptions, and Approvals: What are the best practices when running required Level 2 background checks with the Agency for Health Care Administration (AHCA)? This session goes in-depth on what an AHCA detailed investigation is all about, ways to access the data you need for hiring, how to obtain disqualification exemptions so skilled nurses can join your home care agency, and methods to efficiently navigate AHCA’s regulatory labyrinth. • Third-Party Payor Audits: This session discusses common examples of how home care agencies get into trouble with commercial and government payors; practical ways to protect your entity from audits and investigations; best recordkeeping practices; and how an effective compliance program will keep your staff in line. • How to Conduct Corporate Internal Investigations: Coordinating an internal investigation upon learning of illegal acts (health care fraud, drug diversion, etc.) isn’t just the sound thing to do — it also can help in negotiations with federal investigators regulators, or it can lay the foundation for a solid defense in court. We’ll go over reasons for an internal investigation, how to put one together, who should be conducting it, what the protocol should be, assessing culpability, and employee rights during an investigation. • How Do Recent AHCA Changes Affect Contractors?: If your home care agency relies on 1099 contractors, newly implemented AHCA regulations could restrict their use. Here you’ll get specifics on what could happen if nurse registries get carved out of Medicaid managed care contracts.

When leaders focus on excellent, employee-centric leadership, positive organizational results will follow. Please join J’non Griffin as she quickly shuffles through a crosswalk of anticipated changes between the assessment forms. Unlocking Performance Value in Care at Home: Strategic Use of Analytics Cindy Campbell & Katherine Morrison, WellSky
Providers of home health, hospice, and personal care are increasingly supporting high patient acuity and complex needs within a value-based market. In order to achieve top, key outcomes at the lowest cost, providers need to accelerate their competitive status. The effective use of data can lead to top performance among all home care providers. Using analytics within your clinical modeling and business development builds positive positioning within a value-based health care environment. This session will focus on the power of predictive and performance analytics within home health, hospice, and personal care, identifying key data levers that enable you to boost performance and accelerate your growth. 11:30 AM-12:20 PM • BUSINESS OPERATIONS TRACK • MEYER 2
Partnerships Beyond Medicare: Maximizing Care, Minimizing Risk & Maintaining Referrals Angela Hays, Nuclear Care Partners
The federal Energy Employees Occupational Illness Compensation Program Act (EEOICPA) offers unique benefits to former nuclear workers, but also places Medicare-certified home health providers at risk of denial or recovery of claims for EEOICPA-eligible patients and covered illnesses. Providing care for a patient to remain at home often requires multiple types of home care providers, including Medicare, EEOICPA-certified, and private pay. During this session, participants will learn how working with non-Medicarecertified, EEOICPA-certified providers can maximize patient care while minimizing Medicare recovery risks. —

Balancing Efficiency & Quality: Keeping Things “Copacetic” for Roaring Success in Value-Based Care Delivery Carissa McKenna, McBee & Sparkle Sparks, Sutter Care at Home
With the implementation of the Patient-Driven Groupings Model (PDGM) in 2020, providers have had to adapt to the new system’s incentive to maximize efficiency while maintaining a high quality of care. In their pursuit of efficient, quality care delivery under the PDGM, the most pressing challenges providers have encountered have been adjusting to the two 30-day payment periods within

Growing Your Roots: The Essential Grassroots Marketing Initiatives for Long-Term Growth Sean McHale, Saint Michael’s Home Care
The growth of internet-based marketing has caused some home care providers to lose sight of the need to personalize their marketing efforts. Patients/clients receiving care at home are so accustomed to receiving high-level service, and they want to maintain a personal relationship with the agency providing the care. Home care agencies need to adopt marketing strategies at the grassroots level, and this presentation explains how to leverage community-based initiatives that will plant the seed for future growth. The best part is that grassroots programs are usually free or very inexpensive, you only have to invest your time! 3:30-4:30 PM • MEDICARE TRACK • CITRON CENTER

“Humdinger” Case Conferences: Extracting Maximum Value for Your Organization Peter Caudle, Coastal Home Health and Hospice
What value do case conferences bring to your agency? Are your field clinicians attending your case conferences with a high attendance rate or is it difficult to get them to attend? Are your clinicians engaged in timely, concise, and professional conversations during patient reviews or does patient discussion “creep” occur? Are your case managers owning their leadership responsibilities or do they fail to know “who’s on my case?” During the time your clinical team spends together, are they focusing on ensuring safe and timely discharges, or are they highlighting bobbles in care planning, such as recertifications around medical social worker assistance or unaddressed MediPlanner strategies? Case conferences offer the opportunity to positively impact key aspects of your home health agency: organizational culture, team communication, case manager empowerment, and improving key quality measures. By conducting a consistent, tight, professional, and engaging case conference, agencies can achieve their organizational objectives, since it touches on so many key areas of operations. In this session, you will learn how to maximize the value of case conferences for your organization.

Why Should You Provide Behavioral Health Services Now? Amy Craven & Katherine Vanderhorst, C&V Senior Care Specialists, Inc.
COVID-19 has emphasized the importance of mental health services. According to a June 2021 report, 40% of people had mental health or substance abuse problems. Patients/clients receiving home care experienced unprecedented levels of anxiety and depression due to the increased isolation caused by COVID-19. Still more troubling, the suicide rate in many states now exceeds the COVID-19 mortality rate. Over the past two years, COVID-19 quarantines and closures have strained access to mental health services despite the demonstrated need for them. In this session, we’ll discuss why your organization should provide behavioral health services, and how adding this service can assist patients/clients with managing mental health conditions, which can negatively affect their medical comorbidities. 3:30-4:30 PM • BUSINESS OPERATIONS TRACK • SUNBURST
Actionable Value-Based Dashboards: Understanding & Taking Action on KPIs That Lead to Success Mike Carr, Axxess
In this new era of value-based purchasing (VBP), the first step to success is understanding the value of Key Performance Indicators (KPIs). The next and most important step is to develop a business intelligence dashboard that contains the metrics that matter. Metrics should be easy to understand and should guide operators to take corrective action simultaneously. This will turn a possible negative outcome into a positive one. The goal of the workshop is to provide participants with an understanding of root cause analysis and multiple contributing factors affecting metrics so they can take rapid corrective action. If you’re looking to streamline your operations and achieve financial success with your VBP program, you’ll want to attend this session.

PDGM Operational Summit: Interactive Session to Address Your Outcomes Arnie Cisneros & Kimberly McCormick, Home Health Strategic Management (HHSM)
As home health moves forward with the Medicare Patient-Driven Groupings Model (PDGM) and other IMPACT Act reforms, we encounter home health providers who want to improve their operations for value-based purchasing (VBP). After talking with these agencies about operational areas related to value outcomes, it became apparent that they fail to recognize the improvements they need to achieve those outcomes. While most providers take the “we’re fine” approach in terms of operations, they simultaneously lose clinical and fiscal returns in areas such as intake, scheduling, SOC (Start of Care) control, POC (Plan of Care) management, content management, and matching Star Ratings to HHCAHPS (Home Health Consumer Assessment of Healthcare Providers and Systems). In this interactive session, we will discuss the nuances of operations for the PDGM, VBP, and other future reforms. We will also demonstrate areas of your agency where a review of operations can lead to success today and tomorrow.

Tips and Myths Leading a Multigenerational Workforce Mike Carr & Anita Paxton, Axxess
As a result of the aging demographics of the workforce combined with the reorientation of priorities prompted by the pandemic, many have labeled 2022 “The Great Resignation” or “The Great Reshuffle”. This human capital conundrum has led to the realization that knowledge transfer, retention, and upskilling should be a top priority when managing this multigenerational workforce comprising four and five cohorts. Although technology can boost productivity and efficiency, there are myths surrounding cohorts’ application and competence, which may challenge the culture of an organization. Discover various myths surrounding the generations and learn practical tips for embracing the diversity of a multigenerational team to improve your organization.
Globally, the number of individuals with dementia will nearly triple to greater than 152 million by 2050. There is an impending need and an opportunity for home health agencies to promote interventions to assist people with dementia to maintain their current level of function in the home setting. Individuals with dementia lose independence due to progressive neurological deterioration of the disease. There are numerous strategies that can be employed to help the client with dementia to live at least restrictive level of care, improve quality of life and delay placement. Behavior and functional issues that are addressed are often the precipitant to a move to a higher level of care. What can your agency do to enhance your dementia care offerings and foster revenue growth. 10:50-11:50 AM • MEDICARE TRACK • CITRON CENTER
After Receiving Secret Knock on the Door, How to Improve Your Certified Intake Process J’non Griffin, SimiTree Healthcare Consulting
Home care’s intake process is no longer simply receiving the information given from a referral source and performing the necessary data entry. The PDGM's (Patient-Driven Groupings Model) devil is in the details. Eligibility requirements, homebound criteria, appropriate referring diagnosis and conditions, recent procedures or injuries, special skilled service needs and clinical characteristics all determine home health periods of care and meaningful clinical groupings and payment categories. After your agency receives that elusive and potentially rewarding referral, does your staff know the critical details to ascertain, the pertinent questions that need answered, the components of a complete physician visit note, and how to communicate all of this to clinical and billing staff? As the other teams receive these details, do they feel empowered to follow-up and request additional information if they recognize a gap in needed details? A continuous feed-back loop should be implemented to perfect your intake and communication process. In truth, your entire team needs to know the signs and signals, much like secret handshakes, to implement a successful intake process under the PDGM.

Strategic Development: Process Improvement Initiatives for Times of Transformation Michael Puskarich, McBee
This session will discuss why strategic process improvements are essential during times of change. The effects of the pandemic on your organization’s culture, morale, recruitment, retention, as well as financial outcomes — all combined with the ever-changing regulatory and reimbursement landscape — can make the future seem overwhelming. With the use of strategic process improvements, the impact of the Home Health Value-Based Purchasing (HHVBP) Model, the sustained growth of Medicare Advantage,

This session will help you understand how the strategic planning process works, how it can prepare you for these changes and the ones to come, and, above all, how to roll these initiatives out to your staff so they can ensure your agency will thrive despite the stresses of the operating environment today as well as the rise of these transformational reimbursement models. 2:50-4:10 PM • GENERAL TRACK • CITRON EAST/WEST

Making Your Agency's Customer Service the Bee's Knees
Joe Osentoski, Gateway Home Health Coding & Consulting
John Dalton, Optimum RTS
Never mind the Public Health Emergency (PHE), in addition to the Review Choice Demonstration (RCD), there are multiple Medicare, Medicare Advantage, and commercial insurance reviewers checking records for accurate claim payment. The best defense against these reviews is generating compliant clinical record content at the time the record is created — not after. By linking actual regulatory requirements with known denial reasons, it shows how proper documentation can avoid a negative outcome. This also applies to all RCD options. Having this knowledge is the basis for staff education so the benefit of prevention can be achieved. Current staff can learn how to chart it right; new staff learn from the actual regulations to start right. The topics covered also apply to Medicare Advantage reviews that occur and are not subject to the RCD.

During this session will discuss how professional etiquette and customer service needs to flow from the top of an organization down through the internal staff and out to our patients. These skills will be discussed in-depth and easy-to-accomplish goals will be provided that will make your team the envy of your community.

Compassion Fatigue & Burnout: New Perspectives for Providers, Teams, and Organizations Lourdes Wiley, Amity Healthcare Group, LLC
Compassion fatigue and burnout are leading causes of health care workers leaving their jobs and professions. The “solution” has been vaguely framed as an individual responsibility, as opposed to an organizational or systemic issue. As health care organizations face unprecedented staffing shortages and recruitment and retention problems, it is time to consider a different approach to creating sustainable teams. Rather than pressing individuals who are already stressed to “fix” themselves, workplace wellness requires “self-care” at the individual, team, and organizational levels. Compassion fatigue and burnout affect the entire organization, as well as the patients. The goal of this presentation is to give clinicians and non-clinicians alike a new perspective on how to care for themselves, each other and take responsibility for creating a welcoming and healthy workplace.
